Objective

To observe the effect of Astragalus Polysaccharide Injection combined with intensity modulated conformal radiotherapy in the treatment of advanced cervical cancer.

Methods

A total of 82 patients with advanced cervical cancer admitted from February 2021 to December 2023 were included in this study. The patients were divided into two groups by random number table method，namely radiotherapy group and Chinese patent medicine group，with 41 cases in each group. All patients received conventional chemotherapy treatment，while the patients in the radiotherapy group also received intensity-modulated radiotherapy treatment. Those in the Chinese patent medicine group were treated with Astragalus Polysaccharide Injection on the basis of radiotherapy group，and the data of patients in each group was observed and compared，including the efficacy and adverse reactions，the changes of TCM symptom scores（hypoabdominal distention，weakness，shortness of breath，pale complexion，etc.）before and after treatment，squamous cell cancer（SCC），the changes in the levels of tumor markers such as carbohydrate antigen 125（CA125）and carcinoembryonic antigen（CEA），the changes in immune function indicators such as helper T cells，cytotoxic T cells，helper T cells/cytotoxic T cells，and EORTC Quality of Life scale（European Organization for Research and Treatment of Cancer）Cancer Quality of Life Questionnaire，EORTCQLQ-C30）score changes.

Results

The total effective rate in the Chinese patent medicine group was significantly higher than that in the radiotherapy group（P<0.05）. The decrease rate of leukocyte in the Chinese patent medicine group was lower than that in the radiotherapy group（P<0.05）. The incidence of other adverse reactions was similar between the two groups（P>0.05）. The comparison of TCM syndrome scores（hypoabdominal distention，weakness，shortness of breath，pale complexion，etc.），tumor marker levels（CA125 and CEA），helper T cells，cytotoxic T cells，helper T cells/cytotoxic T cells，EORTCQLQ-C30 score and other indicators in both groups before treatment was not significant（P>0.05）. After treatment，TCM syndrome scores（hypoabdominal distention，weakness，shortness of breath，pale complexion，etc.）and tumor marker levels（SCC，CA125 and CEA）were all decreased in both groups，and EORTC QLQ-C30 scores were all increased，and the decrease and improvement degree of indexes in the Chinese patent medicine group were significantly higher than those in the radiotherapy group（P<0.05）. After treatment，helper T cells decreased，cytotoxic T cells increased，and helper T cells/cytotoxic T cells decreased in both groups. The degree of decline and increase of various indexes in the Chinese patent medicine group was lower than that in the radiotherapy group（P<0.05）.

Conclusion

Polysaccharide Injection combined with intensity modulated conformal radiotherapy has a good effect in the treatment of advanced cervical cancer，which can effectively reduce tumor markers，improve disease conditions and enhance immune function and quality of life of patients. The treatment is tolerable and relatively safe and feasible.

目的

观察黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌效果。

方法

研究共计纳入82例中晚期宫颈癌患者，采取随机数字表法将患者分为两组，分别为放疗组及中成药组，每组41例，入组患者均接受常规化疗治疗，放疗组同时接受调强适形放疗治疗，中成药组在放疗组治疗基础上联合黄芪多糖注射液治疗，观察及对比两组数据情况：疗效与不良反应、治疗前后中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）变化及鳞状上皮细胞癌抗原（squamous cell cancer，SCC）、糖类抗原125（carbohydrate antigen 125，CA125）及癌胚抗原（carcinoembryonic antigen，CEA）等肿瘤标志物水平变化、辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞等免疫功能指标变化、EORTC生存质量测定量表（European organization for research and treatment of cancer quality of life questionnaire，EORTC QLQ-C30）评分变化。

结果

中成药组近期治疗总有效率明显高于放疗组（P<0.05）；中成药组白细胞下降率低于放疗组（P<0.05）；两组其他不良反应率发生相当（P>0.05）；两组治疗前中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）、肿瘤标志物水平（CA125及CEA）、辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞、EORTC QLQ-C30评分等指标比较（P>0.05），治疗后两组中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）及肿瘤标志物水平（SCC、CA125及CEA）均下降，EORTC QLQ-C30评分均提升，中成药组指标下降及提升程度明显高于放疗组（P<0.05）；两组治疗后辅助性T细胞下降、细胞毒性T细胞上升，辅助性T细胞/细胞毒性T细胞下降，中成药组治疗后各项指标下降及上升程度均低于放疗组（P<0.05）。

结论

黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌效果良好，可较好降低患者肿瘤标志物，改善患者病症，提升患者免疫功能与生活质量，治疗患者可耐受，较为安全可行。

