Ischemic stroke，a prevalent type of cerebrovascular disease，is marked by high incidence and mortality rates，with an alarming trend towards younger demographics in China. In this context，tanshinone ⅡA and tetramethylpyrazine have garnered research attention for their multifaceted therapeutic effects and minimal side effects in treating ischemic stroke. These two traditional Chinese medicinal compounds exhibit similar biological effects，including antioxidant，anti-inflammatory，anti-apoptotic properties，protecting blood-brain barrier and promoting neovascularization effects. They also protect the nervous system by modulating key signaling pathways such as Nrf2/HO-1，NF-κB and PI3K/Akt. Clinical studies have demonstrated significant efficacy and good safety profiles for tanshinone ⅡA and tetramethylpyrazine injections in the treatment of stroke. This article aimed to synthesize the research advancements of tanshinone ⅡA and tetramethylpyrazine in ischemic stroke therapy，offering new perspectives for drug development，mechanistic research and treatment strategies for ischemic stroke.
