Advanced heart failure with preserved ejection fraction（HFpEF）is a critical condition in the cardiovascular field，with limited treatment methods and poor prognosis. Recurrent wheezing and edema are the main clinical symptoms in the advanced HFpEF，which is classified as heart water disease according to traditional Chinese medicine（TCM）. The main treatment principle is to warm Yang and promote diuresis，which plays an important role in the treatment of advanced HFpEF. Based on the pathological characteristics of water retention in advanced HFpEF，the author systematically reviewed the etiology，pathogenesis，clinical manifestations，and characteristics of recurrent fluid retention，which is similar to the advanced HFpEF and can serve as the core pathogenesis of advanced HFpEF. Advanced HFpEF treatment in the stable and acute stages should base on the incubation and attack periods of recurrent fluid retention，using a combination of TCM and non pharmacological techniques to improve patients' clinical symptoms and enhance their quality of life，which will promote the ability and status of TCM in the treatment of critical illnesses by exerting multi-target integrated prevention and treatment advantages.
