目的

观察基于“病变于脑，首取督脉”理论，通督调神针刺联合重复经颅磁刺激（Repetitive transcranial magnetic stimulation，rTMS）对卒中后偏瘫患者康复效果及神经功能的影响。

方法

选取2022年6月—2024年6月在湖北中医药大学附属荆门市中医医院就诊的卒中后偏瘫患者90例，随机分为观察组和对照组，每组45例。对照组采用常规康复治疗，观察组在对照组基础上联合通督调神针刺和重复经颅磁刺激治疗，治疗周期为8周。观察两组患者治疗前后的神经功能、运动功能、日常生活能力、生活质量、中医证候积分及临床疗效有无统计学意义。

结果

治疗后两组患者美国国立卫生研究院卒中量表（National institutes of health stroke scale，NIHSS）评分均降低，且观察组更低（P<0.05）；治疗后两组患者简化版Fugl-Meyer运动功能评定量表（Simplified fugl-meyer motor function assessment scale，FMA）、Barthel指数日常生活活动能力评定量表（Barthel index of activities of daily living，BI）及脑卒中特异性生活质量量表（Stroke-specific quality of life scale，SS-QIL）均升高，且观察组更高（P<0.05）；治疗后两组患者中医证候积分均降低，且观察组更低（P<0.05）；观察组总有效率为93.33%（42/45），明显高于对照组的73.33%（33/45）（P<0.05）。

结论

在“病变于脑，首取督脉”理论指导下，通督调神针刺联合重复经颅磁刺激能够显著促进卒中后偏瘫患者的神经功能恢复，改善运动功能和生活自理能力，具有较好的临床疗效。

Objective

This study aims to observe the rehabilitation effects and neurological impact of acupuncture on the Du meridian combined with repetitive transcranial magnetic stimulation (rTMS) on post-stroke hemiplegia patients based on the theory that “primary treatment of brain lesions should target the Du meridian”.

Method

Ninety post-stroke hemiplegia patients treated in Jingmen Hospital of Traditional Chinese Medicine Affiliated to Hubei University of Chinese Medicine from June 2022 to June 2024 were randomly divided into an observation group and a control group, each consisting of 45 patients. The control group received conventional rehabilitation treatment, while the observation group additionally underwent acupuncture on the Du meridian and rTMS for an 8-week treatment period. The differences in neurological deficit scores (NIHSS), motor function recovery, daily living ability, quality of life, traditional Chinese medicine syndrome scores and clinical efficacy before and after treatment were observed between the two groups.

Results

After treatment, both groups showed reduced NIHSS scores, with the observation group having significantly lower scores (P<0.05). Fugl-Meyer motor scores, Barthel Index scores and stroke-specific quality of life scale (SS-QOL) scores increased in both groups after treatment, with the observation group showing significantly higher improvements (P<0.05). Additionally, traditional Chinese medicine syndrome scores decreased in both groups, with the observation group exhibiting significantly lower scores (P<0.05). The overall effective rate in the observation group was 93.33% (42/45), which was significantly higher than that (73.33%, 33/45) in the control group (P<0.05).

Conclusion

Guided by the theory of “primary treatment of brain lesions via the Du meridian”, acupuncture on the Du meridian combined with rTMS can significantly promote the neurological recovery, improve the motor function and self-care abilities in post-stroke hemiplegia patients, demonstrating substantial clinical efficacy.
