ArchiveThe incidence of tumor-related malnutrition is high, and significantly affects the prognosis and quality of life of patients.Nutritional support therapy has been established as an important component of comprehensive tumor treatment. However, the clinical practice of oncology nutrition care in China still lacks unified norms and a complete process management system. This consensus, based on a multidisciplinary collaboration mechanism and an information platform, has constructed a nursing management model for nutritional support and care for tumor patients. The main content covers admission nutritional screening and comprehensive assessment based on the Nutritional Risk Screening 2002(NRS-2002) and the Patient Generated Subjective Global Assessment(PG-SGA),as well as a stepwise graded intervention path during hospitalization, and an Internet-based continuous care closed-loop management after discharge.Standardized procedures have been formulated for the prevention of complications and nursing operations during the nutritional support process. The aim is to provide scientific, systematic, and uniform practical basis for clinical nurses, thereby optimizing the nursing process, improving the clinical outcome of patients, and enhancing their quality of life.
To identify the influencing factors of presentation delay among patients with colorectal cancer, and to quantify the dose-response relationship and stratified differences between colorectal cancer awareness and presentation delay,in order to provide a basis for precise intervention.
A cross-sectional study was performed involving 910 patients diagnosed with colorectal cancer from January 2024 to August 2025 at a tertiary grade A hospital in Sichuan province.Demographic characteristics, health treatment behaviors and colorectal cancer awareness level were collected via questionnaires and electronic medical records.Linear regression, restricted cubic spline(RCS) model and stratified analysis were used for statistical analysis.
The median presentation delay of colorectal cancer patients was 84 days, and 48.79% of patients suffered presentation delay exceeding 90 days.Linear regression analysis indicated that educational level, per capita monthly household income, type of medical insurance and having medical workers in the family were influencing factors for presentation delay(all P<0.05).After adjusting for confounders such as educational level, per capita household income, medical insurance type and family medical background, the restricted cubic spline model revealed a non-linear dose-response relationship between colorectal cancer awareness and presentation delay.
Presentation delay in colorectal patients is affected by multiple factors, and a dose-response relationship exists between colorectal cancer awareness and presentation delay.Targeted health education for high-risk groups, optimized medical security and improved accessibility of medical resources are suggested to shorten presentation delay.
To explore the multiple influencing pathways of death anxiety and death attitude in patients with advanced lung cancer,so as to provide a reference for formulating precise death education strategies.
A cross-sectional survey design was adopted.A total of 164 patients with advanced lung cancer from a tertiary grade A cancer specialty hospital in Yunnan province were selected as the research subjects.General information questionnaires, the Chinese version of the Death Anxiety Scale(CT-DAS),and the Chinese version of the Death Attitude Profile-Revised(DAP-R) were used for the investigation.Data analysis was conducted using the fuzzy-set qualitative comparative analysis(fsQCA) method to identify the antecedent condition configurations leading to high levels of death anxiety, negative and positive death attitudes.
There were 6 configuration paths of death anxiety in patients with advanced lung cancer, presenting 5 different patterns, namely long-term isolation-communication deficiency type, high load-silence type, prognosis fear type, traumatic notification type, and spiritual struggle-silence negative type.Negative death attitudes had 8 configuration paths, presenting 5 different patterns, namely exhaustion silence type, early shock type, sudden high malignancy type, high malignancy load type,long-term aphasia type.Positive death attitudes had 6 configuration paths, presenting 4 different patterns, namely open communication type,long-term adaptation type, positive planning type, and focused on the present type.
The death anxiety and death attitudes of patients with advanced lung cancer are the result of multiple factors coupling. The multiple causal paths discovered in this study could provide a basis for formulating precise death education strategies based on the combination of patients' individual characteristics, and are of great significance for promoting the development of clinical death education.
To investigate the association between helicobacter pylori infection and serum homocysteine(Hcy) levels as well as brachial-ankle pulse wave velocity(baPWV),and to assess whether helicobacter pylori infection is an independent risk factor for hyperhomocysteinemia and arterial stiffness.
A total of 6 207 adults who underwent physical examinations at our hospital from January 2022 to December 2023 were included. The subjects were divided into the helicobacter pylori positive group and the helicobacter pylori negative group based on the results of the 13C urea breath test.Linear regression analysis was used to evaluate the linear association between helicobacter pylori infection and Hcy as well as baPWV.Further, multivariate logistic regression was employed to construct models for high Hcy(≥15 μmol/L) and high baPWV(≥1 800 cm/s),and sensitivity analysis was conducted.
There was no significant linear correlation between helicobacter pylori infection and Hcy or baPWV.High Hcy increased the risk of helicobacter pylori positive individuals(P<0.001),and the risk of having high baPWV was significantly increased(P<0.001).The results of the sensitivity analysis were consistent.
There is no significant linear correlation between helicobacter pylori infection and Hcy or baPWV.However, helicobacter pylori infection significantly increases the risk of high Hcy levels and arterial stiffness.Helicobacter pylori infection may affect vascular elasticity through inflammatory responses, nutrient absorption disorders, or metabolic pathways.

