The concept of "Internal Deficiency and Latent Wind" was proposed by the Miao Xiyong, a physician in Ming Dynasty, based on the understanding of sages, as a new insight into the pathogenesis of stroke. He believed that in the hot and humid southern regions, people are prone to stroke due to the consumption of true yin and dual deficiency of yin and yang; internal heat becomes rampant, and phlegm - heat congests the airways, leading to the obstruction of the meridians and the onset of stroke. This article starts from the theoretical understanding of 'Internal Deficiency and latent Wind' and, combined with the characteristics of cold regions, suggests that the pathogenesis of ischemic stroke in cold areas is related to the depletion of the liver and kidneys, deficiency of yin and yang; internal phlegm - dampness and heat - stasis interplay; external cold invasion and the mutual stimulation of wind and cold; as well as the combined invasion of external and internal dryness. Treatment should pay attention to the climate and physical characteristics of cold regions, focus on dispersing external wind and removing phlegm - stasis, and emphasize replenishing the true yin of the liver and kidneys. Additionally, moistening agents should be used with discretion to nourish yin, moisten dryness, and open the Xuanfu, with the aim of providing a reference for the clinical prevention and treatment of ischemic stroke in cold regions.

To investigate the mechanism of Yangxin Granules in regulating GSK3β, COX2 and AQP2 to intervene in water - sodium retention in heart failure rats. Methods: One hundred SPF - grade SD rats were randomly divided into sham operation group (10 rats) and modeling group (90 rats). The modeling group underwent ligation of the left anterior descending coronary artery for 2 weeks to establish a post - myocardial infarction heart failure model. The modeling group was then randomly divided into model group, Yangxin Granules low - dose, medium - dose, and high - dose groups, tolvaptan group, and combined intervention group (tolvaptan + Yangxin Granules). All groups received once - daily drug interventions. Urine output was collected at 6, 12, and 24 h during weeks 2 and 4 of treatment. After the final administration, cardiac ultrasound was performed to record left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS). Urine output and body weight were recorded, and heart and kidney indices were calculated. Serum brain natriuretic peptide (BNP) levels were measured by ELISA. Myocardial tissue pathology was observed via HE and Masson staining. Western blot was used to detect expression levels of GSK3β, COX2, and AQP2 proteins in renal tissues. Results: Compared with the model group, Yangxin Granules increased urine output(P < 0.05), decreased serum BNP levels (P <0.05), improved cardiac function (LVEF and LVFS, P < 0.05, P <0.01), and upregulated COX2 expression (P <0.05). Compared with the model group, the tolvaptan and combination groups showed downregulated GSK3β and AQP2 expression (P <0.01). Yangxin Granules alleviated myocardial tissue damage and inflammatory cell infiltration. Conclusion: Yangxin Granules can regulate renal tubular GSK3β and COX2 expression, thereby inhibiting AQP2, reducing water reabsorption, thus exerting therapeutic effects on water - sodium retention in heart failure.

