Atherosclerosis（AS）is a chronic vasculitis disease，which is the main pathological basis of cardiovascular and cerebrovascular diseases. The occurrence and development of AS involve various pathological mechanisms such as inflammatory response，oxidative stress，lipid infiltration，dysfunction of vascular endothelial cells，proliferation of vascular smooth muscle cells，macrophage polarization and imbalance of autophagy homeostasis. Sanqi（Notoginseng Radix et Rhizoma）is one of the traditional Chinese medicines for promoting blood circulation and removing blood stasis. The combination of Sanqi（Notoginseng Radix et Rhizoma）and multiple traditional Chinese medicines is commonly used in clinical practice for the treatment of AS，with definite therapeutic effects. Modern pharmacology shows that the active ingredients of Sanqi（Notoginseng Radix et Rhizoma）mainly include saponins，Panax notoginseng saponins，flavonoids，etc. As a saponin class，Panax notoginseng saponins（PNS）are important indicators for evaluating the quality of Sanqi（Notoginseng Radix et Rhizoma）and the main material basis for exerting pharmacological effects. It contains multiple monomers and has been proven to have various pharmacological effects such as anti-AS and improving cerebral ischemia-reperfusion injury. This article analyzed the pharmacological effects of the active ingredients of Sanqi（Notoginseng Radix et Rhizoma）and its anti-AS pharmacological mechanisms，summarized the mechanism of action of commonly used clinical anti-AS formulas of Sanqi（Notoginseng Radix et Rhizoma），and explored the impact of Sanqi（Notoginseng Radix et Rhizoma）processing methods on its anti-AS effects，in order to provide reference for better application of Sanqi（Notoginseng Radix et Rhizoma）in the treatment of AS and in-depth pharmacological research.
