Metabolic diseases have shown a global trend of high prevalence in recent years, causing a heavy social burden. In Chinese medical theory, the spleen is responsible for transporting water and grain throughout the whole body. The dysfunction of the spleen in transportation would affect the fluidity of the human body, resulting in phlegm and dampness, eventually leading to metabolic disturbance. However, the microscopic mechanism of the above phenomenon remains unclear. Intestinal homeostasis refers to the dynamic state of equilibrium formed by the interaction between the host mucosal barrier and the intestinal microecology. The factors such as intestinal flora and its metabolites, intestinal barrier, intestinal mucosal immunity and the host intestinal function are involved. According to the theory of visceral manifestations, the intestinal tract belongs to the spleen category in traditional Chinese medicine, and the destruction of intestinal homeostasis is closely related to metabolic diseases. This article proposes that the “intestinal origin theory”of metabolic diseases could provide a new scientific interpretation of the traditional Chinese medicine theory of spleen dysfunction in transportation, explain its scientific connotation and provide a novel intervention target for the prevention and treatment of metabolic diseases by traditional Chinese medicine.

Ruxiang(Frankincense)has been imported since the ancient times and is one of the commonly used traditional Chinese medicines in clinical practice. However, there is a discrepancy between the actual used species and those recorded in the Chinese Pharmacopoeia. Combining field investigations with textual research helps to clarify the truth. This study aims to restore the historical context of Ruxiang(Frankincense), conducting the field investigations in Oman, the main producing area of Ruxiang(Frankincense), covering the distribution of Ruxiang(Boswellia)plants, the origin of their Latin names, harvesting and processing methods, specifications and producing areas. It also integrates the researches such as consulting specimens from the Royal Botanic Gardens, Kew, investigations at port drug inspection institutes and medicinal material markets, as well as sorting out the historical literature, to explore the historical evolution and changes of Ruxiang(Frankincense)in terms of producing areas and species. The results show that Ruxiang(Frankincense)was introduced to China as early as in the Han Dynasty via land and sea routes, originating from the Arabian region, and is the resin exuded from the bark of Arabic Ruxiangshu(Boswellia sacra). Both Kashi Ruxiangshu(Boswellia carterii)and Kashi Ruxiangshu(Boswellia bhaw-dajiana)recorded in the Chinese Pharmacopoeia are synonyms of Arabic Ruxiangshu(Boswellia sacra). Ruxiang(Frankincense)is obtained by cutting the bark with a special tool called “menguf”;after the exuded resin dries, it is harvested using the same “menguf”. It is classified into four types according to its producing area, with the best quality coming from inland regions with a dry and hot climate. During the mid to late Tang Dynasty, maritime imports became the main source, still dominated by Arabic Ruxiangshu(Boswellia sacra). In the Song Dynasty, new producing areas of frankincense emerged in East African countries such as Somalia, and the origin of Ruxiang(Frankincense)included multiple species of the Boswellia genus. In the Ming and Qing dynasties, Sri Lanka and India were added as the new producing areas, and Chiye Ruxiangshu(Boswellia serrata)was included as an additional origin of frankincense. In modern times, Sudan has become the main producing area, with Zhipi Ruxiangshu(Boswellia papyrifera)as the primary source of Ruxiang(Frankincense), which is inconsistent with both the records in the Chinese Pharmacopoeia and the historical facts that Oman was the main producing area and Arabic Ruxiangshu(Boswellia sacra)was the primary origin of Ruxiang(Frankincense)in the past dynasties. The research results are helpful for restoring the ancient species and producing areas of Ruxiang(Frankincense), and provide references for the revision of the Latin names and species of Ruxiang(Frankincense)in the next edition of the Chinese Pharmacopoeia and the addition of producing countries of frankincense in the Catalogue ofNon-first-time Imported Medicinal Materials.

