    Peripheral facial paralysis (PFP) is a functional disorder of facial muscles primarily caused by damage to the facial nerve or its nucleus, commonly resulting from conditions such as stroke, intracranial infections, or otitis media. The majority of cases are idiopathic facial paralysis, which is associated with factors like cold exposure and seasonal changes. PFP has a rapid onset and is often seen on one side of the face. It can cause paralysis or paralysis of the facial muscles in patients, presenting symptoms such as drooling, facial expression imbalance, inability to close eyes, and limited ability to raise eyebrows, which brings inconvenience to patients' lives and requires timely treatment. Western medicine mostly conducts drug treatment based on the etiology, but there are still some patients with sequelae of PFP. Traditional Chinese medicine theory has unique insights into PFP. It holds that PFP is mostly caused by wind pathogen invading the human body and blocking the circulation of qi and blood in the meridians. Stimulating specific acupoints with acupuncture can promote blood circulation and remove blood stasis, unblock the meridians, dispel cold and dampness, adjust the overall state of the body, improve blood supply to the facial muscles, promote nerve regeneration, and facilitate the patient's recovery. Now, this study aims to further explore the mechanism of acupuncture in the treatment of PFP and the therapeutic effect of acupuncture on PFP at different times, providing a basis for improving the therapeutic effect of acupuncture on PFP and accelerating the recovery process.

    Objective:

    To observe the clinical efficacy of special acupoint acupuncture in the treatment of abducens nerve palsy, and compare the efficacy with traditional acupuncture and drugs in the treatment of abducens nerve palsy.

    Methods:

    The clinical data of 97 patients with abducens nerve palsy treated by Department of Ophthalmology, Baoding Hospital of Guang'anmen Hospital, China Academy of Chinese Medical Sciences from June 2020 to March 2022 were analyzed retrospectively, including 40 cases receiving special acupoint acupuncture therapy (special group), 30 cases receiving traditional acupuncture therapy (traditional group) and 30 cases receiving pure drug therapy (drug group). The onset time, clinical efficacy and course of treatment of patients in each group were compared and analyzed.

    Results:

    The onset time of special group was significantly faster than that of traditional group and drug group (P < 0.05); The total clinical effective rate of special group was significantly higher than that of traditional group and drug group (P <0.05); The duration of treatment in the special group was significantly shorter than that in the traditional group and the drug group (P < 0.05); The satisfaction of patients in the special group was significantly higher than that in the traditional group and the drug group (P <0.05).

    Conclusion:

    Compared with traditional acupuncture and drug therapy, special acupoint acupuncture has faster effect, shorter course of treatment, more significant curative effect and higher satisfaction. It is worth popularizing and applying.

    "Insomnia due to the kidney's failure to store the will" is a sub-type of insomnia within the TCM "Five-Spirit" diagnostic classification system for insomnia, characterized by the direct symptom of waking early after falling asleep at night. Its disease location is in the kidney, and the pathogenesis is the failure of the kidney-will to rest peacefully. The therapeutic method of calming the kidney will can be adopted for treatment. The medicinal herb Yuanzhi (Radix Polygalae), first documented in the Shennong Bencao Jing (Shennong's Collection of Materia Medica), was said to have the function of "strengthening the will and doubling strength". Subsequent physicians through the ages have extensively discussed it, believing that Yuanzhi has the function of "calming the kidney will". The herb pairs Yuanzhi-Fuling (or Fushen), Yuanzhi-Shudihuang, Yuanzhi-Yizhiren, Yuanzhi-Suanzaoren, and the compound formulas Huanshao pill, Kongsheng Zhenzhong pill, and Qifu decoction can all calm the kidney will to treat insomnia due to the kidney's failure to store the will. Among the herb pairs, Yuanzhi-Fuling (or Fushen) and Yuanzhi-Suanzaoren calm the heart spirit and soothe the kidney will; Yuanzhi-Shudihuang enriches kidney essence and nourishes kidney water to calm the kidney will; Yuanzhi-Yizhiren boosts intelligence, stabilizes qi, and calms the kidney will. Among the compound formulas, Huanshao pill tonifies the spleen and kidney, stabilizes the will, and calms the spirit; Kongsheng Zhenzhong pill tonifies the kidney and heart, calming the spirit and stabilizing the will; Qifu decoction tonifies the kidney and benefits the marrow, stabilizing the will and nourishing the spirit. Modern research indicates that the mechanism of Yuanzhi in treating this type of insomnia is related to improving neuroinflammation, oxidative stress, and regulating neurotransmitters, which can provide references for clinical and mechanistic research on Yuanzhi for this condition.

