    Objective

To investigate the mechanism of action of curculigoside(CUR)antioxidation in alleviating apoptosis of ovarian granulosa cells(GCs)in mice with premature ovarian failure(POF).

    Method

Mice after acclimatization feeding were divided into blank, model,CUR low-dose,CUR mid-dose and CUR high-dose groups, and estradiol valerate group. The mouse model was replicated by intraperitoneal injection of cyclophosphamide(CTX). After modeling, the mice were treated by gavage for 14 consecutive days and vaginal smears were performed daily, the body mass of the mice was weighed and the mice were killed 24 h after the last administration and the blood and ovary tissues were extracted, and the ovary tissues were weighed bilaterally to calculate the ovarian index. The levels of serum follicle stimulating hormone(FSH),anti-Müllerian hormone(AMH)and estradiol(E2)were measured. The ovaries were stained with hematoxylin-eosin(HE)staining to observe follicular development and corpus luteum morphology, and the apoptosis level of GCs was determined using terminal deoxynucleotidyl transferase-mediated dUTPNick-End Labeling(TUNEL),and the superoxide dismutase(SOD)activity and malondialdehyde(MDA)level were measured using protein peroxidase, glutathione peroxidase(GSH-Px)content and malondialdehyde(MDA)level in the ovary tissue. The protein expressions of ovarian protein kinase B(AKT),p-AKT,mammalian target of rapamycin(mTOR)and p-mTOR were detected using protein blotting(WB).

    Results

Compared with the blank group, the mice in the model group showed disorganized estrous cycle, decreased ovarian index, large number of atretic follicles, high level of apoptosis of GCs and significantly upregulated levels of FSH,whereas AMH and E2 showed a significant decrease, suggesting a successful replication of the POF mouse model. Compared with the model group, the administration of CUR intervention was able to increase the antioxidant level, activate the AKT/mTOR signaling pathway, improve the estrous cycle and serum hormone levels, reduce apoptosis of GCs,and promote follicular development in POF mice, thus restoring the ovarian reserve function.

    Conclusion

CUR ameliorated mouse POF caused by CTX,and the mechanism may be related to the activation of Akt/mTOR signaling pathway to exert antioxidant effects to alleviate the apoptosis of GCs in POF model mice.

Shengma Biejia Decoction(升麻鳖甲汤),recorded in the Synopsis of Prescriptions of Golden Chamber, consists of a combination of Shengma(Cimicifugae Rhizoma),Biejia(Trionycis Carapax),Danggui(Angelicae Sinensis Radix),Gancao(Glycyrrhizae Radix et Rhizoma),Shujiao(Zanthoxyli Pericarpium)and Xionghuang(Realgar),and can be used to treat Yang toxin. On the basis of this prescriptions,Shujiao(Zanthoxyli Pericarpium)and Xionghuang(Realgar)are subtracted to treat Yin toxin. Physicians in past dynasties held varying perspectives on the drug addition and subtraction of Shengma Biejia Decoction. The toxicity of Xionghuang(Realgar)in Shengma Biejia Decoction restricts its clinical applications. Physicians hold discrepant perspectives on the interpretation of“Yin-Yang”of the Yin-Yang toxin, with some linking it to the cold and heat nature of diseases and others connecting it to the differentiation between exterior and interior diseases, causing debate over drug addition and subtraction of Shengma Biejia Decoction. Ancient physicians thought that Yin-Yang toxin was exogenous diseases such as typhoid fever and epidemic disease, and treated it with Shengma Biejia Decoction. In modern clinic,Shengma Biejia Decoction is widely used to treat dermatological diseases, ophthalmological and otolaryngological diseases and internal diseases including hematological diseases and immunological diseases, and its pharmacodynamic mechanism is related to regulating immunity and inhibiting tumor cell viability. Most physicians believe that the usage of Xionghuang(Realgar)in this prescription should be taken with water in low doses for a short duration. Shengma Biejia Decoction with Xionghuang(Realgar)is commonly used for dermatological diseases, hematological diseases and immunological diseases, particularly for treating urticaria, leukemia and systemic lupus erythematosus. This paper addressed the problems of drug addition and subtraction of Shengma Biejia Decoction, reviewed the advancements in clinical application and pharmacodynamic mechanism of Shengma Biejia Decoction, and conducted statistical analysis on the clinical utilization of Shengma Biejia Decoction with Xionghuang(Realgar). This review aimed to offer guidance for the clinical application of Shengma Biejia Decoction and inform future basic and theoretical research in this area.

The term“Stomach Preponderance and Spleen Weakness”was initially used to explain the pathological mechanism of“spleen constipation syndrome”in which the spleen's inability to properly transport body fluids leads to abnormal fluid distribution, resulting in insufficient fluid supply to the intestines and causing constipation. Over time, this concept evolved to describe a broader state of impaired Qi dynamics, where spleen deficiency disrupts the proper transportation and transformation of fluids. This leads to fluid accumulation in the stomach, stagnation, and subsequent heat generation due to fluid retention. The pattern of“Stomach Preponderance and Spleen Weakness”is linked to dysfunction in the spleen and stomach in three key areas:transport and absorption, the regulation of ascending and descending, and the balance of moistening and drying. These dysfunctions form an important pathological basis for the onset of gastrointestinal and various visceral diseases. Gastrointestinal dysfunction is a common non-motor symptom(NMS)in Parkinson's disease(PD),manifesting in both the prodromal and disease stages. Key symptoms include difficulty swallowing, excessive salivation, delayed gastric emptying, bloating, and constipation. Gastrointestinal dysfunction is an important pathological factor in the development of PD,and its mechanisms are related to disturbances in the intestinal microbiota and its metabolites, damage to the intestinal barrier, inflammatory responses, and the aggregation ofα-synuclein(α-syn)in both the intestines and the central nervous system(CNS). Excessive dietary intake or a deficiency in spleen Qi can lead to food stagnation in the stomach, preventing timely discharge and triggering pathological changes such as dysbiosis of the intestinal microbiota and activation of inflammatory responses. The failure of the stomach's descending function and obstruction can lead to the stagnation of heart and lung Qi in the upper body, while the spleen's weakened ability to lift results in dysfunction of liver and kidney essence. This may delay intestinal transit, increase intestinal permeability, and promote the translocation of pathogen metabolites, along with other pathological changes. Dampness in the spleen or excessive dryness in the stomach can damage the intestinal barrier, resulting in inflammation andα-syn accumulation.“Stomach Preponderance and Spleen Weakness”is closely associated with the occurrence of gastrointestinal dysfunction in PD. Exploring the relationship between these concepts can deepen the understanding of PD from a Traditional Chinese Medicine(TCM)perspective and offer new insights for clinical differentiation and treatment strategies.