Banxia(Pinelliae Rhizoma)contains various chemical components such as alkaloids, organic acids, volatile oils, amino acids and sterols. Traditionally, it is known for its effects of drying dampness and resolving phlegm, descending counterflow to stop vomiting, and dispersing masses and nodules. Modern research has revealed that Banxia(Pinelliae Rhizoma)also possesses pharmacological activities, including antitumor, anti-inflammatory, antibacterial and antiepileptic effects. However, it is associated with adverse reactions such as mucosal irritation, hepatorenal toxicity and embryotoxicity. Due to its “toxic”nature, Banxia(Pinelliae Rhizoma)must be processed before oral administration. This article systematically reviewed the chemical composition, pharmacological effects and toxicity of Banxia(Pinelliae Rhizoma). Furthermore, it introduced the core concept of the quality marker(Q-marker)of traditional Chinese medicine. Based on this, potential Q-markers of Banxia(Pinelliae Rhizoma)were predicted and analyzed from multiple perspectives, including plant phylogeny and chemical specificity, traditional medicinal properties, chemical transformations during processing, component measurability and compositional expression in compatibility environments. The aim was to provide a theoretical foundation for establishing a scientific and comprehensive quality standard for Banxia(Pinelliae Rhizoma).

Osteoporosis(OP)is a kind of systemic bone disease characterized by decreased bone mass and degeneration of bone microstructure, and its incidence is increasing with the increase of population aging. Because of the limitations of the current treatment methods, the search for safer and more effective treatment strategies has become a research hotspot. Bone marrow mesenchymal stem cells(BMSCs)play an important role in the pathogenesis of osteoporosis, and their abnormal function is closely related to the occurrence of osteoporosis. Icariin(ICA), as a commonly used Chinese medicine for tonifying kidney and strengthening bone, has the potential to regulate BMSCs. In this review, ICA regulates the mechanism of BMSCs in the treatment of osteoporosis. ICA can promote the differentiation of BMSCs into osteoblasts, inhibit their differentiation into adipocytes, and improve their migration and homing ability through various ways, thus promoting bone formation and inhibiting bone resorption. The mechanism of ICA involves a variety of signaling pathways, including Wnt/β-catenin, BMP-2/RunX2/OSX, PPARγ, etc. By regulating these signaling pathways, ICA can affect the gene expressions of BMSCs, and thus affect bone metabolic balance. In addition, ICA also has the effects of promoting osteoblast proliferation, inhibiting osteoclast function and anti-inflammation, which together exert the effect of anti-osteoporosis, and is expected to become a new choice for treating OP.

Female androgenetic alopecia is one of the most common types of hair loss in women, and its pathogenesis is not fully understood. Modern medicine often has problems with long treatment cycles and poor patient compliance in the treatment of female androgenetic alopecia. Traditional Chinese medicine has a long history and significant therapeutic effects in treating female androgenetic alopecia. There are various methods of syndrome differentiation and treatment, which have the characteristics of multi-target, holistic and local treatment. Hair is the sign of blood condition. When the blood is sufficient and nourished, the hair can be moistened. The concept of “blood being basis of women”runs through the entire life process of women. Throughout the history, the medical practitioners have attached great importance to the maintenance of Yin and blood in the diagnosis and treatment of female diseases, which plays an important role. Based on the concept of “blood being basis of women”, it is believed that the key pathogenesis of female androgenetic alopecia lies in three aspects: blood deficiency, blood heat, blood stasis. In response to the different pathogenesis of blood deficiency, blood heat and blood stasis, this paper proposed to treat female androgenetic alopecia from the perspective of blood, emphasizing the four key treatment points of promoting blood circulation for removing blood stasis, nourishing and replenishing blood, clearing heat and promoting diuresis, cooling blood for calming endogenous wind, in order to provide new directions for clinical and scientific research in the diagnosis and treatment of female androgenetic alopecia from multiple perspectives and levels.

    Objective

To investigate the effect of Guben Qingyuan Prescription(固本清源方)on inhibiting cancer-associated fibroblast(CAF) in the development of resistance to enzalutamide.

