Objective

To explore the therapeutic effect of Wuling Capsules（乌灵胶囊）combined with Shenqi Wuweizi Tablets（参芪五味子片）on elderly patients with stable angina pectoris and anxiety and depression.

Method

A total of 126 elderly patients with stable angina pectoris combined with anxiety and depression admitted to the cardiology department of the hospital from February 2022 to February 2023 were selected and randomly divided into experimental group and control group，with 63 cases in each group. The control group patients received Western medicine treatment，including Bisoprolol Fumarate Tablets，Xinkang Sustained-Release Tablets and Aspirin Enteric Coated Tablets. On the basis of the control group，the experimental group received Wuling Capsules combined with Shenqi Wuweizi Tablets. The efficacy of stable angina pectoris，electrocardiogram efficacy，traditional Chinese medicine syndrome efficacy，the scores of anxiety and depression，the sleep quality score，the levels of plasma atrial natriuretic peptide（ANP），endothelin（ET），cholesterol（TC），triacylglycerol（TG），norepinephrine（NE），5-hydroxytryptamine（5-HT）and dopamine（DA）and the occurrence of adverse reactions of two groups were compared.

Results

Compared with that of the control group，the treatment effect of stable angina pectoris in the experimental group was better（P<0.05）. Compared with those before treatment，the self-rating anxiety scale（SAS），self-rating depression scale（SDS）and Pittsburgh sleep quality index（PSQI）scores of the two groups decreased after treatment. Compared with the control group，the SAS，SDSand PSQI scores of the experimental group were lower after treatment（P<0.05）. After treatment，compared with those before treatment，the levels of ANP，ET，TCand TG in the two groups decreased. After treatment，compared with the control group，the experimental group had lower levels of ANP，ET，TCand TG，with statistically significant differences（P<0.05）. Compared with those before treatment，the levels of NE，5-HT and DA in the two groups increased after treatment. Compared with the control group，the experimental group had higher levels of NE，5-HT and DA after treatment（P<0.05）. There was no difference in the occurrence of adverse reactions between the two groups（P>0.05）.

Conclusion

Wuling Capsules combined with Shenqi Wuweizi Tablets can improve the symptoms of angina pectoris in elderly patients with stable angina pectoris and anxiety，alleviate negative emotions such as anxiety and depression and down-regulate the levels of ANP，ET，TC，TG and monoamine neurotransmitters，making it suitable for clinical application.

目的

探究乌灵胶囊联合参芪五味子片对老年稳定型心绞痛并焦虑抑郁状态的疗效。

方法

选取2022年2月—2023年2月医院心内科收治的老年稳定型心绞痛并焦虑抑郁患者126例，随机分为试验组、对照组，各63例。对照组患者接受西医治疗，富马酸比索洛尔片、欣康缓释片联合阿司匹林肠溶片。试验组在对照组的基础上予以乌灵胶囊联合参芪五味子片。对比两组稳定型心绞痛疗效、心电图疗效、中医证候疗效、焦虑抑郁、睡眠质量评分、血浆心钠素（Atrial natriuretic peptide，ANP）、内皮素（endothelin，ET）、胆固醇（cholesterol，TC）、三酰甘油（triacylglycerol，TG）、去甲肾上腺素（noradrenaline，NE）、5-羟色胺（5-hydroxytryptamine，5-HT）和多巴胺（dopamine，DA）、不良反应发生情况。

结果

与对照组相比，试验组稳定型心绞痛治疗效果、心电图疗效、中医症状疗效较优（P<0.05）。与治疗前比，治疗后两组焦虑自评量表（self-rating anxiety scale，SAS）、抑郁自评量表（Self-Rating Depression Scale，SDS）及匹兹堡睡眠质量指数量表（Pittsburgh Sleep Quality Index，PSQI）评分降低；与对照组比，治疗后试验组SAS、SDS及PSQI评分较低，差异有统计学意义（P<0.05）。治疗后两组患者ANP、ET、TC及TG水平降低；与对照组比较，试验组ANP、ET、TC以及TG水平较低（P<0.05）。与治疗前比，治疗后两组NE、5-HT、DA水平升高；与对照组比，治疗后试验组NE、5-HT、DA水平较高（P<0.05）。两组不良反应发生情况对比差异无统计学意义（P>0.05）。

