To study the clinical efficacy of Mahuang Lianqiao Chixiaodou Decoction（麻黄连翘赤小豆汤）combined with routine treatment of primary nephrotic syndrome（PNS）with damp-heat syndrome.
A total of 120 patients with PNS in the hospital from March 2022 to March 2024 were included and assigned into observation and control group randomly. The control group received conventional treatment. The observation group received Mahuang Lianqiao Chixiaodou Decoction additionally. The clinical efficacy, TCMsyndrome scores, the levels of urea nitrogen（BUN）, creatinine（Scr）, albumin（ALB）, 24 h urine protein, T lymphocyte CD4+, CD8+, CD4+/CD8+, triglycerides（TG）and total cholesterol（TC）and occurrence of adverse reactions were compared between the two groups after treatment.
The observation group detected a higher clinical efficacy rate compared to control group（P<0.05）. After treatment, the observation group had lower scores on TCM syndromes compared to control group（P<0.05）. The levels of BUN, Scr and 24 h urinary protein decreased, ALB increased and the improvement was more obvious in the observation group（P<0.05）. The levels of CD4+ and CD4+/CD8+ in both groups were increased than those before treatment, the levels of CD8+, TG and TC decreased, and the improvement was more obvious in the observation group（P<0.05）. No statistical difference was found in adverse reaction rate between two groups（P>0.05）.
Mahuang Lianqiao Chixiaodou Decoction has a high efficacy in treating PNS with damp-heat syndrome. It can improve the renal function, blood lipids and immune function of the patients and relieve the clinical symptoms while ensuring safety.