目的

观察黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌效果。

方法

研究共计纳入82例中晚期宫颈癌患者，采取随机数字表法将患者分为两组，分别为放疗组及中成药组，每组41例，入组患者均接受常规化疗治疗，放疗组同时接受调强适形放疗治疗，中成药组在放疗组治疗基础上联合黄芪多糖注射液治疗，观察及对比两组数据情况：疗效与不良反应、治疗前后中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）变化及鳞状上皮细胞癌抗原（squamous cell cancer，SCC）、糖类抗原125（carbohydrate antigen 125，CA125）及癌胚抗原（carcinoembryonic antigen，CEA）等肿瘤标志物水平变化、辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞等免疫功能指标变化、EORTC生存质量测定量表（European organization for research and treatment of cancer quality of life questionnaire，EORTC QLQ-C30）评分变化。

结果

中成药组近期治疗总有效率明显高于放疗组（P<0.05）；中成药组白细胞下降率低于放疗组（P<0.05）；两组其他不良反应率发生相当（P>0.05）；两组治疗前中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）、肿瘤标志物水平（CA125及CEA）、辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞、EORTC QLQ-C30评分等指标比较（P>0.05），治疗后两组中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）及肿瘤标志物水平（SCC、CA125及CEA）均下降，EORTC QLQ-C30评分均提升，中成药组指标下降及提升程度明显高于放疗组（P<0.05）；两组治疗后辅助性T细胞下降、细胞毒性T细胞上升，辅助性T细胞/细胞毒性T细胞下降，中成药组治疗后各项指标下降及上升程度均低于放疗组（P<0.05）。

结论

黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌效果良好，可较好降低患者肿瘤标志物，改善患者病症，提升患者免疫功能与生活质量，治疗患者可耐受，较为安全可行。

Objective

To observe the effect of Astragalus Polysaccharide Injection combined with intensity modulated conformal radiotherapy in the treatment of advanced cervical cancer.

Methods

A total of 82 patients with advanced cervical cancer admitted from February 2021 to December 2023 were included in this study. The patients were divided into two groups by random number table method，namely radiotherapy group and Chinese patent medicine group，with 41 cases in each group. All patients received conventional chemotherapy treatment，while the patients in the radiotherapy group also received intensity-modulated radiotherapy treatment. Those in the Chinese patent medicine group were treated with Astragalus Polysaccharide Injection on the basis of radiotherapy group，and the data of patients in each group was observed and compared，including the efficacy and adverse reactions，the changes of TCM symptom scores（hypoabdominal distention，weakness，shortness of breath，pale complexion，etc.）before and after treatment，squamous cell cancer（SCC），the changes in the levels of tumor markers such as carbohydrate antigen 125（CA125）and carcinoembryonic antigen（CEA），the changes in immune function indicators such as helper T cells，cytotoxic T cells，helper T cells/cytotoxic T cells，and EORTC Quality of Life scale（European Organization for Research and Treatment of Cancer）Cancer Quality of Life Questionnaire，EORTCQLQ-C30）score changes.

Results

The total effective rate in the Chinese patent medicine group was significantly higher than that in the radiotherapy group（P<0.05）. The decrease rate of leukocyte in the Chinese patent medicine group was lower than that in the radiotherapy group（P<0.05）. The incidence of other adverse reactions was similar between the two groups（P>0.05）. The comparison of TCM syndrome scores（hypoabdominal distention，weakness，shortness of breath，pale complexion，etc.），tumor marker levels（CA125 and CEA），helper T cells，cytotoxic T cells，helper T cells/cytotoxic T cells，EORTCQLQ-C30 score and other indicators in both groups before treatment was not significant（P>0.05）. After treatment，TCM syndrome scores（hypoabdominal distention，weakness，shortness of breath，pale complexion，etc.）and tumor marker levels（SCC，CA125 and CEA）were all decreased in both groups，and EORTC QLQ-C30 scores were all increased，and the decrease and improvement degree of indexes in the Chinese patent medicine group were significantly higher than those in the radiotherapy group（P<0.05）. After treatment，helper T cells decreased，cytotoxic T cells increased，and helper T cells/cytotoxic T cells decreased in both groups. The degree of decline and increase of various indexes in the Chinese patent medicine group was lower than that in the radiotherapy group（P<0.05）.

Conclusion

Polysaccharide Injection combined with intensity modulated conformal radiotherapy has a good effect in the treatment of advanced cervical cancer，which can effectively reduce tumor markers，improve disease conditions and enhance immune function and quality of life of patients. The treatment is tolerable and relatively safe and feasible.