    Methods

A tumor-bearing mouse model of enzalutamide-resistant prostate cancer cells was established to observe the effects of Guben Qingyuan Prescription combined with enzalutamide on tumor growth and CAF in tumor tissues. The effects of Guben Qingyuan Prescription on CAF transformation were assessed using immunofluorescence. CAF proliferation was evaluated by the CCK8 assay, and the influence of Guben Qingyuan Prescription on CAF-derived factors was measured via enzyme-linked immunosorbent assay(ELISA).

    Results

The tumor inhibition rate of the Guben Qingyuan Prescription combined with enzalutamide ranged from 29% to 47%. In the medium - and high-dose groups, the combination treatment showed a statistically significant difference compared to the enzalutamide-only group(P<0.01). Immunofluorescence and immunohistochemistry results demonstrated that the expressions of CAF markers fibroblast activation protein-α(FAP)and α-smooth muscle actin(α-SMA)in tumor tissues of enzalutamide-resistant mice were significantly higher than those in the group inoculated with prostate cancer C4-2B cells. The medium - and high-dose Guben Qingyuan Prescription combined with enzalutamide groups significantly inhibited the expressions of FAP and α-SMA compared to the enzalutamide-only group(P<0.05). Additionally, immunofluorescence showed that serum containing medium - and high-dose Guben Qingyuan Prescription groups could inhibit the expressions of FAP and α-SMA in vitro. After 48 and 72 hours of treatment, the inhibition rates of the medium - and high-dose Guben Qingyuan Prescription groups significantly increased, with a statistically significant difference compared to the control group(P<0.05). Further studies revealed that Guben Qingyuan Prescription reduced the concentrations of transforming growth factor-β1(TGF-β1), tumor necrosis factor-α(TNF-α), stromal cell-derived factor-1(SDF-1), Wingless/Integrated-3a(Wnt3a)and matrix metalloproteinase 2(MMP2)in CAF supernatants.

    Conclusion

Guben Qingyuan Prescription has a significant effect in delaying resistance to enzalutamide, a novel endocrine therapy for prostate cancer, and its mechanism of action may be related to the inhibition of CAF.

    Objective

The aim of this study was to investigate the effects of myristicin on the regulation of endonuclease/exonuclease/phosphatase family domain containing 1(EEPD1)expression and its subsequent impact on the proliferation, migration, invasion and apoptosis of pulmonary squamous cell carcinoma.

    Method

Molecular docking techniques were employed to analyze the interaction between myristicin and the target protein EEPD1. Immunohistochemistry method was used to assess the levels of EEPD1 protein in lung squamous carcinoma and adjacent paracancerous tissues on tissue chips. The gene expression activity in cell lines was evaluated using real-time quantitative polymerase chain reaction(qRT-PCR), while the effect of myristicin on the proliferation of lung squamous carcinoma cells KNS-62 and NCI-H226 was assessed with Cell Counting Kit-8(CCK-8). Scratch assay and Transwell migration and invasion experiments were conducted to examine the impact of myristicin on the migration and invasion capabilities of lung squamous carcinoma cells. Flow cytometry method was utilized to determine the influence of myristicin on the apoptosis of lung squamous carcinoma cells, and Western blot(WB)was employed to analyze the expression of EEPD1 protein in these cells.

    Results

Myristicin could form specific binding with the EEPD1 protein. Higher EEPD1 positive signal intensity was observed in lung squamous carcinoma tissue samples compared to adjacent paracancerous tissue control areas. CCK-8 assay results demonstrated that myristicin significantly inhibited the proliferation of lung squamous carcinoma cells. A slower scratch healing rate suggested that myristicin could inhibit the migration of lung squamous carcinoma cells. Transwell assay results similarly showed that myristicin could inhibit the migration and invasion of lung squamous carcinoma cells. Flow cytometry data revealed that myristicin promoted the apoptosis of lung squamous carcinoma cells, while WB analysis indicated that myristicin reduced the expression level of EEPD1 protein.

    Conclusion

In vitro experimental evidence supports that myristicin exerts its anticancer effects by down-regulating the expression of EEPD1 protein, specifically manifested in inhibiting the proliferation, migration and invasion of pulmonary squamous cell carcinoma and promoting cell apoptosis.

