Jujuboside B(JuB),one of themain constituents of herbal medicine Suanzaoren(Ziziphi Spinosae Semen),possess various pharmacological activities, including anti-inflammatory,antiplatelet and anti-restenosis potentials on the cardiovascular system.However,anti-arrhythmia and underlying mechanisms of JuB are uncertain. This study explored the effects of JuB on the sodium currents(INa)in rat ventricular myocytes using whole-cell patch-clamp techniques and molecular docking studies. The whole-cell patch-clamp technique showed that the current-voltage(Ⅰ-Ⅴ)curves of INa shifted along the positive axis in a dose-dependent manner with JuB.Besides, the administration of JuB caused the steady-state activation curve to shift positively along the voltage axis,i.e.,a depolarization trend but the steady-state inactivation curve of INa shifted negatively, which represented that JuB suppressed the opening process of the Na+channel and may had a strong affinity for binding to sodium channels during inactivation.Themolecular docking study exhibited that JuB was mainly docked in the trans membrane helices of voltage-sensing domain(VSD)ⅢandⅣ of rNav1.5 and rNav1.5/QQQ.The study preliminarily confirmed that JuB can inhibited voltage-gated sodium channel of cardiacmyocytes for the first time.

Taking the odor as the research object, this paper revealed the scientific connotation of the qualitative standard that“the drugwith strong odor has good quality”of Tianma(Gastrodiae Rhizoma).

    Methods

The contents of9monomer components and polyphenols in 53 batches of Tianma(Gastrodiae Rhizoma)were determined by high performance liquid chromatography and Folin-Ciocalteu colorimetricmethod. The odor of the samples was quantified by electronic nose technology, and the correlation between the two was analyzed by bivariate correlation method.Taking the quantitative characteristic value of odor as the index, the qualitative identificationmodel of gas-based quality evaluation of differentgrades of Tianma(Gastrodiae Rhizoma)was established by using chemometrics method.

    Results

Gastrodin, protocatechuic acid,4-hydroxybenzaldehyde,vanillin,parishin B and polyphenols were all positively correlated with the odor.Differentgrades of Tianma(Gastrodiae Rhizoma)can be distinguished by PCA and DFA models.

    Conclusion

The stronger the odor of Tianma(Gastrodiae Rhizoma)is,the higher the contents of the above-mentioned pharmacodynamic components are, which reveals the traditional mechanism that“the drug with strong odor has good quality”of Tianma(Gastrodiae Rhizoma). The establishment of a qualitative identification model based on objective quantification of odor can be used for the identification of differentgrades of Tianma(Gastrodiae Rhizoma),which lays a foundation for the quality evaluation of Tianma(Gastrodiae Rhizoma).

Due to the limitations of ancient anatomy,Huangdi Neijing is relatively brief in describing the internal part of the meridian tendons in the human body cavity, which is in sharp contrast to the detailed description of the lay meridian tendons on the body surface and limbs. This difference resulted in the later generations in the development and application of the theory of meridian tendons, more attention was paid to the internal meridian tendons, while the importance of the external meridian tendons was ignored. However, modern medical practice has revealed the close relationship between the meridian tendon and the Zang-Fu organs, which urgently requires us to construct a comprehensive theoretical system to cover both internal and external aspects of the meridian tendon,so as to better guide the clinical treatment. The purpose of this paper was to probe into the anatomical essence of meridian tendon, clarify its direct relationship with Zang-Fu organs, enrich the connotation of meridian tendon theory, and provide solid theoretical support and innovative perspective for the application of meridian tendon therapy in the treatment of Zang-Fu diseases.

    Objective

To investigate the effects of vinegar-procssed Chaihu(Radix Bupleuri)and its differentextraction parts on the uptake of cisplatin(DDP)by cells with high expression of organic cation transporter 2(OCT)(hOCT2-HEK293)and cells with high expression ofmultidrug resistance associated protein 2(MRP2)(hMRP2-HEK293)and to preliminarily explore their mechanisms so as to provide data support for the exploration of the mechanism of using vinegar-procssed Chaihu(Radix Bupleuri)to assist in reversing DDPmultidrug resistance in tumor cells in clinical practice.

    Methods

The hOCT2-HEK293 and hMRP2-HEK293 cells were co-incubated with vinegar-procssed Chaihu(Radix Bupleuri)and its different parts for 24 hours, and then DDP was added to continue incubation for 4 hours. The intracellular DDP content of the two types of cells was measured by HPLC.Western Blot and real-time quantitative PCR(RT-PCR)were used tomeasure the protein and gene expressions of OCT2 and MRP2 in two types of cells, respectively.

    Results

In hOCT2-HEK293 cells, the total extract, polysaccharide fraction and small molecule water-soluble fraction of vinegar-procssed Chaihu(Radix Bupleuri)can significantly promote the uptake of DDP,and all promote the protein and gene expressions of OCT2.In hMRP2-HEK293 cells, the total extract, polysaccharide fraction and small molecule water-soluble fraction of vinegar-procssed Chaihu(Radix Bupleuri)also significantly promoted the uptake of DDP,but only the total extract could significantly reduce the expression of MRP2 protein.