宫颈癌是女性常见恶性肿瘤，多数患者发病、经确诊已发展为疾病中晚期，危及患者生命安全，需积极进行治疗[1-3]。目前，临床治疗中晚期宫颈癌多采取同步放化疗，取得一定效果[4-6]。为了提升中晚期宫颈癌患者治疗效果，给予患者化疗治疗同时，联合调强适形放疗治疗，取得一定结果[7-8]。调强适形放疗可较好杀灭肿瘤，有助于抑制癌细胞的复发及扩散，治疗效果较为显著，临床较为关注[9]。近年，中医药治疗中晚期宫颈癌取得一定结果，相关报道较多[10-11]。黄芪多糖注射液为益气中成药，可较好提升患者免疫功能，治疗中晚期宫颈癌疗效甚佳，该药通过提升机体免疫力来抑制细胞，延长患者生存期及提升患者生活质量，值得深入探讨。故而本次纳入82例中晚期宫颈癌患者进行分组，观察黄芪多糖注射液联合调强适形放疗治疗效果，报道如下。
1　资料与方法

1.1　一般资料
研究共计纳入2021年2月—2023年12月收治的82例中晚期宫颈癌患者，采取随机数字表法将患者分为两组，分别为放疗组及中成药组，每组41例。放疗组41例，年龄38～69岁，平均（51.82±2.73）岁，病理类型：鳞癌29例，腺鳞癌4例，腺癌8例。中成药组41例，年龄36～68岁，平均（51.79±2.80）岁，病理类型：鳞癌27例，腺鳞癌5例，腺癌9例。两组患者临床资料比较，差异无统计学意义（P>0.05）。
1.2　纳入排除标准
纳入标准：确诊为中晚期宫颈癌，中西医诊断参照《妇产科学》与《中药新药临床研究指导原则》[12]；自愿加入研究；可接受本次化疗、黄芪多糖注射液、调强适形放疗治疗；伦理批准研究进行。排除标准：合并其他恶性肿瘤；无法接受本次治疗；合并精神疾病；过敏体质；接受其他研究；心理疾病等。
1.3　治疗方法
入组患者均接受化疗治疗，均采取单药顺铂化疗。静脉滴注顺铂治疗，剂量为每周40 mg/m2，连续化疗3个周期。放疗组：在化疗基础上应用调强适形放疗治疗。根据检查影像学结果明确临床靶区与肿瘤靶区体积，以临床靶区各方向外放5 mm，将其作为计划靶区，放疗的剂量分别为45～50.4 Gy，55～60 Gy，分为25～28次照射，首先进行盆腔外照射，照射剂量30 Gy后开始进行后装放疗，每周1次，每次5～7 Gy，共4次；中成药组：在放疗组治疗基础上联合黄芪多糖注射液（天津赛诺制药有限公司；国药准字Z20040086；规格：250 mg/瓶）治疗。1次/d，250 mg/次，与化疗共同结束治疗。
1.4　观察指标
①观察及对比两组近期临床疗效[13]。患者肿瘤病灶全部消失，持续4周以上为完全缓解；患者肿瘤病灶消失50%及以上，持续4周以上时间为部分缓解；患者肿瘤病灶消失不足50%，或增大不足25%为稳定；患者治疗后未达到以上标准为进展。总有效率＝完全缓解率＋部分缓解率。②观察及对比两组不良反应。③观察及对比两组下腹坠胀、乏力、气短、面色萎黄等中医证候积分，各项积分0、2、4、6分，分别为无症状、轻度、中度与重度[14]。④观察及对比两组治疗前后鳞状上皮细胞癌抗原（squamous cell cancer，SCC）、糖类抗原125（carbohydrate antigen 125，CA125）及癌胚抗原（carcinoembryonic antigen，CEA）等肿瘤标志物水平变化，测定方法为酶联免疫吸附。⑤观察及对比两组治疗前后辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞等免疫功能指标变化，采取流式细胞仪进行测定。⑥观察及对比两组治疗前后EORTC生存质量测定量表（European organization for research and treatment of cancer quality of life questionnaire，EORTC QLQ-C30）评分变化，量表包括情感功能、躯体功能、社会职能、认知功能及角色功能，各项100分，越高表示越佳[15]。
1.5　统计学方法
将两组患者的所有临床数据均录入SPSS 28.0软件中进行处理，两组计量资料（下腹坠胀、乏力、气短、面色萎黄等中医证候积分、SCC、CA125及CEA、辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞、EORTC QLQ-C30评分）和计数资料（疗效和不良反应等）分别进行t/χ2检验，当P<0.05时，说明数据比较差异存在统计学意义。

