Objective

To evaluate the application of Guizhi Longgu Muli Decoction（桂枝龙骨牡蛎汤）combined with repetitive transcranial magnetic stimulation in chronic obstructive pulmonary disease（COPD）with anxiety and depression.

Method

A prospective study was conducted to select 98 COPD patients with anxiety and depression who received a systematic diagnosis and treatment plan，and to cooperate in completing the follow-up process. According to the random number table method，the patients were divided into two groups，with 49 cases in each group. The control group received repeated transcranial magnetic stimulation and continued to take it for 8 weeks. The observation group was given Guizhi Longgu Muli Decoction combined with repeated transcranial magnetic stimulation for 8 weeks. The clinical efficacy and adverse reactions were evaluated. The levels of interleukin-1β（IL-1β），interleukin-6（IL-6），C-reactive protein（CRP），self rating anxiety scale（SAS），self rating depression scale（SDS）before and after 8 weeks of treatment were compared.

Results

The clinical efficacy of the control group was lower than that of the observation group，and the difference was statistically significant（P<0.05）. The levels of serum IL-1β，IL-6 and CRP in the control group after 8 weeks of treatment in the observation group［（32.81±5.80）ng/mL，（20.15±3.56）ng/mL，（8.46±2.15）mg/L］were higher than those in the observation group［（28.63±5.47）ng/mL，（14.08±3.12）ng/mL，（5.13±1.86）mg/L］，with statistical significance（P<0.05）. After 8 weeks of treatment，the SAS score［（49.81±1.63）points］and SDSscore［（52.10±1.22）points］in the control group were higher than those in the observation group［（48.42±1.56）points］and［（50.06±1.16）points］，with statistical significance（P<0.05）. There was no statistically significant difference in total adverse reactions between the two groups（P>0.05）.

Conclusion

Guizhi Longgu Muli Decoction combined with repeated transcranial magnetic stimulation can improve the symptoms of anxiety and depression in COPD patients with anxiety and depression，reduce inflammatory reaction，and has high safety.

目的

评估桂枝龙骨牡蛎汤联合重复经颅磁刺激在慢性阻塞性肺疾病（Chronic obstructive pulmonary disease，COPD）合并焦虑抑郁症中的应用。

方法

采用前瞻性研究方式，选取COPD合并焦虑抑郁症患者98例。按照随机数字表分组，每组49例。对照组予以重复经颅磁刺激，持续8周；观察组予以桂枝龙骨牡蛎汤联合重复经颅磁刺激，持续8周。评价疗效和不良反应，治疗前和治疗8周评定白介素-1β（Interleukin-1β，IL-1β）、白介素-6（Interleukin-6，IL-6）、C反应蛋白（C-reactive protein，CRP）和焦虑自评量表（Self-rating anxiety scale，SAS）、抑郁自评量表（Self-rating depression scale，SDS），相互比较。

结果

对照组的临床疗效低于观察组，差异有统计学意义（P<0.05）。治疗8周对照组血清IL-1β［（32.81±5.80）ng/mL］、IL-6［（20.15±3.56）ng/mL］、CRP［（8.46±2.15）mg/mL］高于观察组［（28.63±5.47）ng/mL、（14.08±3.12）ng/mL、（5.13±1.86）ng/L］，差异有统计学意义（P<0.05）。治疗8周对照组SAS评分［（49.81±1.63）分］、SDS评分［（52.10±1.22）分］高于观察组［（48.42±1.56）分、（50.06±1.16）分］，差异有统计学意义（P<0.05）。两组总不良反应比较，差异无统计学意义（P>0.05）。

结论

桂枝龙骨牡蛎汤联合重复经颅磁刺激应用于COPD合并焦虑抑郁症，可以改善焦虑、抑郁症状，减轻炎症反应，并且安全性高。

目的

评估桂枝龙骨牡蛎汤联合重复经颅磁刺激在慢性阻塞性肺疾病（Chronic obstructive pulmonary disease，COPD）合并焦虑抑郁症中的应用。