    Objective:

    To analyze the differences in chemical components between two types of Lycii cortex derived from Lycium chinense Mill. and Lycium barbarum L.

    Methods:

    HPLC was used to establish the fingerprint profiles of the two kinds of Lycii cortex samples, and the common pattern diagrams were compared for similarity analysis. An HPLC method was developed for the determination of four components: (E)-N-caffeoylputrescine, scopoline, kukoamine B, kukoamine A. Combined with principal component analysis (PCA) and orthogonal partial least squares discriminant analysis (OPLS-DA), the differences in chemical components of Lycii cortex from different origins were compared.

    Results:

    HPLC fingerprints of Lycii cortex from different origins were established. Lycii cortex originated from Lycium chinense Mill. had 14 common peaks, while Lycii cortex originated from Lycium barbarum L. had 11 common peaks. Five common peaks were identified: (E)-N-caffeoylputrescine (Peak 1), scopolin (Peak 2), kukoamine B (Peak 3), kukoamine A (Peak 5), and scopoletin (Peak 6). The quantitative analysis showed good linearity for the four components within the concentration ranges of 1.37 ~ 87.80 μg·mL-1(r=0.999 9), 1.97 ~ 125.83 μg·mL-1(r=0.999 7), 6.31 ~ 403.76 μg·mL-1(r = 0.999 9), and 10.67 ~ 341.57 μg·mL-1 (r = 0.999 8), with average recoveries of 100.52% ~ 103.84% and RSD of 2.33% ~ 3.42%. The content determination results showed that the average percentages of the four components in Lycii cortex originated from Lycium barbarum L. were higher than in Lycium chinense Mill. Chemical pattern recognition analysis classified the two types of Lycii cortex into two categories, and (E)-N-caffeoylputrescine played a prominent role in model construction.

    Conclusion:

    The established method is accurate and reliable, which can be utilized to distinguish the two varieties of Lycii Cortex and provide a reference for the quality control of them.

    Objective:

    To observe the effect of Bear bile and UDCA on circulating blood Ly6Chigh monocyte chemotaxis receptor CCR2 and its ligand CCL2 (MCP-1) in model mice with Immune Thrombocytopenia (ITP), and explore the possible mechanism of regulating Ly6Chigh monocyte migration and reference studies on bear bile and its replacements in ITP management.

    Methods:

    Following established methods, an ITP model was induced via immunization in 50 BALB/c mice, randomLy allocated into five groups: normal control, model control, prednisone treatment, bear bile treatment, and UDCA treatment, from the 6th day of making model, mice in the each group was administered 0.4 mL/d volume of corresponding drug by irrigation treatment. From the 6 h, 7th day, 15th day, automatic blood cytometer was used to dynamic detect platelet count. after treatment 10 day, Flow cytometry was used to detect the changes of the Ly6Chigh monocyte in circulating blood, Enzyme-linked immunosorbent Assay was used to detect the levels of serum MCP-1 (monocyte chemotactic protein-1).

    Results:

    After the 6 hour of making model, the counts of platelet decreased obviously in all group, reaching the lowest point on the 7th day, and remained at a relatively low level until the 15th day. There was a statistically significant difference compared to the normal group (P < 0.05). After treatment, the platelet counts of bear bile, prednisone and UDCA group increased obviously (P < 0.01). Flow cytometry results showed that compared with the model group, the positive rate of circulating Ly6Chigh monocytes and its surface chemokine receptor CCR2 in bear bile, UDCA and prednesone groups decreased (P < 0.01, P < 0.001), while CX3CR1 expression increased (P < 0.001). Enzyme-linked immunosorbent Assay results showed that compared with the model group, the levels of MCP-1 in bear bile, UDCA and prednesone groups decreased(P < 0.001).

    Conclusion:

    The Bear bile and UDCA can downregulate the expression of CCR2 by reducing the level of MCP-1 in circulating blood, thus governs CCL2-CCR2 mediated Ly6Chigh monocyte emigration, reduce the Ly6Chigh monocytes in the circulating blood, this represents a crucial mechanism driving Ly6Chigh monocyte accumulation during ITP progression and worsening.

    Objective:

    To investigate the mechanism by which Yiqi Huoxue Lishui Decoction ameliorates hepatic fibrosis (HF) in rats by inhibiting hepatocyte ferroptosis via activation of the nuclear factor-E2-related factor 2 (Nrf2)/glutathione peroxidase 4 (GPX4) signaling pathway.

