To investigate the effect of Suhexiang Pill（苏合香丸）combined with alteplase in the treatment of acute ischemic stroke and its influence on the level of coagulation index.
A total of 102 patients with acute ischemic stroke admitted to the hospital were randomly divided into control group（52 cases）and observation group（50 cases）. After admission，all patients were given symptomatic supportive treatment such as oxygen inhalation，anti-platelet aggregation，maintenance of water and electrolyte balance，nutritional nerve and reduction of intracranial pressure. The control group was treated with alteplase injection thrombolytic therapy，and the observation group was treated with Suhexiang Pill（1 pill/time，once a day）. Both groups were treated for 2 weeks. After treatment，the therapeutic effect，Modified Barthel index（MBI）scores，National institutes of health stroke scale）（NIHSS）scores，Stroke-specific quality of life scale（SS-QOL）scores and TCM syndrome scores，the levels of D-dimer（D-D），prothrombin time（PT），thrombin time（TT）and activated partial thromboplastin time（APTT），the serum levels of endothelin-1（ET-1）and endothelial nitric oxide synthase（eNOS），the levels of hypersensitive C-reactive protein（hs-CRP），tumor necrosis factior-α（TNF-α）and interleukin-6（IL-6）were compared between the two groups.
After treatment，the curative effect of the observation group was significantly higher（P<0.05）. After treatment，TCM syndrome scores，MBI scores，NIHSS scores and SS-QOL scores，the levels of D-D，PT，TT and APTT of the two groups were significantly improved（P<0.05）. The improvement was more obvious in the observation group（P<0.05）. After treatment，the serum levels of hs-CRP，TNF-α and IL-6，as well as the serum levels of ET-1 and eNOS in two groups were significantly decreased（P<0.05），and the decrease was more significant in the observation group（P<0.05）.
Suhexiang Pill combined with alteplase has a good effect on acute ischemic stroke.
