Objective:

To investigate the correlation among cytokine levels, traditional Chinese medicine (TCM) syndromes, and clinical efficacy in patients with radiation-induced vaginitis after cervical cancer who were treated with heat-clearing and detoxifying herbal irrigation.

Methods:

A total of 124 patients with radiation-induced vaginitis following cervical cancer admitted to our hospital from February 2023 to May 2025 were enrolled and randomly divided into a control group (n=62, receiving conventional vaginal douching) and an observation group (n=62, receiving heat-clearing and detoxifying herbal irrigation). Curative effect, adverse reactions, cytokine levels, TCM syndrome scores, and the correlations between cytokines and TCM syndromes or efficacy in the observation group were evaluated before and after treatment.

Results:

Following treatment, the observation group's (93.55 percent) overall effective rate was significantly greater than the control group's (80.64 percent) (P<0.05), and both groups' total effective rates exceeded 80%. There was no statistically significant difference between the observation group's (3.22 percent, P>0.05) and control group's (8.05 percent) overall incidence of adverse responses. Post-treatment, cytokine levels declined significantly in both groups (P<0.05), with the observation group experiencing a larger decline (P<0.05). All TCM syndrome scores were improved after treatment (P<0.05), and the improvement was superior in the observation group (P<0.05). In the observation group after treatment, only “profuse leukorrhea” showed a significant negative correlation with IL-6 level (P<0.01), while “yellow color with foul fishy odor” was positively correlated with IL-8 level (P<0.01).

Conclusion:

Heat-clearing and detoxifying herbal irrigation can effectively ameliorate inflammatory symptoms and TCM syndromes in patients with radiation-induced vaginitis after cervical cancer, demonstrating favorable efficacy and safety, and is worthy of clinical application and promotion. The correlation results suggest that its mechanism may involve down-regulating inflammatory cytokines, thereby improving TCM syndromes and ultimately achieving better clinical outcomes.