    Method:

    Forty Sprague-Dawley (SD) rats were randomly divided into five groups (n =8 per group) : the control group (Control), model group (Model), traditional Chinese medicine group (TCM), traditional Chinese medicine + ferroptosis inhibitor group (TCM + Fer-1), and traditional Chinese medicine + ferroptosis inhibitor + autophagy inhibitor group (TCM + Fer-1 + 3-MA). The model was established by intraperitoneal injection of a 50% carbon tetrachloride (CCl4) solution (1.5 mL/kg), administered twice weekly for 4 weeks. Treatment groups received their respective interventions: the Yiqi Huoxue Lishui Decoction was administered via oral gavage (2 mL/100 g), while the ferroptosis inhibitor Ferrostatin-1(Fer-1) (1 mg/kg) and the autophagy inhibitor 3-Methyladenine (3-MA) (15 mg/kg) were administered via intraperitoneal injection. All administrations were performed once daily for 4 weeks, while the other groups were given an equivalent volume of normal saline. Hepatic histopathological changes were evaluated using hematoxylin-eosin (HE) and Masson staining. Serum levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), tumor necrosis factor-α (TNF-α), superoxide dismutase (SOD), malondialdehyde (MDA), and Fe2+ content in serum and liver tissue were measured. Western blot (WB) was employed to assess protein expression of GPX4, Nrf2, solute carrier family 7 member 11 (SLC7A11), transferring receptor 1 (TFR1), Beclin-1, and the LC3 II/I ratio. Quantitative real-time polymerase chain reaction (qPCR) was utilized to determine mRNA expression levels of Nrf2 and GPX4.

    Results:

    Compared with the Control group, the Model group exhibited severe hepatic steatosis, significant fibrosis, elevated collagen deposition, increased serum AST, ALT, MDA, TNF-α, and Fe2+ levels, and reduced SOD activity (P < 0.05). Protein expression of GPX4, Nrf2, and SLC7A11 was upregulated, while TFR1 and Beclin-1 expression increased, and mRNA levels of GPX4 and Nrf2 were markedly suppressed (P < 0. 05). Intervention with Yiqi Huoxue Lishui Decoction and/or inhibitors significantly attenuated histopathological damage, reduced AST, ALT, MDA, TNF-α, Fe2+ levels, and downregulated TFR1 and Beclin-1 expression (P < 0.05). Concurrently, SOD activity, GPX4 and Nrf2 protein/mRNA expression, and SLC7A11 protein levels were elevated (P <0. 05). No significant difference was observed in the LC3 Ⅱ/Ⅰ ratio across groups(P > 0.05).

    Conclusion:

    Yiqi Huoxue Lishui Decoction may ameliorate hepatic fibrosis by inhibiting ferroptosis through activation of the Nrf2/GPX4 signaling pathway.

    Based on the physician Chen Shiduo's theory of "yang-fire and yin-fire in Qing Dynasty," this paper proposes a new perspective for treating atrial fibrillation (AF) from the concept of "pericardial fire. "The article points out that modern AF treatment has limitations such as bleeding risks from anticoagulation, side effects of medications, and high recurrence rates after interventional procedures. Furthermore, in the treatment of heart fire, traditional Chinese medicine (TCM) conventionally focuses on clearing heart fire, relatively neglecting the pathogenesis of the pericardium. Chen Shiduo believed that pericardial fire is a type of "yin-fire," characterized by a slow nature and a tendency to rise upward. It often arises from deficiency of heart qi and depletion of kidney water, leading to floating ministerial fire and stirring of deficient yang, which manifests as palpitations. The treatment emphasizes that "ministerial fire should be tonified, not drained," advocating for methods such as strongly tonifying heart qi, enriching kidney yin to restrain yang, or soothing liver qi to extinguish the flame, thereby returning the ministerial fire to its proper place, rather than simply clearing and draining it. The paper presents a clinical case of AF treated with a modified formula based on Chen's Guipi Decoction and Liuwei Dihuang Pills, demonstrating the clinical reasoning and therapeutic efficacy guided by this theory. This reflects TCM's holistic principle of "treating the root cause" and provides a new theoretical reference for the integrated treatment of AF with Chinese and Western medicine.

    Objective:

    To investigate the effects of Gegen Decoction combined with probiotics on symptom scores, time to negative conversion of fecal reducing substances, fecal pH value, and microbiome-related indicators in pediatric acute diarrhea.

