This study aims to explore the clinical safety and efficacy of Shenfu injection combined with western medicine in the treatment of septic shock at the level of evidence-based medicine. SinoMed, Embase, PubMed and Cochrane databases were searched for randomized controlled trials (RCTs) of Shenfu injection in the treatment of septic shock, with key indicators being 28-day mortality, incidence of multiple organ dysfunction syndrome (MODS), length of ICU stay, blood lactate level and blood lactate clearance rate. Meta-analysis was performed using Review Manager 5.4 and Stata 15.0 software. A total of 43 RCTS were included. The results showed that the 28-day mortality rate (RR=0.82, 95%CI: 0.70 to 0.95) and the incidence rate of MODS (RR=0.67, 95%CI: 0.56 to 0.81), ICU length of stay (MD=-1.95, 95%CI: -2.61 to -1.30), blood lactate level at 6 hours after treatment (MD=-1.10, 95%CI: -1.47 to -0.72), and blood lactate level at 24 hours after treatment (MD=-0.87, 95%CI: -1.12 to -0.63) were all lower (P<0.001), compared with the control group (receiving conventional western medicine treatment), while the clearance rate of blood lactic acid at 24 hours after treatment in the Shenfu group (MD=9.79, 95%CI: 6.33 to 13.25) was higher. It is concluded that compared to conventional western medicine treatment, Shenfu injection can significantly reduce the 28-day mortality, the incidence of MODS, the length of ICU stay, the level of blood lactate 6 hours after treatment, the level of blood lactate 24 hours after treatment, improve the clearance rate of blood lactate 24 hours after treatment, and significantly improve the prognosis of patients with septic shock compared with conventional western medical therapy.