    Conclusion

In tumor cells, the reversal of multidrug resistance to anti-tumor drugs by vinegar-procssed Chaihu(Radix Bupleuri)may be related to the up-regulation of OCT2 protein and mRNA expressions in polysaccharide and small molecule water-soluble sites,or the down-regulation ofMRP2 protein expression in vinegar-procssed Chaihu(Radix Bupleuri)total extract.Preliminary speculation suggests that affecting the functions of OCT2 and MRP2 is one of the mechanisms by which vinegar-procssed Chaihu(Radix Bupleuri)enhances the meridian stimulation.

Atherosclerosis(AS)is a chronic and complex disease that occurs in the arterial wall and underlies a variety of cardiovascular pathologies.Nuclear factor erythroid 2-related factor 2(Nrf2)is an anti-oxidative stress high-sensitivity transcription factor that interacts with antioxidant response element(ARE)to activate downstream gene transcription and exerts various protective effects such as anti-inflammation,antioxidant and detoxification, thus protecting cellular components such as endothelial cells, macrophages, vascular smooth muscle cells, endothelial progenitor cells and so on in arterial vascular tissue, and plays an important role in the development of AS.Traditional Chinese medicine(TCM)has significant advantages in the treatment of AS due to its multi-targets and multi-pathways,stable efficacy, individualisation and safety. This article summarized the mechanism of Nrf2 regulation by TCM monomers,TCM extracts,TCM combinations and proprietary Chinese medicines in the prevention and treatment of AS,with a view to providing a theoretical basis for clinical research and drug development in the prevention and treatment of AS.

    Objective

To study the effect of Wendantang Huacaifang(WDTHCF)on atrial energy metabolic remodeling in paroxysmal atrial fibrillation(PAF)rats based on fatty acid metabolic pathway.

    Methods

Male SD rats were selected and randomly divided into the normal group, the model group, the WDTHCF low, medium and high dose groups, and the amiodarone hydrochloride group, and the PAF model was induced by using Ach-CaCl2.After the model was successfully established, the WDTHCF low, medium and high dose groups and amiodarone hydrochloride were given by gavage. The duration of atrial fibrillation was monitored using electrocardiography. The formation of atrial lipid droplets was detected by oil-red O staining. The contents of free fatty acid(FFA)and triglyceride(TG)of atrial tissues were detected.RT-qPCR was used to detect the mRNA expressions of aAMP-activated protein kinase(AMPK),acetyl coenzyme A carboxylase(ACC),carnitine palmitoyltransferase-1B(CPT-1B),fatty acid translocase/cluster of differentiation 36(FAT/CD36),fatty acid transport protein(FATP)and fatty acid binding protein 3(FABP3).Western blot was used to detect the protein expressions of AMPK,p-AMPK,ACC,p-ACC and CPT-1B and immunohistochemistry method was used to detect myocardial translocation of atrial FAT/CD36.

    Results

Compared with the normal group, the rats in themodel group showed increased duration of AF(P<0.05),elevated contents of FFA and TG in the atria(P<0.05),significant lipid droplet formation in the atria, decreased mRNA expressions of AMPK,ACC,CPT-1B,FAT/CD36,FATP and FABP3(P<0.05),and decreased atrial tissue AMPK phosphorylation,ACC phosphorylation and relative protein expression of CPT-1B were significantly reduced(P<0.05),and the membrane translocation of FAT/CD36 in the atria was reduced(P<0.05).After treatment with WDTHCF,PAF duration was shortened(P<0.05),and the contents of FFA and TG in the atria decreased(P<0.05),lipid droplet formation in atria decreased, the mRNA expressions of AMPK,ACC,CPT-1B,FAT/CD36,FATP and FABP3 in atrial tissues were elevated(P<0.05),and the relative protein expressions of AMPK phosphorylation,ACC phosphorylation and CPT-1B in atrial tissues were elevated(P<0.05),and the membrane translocation of FAT/CD36 in atria was increased.

    Conclusion

WDTHCF improved atrial energy metabolic remodeling in phlegm-turbid and bloodstasis type of PAF rats through AMPK regulating the ACC/CPT-1Bmediated fatty acid metabolism pathway.

    Objective

To investigate the impact of Schisandrol A(SA)on lipopolysaccharide(LPS)-induced and differentiated small intestinal epithelial cell damage in human colorectal cancer Caco-2 cells.

