To explore the effect of Xuanfei Huazhuo Formula（宣肺化浊方）on the expressions of CD86 and CD206 in alveolar lavage fluid of patients with sepsis lung injury with turbid toxin accumulation syndrome.
Sixty patients with sepsis induced lung injury（turbid toxin internal accumulation syndrome）who visited Hebei Provincial Hospital of Traditional Chinese Medicine from September 2022 to September 2023 were selected as the study subjects.The participants were randomly divided into an experimental group and a control group using a random coding table，with 30 cases in each group.The control group were given an equal amount of warm water as a control，while the experimental group were given nasal feeding of Xuanfei Huazhuo Formula on the basis of routine treatment.The total treatment period for both groups was 7 days.The 28-d mortality rate，clinical efficacy，severity of the condition，serum inflammatory response indicators，blood gas indicators，respiratory function indicators and expressions of CD86 and CD206 in alveolar macrophage phenotype in alveolar lavage fluid were compared between two groups.
Compared to the control group，the experimental group showed a higher overall effective rate and a lower 28-day mortality rate（P<0.05）.After treatment，the Acute Physiology and Chronic Health Evaluation（APACHE）Ⅱscore and lung injury score（LIS）of both groups decreased（P<0.05），and compared to the control group，the LIS score of the experimental group was lower（P<0.05）.After treatment，the levels of C-reactive protein（CRP），procalcitonin（PCT），interleukin-6（IL-6），tumor necrosis factor-α（TNF-α）and interleukin-10（IL-10）in both groups were reduced（P<0.05），and compared with the control group，the experimental group had lower levels of these indicators（P<0.05）.After treatment，the levels of arterial partial pressure of oxygen（PaO2）and PaO2/FiO2 in both groups increased，while the levels of extravascular lung water index（EVLWI）and pulmonary vascular permeability index（PVPI）decreased（P<0.05）. Compared with the control group，the experimental group performed better on these indicators（P<0.05）.After treatment，the expression of CD68 increased and the expression of CD206 decreased in the alveolar lavage fluid of both groups（P<0.05），and compared to the control group，the experimental group had higher expression of CD68 and lower expression of CD206（P<0.05）.
Xuanfei Huazhuo Formula has definite therapeutic effect in treating patients with sepsis induced lung injury due to the accumulation of turbid toxin.Its mechanism of action may be closely related to the regulation of CD86 and CD206 in alveolar lavage fluid，and has clinical application and promotion value.