Objective: To explore the mechanism of liver injury induced by Dictamni Cortex water extract through interference with lipid metabolism in the liver. Methods: Thirty - two SPF - grade male SD rats were randomly divided into four groups: control group, high - dose Dictamni Cortex group (8.1 g/kg), medium - dose group (2.7 g/kg), and low - dose group (0.9 g/kg), with 8 rats in each group. The rats were administered by gavage for 8 weeks. Liver weight, liver - to - body weight ratio, and liver toxicity - related indicators were measured to observe changes in the rats' liver. Hematoxylin - eosin (HE) staining and Oil Red O staining were used to assess the pathological changes and lipid droplet accumulation in liver tissues. The mRNA levels of AMPK, PPAR - α, IL - 1β, and TNF - α were detected in liver tissues. Lipid levels in the liver were analyzed by LC - MS/MS, and multivariate statistical analysis was used to identify differentially expressed lipid metabolites. Results: Compared with the control group, the liver - to - body weight ratio was increased in all Dictamni Cortex dose groups(P < 0.01, P < 0.001). In the high - dose group, AST、ALT、ALB levels were significantly increased (P < 0.05, P < 0.001). HE and Oil Red O staining showed that the high - dose group significantly induced fatty liver lesions in rats. The mRNA levels of inflammatory factors IL - 1β and TNF- α were significantly increased (P < 0.01), while the mRNA levels of PPAR - α, were significantly reduced (P <0.01). Lipidomics analysis showed that, compared to the control group, 156 lipid species were significantly altered in the high - dose group, including glycerolipids, phosphatidylcholine, sphingolipids, etc. Specific lipids such as PC(16∶0/18∶2), PC(16∶0/18∶1)、PC(14∶0/18∶2)、PC(15∶0/18∶2)、TG(16∶0/16∶0/16∶0), TG(18∶0/16∶0/16∶0)、SM(d42∶1), SM (d42∶2), SM (d42∶3) showed significantly variation levels after treatment. These results suggest that the high - dose Dictamni Cortex water extract disturbed lipid metabolism in the liver of rats. Conclusion: The water extract of Dictamni Cortex induces liver injury in rats by interfering with lipid metabolism, particularly sphingolipids (SM), ceramides (Cer), triglycerides (TG), and phosphatidylcholine (PC).

The Jiexi acupoint (ST 41) belongs to the Stomach Meridian of Foot - Yangming and has a special anatomical location. Its clinical application reflects the holistic concept of "treating upper diseases from lower." This principle originates from the Huangdi Neijing, which states, "For diseases in the upper body, treat the lower body." This article explores Jiexi's therapeutic approach of treating upper diseases from below from five perspectives: tracing the origins of Jiexi, analyzing its nature, and clarifying its application, clarifying the use of Jiexi Point, enlightenment on the use of Jiexi Point. The Jiexi acupoint is also noted in the phrase "treat muscular atrophy by selecting points on the Yangming meridian," demonstrating its efficacy in treating ankle atrophy. As a key point on the stomach meridian, it helps to alleviate local edema and clear heat in the Stomach meridian. The stomach meridian of Foot - Yangming extends to the hairline and forehead, so needling the Jiexi acupoint can treat upper body conditions, such as headaches and brain disorders, by regulating them from below. In the five elements, Jiexi belongs to fire and shares an affinity with the eyes, heart, and pericardium. The upper portion of the Foot - Yangming meridian connects to the heart, while the major collateral vessel of the stomach, known as Xuli, is located at the point of the heart's pulsation. This connection lays the foundation for treating eye diseases, heart - related conditions, and mental disorders. As an acupoint on the meridian, Jiexi is also indicated for treating coughs, fevers, and chills. Additionally, the theory of "treating edema by targeting the meridian" suggests that Jiexi can be used to address both external coughs and stomach - related coughs accompanied by edema. Jiexi point has a wide range of clinical applications in combination with other points, and it is rich in special needling techniques. Jiexi and Yang Jiao points, both belonging to the fire element, can be used together to treat palpitations. A classic case is appended to verify their effectiveness.

Professor Xie Lin points out that the core pathogenesis of cervical spondylosis lies in the blockage of muscles, bones, and blood vessels, as well as the malnutrition of the muscles and bones. In clinical practice, the diagnosis and treatment should be based on the four diagnostic methods, to clarify the condition of deficiency or excess, and differentiate the symptoms. The treatment principle should focus on muscles and bones, with the main strategy being the elimination of excess and supplementation of deficiency, and the active use of horn medicine. In cases of wind, cold, phlegm, and blood stasis blocking the channels, it is necessary to expel the pathogenic factors, unblock the channels, and benefit the muscles and bones. When the internal organs are deficient in qi and blood, the treatment should aim to nourish the base, supplement deficiency, and nourish the muscles and bones. Additionally, associated symptoms should be considered, and treatments for muscle relaxation, liver wind elimination, and the relaxation of muscles and tendons should be used. Clinical practice has shown good results, as evidenced by the case examples provided.