结论

乌灵胶囊联合参芪五味子片能改善老年稳定型心绞痛并焦虑患者的心绞痛症状，缓解焦虑抑郁等不良情绪，适宜临床应用。

目的

探究乌灵胶囊联合参芪五味子片对老年稳定型心绞痛并焦虑抑郁状态的疗效。

方法

选取2022年2月—2023年2月医院心内科收治的老年稳定型心绞痛并焦虑抑郁患者126例，随机分为试验组、对照组，各63例。对照组患者接受西医治疗，富马酸比索洛尔片、欣康缓释片联合阿司匹林肠溶片。试验组在对照组的基础上予以乌灵胶囊联合参芪五味子片。对比两组稳定型心绞痛疗效、心电图疗效、中医证候疗效、焦虑抑郁、睡眠质量评分、血浆心钠素（Atrial natriuretic peptide，ANP）、内皮素（endothelin，ET）、胆固醇（cholesterol，TC）、三酰甘油（triacylglycerol，TG）、去甲肾上腺素（noradrenaline，NE）、5-羟色胺（5-hydroxytryptamine，5-HT）和多巴胺（dopamine，DA）、不良反应发生情况。

结果

与对照组相比，试验组稳定型心绞痛治疗效果、心电图疗效、中医症状疗效较优（P<0.05）。与治疗前比，治疗后两组焦虑自评量表（self-rating anxiety scale，SAS）、抑郁自评量表（Self-Rating Depression Scale，SDS）及匹兹堡睡眠质量指数量表（Pittsburgh Sleep Quality Index，PSQI）评分降低；与对照组比，治疗后试验组SAS、SDS及PSQI评分较低，差异有统计学意义（P<0.05）。治疗后两组患者ANP、ET、TC及TG水平降低；与对照组比较，试验组ANP、ET、TC以及TG水平较低（P<0.05）。与治疗前比，治疗后两组NE、5-HT、DA水平升高；与对照组比，治疗后试验组NE、5-HT、DA水平较高（P<0.05）。两组不良反应发生情况对比差异无统计学意义（P>0.05）。

结论

乌灵胶囊联合参芪五味子片能改善老年稳定型心绞痛并焦虑患者的心绞痛症状，缓解焦虑抑郁等不良情绪，适宜临床应用。

Objective

To explore the therapeutic effect of Wuling Capsules（乌灵胶囊）combined with Shenqi Wuweizi Tablets（参芪五味子片）on elderly patients with stable angina pectoris and anxiety and depression.

Method

A total of 126 elderly patients with stable angina pectoris combined with anxiety and depression admitted to the cardiology department of the hospital from February 2022 to February 2023 were selected and randomly divided into experimental group and control group，with 63 cases in each group. The control group patients received Western medicine treatment，including Bisoprolol Fumarate Tablets，Xinkang Sustained-Release Tablets and Aspirin Enteric Coated Tablets. On the basis of the control group，the experimental group received Wuling Capsules combined with Shenqi Wuweizi Tablets. The efficacy of stable angina pectoris，electrocardiogram efficacy，traditional Chinese medicine syndrome efficacy，the scores of anxiety and depression，the sleep quality score，the levels of plasma atrial natriuretic peptide（ANP），endothelin（ET），cholesterol（TC），triacylglycerol（TG），norepinephrine（NE），5-hydroxytryptamine（5-HT）and dopamine（DA）and the occurrence of adverse reactions of two groups were compared.

Results

Compared with that of the control group，the treatment effect of stable angina pectoris in the experimental group was better（P<0.05）. Compared with those before treatment，the self-rating anxiety scale（SAS），self-rating depression scale（SDS）and Pittsburgh sleep quality index（PSQI）scores of the two groups decreased after treatment. Compared with the control group，the SAS，SDSand PSQI scores of the experimental group were lower after treatment（P<0.05）. After treatment，compared with those before treatment，the levels of ANP，ET，TCand TG in the two groups decreased. After treatment，compared with the control group，the experimental group had lower levels of ANP，ET，TCand TG，with statistically significant differences（P<0.05）. Compared with those before treatment，the levels of NE，5-HT and DA in the two groups increased after treatment. Compared with the control group，the experimental group had higher levels of NE，5-HT and DA after treatment（P<0.05）. There was no difference in the occurrence of adverse reactions between the two groups（P>0.05）.

