The theory of“water entering meridians, and blood being formed”originates from ZHANG Zhongjing's Treatise on Febrile Diseases-Pulse Diagnosis and is a comprehensive summary of the absorption, circulation and transformation of water in traditional Chinese medicine. The process of water entering the meridians and transforming into blood marks the beginning of its circulation and function within the body. During this process, the spleen, meridians and kidney play vital roles. When water fails to enter the meridians to transform into blood,it forms pathological products such as phlegm, retained fluid and dampness. These pathological products, when accumulated in the lower limbs, obstruct normal blood flow and lead to the mutual obstruction of water and blood, resulting in edema. This mechanism bears a resemblance to the pathogenesis of chronic venous edema of the lower limbs, where venous hypertension in the lower limbs causes tissue fluid reflux obstruction and changes in vascular permeability. Based on the theory of“water entering meridians, and blood being formed”,Professor CAO Jianchun, the chief physician at the Peripheral Vascular Department of Dongfang Hospital has proposed that the treatment of chronic venous edema of the lower limbs should start from the physiological and pathological mechanisms of water entering the meridian circulation, adopt the method of promoting blood circulation and urination, reuse Huangqi(Astragali Radix)to invigorate spleen,Zelan(Lycopi Herba)to promote blood circulation,Zexie(Alismatis Rhizoma)to promote urination, and supplement with Guizhi(Cinnamomi Ramulus)and other spleen-invigorating,urination-promoting and blood-activating drugs,to achieve the process of invigorating spleen and tonifying Qi,regulating the absorption of water into the meridians to treat the root cause, promoting blood circulation and removing blood stasis, improving the process of water returning to the meridians and transforming blood between the skin and flesh, promoting urination and reducing swelling, and promoting the excretion of water through the meridians and kidney to the human body for external treatment.