    Methods

Caco-2 cells were random ly divided into a normal control group(without drug intervention,Con),a model group(0.7 mg·mL-1,LPS)and SA(2.5,5,10 μmol·L-1)dose groups. The cell viability was assessed by the CCK-8 method. The cell apoptosis, cell cycle, mitochondrial membrane potential and intracellular reactive oxygen species(ROS)were detected by flow cytometry. The levels of inflammatory factors in the cell supernatant were measured using ELISA.Oxidative stress was assessed using an enzyme-labeled method. The expressions of proteins related to the NF-κB signaling pathway and apoptosis-related pathways were determined by Western Blot analysis.

    Results

The findings indicated that compared with the LPS group,SA at concentrations between 2.5-10μmol·L-1 exhibited a significant protective effect on cells, exhibiting a certain dose-dependent relationship(P<0.01,P<0.001).It reduced the contents of malondialdehyde(MDA),lactate dehydrogenase(LDH)and ROS(P<0.05,P<0.01,P<0.001),increased the activities of superoxide dismutase(SOD)and GSH(P<0.01,P<0.001),inhibited the secretion of pro-inflammatory factors tumor necrosis factor-α(TNF-α),interleukin-6IL-6 and interleukin-18(IL-18)(P<0.05,P<0.01,P<0.001),promoted the release of anti-inflammatory factor interleukin-10(IL-10)(P<0.001),increased mitochondrial membrane potential(P<0.05,P<0.01),improved cell cycle arrest(P<0.05)and modulated the expression levels of inflammation and apoptosis-related pathway proteins(P<0.05,P<0.01,P<0.001).

    Conclusion

This study has confirmed in vitro that SA is capable of improving LPS-induced damage to Caco-2 cells. The mechanism of action appears to be related to the inhibition of the NF-κB signaling pathway.

    Objective

To investigate the efficacy and safety of Sanhuangtangshenkang(三黄糖肾康)combined with Dapagliflozin in the treatment of Qi deficiency and blood stasis type of early diabetic nephropathy(DN).

    Methods

A randomized,double-blind and placebo-controlled clinical trial was conducted involving 120 patients from the Endocrinology and Nephrology Departments at Taizhou Hospital of Traditional Chinese Medicine from June 2021 to June 2023.The participants were allocated into two groups. The experimental group(60 patients)received standard care augmented with Sanhuangtangshenkang and Dapagliflozin, while the control group(60 patients)was treated with standard care plus placebo granules and Dapagliflozin. The duration of treatmentwas 12 weeks. The primary endpoints included the efficacy of treatment, the renal function, the inflammatory markers, the enhancement of traditional Chinese medicine(TCM)syndromes, and the frequency of adverse events.

    Results

A total of 117 patients were successfully enrolled,59 in the experimental group and 58 in the control group. The experimental group showed a significantly higher overall effective rate compared to the control group(98.30% versus70.68%,P<0.05).Biochemical indicators such as the urine albumin-to-creatinine ratio(UACR),24-hour urinary microalbumin(UMA)and 24-hour urinary protein were significantly improved in the experimental group compared to those in the control group after 12 weeks of treatment(P<0.05).Inflammatory markers including serum amyloid A(SAA),interleukin-6(IL-6),C-reactive protein(CRP)and homocysteine(Hcy)also showed more significant reductions in the experimental group, especially after 12 weeks of treatment(P<0.05).The improvements in renal function and other biochemical indicators, such as glycated hemoglobin(HbA1c),estimated glomerular filtration rate(eGFR)and total cholesterol(TC),were also notably better in the experimental group(P<0.05).TCM symptom scores further confirmed the improvement in various indicators in the experimental group after treatment(P<0.05).Although there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).The experimental group exhibited a lower rate of adverse reactions(3.38%)compared to the control group(8.62%).

    Conclusion

The combination of Sanhuangtangshenkang with Dapagliflozin is highly effective and safe for improving renal function, glycemic control and TCM syndromes in patients with early-stage DN characterized by Qi deficiency and blood stasis, demonstrating both enhanced safety and superior efficacy.

Multiplemyeloma(MM)is one of the most common malignant tumors of the hematologic system, with multiple myeloma bone disease(MBD)as a hematological complication.MBD is prominently characterized by pathological fractures, osteoporosis and osteolytic lesions, all of which severely impact patients'quality of life and survival rates. The primary mechanisms of MBD involve bone destruction and impaired osteogenesis, especially the abnormal activation of the receptor activator of nuclear factor kappa B(RANK)/ligand of RANK(RANK-L)/osteoprotegerin(OPG)system,along with an imbalance in the regulation of osteoclast and osteoblast function by pro-inflammatory cytokines in the bone marrow microenvironment. In recent years, substantial progress has been achieved in MBD treatment through integrated traditional Chinese and Western therapeutic strategies. This combined approach harnesses multiple therapeutic advantages to alleviate bone pain, enhance bone strength and reduce the risk of complications. To standardize this integrated diagnosis and treatment protocol,an expert team has developed a consensus based on evidence-based medicine and regional resources in traditional Chinese and Western medicine within the Lingnan area. This consensus aims to provide guidance for clinical practice and improve patients'quality of life and prognosis.