2.1　近期临床疗效比较
中成药组近期治疗总有效率明显高于放疗组（P<0.05）。见表1。
2.2　治疗不良反应比较
中成药组白细胞下降率低于放疗组（P<0.05）；两组其他不良反应率发生相当（P>0.05）。见表2。
2.3　治疗前后中医证候积分比较
比较两组治疗前中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）（P>0.05），治疗后两组中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）均降低，中成药组治疗后中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）比放疗组更低（P<0.05）。见表3。
2.4　治疗前后肿瘤标志物水平比较
比较两组治疗前肿瘤标志物水平（SCC、CA125及CEA），P>0.05，治疗后两组肿瘤标志物水平（SCC、CA125及CEA）均降低，中成药组治疗后肿瘤标志物水平（SCC、CA125及CEA）比放疗组更低（P<0.05）。见表4。
2.5　治疗前后辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞比较
比较两组治疗前辅助性T细胞、细胞毒性T细胞、辅助性T细胞/细胞毒性T细胞，P>0.05；两组治疗后辅助性T细胞下降、细胞毒性T细胞上升，辅助性T细胞/细胞毒性T细胞下降，中成药组治疗后各项指标下降及上升程度均低于放疗组（P<0.05）。见表5。
2.6　治疗前后EORTC QLQ-C30评分比较
比较两组治疗前EORTC QLQ-C30评分（P>0.05），治疗后两组EORTC QLQ-C30评分均提升，中成药组治疗后EORTC QLQ-C30评分比放疗组更高（P<0.05）。见表6。

中医并无宫颈癌病名，将其纳入“带下”“崩漏”“癥瘕”“虚劳”等范畴[20-23]。中医对该病早有记载。《千金要方》曰：“妇人崩中漏下，赤白青黑，腐臭不可近……所下之物，一曰状如膏，二曰如黑血，三曰如紫汁，四曰如赤肉，五曰如脓血”。其病变在子宫颈，属于正虚邪实之证[24-25]。患者正气虚损引发肿瘤，而放化疗加重机体气血损耗，气血津液不足，正气加重，损伤脏腑，脾肺等功能失常，故而治疗当以益气健脾、补虚之法[26]。黄芪多糖注射液属于中成药，由黄芪提炼而来，具有较好益气升阳、利水消肿、补益脾肺及托毒生肌等功效[27-30]。现代药理指出，黄芪多糖注射液中黄芪多糖可增强细胞活性，提升机体免疫功能，进而达到抗肿瘤目的[31-33]。
调强适形放疗治疗中晚期宫颈癌取得较好临床效果，临床较为重视[34]。调强适形放疗为调整入射角度及增加射野数量、改变射野权重，提升肿瘤病灶局部照射的剂量，以杀灭肿瘤细胞，最大限度保护正常组织，避免损伤正常组织，达到治疗目的，与黄芪多糖注射液联合可较好提升中晚期宫颈癌治疗效果[35]。本次数据显示，中成药组患者近期治疗总有效率明显高于放疗组患者，黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌效果良好，分析原因为在放疗治疗基础上，联合调强适形放疗可更好地杀灭肿瘤细胞，并联合黄芪多糖注射液，增强机体免疫力，进一步抗肿瘤，达到治疗目的。中成药组患者白细胞下降率低于放疗组，中医证候积分（下腹坠胀、乏力、气短、面色萎黄等）低于放疗组患者；两组其他不良反应率发生相当，提示中晚期宫颈癌患者采取化疗与放疗治疗多存在脱发、胃肠道反应、肝肾功能损伤、白细胞下降、放射性皮炎等不良反应，而黄芪多糖注射液可提升机体免疫力，故而可较好降低患者白细胞下降发生率。SCC、CA125及CEA为临床常用肿瘤标志物，中晚期宫颈癌患者机体内SCC、CA125及CEA水平显著上升，检测患者机体SCC、CA125及CEA水平变化可了解患者病情变化。中成药组患者治疗后的SCC、CA125及CEA均低于放疗组，提示黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌可较好降低机体肿瘤标志物，抗肿瘤作用良好。本次治疗后数据显示，两组患者治疗后辅助性T细胞下降、细胞毒性T细胞上升，辅助性T细胞/细胞毒性T细胞下降，中成药组患者治疗后各项指标下降及上升程度均低于放疗组，提示本次治疗可提升中晚期宫颈癌患者免疫功能。分析原因，中晚期宫颈癌患者应用放化疗治疗损伤机体免疫功能，辅助性T细胞下降、细胞毒性T细胞上升，辅助性T细胞/细胞毒性T细胞下降提示免疫功能降低，而黄芪多糖注射液可显著增强机体免疫功能，故而可改善中成药组患者免疫指标。治疗后，中成药组患者EORTCQLQ-C30评分均高于放疗组，提示黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌可较好提升患者生活质量，分析原因为，黄芪多糖注射液使得患者免疫功能提升，临床疗效提升，部分不良反应下降等有关。
综上所述，黄芪多糖注射液联合调强适形放疗治疗中晚期宫颈癌效果良好，可较好改善患者症状及降低肿瘤标志物，提升患者免疫功能和生活质量，患者可耐受其不良反应，治疗可行。