方法

采用前瞻性研究方式，选取COPD合并焦虑抑郁症患者98例。按照随机数字表分组，每组49例。对照组予以重复经颅磁刺激，持续8周；观察组予以桂枝龙骨牡蛎汤联合重复经颅磁刺激，持续8周。评价疗效和不良反应，治疗前和治疗8周评定白介素-1β（Interleukin-1β，IL-1β）、白介素-6（Interleukin-6，IL-6）、C反应蛋白（C-reactive protein，CRP）和焦虑自评量表（Self-rating anxiety scale，SAS）、抑郁自评量表（Self-rating depression scale，SDS），相互比较。

结果

对照组的临床疗效低于观察组，差异有统计学意义（P<0.05）。治疗8周对照组血清IL-1β［（32.81±5.80）ng/mL］、IL-6［（20.15±3.56）ng/mL］、CRP［（8.46±2.15）mg/mL］高于观察组［（28.63±5.47）ng/mL、（14.08±3.12）ng/mL、（5.13±1.86）ng/L］，差异有统计学意义（P<0.05）。治疗8周对照组SAS评分［（49.81±1.63）分］、SDS评分［（52.10±1.22）分］高于观察组［（48.42±1.56）分、（50.06±1.16）分］，差异有统计学意义（P<0.05）。两组总不良反应比较，差异无统计学意义（P>0.05）。

结论

桂枝龙骨牡蛎汤联合重复经颅磁刺激应用于COPD合并焦虑抑郁症，可以改善焦虑、抑郁症状，减轻炎症反应，并且安全性高。

Objective

To evaluate the application of Guizhi Longgu Muli Decoction（桂枝龙骨牡蛎汤）combined with repetitive transcranial magnetic stimulation in chronic obstructive pulmonary disease（COPD）with anxiety and depression.

Method

A prospective study was conducted to select 98 COPD patients with anxiety and depression who received a systematic diagnosis and treatment plan，and to cooperate in completing the follow-up process. According to the random number table method，the patients were divided into two groups，with 49 cases in each group. The control group received repeated transcranial magnetic stimulation and continued to take it for 8 weeks. The observation group was given Guizhi Longgu Muli Decoction combined with repeated transcranial magnetic stimulation for 8 weeks. The clinical efficacy and adverse reactions were evaluated. The levels of interleukin-1β（IL-1β），interleukin-6（IL-6），C-reactive protein（CRP），self rating anxiety scale（SAS），self rating depression scale（SDS）before and after 8 weeks of treatment were compared.

Results

The clinical efficacy of the control group was lower than that of the observation group，and the difference was statistically significant（P<0.05）. The levels of serum IL-1β，IL-6 and CRP in the control group after 8 weeks of treatment in the observation group［（32.81±5.80）ng/mL，（20.15±3.56）ng/mL，（8.46±2.15）mg/L］were higher than those in the observation group［（28.63±5.47）ng/mL，（14.08±3.12）ng/mL，（5.13±1.86）mg/L］，with statistical significance（P<0.05）. After 8 weeks of treatment，the SAS score［（49.81±1.63）points］and SDSscore［（52.10±1.22）points］in the control group were higher than those in the observation group［（48.42±1.56）points］and［（50.06±1.16）points］，with statistical significance（P<0.05）. There was no statistically significant difference in total adverse reactions between the two groups（P>0.05）.

Conclusion

Guizhi Longgu Muli Decoction combined with repeated transcranial magnetic stimulation can improve the symptoms of anxiety and depression in COPD patients with anxiety and depression，reduce inflammatory reaction，and has high safety.

慢性阻塞性肺疾病（Chronic obstructive pulmonary disease，COPD）是一种气道炎症性疾病，随着病程延长，肺功能持续下降，直至出现不可逆表现。由于COPD的病程较长，即使控制得当，也会造成一些身心损害。有报道发现[1]，COPD患者普遍存在情志淡漠、活动减少或烦躁易怒、易激惹等不良情绪。医学界认为对于COPD合并焦虑抑郁症患者，通过及时有效地处理，对于病情控制起到积极作用[2]。重复经颅磁刺激是一类非药物疗法，与药物治疗比，具有无创、操作简单以及安全性高的特点，通过促使神经元代谢、激发脑功能区等机制，对情感有正向调节作用。有一些研究证实重复经颅磁刺激对于抑郁恢复有益，为焦虑抑郁症患者提供了更多选择[3]。随着现代中医的发展，对于COPD合并症有了更深入地认识，现代中医认为COPD合并情绪疾病多为肺气虚与脏腑阴阳紊乱所致[4]。《素问·阴阳应象大论篇》云：“人有五脏化五气，以生喜怒悲忧恐”，患者病久，脏腑阴阳紊乱，容易变生情志之病。桂枝加龙骨牡蛎汤出自汉·张仲景《金匮要略》，具有调和阴阳、重镇安神之功，临床上常应用于情志疾病[5]。为进一步丰富治疗方案，本次选择COPD合并焦虑抑郁症患者，评估桂枝龙骨牡蛎汤联合重复经颅磁刺激的应用效果。现报道如下。
1　资料与方法

