Objective

To explore the therapeutic effect of Bufei Xichuan Decoction（补肺息喘汤）on stable stage chronic obstructive pulmonary disease（SCOPD）patients and its impact on the levels of inflammatory factors，Toll like receptor 4（TLR4）and chemokine CXC ligand 13（CXCL13）in patients.

Method

A total of 160 SCOPD patients from April 2022 to April 2024 in the hospital were divided into observation group and control group，each with 80 cases. The patients of the control group inhaled indatelopram bromide，while those of the observation group took Bufei Xichuan Decoction on the basis of the control group. The treatment period for both groups was 8 weeks. The therapeutic effects of two groups was compared. The changes in lung function，6-minute walking distance（6MWT），COPD assessment test（CAT）questionnaire scores，inflammatory factors，TLR4 and CXCL13 before and after treatment were compared.

Result

The effective rate of the observation group was higher than that of the control group（P<0.05）. After 8 weeks of treatment，the values of FEV1/FVC，PEF，FEV1 and 6MWT of the two groups were higher than those before treatment，while the CAT score，the levels of tumor necrosis factor-α（TNF-α），interleukin-6（IL-6），interleukin-8（IL-8），TLR4 and CXCL13 were lower than those before treatment（P<0.05）. The values of FEV1/FVC，PEF，FEV1 and 6MWT of the observation group were higher than those of the control group，while the CAT score，the levels of TNF-α，IL-6，IL-8，TLR4 and CXCL13 were lower than those of the control group after treatment（P<0.05）.

Conclusion

-Bufei Xichuan Decoction has a significant therapeutic effect on SCOPD patients，reducing cellular inflammatory response and lowering the levels of TLR4 and CXCL13，which is worthy of clinical reference.

目的

探讨补肺息喘汤对慢性阻塞性肺疾病稳定期（SCOPD）患者疗效及对炎症因子、TLR4和CXCL13水平影响。

方法

选择医院于2022年4月—2024年4月SCOPD患者160例，将其随机分为观察组80例与对照组80例。对照组吸入茚达特罗格隆溴铵，观察组在对照组基础上服用补肺息喘汤。两组治疗周期8周。比较两组治疗疗效；治疗前后肺功能，6MWT和CAT问卷评分，炎症因子、TLR4和CXCL13水平变化。

结果

观察组总有效率高于对照组（P<0.05）。两组治疗8周SCOPD患者FEV1/FVC、PEF、FEV1和6MWT高于治疗前，而CAT评分、TNF-α、IL-6、IL-8、TLR4和CXCL13水平低于治疗前（P<0.05）；观察组治疗8周SCOPD患者FEV1/FVC、PEF、FEV1和6MWT高于对照组，而CAT评分、TNF-α、IL-6、IL-8、TLR4和CXCL13水平低于对照组（P<0.05）。

结论

补肺息喘汤对SCOPD患者治疗疗效显著，可减轻细胞炎症反应，及降低TLR4和CXCL13水平，值得临床借鉴。

目的

探讨补肺息喘汤对慢性阻塞性肺疾病稳定期（SCOPD）患者疗效及对炎症因子、TLR4和CXCL13水平影响。

方法

选择医院于2022年4月—2024年4月SCOPD患者160例，将其随机分为观察组80例与对照组80例。对照组吸入茚达特罗格隆溴铵，观察组在对照组基础上服用补肺息喘汤。两组治疗周期8周。比较两组治疗疗效；治疗前后肺功能，6MWT和CAT问卷评分，炎症因子、TLR4和CXCL13水平变化。

结果

观察组总有效率高于对照组（P<0.05）。两组治疗8周SCOPD患者FEV1/FVC、PEF、FEV1和6MWT高于治疗前，而CAT评分、TNF-α、IL-6、IL-8、TLR4和CXCL13水平低于治疗前（P<0.05）；观察组治疗8周SCOPD患者FEV1/FVC、PEF、FEV1和6MWT高于对照组，而CAT评分、TNF-α、IL-6、IL-8、TLR4和CXCL13水平低于对照组（P<0.05）。

结论

补肺息喘汤对SCOPD患者治疗疗效显著，可减轻细胞炎症反应，及降低TLR4和CXCL13水平，值得临床借鉴。

Objective

To explore the therapeutic effect of Bufei Xichuan Decoction（补肺息喘汤）on stable stage chronic obstructive pulmonary disease（SCOPD）patients and its impact on the levels of inflammatory factors，Toll like receptor 4（TLR4）and chemokine CXC ligand 13（CXCL13）in patients.

A total of 160 SCOPD patients from April 2022 to April 2024 in the hospital were divided into observation group and control group，each with 80 cases. The patients of the control group inhaled indatelopram bromide，while those of the observation group took Bufei Xichuan Decoction on the basis of the control group. The treatment period for both groups was 8 weeks. The therapeutic effects of two groups was compared. The changes in lung function，6-minute walking distance（6MWT），COPD assessment test（CAT）questionnaire scores，inflammatory factors，TLR4 and CXCL13 before and after treatment were compared.