Conclusion

Wuling Capsules combined with Shenqi Wuweizi Tablets can improve the symptoms of angina pectoris in elderly patients with stable angina pectoris and anxiety，alleviate negative emotions such as anxiety and depression and down-regulate the levels of ANP，ET，TC，TG and monoamine neurotransmitters，making it suitable for clinical application.

稳定型心绞痛是一种临床常见的心血管病[1-2]，以老年患者居多，疾病可能诱发心绞痛、心肌梗死、心力衰竭、心律失常甚至心脏性猝死等[3]，是当今威胁居民生命健康的重要疾病之一[4]。本病具有较高比例焦虑抑郁情况，临床可见躯体化症状、强迫症状及睡眠障碍等[5]。乌灵胶囊能够补肾健脑、养心安神，可临床用于改善神经衰弱、抑郁症以及老年病等[6]。参芪五味子片具有健脾益气、宁心安神的功效。目前暂无乌灵胶囊与参芪五味子片联合应用的相关研究，基于此本项目就乌灵胶囊联合参芪五味子片对老年稳定型心绞痛并焦虑抑郁状态的疗效进行观察，报道如下。
1　资料与方法

1.1　临床资料
选取2022年2月—2023年2月医院心内科收治的老年稳定型心绞痛并焦虑抑郁患者126例，根据随机数字表法进行分组，分为试验组、对照组两组。试验组63例，男30例，女33例；年龄65～88岁，平均（73.92±8.12）岁；病程2～15年，平均（8.44±0.93）年。对照组63例，男29例，女34例；年龄66～87岁，平均（73.51±8.15）岁；病程3～15年，平均（8.50±0.91）年。两组患者一般资料相仿相比无差异（P>0.05）。
1.2　纳入标准
（1）诊断符合《内科学》[7]中稳定型心绞痛的诊断标准；（2）冠心病心绞痛Ⅰ～Ⅲ级；（3）同时合并焦虑抑郁症状符合《中国精神障碍分类与诊断标准》[8]中的诊断标准；（4）年龄超过65岁的老年患者；（5）本人均自愿参与本试验，经院医学伦理委员会批准。
1.3　排除标准
（1）对应用药物有过敏反应者；（2）近期急性感染；（3）合并重度心肺功能不全，重度心律失常者，存在严重肝、肾疾病者；（4）无法正常交流者；（5）正应用抗抑郁类药物；（6）病历不全者。
1.4　治疗方法
对照组患者接受西医治疗，富马酸比索洛尔片（规格：5 mg×10片，国药准字HJ20160474，Merck Serono GmbH）自小剂量2.5 mg每日口服开始，逐级增高剂量，以缓解症状、心率不低于60～65次/min为宜；欣康缓释片（规格：40 mg，国药准字H19991039，鲁南贝特制药有限公司）40 mg每日口服，若存在药物不良反应则予以单硝酸异山梨酯片20 mg，每日2次口服；阿司匹林肠溶片（规格：每片75 mg，国药准字HJ20160685，拜耳医药保健有限公司）100 mg，每天1次口服；立普妥（规格：10 mg×7 s/盒，国药准字H20051407，辉瑞制药有限公司）10 mg，日1次口服。
试验组在对照组的基础上予以乌灵胶囊联合参芪五味子片，乌灵胶囊（规格：0.33 g×36粒，国药准字Z19990048，浙江佐力药业股份有限公司）0.99 g口服，日3次。参芪五味子片（由党参、黄芪、五味子、酸枣仁组成，国药准字Z62020288，康县独一味生物制药有限公司），每次3片口服，日3次。
1.5　观察指标
1.5.1　稳定型心绞痛疗效评定标准
参考《中药新药临床研究指导原则》[9]进行疗效评估。显效为心绞痛相关症状减轻或消失，心绞痛发作次数减少超出80%；有效为心绞痛程度改善1度，心绞痛的发作次数减少为50%～80%；无效为临床症状无改变甚至存在疾病进展。总有效率为显效与有效率之和。
1.5.2　心电图疗效判定原则
显效为心电图恢复至“大致正常”或达到“正常心电图”，基本不需口服硝酸甘油；有效为治疗后S-T段回升低于0.05 mV、主要导联T波改变变浅，硝酸甘油用量降低超过一半；无效为心电图基本与治疗前相同；加重为S-T段较治疗前下降超过0.05 mV，在主要导联倒置T波加深或直立T波变平坦，平坦T波变倒置，严重者可能合并异位心律、房室传导阻滞、室内传导阻滞等。
1.5.3　中医证候疗效评估标准
显效为中医临床主次症基本或完全消失，治疗后证候积分为0或下降超过70%；有效为治疗后70%>证候积分减少≥30%；无效为0≤治疗后证候积分减少<30%；加重为证候积分减少<0或治疗后积分超过治疗前积分。
1.5.4　焦虑抑郁和睡眠质量
治疗前、治疗后采用焦虑自评量表（SAS）评分、抑郁自评量表（SDS）评分对焦虑抑郁情况进行评估，SAS评分分界值为50分，SDS评分标准分界值为53分，分数越高提示焦虑抑郁症状越严重。匹兹堡睡眠质量指数（PSQI）总分。采用PSQI对睡眠质量进行评估，总分值为21分，得分越高提示睡眠质量越好。
1.5.5　实验室指标检测
于治疗前和治疗后次日取清晨空腹静脉血5 mL，分装于特制试管内待检，采用放射免疫法对血浆心钠素（ANP）、内皮素（ET）进行检测；应用日立7060型全自动生化分析仪对血浆胆固醇（TC）及三酰甘油（TG）进行检测；采取双抗夹心酶联免疫吸附法对去甲肾上腺素（NE）、5-羟色胺（5-HT）和多巴胺（DA）水平进行监测。
1.5.6　不良反应发生情况
记录并对比两组不良反应发生率。
1.6　统计学方法
应用SPSS 23.0统计进行统计学分析，计量资料采取±标准差表示并应用t检验，计数资料采取率（%）表示并予以χ2检验，P<0.05提示有统计学意义。
2　结果