Tic disorders often co - occur with sleep disorder, mood disorder, learning disorder and other physical and mental diseases, it's characterized by spirit - body disharmony, can be classified as Jueyin wind wood disease. The theory of Jueyin Conghua is believed that Jueyin wind wood is smooth and follows the Shaoyang phase fire, if the wood qi is smooth, the phase fire will transform into the wind wood. Guided by the "Jueyin Conghua theory", the professor Cui xia argues that the key to the pathogenesis of tic disorders and comorbidities lies in the compromised Jueyin pivotal mechanism and aberrant Conghua. The tendency of disease depends on the rise and fall of Jueyin phase fire. If it is too high, the wood will produce fierce wind from fire, and if it is too low, cold and heat will be mixed to generate wind In cases of acute and severe tics, the treatment must concentrate solely on regulating the body, with minor attention to the spirit, prioritizing the method of subduing yang and extinguishing wind. For chronic and mild tics presenting with comorbidity, a concurrent treatment approach is required. This entails simultaneous harmonization of both body and spirit, through dispersing stagnation and nourishing to pacify wind, thereby regulating the qi and blood of the root organ. At the same time, methods such as breath and mental regulation, daoyin (Therapeutic Exercise) and tuina (Chinese medical massage), and spiritual cultivation should be integrated to achieve a combined internal and external therapeutic strategy, enabling a comprehensive intervention for the condition. A case is attached for clinical reference.

Pediatric sinusitis is prevalent among children, which is mainly manifested by a large amount of turbid nasal discharge. Western medicine treats this disease mostly with oral antibiotics and nasal hormones, which have the disadvantages of uncertain efficacy and drug resistance. Traditional Chinese Medicine (TCM) treats this disease through oral and external use of Chinese herbal drugs, offering a relatively safe and low - side - effect treatment option. In this study, we believe that the onset of pediatric sinusitis is related to the external pathogens to the lungs closing to the surface and inside; the heat of the gallbladder leading to the congestion of the nose and orifices; spleen and stomach disorders causing to the imbalance of qi movement. The primary pathogenesis involves internal heat stagnation and obstruction of the nasal passages. The following methods should be used in treating this disease: to resolve the pathogens both external and internal, to dispel external evils and clear the heat in the lungs at the same time; pungent and bitter medicines should be used together, to relieve the heat in the gallbladder; to elevate the clearness and lower the turbidity, regulating the spleen and the stomach Qi. Tongqiao Shengjiang Powder is derived from "Shengjiang Powder", a classic formula for treating epidemics, and is modified by the addition of Magnolia biondii Pamp., ueraria lobata(Willd.) Ohwi, and Glycyrrhiza uralensis Fisch. This formula has excellent therapeutic effects for treating pediatric sinusitis. It has the efficacy of resolving external pathogens and clearing the heat in the lungs, elevating the clearness and lowering the turbidity, and improving nasal ventilation. Although its medicinal flavor is simple, the formula is comprehensive and covers several treatments. It can effectively treat pediatric nasosinusitis of various TCM syndrome types through addition or subtraction. In this paper, one case is attached for clinical reference.