Result

The effective rate of the observation group was higher than that of the control group（P<0.05）. After 8 weeks of treatment，the values of FEV1/FVC，PEF，FEV1 and 6MWT of the two groups were higher than those before treatment，while the CAT score，the levels of tumor necrosis factor-α（TNF-α），interleukin-6（IL-6），interleukin-8（IL-8），TLR4 and CXCL13 were lower than those before treatment（P<0.05）. The values of FEV1/FVC，PEF，FEV1 and 6MWT of the observation group were higher than those of the control group，while the CAT score，the levels of TNF-α，IL-6，IL-8，TLR4 and CXCL13 were lower than those of the control group after treatment（P<0.05）.

Conclusion

-Bufei Xichuan Decoction has a significant therapeutic effect on SCOPD patients，reducing cellular inflammatory response and lowering the levels of TLR4 and CXCL13，which is worthy of clinical reference.

慢性阻塞性肺疾病（COPD）是呼吸系统常见的一种疾病，调查显示其患病率呈增长趋势，导致患者身心健康受到严重影响[1-2]。COPD不仅病程迁延，病情复杂，并且呈进行性加重[3-4]。针对COPD患者采取及时有效的治疗方法具有重要意义。现阶段，临床上关于COPD尚无特效治疗方法，西医治疗以缓解临床症状为主，但其对延缓肺功能并不十分理想[5-6]。随着近年来中医药的深入研究，通过辨证论治用于COPD患者治疗不仅可有效缓解临床症状，并且可发挥抗炎及减缓疾病进展等优势[7]。本研究选择我院于2022年4月—2024年4月SCOPD患者160例，以期探讨补肺息喘汤对慢性阻塞性肺疾病稳定期（SCOPD）患者疗效及对炎症因子、TLR4和CXCL13水平影响。
1　资料与方法

1.1　诊断标准
西医诊断：符合《慢性阻塞性肺疾病诊治指南》[8]标准。
中医辨证[9]：辨证属肺脾肾虚证。（1）主症：咳嗽，喘息气短，神疲乏力，食少腹胀，腰膝酸软；（2）次症：咯痰不爽，胸闷，口干咽干，面目浮肿，夜尿多，耳鸣盗汗；（3）舌脉：舌淡胖大有齿痕，苔白，脉细缓弱。
1.2　一般资料
选择我院于2022年4月—2024年4月SCOPD患者160例，将其随机分为观察组80例与对照组80例。观察组中，男54例，女26例；年龄（60.86±6.52）岁，最小42岁，最大74岁；病程（9.84±2.13）年，最短2年，最长16年。对照组中，男56例，女24例；年龄（60.37±7.03）岁，最小41岁，最大72岁；病程（9.61±1.82）年，最短3年，最长17年。两组一般资料无统计意义（P>0.05）。
1.3　纳入标准
（1）符合SCOPD标准；（2）中医辨证属肺脾肾虚证；（3）临床资料完整；（4）签署知情同意书。
1.4　排除标准
（1）COPD急性期；（2）伴其他呼吸系统疾病；（3）精神疾病者；（4）伴重要脏器严重异常者；（5）依从性差；（6）本研究方案过敏者。
1.5　治疗方法
对照组：给予茚达特罗格隆溴铵，每次吸入1粒，每天吸入1次；观察组：在对照组基础上服用补肺息喘汤，处方：炙黄芪30 g，太子参20 g，白术15 g，党参20 g，红景天15 g，菟丝子15 g，茯苓15 g，穿山龙15 g，巴戟天15 g，川厚朴20 g，首乌藤20 g，川芎10 g，紫苏子10 g，桔梗10 g，郁金10 g，石菖蒲10 g，取上述诸药，水煎取汁450 mL，150 mL/次，每日3次温服。两组治疗周期8周。
1.6　疗效判定标准
（1）显效：患者证候积分减少>70%；（2）有效：患者证候积分减少30%～70%；（3）无效：患者证候积分减少<30%。
1.7　观察指标
（1）比较两组肺功能变化，包括FEV1/FVC、PEF和FEV1。（2）比较两组6 min步行距离（6MWT）和COPD评估测试（CAT）问卷评分，其中6MWT试验测定患者6 min内在硬地、平台地上行走的距离；CAT评分内容包括精神、睡眠、气喘及咳嗽等情况，评分0～40分，生存质量与CAT评分负相关。（3）比较两组炎症因子变化，收集肘静脉血5 mL，离心10 min，取上清液，ELISA法测定TNF-α、IL-6和IL-8水平。（4）比较两组CXCL13和TLR4水平变化，收集外周静脉血5 mL，离心10 min，取上清液，ELISA法测定CXCL13和TLR4水平。
1.8　统计学方法
运用SPSS 26.0处理。计数资料（疗效）用例（%）表示，行χ2检验；计量资料（肺功能、6MWT、CAT评分、炎症因子、CXCL13和TLR4）用（）表示，行t检验。P<0.05差异有统计学意义。
2　结果

