目的

探讨针灸对腹腔镜子宫切除术全麻复合腹横平面阻滞患者胃肠功能恢复的作用。

方法

选取2023年1月—2023年12月于医院进行腹腔镜子宫切除术的患者78例，按照随机数字表法均分成两组，各39例，对照组在采用全麻复合腹横平面阻滞下进行手术，针灸干预组在对照组基础上增加术后针灸干预，比较两组患者的术后疼痛情况，胃肠功能恢复时间，胃肠道症状问卷（gastrointestinal symptom score，GIS）评分，炎症因子水平，术后并发症发生率。

结果

术后24、48 h，针灸干预组与对照组在疼痛视觉模拟评分（visual analogue scale，VAS）比较差异无统计学意义（P>0.05）；术后72 h针灸干预组VAS评分为（4.16±1.07）分，显著低于对照组[（4.89±1.18）分]，差异有统计学意义（P<0.05）。针灸干预组术后腹胀消失时间（31.42±4.51）h、首次排气时间为（36.94±5.30）h、首次排便时间为（51.79±7.15）h均显著短于对照组[（33.62±3.94）h、（41.22±5.08）h、（56.30±8.16）h]，两组比较差异存在统计学意义（P<0.05）。术后24 h，针灸干预组与对照组GIS评分差异无统计学意义（P>0.05）；术后48、72 h针灸干预组GIS评分为（18.62±1.72）分显著低于对照组[（21.38±1.94）分]，两组比较差异具有统计学意义（P<0.05）。术后1 d，两组患者肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）、白细胞介素-6（interleukin-6，IL-6）、可溶性白细胞介素-2（soluble interleukin 2，sIL-2）水平比较，差异无统计学意义（P>0.05）；出院时，两组以上各项指标均降低，并且针灸干预组的血清TNF-α为（18.70±2.11）pg/mL、IL-6为（17.25±2.51）pg/mL、sIL-2为（60.82±6.08）pg/mL水平低于对照组[（21.49±1.97）pg/mL、（19.32±2.60）pg/mL、（64.26±5.76）pg/mL]，两组比较差异有统计学意义（P<0.05）。针灸组并发症发生率（7.69%），低于对照组（12.82%），两组比较差异无统计学意义（P>0.05）。

结论

针灸在腹腔镜子宫切除术全麻复合腹横平面阻滞患者中的应用效果满意，能够缓解患者术后疼痛、促进胃肠功能恢复，还可以减轻炎症反应，有利于患者的术后康复，值得应用。

Objective

To explore the effect of acupuncture and moxibustion on the recovery of gastrointestinal function in the patients with laparoscopic hysterectomy under general anesthesia combined with abdominal transverse plane block.

Methods

Seventy-eight patients who underwent laparoscopic hysterectomy in the hospital from January 2023 to December 2023 were selected and divided into two groups according to the random number table method, 39 patients in each group. The control group was operated under general anesthesia combined with abdominal transverse plane block. The acupuncture and moxibustion intervention group added postoperative acupuncture and moxibustion intervention on the basis of the control group. The postoperative pain, gastrointestinal function recovery time, gastrointestinal symptom scores (GIS), inflammatory factor levels and postoperative complication rates of the patients in the two groups were compared.

Results

There was no significant difference in visual analog scale (VAS) scores between the acupuncture and moxibustion intervention group and the control group 24 and 48 hours after operation (P>0.05). The VAS score of the acupuncture and moxibustion intervention group [(4.16±1.07) points] was significantly lower than that of the control group [(4.89±1.18) points]72 hours after operation, with a statistical difference (P<0.05). The disappearance time of abdominal distension [(31.42±4.51) h], the time of first exhaust [(36.94±5.30) h] and the time of first defecation [(51.79±7.15) h] in the acupuncture and moxibustion intervention group were significantly shorter than those in the control group [(33.62±3.94) h, (41.22±5.08) h, (56.30±8.16) h]. There was a statistical difference between the two groups (P<0.05). There was no significant difference in GIS scores between the acupuncture and moxibustion intervention group and the control group 24 hours after operation (P>0.05). The GIS score of the acupuncture and moxibustion intervention group [(18.62±1.72) points] was significantly lower than that of the control group [(21.38±1.94) points]48 and 72 hours after operation, with a statistical difference between the two groups (P<0.05). On postoperative day 1, there was no statistically significant difference in the levels of tumor necrosis factor-α(TNF-α), interleukin-6 (IL-6) or soluble interleukin-2 (sIL-2) between the two groups (P>0.05). At the time of discharge, the above indexes of the two groups were reduced, and the levels of serum TNF-α, IL-6 and sIL-2 in the acupuncture and moxibustion intervention group were (18.70±2.11) pg/mL, (17.25±2.51) pg/mL and (60.82±6.08) pg/mL, respectively, lower than those in the control group [(21.49±1.97) pg/mL, (19.32±2.60) pg/mL and (64.26±5.76) pg/mL], with a statistical difference between the two groups (P<0.05). The incidence of postoperative complications in the acupuncture and moxibustion intervention group was 7.69%, lower than that in the control group (12.82%), but there was no statistical difference between the two groups (P>0.05).

Conclusion

The application of acupuncture and moxibustion in the patients with laparoscopic hysterectomy under general anesthesia combined with abdominal transverse plane block has a satisfactory effect. It can alleviate the postoperative pain, promote the recovery of gastrointestinal function and also reduce inflammatory reaction, which is conducive to postoperative rehabilitation of the patients. It is worthy of applying.