    Objective

This study explored the pharmacological effect and mechanism of wogonin on lung metastasis of breast cancer after myocardial infarction, and provided potential drugs and experimental basis for the treatment of lung metastasis of breast cancer.

    Methods

In the experiment, the mouse model of myocardial infarction and breast cancer was established by 6-7 weeks old female balb/c mice, and the effect of wogonin on lung metastasis of breast cancer after myocardial infarction and macrophages in mouse tumors was detected. THP-1, RAW264.7 and bone marrow-derived macrophages(BMDM)cells were induced in vitro to construct macrophage polarization model, and the effect of wogonin on macrophage polarization and the changes of key proteins regulating polarization were detected.

    Results

The results showed that compared with those of the model group, the number of lung tumor metastasis, the weight of breast tumor, cardiac function and cardiac inflammation were significantly reduced, and the expression of M2 macrophages in tumor was significantly reduced in the wogonin group. In vitro experiments showed that wogonin significantly inhibited the M2 polarization of macrophages, reduced the phosphorylation of signal transducer and activator of transcription 6(STAT6)and inhibited the migration of MDA-MB 231 cells.

    Conclusions

The above results indicate that wogonin can inhibit the lung metastasis of breast adenocarcinoma after myocardial infarction, and its mechanism may be realized by inhibiting the phosphorylation of STAT6 and then affecting the M2 polarization of macrophages in the tumor.

Quercetin, as an active ingredient in traditional Chinese medicine, shows remarkable potential in the treatment of acute myeloid leukemia(AML). Quercetin inhibits AML cell proliferation by regulating cell cycle proteins and indirectly regulates the cell cycle through heat shock proteins(HSP)and others. Quercetin induces apoptosis in AML cells by various mechanisms, such as regulating apoptotic protein expression, affecting mitochondrial function, inhibiting the phosphatidylin-ositol-3-kinase(PI3K)/protein kinase B(AKT)pathway and modulating the ERK and AMPK signaling pathways. Quercetin can affect AML through vascular endothelial growth factor(VEGF)expression.in AML cells, quercetin promotes cellular autophagy and affects autophagy key junctions and signaling pathways such as PI3K, hypoxia inducible factor-1 α(HIF-1 α)and mammalian target of rapamycin(mTOR), but exhibits a protective effect in MV4-11. Quercetin exerts antioxidant damage and affects oxidative stress in AML by regulating nuclear factor erythroid 2-related factor 2(Nrf2), nuclear factor kappa-B(NF-κB)and PI3K/AKT pathways. In addition, quercetin has epigenetic regulatory effects, inducing DNA demethylation, inhibiting HDAC activity and promoting histone H3 and H4 acetylation, thus inhibiting AML. This paper reviewed the therapeutic effects of quercetin on AML through multiple mechanisms, providing new strategies and directions for traditional Chinese medicine in the field of AML treatment.

Rheumatoid arthritis(RA)is a systemic chronic autoimmune disease with a complex pathogenesis that has not been fully elucidated to date, hindering the development of new anti-RA drugs and imposing a heavy burden on human health and socioeconomics. It is currently believed that the occurrence of RA is mainly related to environmental factors, genetic factors, immune disorders and so on. Currently, the main drugs used in the treatment of RA are disease-modifying anti-rheumatic drugs(DMARDs), non-steroidal anti-inflammatory drugs, glucocorticosteroids and biologics, which have certain efficacy, but suffer from the disadvantages of expensive treatment costs and more adverse reactions. In contrast, traditional Chinese medicinal herbs of the aconite group, which have the efficacy of dispelling wind and removing dampness, warming meridians and relieving pain, are often used in clinical compounding for the treatment of RA, and have demonstrated significant advantages in the treatment of RA. Therefore, this paper systematically investigated the progress of the anti-RA mechanism of aconite herbs from aconite herbs, the active ingredients, medicinal pairs and compound formulas, aiming to provide a scientific basis for the clinical application and modernization of aconite herbs in the treatment of RA.