2.1　两组疗效比较
观察组总有效率高于对照组（P<0.05）。见表1。
2.2　两组SCOPD患者肺功能比较
两组治疗8周SCOPD患者FEV1/FVC、PEF和FEV1高于治疗前（P<0.05）；观察组治疗8周SCOPD患者FEV1/FVC、PEF和FEV1高于对照组（P<0.05）。见表2。
2.3　两组SCOPD患者6MWT和CAT评分比较
两组治疗8周SCOPD患者6MWT高于治疗前而CAT评分低于治疗前（P<0.05）；观察组治疗8周SCOPD患者6MWT高于对照组，而CAT评分低于对照组（P<0.05）。见表3。
2.4　两组SCOPD患者炎症因子水平比较
两组治疗8周SCOPD患者血清TNF-α、IL-6和IL-8水平低于治疗前（P<0.05）；且观察组低于对照组（P<0.05）。见表4。
2.5　两组SCOPD患者CXCL13和TLR4水平比较
两组治疗8周SCOPD患者血清TLR4和CXCL13水平低于治疗前（P<0.05）；且观察组低于对照组（P<0.05）。见表5。
3　讨论

COPD特点主要是气流在气道中持续受阻，并且疾病伴有呼吸症状的发展，当肺泡、支气管以及气管在接触过多的刺激性粉尘及气体，通常会诱发此病发生[10-12]。关于COPD诸多问题尚未被攻克，并且在相关治疗进展上仍待进一步研究，从而给患者预防及治疗造成巨大挑战。目前，西医关于COPD优势在于急性加重期的治疗，但关于SCOPD患者仍缺乏有效的治疗手段。
中医学认为COPD属“喘证”“久咳”等范畴。中医理论认为COPD病变部位首先在肺，继而会导致脾、肾受影响[13-14]。COPD发病与肺、脾、肾关系紧密。肺虚气失致反复咳嗽、气喘，使喘息、气短加重；肺病累脾，脾失健运，则会造成痰饮内生，而使肺脾两虚；肺为气之主，肾为气之根，若久病肺虚及肾，肺不主气，肾气衰惫，肾虚则不纳气，而造成呼多吸少，气短不足为息，动则喘促尤甚[15]。因此，应以补肺健脾益肾为治疗法则。本文应用补肺息喘汤中，炙黄芪具有补气健脾之功；太子参具有润肺益气健脾之功；白术和党参具有益气健脾之功；红景天具有止咳平喘，补气清肺之功；菟丝子具有温补肾阳之功；茯苓具有健脾利水渗湿之功；穿山龙具有止咳平喘祛痰之功；巴戟天具有补肾阳之功；川厚朴具有行气降逆平喘之功；首乌藤具有祛风通络之功；川芎具有活血行气之功；紫苏子具有开宣肺气之功；桔梗具有宣肺利咽，祛痰之功；郁金具有活血行气解郁之功；石菖蒲具有豁痰开窍之功。纵观全方可奏补肺健脾益肾，行气活血之功。现代药理研究表明，补肺息喘汤具有抗炎，增强机体免疫功能，止咳，平喘，祛痰之功[16]。本研究结果显示，观察组总有效率高于对照组，FEV1/FVC、PEF和FEV1高于对照组，6MWT高于对照组而CAT评分低于对照组，由此表明补肺息喘汤可使治疗疗效显著提高，肺功能显著改善，运动耐力显著提高，生活质量显著提高。
炎症因子与COPD发生、发展密切相关[17-20]。TNF-α可加剧局部炎性浸润，从而促进肺组织纤维化[21]。IL-6是重要的一种炎症标志物，有学者研究发现，COPD患者血清IL-6水平较健康体检者明显升高[22]。IL-8可使气道局部炎症反应加重，从而使气道高反应性增强[23]。本研究结果显示，补肺息喘汤可显著降低TNF-α、IL-6和IL-8水平，减轻炎症反应。在COPD不同发展阶段中TLR4均发挥重要调控作用，且其可能参与肺组织损伤的发生、发展[24-25]。CXCL13主要由滤泡基质细胞产生，在自身免疫系统疾病中发挥重要作用。研究报道显示，COPD患者血清CXCL13水平高于健康体检者，可见COPD患者血清CXCL13水平明显升高[26]。本研究结果显示，补肺息喘汤治疗可显著降低血清TLR4和CXCL13水平。
综上所述，补肺息喘汤对SCOPD患者治疗疗效显著，可减轻细胞炎症反应，降低TLR4和CXCL13水平，值得临床借鉴。