Objective: To investigate the clinical efficacy of the addition and subtraction of Duhuo Jisheng Decoction on the residual numbness of nerve root after intervertebral foramenoscopy. Methods: 80 patients with residual numbness of nerve roots after undergoing intervertebral foramen endoscopy treatment at Shangqiu Traditional Chinese Medicine Hospital were selected as the research subjects, and they were divided into 40 cases each in the conventional group(treated with methylcobalamin) and the traditional Chinese medicine group (treated with the modified Duhuo Jisheng Decoction on the basis of the conventional group) according to the method of the randomized numerical table generated by the computer for 4 consecutive weeks of treatment. The clinical effects of the two groups were compared. The numbness degree of lower limbs, lumbar function, nerve conduction velocity, evoked somatosensory potential, inflammatory factors and quality of life of the two groups were observed before and after treatment. Compare the adverse reactions and recurrence rates at 3 and 6 months between two groups. Results: After 4 weeks of treatment, the effective rate of the traditional Chinese medicine group was 92.50% (37/40), which was significantly higher than the 72.50%(29/40) effective rate of the conventional group, and the difference was statistically significant (P < 0.05). After treatment, the acupuncture test, visual analog scale (VAS) for numbness, nylon rope test score, Oswestry dysfunction index (ODI), lumbar spinal potential, cauda equina potential, cortical potential, and serum levels of tumor necrosis factor - α (TNF - α), high - sensitivity C - reactive protein (hs CRP), and interleukin - 1β (IL - 1β) in the traditional Chinese medicine group were significantly reduced compared to the conventional group (P < 0.05, P <0.01). The Japanese Orthopaedic Association's back pain score (JOA) and the motor conduction velocity of the tibial and common peroneal nerves were significantly increased compared to the conventional group (P < 0.01). After treatment, the role function, physical function, social function, and material life scores of the traditional Chinese medicine group were significantly higher than those of the conventional group (P < 0.01). The recurrence rate of the traditional Chinese medicine group was 2.50% (1/40) after a 6 - month follow - up, which was significantly lower than the conventional group's 15.00% (6/40), and the difference was statistically significant (P <0.05).Conclusion: The modified Duhuo Jisheng Decoction has a significant effect in treating residual numbness of nerve roots after intervertebral foramen endoscopy. It may improve nerve conduction velocity, alleviate numbness symptoms, improve lumbar spine function, and enhance quality of life by inhibiting inflammatory reactions, and has high safety.

Warming and Resolving method is one of the fundamental methods of clinical treatment of traditional Chinese medicine, and it can be seen in the Suwen - The Union of Heaven and Human Beings that the idea of the equilibrium between Yin and Yang can be clarified. The principle that yin and yang imbalance can lead to diseases in the human body. The treatment method of "Warming and Resolving" refers to using Warm Traditional Chinese Medicinal to correct the pathological state of yang deficiency in the body, and combining it with products that promote qi circulation, blood circulation, and blood stasis to resolve pathological results such as phlegm dampness, blood stasis, and pathological disorders caused by yang deficiency and inability to transport and transform. In clinical practice, early and mid stage liver cancer is commonly diagnosed with excess symptoms, while in the late stage, the syndrome gradually shifts from excess to deficiency, resulting in deficiency of positive qi and liver and kidney. On the basis of syndrome differentiation, modern doctors can achieve effective treatment of liver cancer by orally taking decoctions according to the theory of "Warming and Resolving". This article reviews the mechanism and research progress of some warm and warm drugs in the treatment of liver cancer from the perspective of modern pharmacology, providing reference for clinical medication.

Chronic musculoskeletal pain (CMP) is the most common type of chronic pain and includes cervical spondylosis, frozen shoulder, knee osteoarthritis, myofascial pain syndrome, rheumatoid arthritis, and fibromyalgia. At present, the clinical treatment of CMP is mainly based on drug therapy, but there are many adverse reactions in the treatment of Western medicine and there are differences in drug efficacy between different indications. Acupotomy therapy combines the advantages of traditional Chinese medicine acupuncture and Western medical surgical therapy, and is widely used in the treatment of CMP in clinical practice. Acupotomy therapy can improve the structure and function of the musculoskeletal system, affect the conduction of pain, and reduce inflammation through local release, thereby relieving pain and improving limb function. This article summarizes the application and progress of the underlying mechanisms of Acupotomy therapy in the field of CMP, aiming to provide a reference for the prevention and treatment of CMP and the clinical application of Acupotomy therapy.