1.1　临床资料
采用前瞻性研究方式，选取2022年1月—2023年4月接受系统诊疗方案的COPD合并焦虑抑郁症患者。有2例未完成检查和治疗，有1例失随，最终98例COPD合并焦虑抑郁症进入对照试验，并配合完成随访过程。调查患者的资料，包括54例男性和44例女性；年龄48～73岁，平均（60.18±6.27）岁；COPD病程7～19年，平均（12.53±3.58）年；焦虑自评量表（Self-rating anxiety scale，SAS）：52～57分，平均（54.87±2.05）分；抑郁自评量表（Self-rating depression scale，SDS）：55～60分，平均（57.41±1.56）分。按照随机数字表分组，每组49例。研究经医学伦理委员部门的审查，经分析两组之间各项数据分析，具有可比价值（P>0.05）。见表1。
1.2　诊断标准
1.2.1　西医诊断
参照《慢性阻塞性肺疾病诊治指南（2021年修订版）》[6]中标准，有明确的高危因素，伴有呼吸困难、慢性咳嗽或咳痰等表现，吸入支气管舒张剂后第1秒用力呼气容积/用力肺活量<70%，除外其他疾病；当SAS评分>50分和SDS评分>53分，即可确诊焦虑抑郁症。
1.2.2　中医诊断
由我院高级职称中医师进行四诊，参照《中药新药临床指导原则（试行）》[7]和《中医病证诊断疗效标准》[8]，主证：情绪淡漠，精神不振或烦躁易怒；次证：咳嗽不止、咳痰、喘息气促、腹胀纳呆、乏力、胸闷、神疲懒言、不寐、心悸头晕。舌红苔黄，脉沉细或沉缓，反符合主证2项或次证4项以上，即可确诊。
1.3　纳入标准
1）具备上述中西医诊断标准，年龄>18岁，未接受任何用药和干预；2）干预前与患者签订知情同意书，完全了解本次研究内容，具备配合能力；3）处于病情稳定阶段，生命体征平稳、意识清楚、认知能力正常。
1.4　排除标准
1）有其他肺部原发疾病、外伤、气胸、重症肺炎，或其他心肺功能不全、占位性病变、器官移植；2）有原发精神障碍、神经官能症、绝经期前后诸证以及先天发育异常；3）过敏体质，对中药成分禁忌；4）癫痫大发作、阿尔茨海默病、帕金森等无法配合；5）妊娠或哺乳期；6）干预前1个月服用抗精神药、镇静药。
1.5　治疗方法
参照“COPD”诊治原则，改善生活方式和饮食习惯，戒烟限酒，予以止咳化痰、平喘、抗感染等对症处理，配合支气管扩张剂、糖皮质激素等，积极控制病情；有呼吸困难或心慌者，予以低流量吸氧，调节水及电解质紊乱。服用氟哌噻吨美利曲辛片，1片/次口服，1次/d。
1.5.1　对照组
予以重复经颅磁刺激。参照《重复经颅磁刺激治疗专家共识》[9]中规范，选用我院经颅磁刺激仪，设置刺激频率10 Hz、100%运动阈值，以右侧额叶背外侧为刺激区域，刺激总量10脉冲/串，间隔10s，重复1000次/d，1次/d，每周4次，持续8周。
1.5.2　观察组
予以桂枝龙骨牡蛎汤联合重复经颅磁刺激，重复经颅磁刺激与对照组相同。桂枝龙骨牡蛎汤药用：桂枝15 g，芍药20 g，生姜6片，甘草10 g，大枣12枚，龙骨30 g，牡蛎30 g，随证加减：气虚重者，加黄芪、太子参；咳喘重者，加款冬花、白前；痰多者，加紫菀、法半夏、香橼。使用自动煎药机煎煮上述中药，煎取400 mL，分早晚2次服用，每次服用200 mL，1剂/d，持续8周。
1.6　观察指标
1.6.1　临床疗效
评价患者疗效，记录各项症状的变化，计算中医证候减分率＝（治疗前-治疗后）/治疗前×100%，进行疗效分级。基本治愈：各项焦虑抑郁症状消失，病情稳定，中医证候减分率>95%，生活质量不受影响。显效：各项焦虑抑郁症状显著改善，病情较为稳定，中医证候减分率>75%，生活质量轻微影响。有效：各项焦虑抑郁症状好转，病情一般，中医证候减分率>50%，生活质量有影响。无效：各项焦虑抑郁症状未好转，或恶化，中医证候减分率≤50%。统计基本治愈、显效、有效人数占比。
1.6.2　炎症因子指标
治疗前和治疗8周抽取肘静脉血2～4 mL，送入检验科室，取上层清液，上机检测。取适量的样本，应用酶联免疫检测法测定血清白介素-1β（Interleukin-1β，IL-1β）、白介素-6（Interleukin-6，IL-6）、C反应蛋白（Creactive protein，CRP）水平，试剂盒为美国BD公司提供。
1.6.3　焦虑抑郁状态
治疗前和治疗8周测定焦虑和抑郁状态，包括SAS和SDS。SAS评分和SDS评分由20个项目组成，采用0～4分，换算成百分制，由专业人员辅助评定，不加以任何外界干预。其分值与焦虑抑郁状态呈正相关，分值高提示焦虑和抑郁越重。
1.6.4　不良反应
对患者随访，必要时进行系统检查，统计两组不良反应，包括恶心呕吐、头晕、发热、皮疹、肝肾功能等。
1.7　统计学方法
使用SPSS 20.0软件分析。计量资料（炎症因子指标和焦虑抑郁状态）使用t检验；计数资料（临床疗效和不良反应）使用卡方检验。以P<0.05为差异有统计学意义。