A total of 259 patients who underwent allogeneic hematopoietic stem cell transplantation at a tertiary grade A hospital in Chongqing from January 2019 to December 2023 were selected as the modeling group.Relevant risk factors were screened through univariate and multivariate analyses, and a risk prediction model was constructed using R language.A total of 75 patients who underwent allogeneic hematopoietic stem cell transplantation in hospital from January 2024 to March 2025 were selected as the validation group. The model was evaluated and validated using the receiver operating characteristic curve area(AUC),Hosmer-Lemeshow goodness-of-fit test, decision curve analysis, and validation of the prediction model adopted.
The incidence of perianal infection in the modeling group was 18.53%,while that in the validation group was 24.00%.The risk factors of perianal infection after allogeneic hematopoietic stem cell transplantation included the history of perianal diseases, diarrhea, duration of neutropenia, and preoperative prognostic nutritional index.A predictive model for perianal infection after allogeneic hematopoietic stem cell transplantation was constructed based on these risk factors. The AUC value of the modeling group was 0.773,with a sensitivity of 0.604,a specificity of 0.853,and a maximum Youden index of 0.457.The AUC value of the validation group was 0.726,with a sensitivity of 0.745,a specificity of 0.897,and a maximum Youden index of 0.442.The decision curve analysis indicated that this model had good clinical benefits within a wide range of risk threshold probabilities.
The predictive model for patients complicated with perianal infection after allogeneic hematopoietic stem cell transplantation has excellent predictive performance. It could provide a scientific basis for the early assessment of perianal infection after allogeneic hematopoietic stem cell transplantation.
To analyze the risk factors for depression in patients with hepatitis B-related cirrhosis(HBC),to establish a decision tree prediction model, and to formulate nursing interventions to reduce the risk of depression in HBC patients.
A total of 200 HBC patients admitted to our hospital from January 2023 to January 2025 were selected as study subjects.Logistic regression analysis was used to identify risk factors influencing the occurrence of depression in HBC patients.A decision tree model for depression occurrence was constructed using the Chi-squared automatic interaction detection(CHAID) algorithm. The predictive performance of the decision tree model was evaluated by plotting the receiver operating characteristic(ROC) curve and calculating the area under the curve(AUC).
According to the Hamilton Depression Scale(HAMD) scores, all patients were divided into the depression disorder group with 58 cases and the non-depression disorder group with 142 cases.Logistic regression analysis showed that the education level, family support, psychological resilience, disease duration, and Child-Pugh classification were all risk factors for depression in HBC patients(P<0.05).The decision tree model identified 5 explanatory variables, namely Child-Pugh classification, psychological resilience, family care level, disease duration, and educational attainment.Among them, the Child-Pugh classification was the most significant influencing factor for the occurrence of depressive disorders in HBC patients. The AUC of the decision tree model was 0.930.After ten-fold cross-validation, the average AUC of the model was 0.914,which showed a relatively small difference from the original model's AUC.There was no significant overfitting in the model. The generalization ability was good. The Hosmer-Lemeshow test results showed that there was no statistically significant difference between the predicted risk of depression disorder by the model and the actual disease status of the patients. The calibration was good.Using HAMD ≥17 as the diagnostic threshold for depression, the model showed a sensitivity of 87.93%,specificity of 91.55%,and Youden's index of 0.795.
Education level, family support, psychological resilience, disease duration, and Child-Pugh classification are important risk factors for depression in HBC patients. The decision tree model based on these factors demonstrates high predictive value.Clinical practice should develop targeted nursing interventions addressing the above risk factors to reduce the incidence of depression in HBC patients.

To explore the impact of long-term care insurance(LTCI) policy on the progression of frailty among older adults in China.
Based on the data from five rounds of the China Health and Retirement Longitudinal Study(CHARLS) from 2011 to 2020,a frailty index covering physical, psychological, cognitive, social, and subjective health dimensions was constructed. The elderly in the pilot cities for long-term care insurance policies were taken as the treatment group, while the non-pilot cities were the control group. The difference-in-differences(DID) model was employed to assess the impact of the policy on the progression of frailty.
After the implementation of the LTCI policy, the progression rates of physical frailty, psychological frailty and cognitive frailty among older adults were significantly slowed.Parallel trend tests and winsorization results supported the robustness of these findings.Heterogeneity analyses revealed that the LTCI policy had more pronounced delaying effects on frailty progression among older adults who were of advanced age, male, residing in rural areas or with lower educational attainment(P<0.05).
The LTCI policy is beneficial in slowing frailty progression in older adults. We recommend further expanding the policy coverage, promoting the development of care service systems, and enhancing policy targeting and equity to better facilitate healthy aging.
To investigate the status quo of the ability levels for disabled elderly people, and to analyze its influencing factors,so as to provide some references for reducing the degree of ability impairment of disabled elderly people and carrying out targeted elderly care services.
