    Objective

To explore whether Chonglou(Rhizoma paridis)has a reversal effect on multidrug resistance of leukemia CEM/VCR and its reversal mechanism.

    Methods

CEM/VCR cells were treated with different concentrations of Chonglou(Rhizoma paridis)for 72 hours, and the mortality rate was calculated, and the appropriate concentration of Chonglou(Rhizoma paridis)reversal agent was selected. MTT colorimetric experiment was used to calculate the inhibitory rate and half inhibitory concentration(IC50)of low, medium and high concentrations(0.1,0.5 and 2.5μg/mL)of vincristine(VCR)acting on Chonglou(Rhizoma paridis)treated cells(hereinafter referred to as the treated group)and unweighted Chonglou(Rhizoma paridis)treated cells(hereinafter referred to as the untreated group)for 24,48 and 72 hours respectively, and to calculate the reversal index(RI)and reversal multiple(IC50)between the two groups. Flow cytometry(FCM)was used to detect the apoptosis rate after VCR was applied to the two groups of cells. RT-qPCR and Western blot were used to detect the mRNA and protein expression levels of Bax,Bcl-2,Cyt-c,P-gp and MRP1 after VCR acted on the two groups of cells.

    Results

The concentration of Chonglou(Rhizoma paridis)reversal agent was 0.008μg/mL. The inhibition rate of VCR in the treatment group was significantly higher than that in the untreated group(P<0.05 or P<0.01). The IC50 value of VCR in the treatment group after 24,48 and 72 hours was reduced from 17.32,13.85 and 6.21μg/mL to 15.37,2.25 and 0.62μg/mL,and the reversal multiples were 1.12 respectively. The results of flow cytometry showed that the apoptosis rate of VCR group increased gradually with the increase of drug concentration, and the apoptosis rate of treated group was higher than that of untreated group under the same conditions. The results of RT-qPCR and Western blot showed that the expressions of Bcl-2,P-gp,MRP1-1 and Bax,Cyt-c genes and proteins were down-regulated and up-regulated by VCR with different concentrations at the same time compared with those of the control group(P<0.05),and this change was more significant in the treatment group.

    Conclusion

Chonglou(Rhizoma paridis)can reverse the multidrug resistance of CEM/VCR cells, which may be related to down-regulating the expressions of P-gp and MRP1,reducing the efflux of chemotherapeutic drugs and increasing the intracellular concentration of VCR,down-regulating the expression of Bcl-2 and up-regulating the expressions of Bax and Cyt-c,inducing cell apoptosis.

Advanced heart failure with preserved ejection fraction(HFpEF)is a critical condition in the cardiovascular field, with limited treatment methods and poor prognosis. Recurrent wheezing and edema are the main clinical symptoms in the advanced HFpEF,which is classified as heart water disease according to traditional Chinese medicine(TCM). The main treatment principle is to warm Yang and promote diuresis, which plays an important role in the treatment of advanced HFpEF. Based on the pathological characteristics of water retention in advanced HFpEF,the author systematically reviewed the etiology, pathogenesis, clinical manifestations, and characteristics of recurrent fluid retention, which is similar to the advanced HFpEF and can serve as the core pathogenesis of advanced HFpEF. Advanced HFpEF treatment in the stable and acute stages should base on the incubation and attack periods of recurrent fluid retention, using a combination of TCM and non pharmacological techniques to improve patients' clinical symptoms and enhance their quality of life, which will promote the ability and status of TCM in the treatment of critical illnesses by exerting multi-target integrated prevention and treatment advantages.