全球范围内COPD呈多发、年轻化的态势，有报道COPD所致住院死亡居于总病死人数的第3位，并且长期服药、不定时急性发作、不确定的住院风险等，也给患者带来沉重的医疗负担。临床上COPD除了器质性病变外，还会带来一些肺外效应，如焦虑、抑郁等情志问题，对于身体健康的影响不容忽视[10]。有学者分析COPD急性发作的诱发因素，其中精神因素和心理原因起到重要的作用，一旦出现COPD合并焦虑抑郁，提示预后欠佳。研究发现[11]，7.0% ～79.1%的COPD合并抑郁症，远高于非COPD人群，其中多数为重度抑郁，需要引起足够重视。目前神经电生理技术不断发展，重复经颅磁刺激对精神障碍、不良情绪等情志疾病有积极作用，体现出独特优势[12]。然而单一疗法也存在限制，仍存在一部分疗效欠佳的病例。本次研究发现：重复经颅磁刺激治疗后SAS评分和SDS评分下降，表明焦虑抑郁症状得到改善，但总有效率仅为83.67%，总体改善效果欠佳，尚存在一些需要持续改进的空间。
目前中西医结合成为临床诊疗的主流思路，可以发挥多靶点、多方位的治疗作用。对于一些脏腑合并症或情志类疾病、疑难杂症，中西医结合治疗更具明显优势。有研究证实中西医结合对于肺病合并情绪障碍的效果[13-14]。中医古籍中早就指出了情志因素与肺系疾病的关联。根据病变特点，将焦虑归于“脏燥”范畴，抑郁症归于“郁证”范畴，现代医学统称为不良情绪，总结其病机为气机郁滞不行。COPD归于“肺胀”“喘证”范畴，患病日久则肺气不行，脏腑衰败，阴阳紊乱[15]。《素问·至真要大论篇》言：“诸气愤郁，皆属于肺”，指出了肺病与焦虑抑郁症的关联。五志皆为五脏气机运动的外在表现，《素问·阴阳应象大论篇》曰：“肺在志为忧”，COPD患者久病必虚，肺气不足，脏腑精气衰败，表现为过度悲伤或忧思[16]。《素问·举痛论篇》言：“悲则气消”，情志因素对于气机运行造成一定不良影响，因此从中医角度来看，COPD与情志因素（焦虑抑郁症）相互关联，形成一种恶性循环。现代中医对于COPD合并抑郁进行了专门论述，从“调节阴阳、重镇安神”来探讨COPD合并焦虑抑郁症的病机特点，为临床治疗提供了思路[17]。