2.1　稳定型心绞痛疗效比较
试验组临床治疗总有效率为93.65%（59/63），对照组总有效率为77.78%（49/63），试验组稳定型心绞痛治疗效果较优（P<0.05）。见表1。
2.2　心电图疗效评价比较
与对照组比较，试验组心电图疗效较优（P<0.05）。见表2。
2.3　中医症状疗效比较
试验组中医症状疗效总有效率为95.24%（60/63），对照组中医症状疗效总有效率为76.19%（48/63），试验组中医症状疗效较优（P<0.05）。见表3。
2.4　焦虑抑郁情况及睡眠质量比较
与治疗前比，治疗后两组SAS、SDS及PSQI评分降低；与对照组比，治疗后试验组SAS、SDS及PSQI评分较低，差异有统计学意义（P<0.05）。见表4。
2.5　ANP、ET、TC以及TG水平比较
治疗后相比治疗前，两组ANP、ET、TC、TG下降；治疗后与对照组比较，试验组患者ANP、ET、TC以及TG水平较低（P<0.05）。见表5。
2.6　单胺类神经递质水平比较
与治疗前比，治疗后两组NE、5-HT、DA水平升高；与对照组比，治疗后试验组NE、5-HT、DA水平较高，差异有统计学意义（P<0.05）。见表6。
2.7　安全性评价
试验组存在恶心1例，嗜睡2例；对照组出现1例头晕，1例嗜睡，2例食欲降低，试验组不良反应发生率4.76%（3/63）与对照组不良反应发生率6.35%（4/63）进行比较，差异无统计学意义（P>0.05）。
3　讨论

