To explore the authentic caregiving experiences of caregivers of left-behind children in underdeveloped regions based on ecosystem theory.
A total of 22 caregivers of left-behind children who underwent physical examinations at a maternal and child health hospital in a county of Yunnan province were selected as study subjects from July 2024 to June 2025 by purposive sampling.Focus group interviews and semi-structured in-depth interviews were conducted.Participant observation was supplemented.Data were analyzed using a combination of directed content analysis and conventional content analysis.
Four core themes and thirteen sub-themes were ultimately identified. The core themes were experiences of physical and mental tearing and resilient stitching at the microsystem level, experiences of tearing at the mesosystem level, experiences of tearing and limited stitching at the exosystem level, and experiences of tearing and resilient stitching at the macrosystem level.
The caregiving process for left-behind children is a dynamic experience involving multiple tearing experiences and limited stitching.Healthcare professionals should play an active role within the family-school-community-policy four-dimensional support network to improve the caregiving experiences of these caregivers.
To explore the application effects of a low glycemic index(GI) diet combined with resistance exercise in patients with gestational diabetes mellitus(GDM).
GDM patients who underwent prenatal check-ups and delivered at Zhangye People's Hospital Affiliated to Hexi University were selected as the research subjects from June to December 2024.The patients were divided into observation group and control group using random number table. The control group received conventional lifestyle management, while the observation group was received low glycemic index diet diet combined with resistance exercise based on conventional lifestyle management.Indicators monitored included glycemic and lipid metabolism parameters[fasting blood glucose(FBG),glycated hemoglobin(HbA1c),total cholesterol (TC),triglycerides(TG),2-hour postprandial blood glucose(2 hPG),fasting insulin(FINS),homeostatic model assessment for insulin resistance(HOMA-IR)],adipokines[leptin(LEP),adiponectin(ADPN),soluble leptin receptor(sLR),LEP/ADPN (L/A)],and delivery outcomes(gestational hypertensive disorders, premature rupture of membranes, cesarean section, preterm birth, cumulative blood loss at 2 hours postpartum, macrosomia, fetal distress, neonatal asphyxia, neonatal hypoglycemia,1-minute Apgar score).
A total of 46 patients in the observation group and 45 patients in the control group completed the intervention. After the intervention, the observation group showed lower levels of FBG,2 hPG,HbA1c,TC,TG,LEP,L/A,FINS,and HOMA-IR compared to both pre-intervention levels and the control group during the same period. The ADPN and sLR levels of observation group were higher than pre-intervention levels and those of the control group. The incidence rates of events such as cesarean section, macrosomia, and neonatal asphyxia of observation group were all lower than those in the control group. The cumulative blood loss at 2 hours postpartum of observation group was less than that in the control group. The 1-minute Apgar score of observation group was higher than that in the control group(all P<0.05).
The combination of low glycemic index diet and resistance exercise can improve insulin sensitivity, optimize glycemic and lipid metabolism indicators, and regulate the balance of the adipokine network to ameliorate the metabolic status of GDM patients.
To explore the illness perceptions of breast cancer patients with lymphedema.
A phenomenological research approach was adopted.Semi-structured interviews were conducted with 15 breast cancer patients with lymphedema who visited the lymphedema clinic of a tertiary grade A hospital in Jiangsu province from June to July 2024.The interview data were analyzed using Colaizzi's seven-step method.
A total of six themes were identified, namely recognizing the chronicity of the disease, symptom relief enhancing treatment confidence, actively adapting to and coping with the illness, lacking an accurate perception of the disease's severity, feeling a lack of control over the illness, and experiencing physical and psychological discomfort as well as life disturbances.
Breast cancer patients with lymphedema generally lacked an accurate understanding of the severity of their condition.Medical staff should strengthen health education for patients, alleviate their physical and psychological discomfort, enhance their treatment confidence and self-management abilities, and improve their sense of control over the illness.
To evaluate the application effectiveness of a mindfulness-based group course in women with unexplained recurrent spontaneous abortion(URSA).
A total of 60 URSA women who attended the reproductive medicine center of our hospital were selected as the study subjects from January to June 2022.The women were divided into an intervention group and a control group using a random number table method, with 30 cases in each group. The control group received routine treatment and mental health education, while the intervention group received routine treatment, mental health education, and a mindfulness-based group course intervention.URSA women were assessed using the Five Facets of Mindfulness Questionnaire(FFMQ),the Self-Rating Depression Scale(SDS),the Self-Rating Anxiety Scale(SAS),the Fertility Problem Inventory(FPI),and the Fertility Quality of Life(FertiQoL) before the intervention and after 6 weeks of intervention.
During the study period,2 URSA women in the control group and 5 URSA women in the intervention group dropped out.Finally,28 URSA women in the control group and 25 URSA women in the intervention group completed the study. After 6 weeks of intervention, the total scores of SDS,SAS,and FPI in the intervention group decreased compared with those before the intervention(all P<0.05),while the total scores of FertiQoL and FFMQ increased compared with those before the intervention(all P<0.05).Moreover,the total scores of SDS and SAS in the intervention group were lower than those in the control group at the same time point(all P<0.05),while the total scores of FertiQoL and FFMQ in the intervention group were higher than those in the control group at the same time point(all P<0.05).After 6 weeks of intervention, the total score of FFMQ in the intervention group was negatively correlated with the total scores of SDS,SAS,and FPI,and positively correlated with the total score of FertiQoL(all P<0.05).
Mindfulness-based intervention could improve the mindfulness level of URSA women and alleviate their psychological distress and improve their quality of life.
