To analyze the curative effect and rehabilitation efficiency of activating blood and dispersing wind acupuncture combined with Tianma Guteng Yin（天麻钩藤饮）in the treatment of stroke patients with hypertension.

A total of 208 patients with stroke combined with hypertension admitted to the hospital from May 2022 to May 2024 were included as the study objects. They were divided into two groups by the random number table method. A hundred and four patients in the control group were treated with conventional western medicine，and the other 104 patients in the observation group were treated with activating blood and dispersing wind acupuncture combined with Tianma Guteng Yin on this basis. The difference of curative effect，24 h ambulatore blood pressure，nerve function，limb function，TCM syndrome scores，vascular endothelial function and Klotho protein level were compared between the two groups.

The total effective rate of the observation group was 97.12%（101/104），which was much higher than 85.58%（89/104）of the control group（P<0.05）. After treatment，the TCM symptom scores，National Institutes of Health Stroke Scale（NIHSS）score and 24 h ambulatore blood pressure in the observation group were lower than those in the control group，and the Fugl-Meyer Assessment（FMA）score was higher than that in the control group（P<0.05）. After treatment，endothelin-1（ET-1）in the observation group was lower than that in the control group，and nitric oxide（NO）and Klotho were higher than those of the control group（P<0.05）.

Activating blood and dispersing wind acupuncture combined with Tianma Guteng Yin can effectively reduce the blood pressure，regulate the levels of NO，ET-1 and Klotho protein，improve the vascular endothelial function，promote the recovery of nerve function and limb function，and plays a positive role in improving the prognosis of patients with stroke and hypertension.
