目的

观察睑板腺功能障碍（meibomian gland dysfunction，MGD）伴蒸发过强型干眼症（evaporative dry eye，EDE）治疗中化湿润目汤的治疗作用以及对泪液中干扰素调节因子的影响。

方法

选取2022年1月—2024年1月医院接诊的MGD伴EDE患者102例，经随机数字表均分为西药组（氧氟沙星滴眼液、玻璃酸钠滴眼液治疗）、综合组（氧氟沙星滴眼液、玻璃酸钠滴眼液、化湿润目汤治疗），各51例。比较治疗前后两组证候积分、泪液中干扰素调节因子4（interferon regulator factor 4，IRF-4）水平、泪膜破裂时间（break up time，BUT）、泪液分泌量；比较两组睑板腺功能分级、治疗效果及不良反应。

结果

治疗后两组证候总积分及泪液中IRF-4水平均低于治疗前（P<0.05），综合组均低于西药组（P<0.05）；治疗后两组BUT均长于治疗前（P<0.05），综合组长于西药组（P<0.05），两组泪液分泌量均高于治疗前，综合组高于西药组（P<0.05）；综合组睑板腺功能1级构成比高于西药组（P<0.05），3级构成比低于西药组（P<0.05）；综合组治疗总有效率高于西药组（P<0.05）。西药组无明显不良反应，综合组轻微腹泻1例，未处理即好转。

结论

化湿润目汤可改善MGD伴EDE患者证候积分、BUT、泪液分泌量、睑板腺功能及治疗效果，降低泪液中IRF-4水平，且不会导致不良反应增加。

Objective

To observe the therapeutic effect of Huashi Runmu Decoction (化湿润目汤) in the treatment of meibomian gland dysfunction (MGD) associated with evaporative dry eye (EDE) and its influence on interferon regulatory factors in tears.

Methods

From January 2022 to January 2024, 102 patients with MGD associated with EDE were selected and randomly divided into two groups: western medicine group (treated with ofloxacin eye drops and sodium hyaluronate eye drops) and comprehensive group (treated with ofloxacin eye drops, sodium hyaluronate eye drops and Huashi Runmu Decoction), with 51 cases in each. The syndrome scores, the levels of interferon regulatory factor 4 (IRF-4) in tears, tear film break-up time (BUT), and tear secretion volume were compared before and after treatment, as well as the grading of meibomian gland function, treatment effect and adverse reactions between the two groups.

Results

After treatment, the total syndrome scores and the levels of IRF-4 in tears were lower than those before treatment in both groups (P<0.05), with the comprehensive group being lower than the western medicine group (P<0.05). The BUT of both groups were longer after treatment (P<0.05), with the comprehensive group being longer than the western medicine group (P<0.05). The tear secretion volume increased after treatment in both groups, with the comprehensive group being higher than the western medicine group (P<0.05). The proportion of grade 1 meibomian gland function was higher in the comprehensive group (P<0.05), and the proportion of grade 3 was lower than in the Western medicine group (P<0.05). The overall treatment effectiveness rate was higher in the comprehensive group (P<0.05). There were no significant adverse reactions in the Western medicine group, while the comprehensive group had one case of mild diarrhea, which improved without treatment.

Conclusion

Huashi Runmu Decoction can improve the syndrome scores, BUT, tear secretion volume, meibomian gland function and treatment effect in the patients with MGD associated with EDE, reduce the levels of IRF-4 in tears, and does not increase adverse reactions.