The incidence and mortality of gastrointestinal tumors are among the highest in China, posing a significant medical burden on society. In search of better therapeutic effects, the treatment of tumors has become increasingly diversified. In recent years, the role of traditional Chinese medicine(TCM)in cancer treatment has been explored and developed. Under the guidance of TCMtheory, herbal medicines can enhance the body's resistance and remove malign influences to combat tumors. Flavonoids, as one of the main effective components in many herbal medicines, have multiple advantages including multi-target effects, multi-pathway interactions and low toxicity. They can also promote gastrointestinal health and prevent gastrointestinal diseases. Therefore, flavonoids have certain advantages in treating gastrointestinal tumors. Modern research has shown that flavonoids such as baicalin, quercetin, luteolin, isoliquiritigenin and hesperidin can exert anti-tumor effects through various mechanisms including regulating autophagy, pyroptosis, mitochondrial apoptosis, ferroptosis, epithelial-mesenchymal transition(EMT), tumor-associated macrophages(TAMs), microRNA(miRNA), angiogenesis, metabolic reprogramming and methylation. This paper reviewed recent domestic and international research on flavonoids in the treatment of gastrointestinal tumors, summarized the mechanisms of flavonoids against gastrointestinal tumors, and supported further discussion on the use of flavonoids in gastrointestinal tumor treatment, promoting in-depth research and clinical development of flavonoids.

Non-alcoholic fatty liver disease(NAFLD)is a common digestive disease, its pathogenesis is not completely clear, clinical symptoms are often not obvious, and the incidence rate is increasing year by year, so it has become one of the digestive diseases that the world's health care system focuses on. In terms of mechanism, the “two strikes”theory of NAFLD is widely accepted, with the first strike being insulin resistance and the second strike being lipid peroxidation and oxidative stress. Western medical treatment is mainly through the regulation of insulin sensitivity, the use of antioxidants and other improvements, and advocates the changes in lifestyle habits and ways of prevention. Although Western medical treatment means are effective, there are still problems such as the development of drug resistance and serious side effects. Traditional Chinese medicine(TCM)is nowadays one of the optimal solutions for the treatment of NAFLD due to a series of advantages such as its holistic nature, versatility, less side effects and equally significant therapeutic effects. This paper reviewed the pharmacological effects and theoretical mechanisms of Linggui Zhugan Decoction in the treatment of NAFLD, aiming to provide a reference for future related studies.

Polycystic ovary syndrome(PCOS)is a common endocrine and metabolic disorder characterized by chronic anovulation, hyperandrogenism and polycystic changes in ovaries. Traditional Chinese medicine theory holds that the occurrence of PCOS is closely related to dysfunction of heart and spleen, especially the obstruction of phlegm and dampness caused by spleen deficiency and excessive dampness, as well as the dysfunction of Qi and blood circulation. The method of “cultivating soil to restrict water”emphasizes regulating the metabolism of water in the body by strengthening spleen, nourishing Qi, promoting dampness and resolving phlegm, thereby achieving the therapeutic goals. This therapy believes that by nourishing spleen and stomach to enhance their transport function, it can effectively control the water and dampness stagnation caused by spleen deficiency, thereby helping to improve the symptoms of PCOS. This article provided a detailed analysis of the application of the theory of heart and spleen in traditional Chinese medicine, and combined modern medical research to elucidate the clinical symptoms and TCM syndrome differentiation characteristics of PCOS. By sorting out the mechanisms of spleen deficiency, excessive dampness, Qi stagnation and blood stasis in PCOS, this article further analyzed the theoretical basis of the “cultivating soil to restrict water”method in improving the symptoms of PCOS patients. Under the guidance of traditional Chinese medicine theory, the “cultivating soil to restrict water”method provides a new perspective and approach for the treatment of PCOS. By enhancing the function of spleen and regulating water metabolism, it can effectively improve the symptoms of PCOS patients.

    Objective

To investigate the effects and mechanisms of extracts from Baihuasheshecao(Hedyotis diffusa Willd)on the proliferation and apoptosis of renal cancer cells.