The key to achieving TCM syndrome differentiation and treatment through artificial intelligence lies in the standardization, normalization, dataization and intelligence of TCM four diagnostic methods. It analyzes the current status of artificial intelligence in the four examinations of inspection, listening and smelling, inquiry and pulse taking and palpation. Inspection adopts high-definition cameras to capture the entire face, tongue images, sublingual veins, lip color, eyes and other parts. The facial segmentation algorithm, color space model, spectral chromaticity and wavelength reflectance are combined to provide ideas for quantitative analysis of the observation. Inspection collects abnormal sounds such as cough and bowel sounds, and abnormal odors such as mouth, sputum and stool, and uses BP and SVM neural networks and electronic nose and other technologies for qualitative and quantitative analysis, providing a new method for the diagnosis of pulmonary and gastrointestinal diseases. Inquiries use the deep learning(DL)model for human-computer interaction to build a many-to-many probability map of pathogenesis and syndromes, providing probability statistics for accurate prescriptions. Pulse taking and palpation have been upgraded from a primary pressure sensor to a pulse wave touch sensor, which can collect three-dimensional pulse maps and dynamically detect blood flow imaging, diversifying the objective indicators of pulse diagnosis. There are still many problems to be solved urgently in the realization of artificial intelligence for the four diagnoses of traditional Chinese medicine:the standardization and quantification rules for the four diagnoses have not yet been improved;the hardware equipment for the individual collection of the four diagnoses is blooming in a hundred ways, but the hardware equipment for the combined collection of the four diagnoses is in its infancy, and the intelligence of the four diagnoses is low. The degree of integration is low;intelligent models, algorithms and big data are still immature in the field of medical video disease identification;big data information security risks lie,and there is a lack of crosscutting talents between medical and industrial departments to lead the development of the industry. This paper discussed and analyzed these issues.

    Objective

To explore the inhibitory activity of phloretin on the RNA polymerase of the novel coronavirus, and predict the target enzyme inhibitory activity of three small analogs molecules.

    Methods

Molecular docking was employed to investigate the binding mode of phloretin with the active site of RNA polymerase. The inhibitory activity of phloretin on RNA polymerase was evaluated using a cell-based assay system for the catalytic reaction of the novel coronavirus RNA polymerase nsp7/nsp8/nsp12. Three small molecules from traditional Chinese medicine which was similar to the structure of rhizobin were selected to predict the inhibitory activity of the targeted enzyme, and the binding mode between them and the target enzyme activity site was studied by molecular docking method.

    Results

Phloretin exhibited favorable binding to the active site of RNA polymerase, occupying the NTPs channel and obstructing the entry of nucleotide substrates, thereby inhibiting RNA polymerase-directed nucleic acid replication. The results of pharmacological testing for the inhibition of RNA polymerase at the cellular level demonstrated that phloretin possesses significant inhibitory activity against the novel coronavirus RNA polymerase, albeit with weaker activity compared to remdesivir. However, phloretin exhibited lower cytotoxicity compared to remdesivir. It is predicted that phloretin analogs can also inhibit RNA polymerase and have pharmacological effects against novel coronavirus.

    Conclusion

This study reveals the RNA polymerase inhibitory activity of phloretin, and predict that the small molecules of the rhizetin analogs also have RNA polymerase inhibitory activity, providing a potential reference for the development of anti-novel coronavirus drugs.

Chronic atrophic gastritis is a modern medical concept, which is categorized in traditional Chinese medicine as“epigastric pain”,“stuffiness of stomach”,“epigastric upset”and other diseases. The disease has the characteristics of insidious onset, long course and complexity. Most of the patients in the clinic show deficiency in origin and excess in superficislity, mix of cold and heat, and the prolonged disease often damages the positive Qi. And the real evils such as dampness, heat, blood stasis and other excess evils block the meridians and depletion of nutrient-Yin,and the loss of nourishment in the gastric mucosa leads to atrophy. Professor YAN Qinghai points out that according to his many years of clinical experience, chronic atrophic gastritis is the syndrome of deficiency in origin and excess in superficislity. The deficiency is Qi deficiency of spleen and stomach, leading to inability to transport and transform. The excess in superficislity is damp-heat obstructing the middle Jiao,or liver depression and Qi stagnation,liver-stomach disharmony, and poor circulation of Qi and blood. In the treatment of chronic atrophic gastritis, chronic atrophic gastritis is mostly treated with the basic principle of strengthening spleen and nourishing Qi,supplemented by methods such as clearing heat and dampness, soothing liver and harmonizing stomach. Combined with YAN Qinghai's own clinical practice,he has proposed a comprehensive treatment approach that combines tonifying, dispelling and regulating methods. Strengthening spleen, tonifying Qi,harmonizing stomach and descending adverse Qi,and clearing away heat and dampness are the common and effective methods for treating chronic atrophic gastritis. In this regard,Prof. YAN Qinghai has proposed the combination of disease differentiation and syndrome differentiation, the combination of macro and micro perspectives, the combination of treatment and mind regulation. The treatment combines the methods of treating the root, dispelling evil and dampness, and harmonizing spleen and stomach qi and blood. The patients' symptoms improved well and the clinical outcome was more significant.
