To observe the clinical efficacy of modified Shangqing powder with channel-guiding herbs in the treatment of acute migraine attacks.
A total of 47 patients with acute migraine attacks who visited the outpatient department of Encephalopathy at Dongzhimen Hospital, Beijing University of Chinese Medicine between July 2023 and June 2024 were selected and divided into Group A and B using a random number table method. In the first phase (P1), Group A received Shangqing powder combined with ibuprofen, while Group B received a placebo combined with ibuprofen. After a washout period, the two groups switched treatments in the second phase (P2). Each patient received both intervention methods sequentially. The effective rate of treatment, VAS scores and accompanying symptom scores before and 2 hours, 24 hours after treatment, and short-term recurrence of migraine within 24 h and 72 h after medication were observed. The possible target points were further explored by comparing the changes of serum indicators before and after treatment.
In the P1, there were significant between-group differences in accompanying symptom scores at 24 hours, VAS scores at 24 hours, and the degree of reduction in VAS scores at 24 hours (P < 0.05). The difference in serum nitric oxide (NO) levels before and after treatment also showed significant between-group differences (P < 0.05, P < 0.01). In the P2, there were no statistically significant differences in VAS scores at 2 hours and 24 hours or accompanying symptom scores between the groups(P > 0.05). Across both phases, the efficacy rate of Shangqing powder at 2 hours post-treatment was 72.20%, significantly higher than the placebo's 34.20% (χ2 =10.716, P =0.001). Significant differences were observed in VAS scores at 24 hours and accompanying symptom scores at 2 hours and 24 hours post-treatment (P <0.05), and the migraine recurrence rate within 72 hours was reduced (P <0.01).
Shangqing powder demonstrates good clinical efficacy in the treatment of acute migraine attacks, providing clinical reference for the intervention of migraines with traditional Chinese medicine.
