    Objective

To explore the application effect of a modified fall risk assessment tool combined with patient-participation interventions in hospitalized elderly patients at high risk of falls.

    Methods

Expert consultation was used to improve the Chinese version of the i Engaging Fall self-Assessment Tool, and a pre-survey was conducted on 270 eligible patients.Using convenience sampling method,high-risk elderly patients admitted to the geriatric department of a hospital in Wuhan city from February 2022 to June 2024 were selected as the research subjects. They were divided into a control group(n=236),observation group 1(n=235),and observation group 2(n=232).The control group used the Chinese Version of the i Engaging Fall Self-Assessment Scale. The Observation group 1 used the Modified Chinese Version of the i Engaging Fall Self-Assessment Scale. The observation group 2 received the Modified Chinese Version of the i Engaging Fall Self-Assessment Scale combined with patient-participation interventions. The incidence of falls, the workload of fall prevention nurses, and fall alertness among three groups were evaluated.

    Results

In the control group,4 cases experienced falls, with a fall incidence rate of 0.195%.In observation group 1,3 cases experienced falls, with an incidence rate of 0.131%;In observation group 2,only one person fell, with an incidence rate of 0.047%.There were no statistically significant differences in terms of the occurrence of falls among three groups(P>0.05).The duration of fall protection in observation group 2 was shorter than that in observation group 1 and control group(P<0.05).The duration of fall assessment was longer than that in observation group 1 and control group(P<0.05).The total score of the Fall Vigilance Scores and the dimensions of activity safety and environmental alert, physical function alert, drug alert, and cognitive behavioral alert in observation group 2 were higher than those in the control group and observation group 1(P<0.05).

    Conclusions

Modified fall assessment tools combined with patient-participation interventions could reduce nursing workload, enhance patients' fall vigilance, and promote patient safety.

To explore the influence of nursing quality supervision model on the role function of nursing staff in the gastroenterology department and the overall nursing quality of the department.

    Methods

A total of 30 nursing staff in the gastroenterology department of Foshan Hospital of Traditional Chinese Medicine from January 2022 to December 2023 were selected as the research subjects. The routine management mode was implemented from January to December 2022.The nursing quality supervision model was implemented from January to December 2023.The overall nursing quality of the gastroenterology department and the regulatory effectiveness, role functions, and practical abilities of nursing staff before and after the implementation of the supervision model were compared.A total of 100 patients with digestive diseases before and after the implementation of the nursing quality supervision model were selected.Their satisfaction with the nursing services provided in the department were compared.

    Results

After supervision, the nursing quality scores of nursing staff in the gastroenterology department(disinfection operation, operation technique, ward nursing, specialized nursing) were higher than that before supervision(P<0.05).The role functions, practical abilities, and regulatory project scores of nursing staff in the gastroenterology department were higher than that before supervision(P<0.05).The satisfaction rate of patients in the gastroenterology department(94.00%) was higher than that before supervision(80.00%)(P<0.05).

    Conclusions

The nursing quality supervision model could enhance the role function, and practical ability of nursing staff in the gastroenterology department, while also improve the nursing quality, regulatory effectiveness, and patients' satisfaction rate in the department.

    Objective

To explore the experience of thirst and needs in stroke patients with nasal feeding, and to provide reference for the construction of thirst management program for stroke patients with nasal feeding.

    Methods

A descriptive qualitative research method was used to select 16 stroke patients with thirst after nasal feeding in the Department of Neurology of a tertiary grade A hospital in Suzhou city from August to October 2024.The purposive sampling method was used to conduct semi-structured interviews.Based on the theory of unpleasant symptoms, the data were analyzed by directional content analysis combined with traditional content analysis.

    Results

A total of five themes were extracted:the characteristics of thirst symptoms(the nature of thirst was burning and knife-like,the time and intensity of occurrence had individual differences, and the degree of distress to patients),the attribution of thirst(physiological factors, psychological factors, environmental factors),the impact of thirst(physical pain, psychological suffering, irrational behavior),the coping of thirst(various coping styles, limitations of coping effects, and concerns about the safety of coping),and unmet needs(emotional and social needs, information needs).

    Conclusions

The problem of thirst in stroke patients with nasal feeding is prominent. It is suggested that nursing staff should strengthen the understanding of thirst symptoms and carry out multi-dimensional evaluation, identify multiple influencing factors of thirst in the early stage, pay attention to the negative impact of thirst on patients, and develop a thirst management model of nursing staff as the leading factor and family as the unit and co-participation.

    Objective

To explore the application effect of modified Seldinger invasive technique in peripherally inserted central venous catheters(PICC) of extremely low birth weight infants.

    Methods

A total of 496 extremely low birth weight infants who underwent PICC catheterization in the neonatal intensive care unit of Children's Hospital of Fudan University from July 2019 to May 2022 were selected as the study subjects. From July 2019 to October 2020,children who urderwent traditional catheterization techniques with a puncture sheath needle served as the control group. From November 2020 to May 2022,children who urderwent the improved Seldinger minimally invasive catheterization technique with a 24G indwelling needle served as the test group.One-time puncture success rate, the incidence of bleeding at the puncture site, the incidence of catheter-related thrombosis, the incidence of limb swelling, and the occurrence of phlebitis after catheterization between the two groups of newborns were compared.

    Results

The one-time puncture success rate of the test group(67.65%) was higher than that of the control group(16.10%).The incidence of bleeding at the puncture site(2.94%) was lower than that of the control group(43.49%).The incidence of phlebitis(6.37%) was lower than that of the control group(12.33%).There were statistically significant differences between the two groups(P<0.05).

    Conclusions

The use of modified seldinger technique minimally invasive technique for PICC catheterization in extremely low birth weight infants could not only improve the success rate of a single puncture, but also reduce the incidence of bleeding and phlebitis at the puncture site.
