Chronic heart failure（CHF）belongs to the category of heart water disease of traditional Chinese medicine（TCM）.The etiology is complex，and the disease location is diversified，especially involves kidney in the late stage.Based on the theory of“kidney governing water”，the connotation of kidney governing water theory was summarized from the aspects of kidney regulating Qi transformation，kidney governing water transformation and five fluids.From the physiological functions and pathological changes of heart，such as heart dominating blood vessels and blood obstruction being water，to understand the physiological correlation between heart and kidney，pathological influence and the evolution of TCM syndromes of heart water disease，the role of this theory in the late syndrome and treatment of heart water disease was analyzed from multiple perspectives，and proposed that the later stage of chronic heart failure was heart and kidney comorbidity.The key syndrome was Yang deficiency and water overflowing syndrome.The key pathogenesis is that heart Yang deficiency is insufficient to help kidney，the colder the kidney water is，the water attacking heart，eventually leading to Yangqi deficiency of heart and kidney，overflowing of water and fluids.
