Outline
Objective: To investigate the efficacy of Chaihu plus Longgu and Muli Decoction combined with Huanglian Wendan Decoction in the treatment of phlegm - fire disturbing the heart type of intractable insomnia and its effect on sleep structure. Methods: Retrospectively selected 92 cases of recalcitrant insomnia patients treated in our hospital as research subjects, the selected time period is January 2022 - October 2024, according to the different treatment methods are divided into 40 cases in the control group (eszopiclone tablets) and 52 cases in the experimental group (control group + Chaihu plus Longgu and Muli Decoction combined with Huanglian Wendan Decoction). The clinical efficacy, TCM scores, sleep structure and sleep quality of the two groups were compared. Safety was analyzed for both groups. Results: After treatment, the effective rate of the experimental group was 96.15%, which was significantly higher than that of the control group, which was 80.00% (P < 0.05). After treatment, the scores of sleep and wakefulness, difficulty in falling asleep, sleeplessness after waking up, and restlessness and dreaminess in the experimental group were significantly lower than those in the control group (P <0.05). After treatment, the levels of rapid eye movement(REM) and non - rapid eye movement (N3) were significantly higher in the experimental group than in the control group (P < 0.05); the levels of non - rapid eye movement (N1) and non - rapid eye movement (N2) were significantly lower in the experimental group than in the control group (P < 0.05). After treatment, patients in the experimental group had significantly lower pulmonary daytime dysfunction, sleep onset, sleep duration, sleep disorder, sleep quality, sleep effectiveness and total score than those in the control group (P < 0.05). After treatment, the incidence of adverse reactions in the experimental group was 15.38% (8/52), higher than the control group's 10.00%(4/40), but there was no significant difference between the two groups (P > 0.05).Conclusion: The combination of traditional Chinese medicine and western medicine (based on the treatment of estazolam tablet and Chaihu plus Longguand Muli Decoction combined with Huanglian Wendan Decoction) has a significant synergistic effect on refractory insomnia of phlegm fire disturbing heart type, which can not only achieve more satisfactory curative effect, but also effectively improve the symptoms of traditional Chinese medicine, significantly improve the sleep quality, optimize the sleep structure, and have higher safety, reflecting the advantages of integrated traditional Chinese and western medicine in the treatment of this disease.