Using a multi-stage stratified sampling method,a total of 420 disabled elderly people in Chengdu city were selected as the research subjects.General information questionnaires and the Elderly Capacity Assessment Scale were used for the investigation.
Among the 420 disabled elderly people,220(52.38%) had mild disability,103(24.52%) had moderate disability,64(15.24%) had severe disability, and 33(7.86%) were completely disabled.Based on the variable selection results of the random forest model,9 core influencing factors were extracted. The results of the ordered multi-class Logistic regression analysis showed that age, educational level, living situation, family per capita monthly income, main caregiver, and life status evaluation were the influencing factors of the ability levels of disabled elderly people(P<0.05).
The proportion of severe and complete disability among disabled elderly people is relatively high.It is recommended that relevant departments promote the elderly ability assessment work.Through relevant interventions,to delay the process of elderly disability, and to improve the quality of life of the elderly.
To explore the trajectory of breast milk sodium ion concentration change in pregnant women with gestational diabetes mellitus and its relationship with delayed onset of lactogenesis.
Using the convenience sampling method,a total of 169 pregnant women with gestational diabetes mellitus from two tertiary grade A hospitals in Guangdong province from August 2024 to April 2025 were selected as the research subjects. They were surveyed using a general information questionnaire.Breast milk sodium ion concentrations were measured at 24 hours,48 hours, and 72 hours after delivery.
The breast milk sodium ion concentrations of pregnant women with gestational diabetes mellitus at 24 hours,48 hours, and 72 hours after delivery were (62.92±8.83) mmol/L,(42.58±9.72) mmol/L,and (27.17±12.95) mmol/L,respectively.Based on the trajectory model, two trajectories of breast milk sodium ion concentration changes were determined:the low sodium ion concentration-rapid decline group(69.8%) and the high sodium ion concentration-slow decline group(30.2%).The incidence of delayed lactation initiation in the low sodium ion concentration-rapid decline group was 13.6%,and that in the high sodium ion concentration-slow decline group was 76.5%.There was statistically significant difference in terms of the incidence of delayed onset of lactogenesis between the two groups(P<0.001).The high sodium ion concentration-slow decline group had a higher risk of delayed onset of lactogenesis as the reference group.
The changes in breast milk sodium ion concentration in pregnant women with gestational diabetes mellitus are correlated with delayed onset of lactogenesis. The trajectory of breast milk sodium ion concentration changes could objectively predict the occurrence of delayed onset of lactogenesis.Clinical medical staff could use the changes in breast milk sodium ion concentration to predict the occurrence of delayed onset of lactogenesis.
To investigate the application value of serum interleukin-23(IL-23),tumor necrosis factor-α(TNF-α) and microRNA-146b-5p(miR-146b-5p) in nursing evaluation of full-term neonates with hypoxic-ischemic encephalopathy(HIE),and to analyze their guiding significance for prognostic nursing management.
A total of 95 full-term neonates diagnosed with HIE admitted to our hospital from January 2020 to December 2024 were enrolled as the HIE group.According to the Diagnostic Criteria and Clinical Grading for Neonatal Hypoxic-Ischemic Encephalopathy, they were subdivided into mild subgroup(29 cases),moderate subgroup(44 cases) and severe subgroup(22 cases).Meanwhile,95 healthy full-term neonates were recruited as the control group.Venous blood samples of all subjects were collected within 1 day after admission to detect serum levels of IL-23 and TNF-α.Quantitative real-time polymerase chain reaction(qRT-PCR) was performed to detect the relative expression of miR-146b-5p.All neonates in the HIE group received standardized nursing interventions, including therapeutic hypothermia care, vital sign monitoring, neurological function protection nursing and so on.After 1 year of follow-up,there was one case of loss to follow-up.The patients were divided into favorable prognosis subgroup(70 cases) and poor prognosis subgroup(24 cases). The correlations of the three indicators with disease severity, nursing evaluation and long-term prognosis were analyzed.
Serum IL-23 and TNF-α levels were significantly higher, while the relative expression of miR-146b-5p was markedly lower in the HIE group compared with the control group(P<0.05).The levels of IL-23,TNF-α and miR-146b-5p were significantly correlated with the severity of HIE(P<0.05).Neonates with poor prognosis exhibited elevated IL-23 and TNF-α levels as well as reduced miR-146b-5p expression relative to those with favorable prognosis(P<0.05).After adjusting for confounding factors,IL-23,TNF-α and miR-146b-5p remained independently correlated with neonatal prognosis(P<0.05).Receiver operating characteristic(ROC) curve analysis revealed that the area under the curve(AUC) of combined detection of IL-23,TNF-α and miR-146b-5p for predicting neonatal prognosis reached 0.945,which possessed significantly superior predictive efficiency compared with single indicator detection(P<0.05).
Serum IL-23,TNF-α,and miR-146b-5p could be used as objective quantitative indicators for the assessment and stratification of the condition of full-term neonates with HIE.The combined detection of these three could improve the accuracy of predicting the prognosis risk,in order to provide scientific support for optimizing nursing plans,to allocate nursing resources, and to improve the long-term outcome of the children.