    Methods:

    A total of 98 children with acute diarrhea treated at Qinhuangdao Hospital of Traditional Chinese Medicine from January 2024 to July 2024 were randomly divided into an experimental group (Gegen Decoction Granules + probiotics) and a control group (probiotics alone), with 49 cases in each group. During follow-up, 3 cases dropped out in the experimental group and 2 cases in the control group, resulting in 46 cases in the experimental group and 47 cases in the control group completing the study. Both groups received oral rehydration and dietary management according to clinical guidelines. The treatment duration was 5 days. Traditional Chinese medicine (TCM) syndrome scores, time to negative conversion of fecal reducing substances, fecal pH value, number of bowel movements within 24 hours, fecal calprotectin, short-chain fatty acids (SCFAs), and fecal secretory immunoglobulin A (sIgA) levels were compared between the two groups before and after treatment. Adverse events were also recorded.

    Results:

    The experimental group showed significantly greater improvements than the control group in terms of symptom score reduction, time to negative conversion of fecal reducing substances, restoration of fecal pH value, increase in total SCFAs, butyrate proportion, and fecal sIgA, as well as reduction in fecal calprotectin (P < 0.05, P < 0.01). The overall response rate was 93.48% (43/46) in the experimental group and 80.85% (38/47) in the control group (P <0.05). There was no significant difference in the incidence of adverse events between the two groups (P > 0.05).

    Conclusion:

    Gegen Decoction combined with probiotics can significantly alleviate symptoms of acute diarrhea in children, promote recovery of carbohydrate absorption, correct the acidic intestinal environment, and modulate microbiome and mucosal immunity indicators, with a favorable safety profile. It represents an effective integrated treatment strategy.

    The incidence of Premature Ovarian Insufficiency (POI) is on the rise year by year. Traditional Chinese medicine holds that the imbalance of the Chong and Ren meridians is the core pathogenesis of POI, and the function of the Chong and Ren meridians is similar to that of the ovary in Western medicine. This article aims to explore the integration of the theory of "regulating the Chong and Ren meridians" in traditional Chinese medicine and the theory of "improving the microenvironment of the ovary" in Western medicine, providing an innovative perspective for the diagnosis and treatment of early-onset ovarian insufficiency in traditional Chinese medicine and revealing the deep consensus of both traditional Chinese and Western medicine in the regulation of female reproduction.

    Objective:

    To study the effect of head and body acupuncture combined with high-low frequency repetitive transcranial brain stimulation (rTMS) on nerve and limb function of hemiplegic patients with cerebral infarction.

    Methods:

    Eighty patients with hemiplegia due to cerebral infarction were selected as the research subjects and were all treated in the Department of Rehabilitation Medicine of Henan Provincial Veterans' Hospital from May 2022 to May 2024. The subjects were randomly and equally divided into the control group and the observation group. Both groups were given conventional medical treatment. The control group was treated with high-low frequency rTMS on the basis of the above, while the observation group was treated with head-body acupuncture and giant needling therapy + high-low frequency rTMS on the basis of conventional medical treatment. The clinical efficacy of the subjects was evaluated. The National Institutes of Health Stroke Scale (NIHSS), Fugl-Meyer Assessment (FMA), Berg Balance Scale (BBS), and activities of Daily Living (Barthel Index) were used to assess the neurological function, limb motor function, balance function, and activities of daily living of the subjects. The cerebral hemodynamics [mean blood flow velocity (Vm) of the middle cerebral artery, pulsatility index (PI)] and serum neurotransmitter levels [brain-derived neurotrophic factor (BDNF), dopamine (DA), 5-hydroxytryptamine (5-HT)] of the subjects were detected before and after the treatment.

    Results:

    The total effective rate in the observation group was significantly higher than that in the control group (P < 0.05). Compared with before treatment, the levels of NIHSS score and pulse index (PI) were decreased (P <0.05), while the levels of FMA, BBS, Barthel score, middle cerebral artery Vm, BDNF, DA and 5-HT were increased (P < 0.05). Compared with the control group, the NIHSS score and PI level of the observation group were lower after treatment (P < 0.05), while the FMA, BBS, Barthel scores, Vm, BDNF, DA, and 5-HT levels were higher (P <0.05).

    Conclusion:

    Head and body acupuncture combined with high-low frequency rTMS can effectively improve the clinical efficacy of patients with cerebral infarction hemiplegia, improve nerve function, limb function and balance function, promote hemodynamics and neurotransmitter release.