本次采用桂枝龙骨牡蛎汤，由桂枝汤加龙骨、牡蛎组成。桂枝汤被誉为“群方之祖”，为调和营卫、调节阴阳的经方之一，其证对外感风寒表虚证，后世医家进一步延伸，临床应用可以此为基础方加减[18]。闫德祺等[19]发现，桂枝加龙骨牡蛎汤对肺不藏魄大鼠，可以调节睡眠、改善神经递质。本次全方中桂枝性味辛温，主解肌、温经通脉、助阳化气。有药理研究发现桂枝对大鼠中枢神经系统有镇静和抗焦虑作用[20]。芍药性味酸寒，养阴、缓中止痛，《神农本草经》言：“主邪气腹痛，除血痹，破坚积”，夏小雯等[21]发现，芍药苷具有抗抑郁的作用，与调节单胺类神经递质、保护神经细胞、抗氧化等有关。大枣性味甘温，补脾益胃、调营卫，《别录》中言：“补中益气，强力，除烦闷……”甘草性味甘平，调和脾胃，《日华子本草》曰“安魂定魄。补五劳七伤，一切虚损、惊悸、烦闷、健忘”。有一些研究认为甘草中的有效成分具有抗抑郁的作用[22-23]。生姜性味辛温，散寒止呕，《神农本草经》曰：“去臭气，通神明”。杨淑等[24]认为生姜对COPD大鼠有抗炎症和改善肺功能的作用。龙骨和牡蛎皆为重镇安神、平肝潜阳之品[25]。张锡纯于《医学衷中参西录》中言：“龙骨入肝以安魂，牡蛎入肺以定魄，魂魄者心神之左辅右弼也”。王秀峰等[26]发现重镇安神中药对于失眠大鼠有调节神经递质的作用，龙骨和牡蛎以降低兴奋性神经递质为主。上述主要合用一则调和营卫、调节阴阳，一则重镇降逆、安神，证对本病的病机特点。
本次结果显示：桂枝龙骨牡蛎汤联合重复经颅磁刺激的总有效率为95.92%，高于对照组（83.67%），并且SAS评分和SDS评分有了明显改善，证实了二者联合应用效果。从本质上来说，COPD为一类慢性气道炎症，多种细胞因子及炎症介质介导且相互作用，当COPD急性加重时，炎症反应呈进行性加重[27]。龙莉等[28]分析COPD合并抑郁与炎症因子的相关性，认为其发生发展均与炎症因子相关。IL-1β、IL-6均为一类白介素炎症物质，闫景霞等[29]发现，与健康人比较，COPD合并抑郁患者血清IL-1β、IL-6水平明显升高，并且与抑郁程度密切相关。血清CRP是一类广谱的炎症因子，当有任何的炎症刺激时，CRP可以进行性升高，呈高表达状态，通过监测血清CRP水平的波动，有助于判断病情变化和预后情况[30]。本次研究以炎症因子作为观察指标，结果显示：两组炎症因子指标均下降，但与对照组比较，治疗8周观察组血清IL-1β、IL-6、CRP水平较低，表明桂枝龙骨牡蛎汤联合重复经颅磁刺激对炎症因子的改善更为明显，效果更好。
综上可见，桂枝龙骨牡蛎汤联合重复经颅磁刺激应用于COPD合并焦虑抑郁症，可以改善焦虑、抑郁症状，减轻炎症反应，并且安全性高。由于本次研究有限，关于二者联用的具体机制和远期效果，仍需要进一步分析。