    Method

Thirty-two BALB/c mice inoculated with renal cancer Renca cells were randomly divided into model group, Baihuasheshecao(Hedyotis diffusa Willd)extract group, 5-fluorouracil(5-FU)group and transforming growth factor-β(TGF-β)/Smad pathway inhibitor(LY2157299)group, with 8 mice in each group. Corresponding doses were administered by gavage, and the model group was given an equal volume of physiological saline by gavage once a day for 14 days. The volume changes of transplanted tumor blocks in mice were recorded. The mice in each group were euthanized 24 hours after the last administration. The tumor mass was weighed, and the tumor inhibition rate was calculated. Western blot was used to detect the expressions of TGF-β1, p-Smad2, p-Smad3, Bcl-xl, Bad and cleaned-Caspase-3. Immunohistochemical method was used to detect the protein expressions of proliferating cell nuclear antigen Ki67 and PCNA. HE and TUNEL staining were performed on tumor tissues to detect the morphology and apoptosis of tumor cells.

    Result

Compared with the model group, the growth rate of transplanted tumors in all three experimental groups was inhibited, and the volume of transplanted tumors significantly decreased after 14 days of administration. Western blot results showed that compared with the model group, the three experimental groups up-regulated the expression levels of pro apoptotic proteins Bad and cleaned-Caspase-3, down-regulated the expressions of anti apoptotic protein Bcl-xl and pathway proteins TGF-β1, p-Smad2 and p-Smad3(P<0.05). The immunohistochemical results showed that the expressions of Ki67 and PCNA in tumor tissues of the three experimental groups were significantly lower than those of the model group(P<0.05). The results of HE staining and TUNEL staining showed that compared with those of the model group, the proliferation ability of renal cancer cells in the tumor tissues of the three experimental groups was inhibited, and the apoptosis rate was significantly increased(P<0.05).

    Conclusion

The extract of Baihuasheshecao(Hedyotis diffusa Willd)can inhibit the proliferation of renal cancer Renca cells and promote their apoptosis, which may be related to the inhibition of TGF-β/Smad signaling pathway activation.

To study the structure of lignans from Dangshen(Codonopsis pilosula)and their anti-hypoxic activities. The ethanol extracts of Dangshen(Codonopsis pilosula)were separated and purified by D-101 column, silica gel column, ODS column and preparation HPLC. The structures were identified by physicochemical properties and various spectroscopic data. Highly differentiated PC12 cells with neuro-like properties were used to establish a chemical hypoxia model by CoCl2 to screen the anti-hypoxic activity of the isolated compounds. The compound with the best anti-hypoxia activity was chosen for further investigation for the effects on lactate dehydrogenase(LDH), reactive oxygen species(ROS)and mitochondrial membrane potential(MMP)under different concentrations. Ten compounds were classified as schisandrin A(1), Gomisin N(2), schisandrol A(3), angylogyl gomisin Q(4), angylogyl gomisin H(5), benzoylgomisin Q(6), gomisin A(7), honokiol(8), magnolol(9)and Codonopiloneolignanin A(10). Compounds 1, 3-7 and 10 could increase the viability of hypoxic PC12 cells. The protection rates of compounds 1, 3, 4 and 6 were more than 20% at 10 μmol/L. Compound 3(schisandrol A)could reduce the leakage of LDH in PC12 cells of hypoxia induced by CoCl2, reduce the production of ROS and slow down the decline of mitochondrial membranepotential caused by hypoxia. Ten lignans were isolated from Dangshen(Codonopsis pilosula). Compounds 1-7 were dibenzo(a, c)cyclooctene lignans, compounds 8-9 were biphenyl lignans, compound 10 was new lignans with unique bridge ring carbon ring skeleton. Compounds 1-9 were isolated from the genus of Codonopsis for the first time. Compounds 1, 3-7 and 10 had a protective effect on neuronal cell damage caused by hypoxia. The protection rate of compounds 1, 3, 4 and 6 were more than 20% at 10 μmol/L. Compound 3(schisandrol A)might exert its protective activity by inhibiting the production of ROS in hypoxic cells and slowing down the degree of MMP depolarization caused by hypoxia.