稳定型心绞痛发病率存在增高趋势，而抑郁症是由多种原因导致的心境障碍或情感障碍疾病[10-15]。上述两种疾病存在共同的发病机制，提示二者相互促进、相互影响，而后者也是稳定型心绞痛常见并发症[16-18]。稳定型心绞痛隶属“心痛”及“胸痹”范畴，病机是心脉痹阻，多呈虚实夹杂证，虚证为气虚、血虚、阴虚、阳虚，实证为气滞、痰凝、血瘀互结，并以气虚血瘀为根本[19-20]。抑郁焦虑是本病常见的合并症，属于中医“郁证”的范畴，多由于情志不遂诱发气机运转不利所导致症状的发生[21-23]。乌灵胶囊临床应用能养肾阴、降心火，还具有养心安神的功效，能够缓解焦虑症状[24-26]。相关研究显示，乌灵胶囊能提高脑内谷氨酸及γ-氨基丁酸（γ-aminobutyri cacid，GABA）水平，有助于谷氨酸脱羧酶活性的提升[27-31]，增加大脑皮层中抑制性神经递质GABA的合成量，达到中枢神经镇静作用[32-33]。参芪五味子片中黄芪、党参共同补益脾肺之气，炒酸枣仁能养心阴、补心肝之血；五味子及酸枣仁能镇静安神、抗焦虑，有效调节植物神经功能。临床应用具有健脾益气、宁心安神，增强免疫功能的功效。前期相关研究发现针对心绞痛并焦虑抑郁患者参芪五味子片起效快[34]，治疗焦虑抑郁效果比阿普唑仑作用持久，且未发现明显不良反应。目前科室将乌灵胶囊与参芪五味子片联合应用于稳定型心绞痛并焦虑抑郁的治疗中，不仅能够增强免疫功能，还能改善失眠。但两者联合应用暂无相关理论基础，因此我们就其进行研究，研究显示，与常规西药治疗组相比，试验组稳定型心绞痛治疗效果、心电图疗效、中医症状疗效较优，提示乌灵胶囊联合参芪五味子片应用于老年稳定型心绞痛并焦虑抑郁状态患者，从中医症状、心电图等多方面均具有显著的疗效提升作用。
从病理角度阐释老年稳定型心绞痛并焦虑抑郁的病机，微血管病变会对患者的认知功能产生损害，诱发抑郁症[35-46]。曾有研究显示血小板在神经递质5-羟色胺（5-HT）作用下被激活，抑郁症患者5-HT分泌增多[37-40]，进而导致其血小板活性比非抑郁症患者活性更高，血小板活性增强黏附力加大，致使粥样硬化斑块形成速度较快[41]；除5-HT外，DA及NE也是最常见单胺类神经递质，均具有调节记忆、精神、情绪、体温和摄食等作用[42]，对肾上腺髓质-外周交感机能状态具有反馈意义。曾有研究显示，外周血5-HT、DA及NE水平与机体情绪、行为关系密切，水平升高提示人兴奋，水平降低则会诱发抑郁焦虑[43]。我们研究显示，与对照组比，治疗后试验组NE、5-HT、DA水平较高，提示乌灵胶囊联合参芪五味子片能够通过提升外周血5-HT、DA及NE水平以提高老年稳定型心绞痛并焦虑抑郁的治疗效果。
老年稳定型心绞痛并焦虑抑郁患者基本病理变化为冠状动脉粥样硬化，而高脂血症是导致动脉粥样硬化的关键因素。当机体内血脂水平升高，能诱发血管内皮细胞膜黏滞升高，内皮损伤，此外粥样硬化斑块部位内皮收缩，平滑肌细胞转变为泡沫细胞，诱发动脉粥样硬化[44]。ANP主要由心房肌特殊颗粒细胞分泌，其水平升高能够促进血管扩张；ET参与动脉粥样硬化的形成，是动脉粥样硬化的标志物[45-46]。我们研究显示，与对照组比较，试验组患者ANP、ET、TC以及TG水平较低，提示推测乌灵胶囊联合参芪五味子片可能通过下调ANP、ET水平从而提高疗效。
本研究通过对老年稳定型心绞痛并焦虑抑郁患者126例进行观察，证实乌灵胶囊联合参芪五味子片能改善老年稳定型心绞痛并焦虑患者的心绞痛症状，缓解焦虑抑郁等不良情绪，下调ANP、ET、TC及TG水平，单胺类神经递质水平，适宜临床应用。
