Effect of Acupuncture Combined with Jiejing Decoction（解痉汤）on Upper Limb Muscle Tension，Motor Function and Life Ability in Patients with Increased Upper Limb Muscle Tension after Stroke

目的

观察针刺联合解痉汤治疗脑卒中后上肢肌张力增高的临床疗效。

方法

选择医院2021年6月—2022年12月诊治的84例脑卒中后上肢肌张力增高患者作为研究对象，根据随机数字表法随机均分为两组，每组42例。对照组采用解痉汤治疗，观察组采用针刺联合解痉汤治疗，治疗4周。观察比较两组总有效率，肌张力，上肢运动功能，日常生活能力以及不良反应，以中医症状积分变化判断疗效、改良Ashworth分级标准评估肌张力、Fugl-Meyer运动积分（Fugl-Meyer assessment scale，FMA）评估上肢运动功能、Barthel指数评估日常生活能力。

结果

观察组总有效率（97.62%，41/42）显著高于对照组（80.95%，34/42），组间差异有统计学意义（P<0.05）。治疗前两组肌张力分级情况差异无统计学意义（P>0.05），治疗后观察组显著优于对照组，差异有统计学意义（P<0.05）。治疗前两组患侧桡神经、尺神经及正中神经反射波H波最大波幅与M波最大波幅比值（Hmax/Mmax，H/M）比值差异无统计学意义（P>0.05），治疗后观察组患侧桡神经、尺神经及正中神经反射波H/M显著低于对照组，差异有统计学意义（P<0.05）。治疗前两组患者的上肢FMA评分及Barthel指数评分比较，差异均无统计学意义（P>0.05），治疗后两组上肢FMA评分及Barthel指数评分均显著升高，并且观察组显著高于对照组（P<0.05）。两组均无严重不良反应发生。

结论

针刺联合解痉汤治疗脑卒中后上肢肌张力增高的临床疗效满意，能够显著改善患者的上肢肌张力状况，并且提高上肢运动功能和日常生活能力，并且安全性良好。

Objective

To observe the clinical effect of acupuncture combined with Jiejing Decoction（解痉汤）in the treatment of upper limb muscle tension after stroke.

Methods

Eighty-four patients with increased upper limb muscle tension after stroke diagnosed and treated in the hospital from June 2021 to December 2022 were selected as the research objects. According to the random number table method，they were randomly divided into two groups，with 42 cases in each group. The control group was treated with Jiejing Decoction and the observation group was treated with acupuncture combined with Jiejing Decoction for 4 weeks. The total effective rate，muscle tension，upper limb motor function and daily living ability of the two groups were observed and compared. The curative effect was judged by the change of TCM symptom scores，the muscle tension was evaluated by modified Ashworth grading standard，the upper limb motor function was evaluated by Fugl-Meyer motor score（FMA），and the daily living ability was evaluated by Barthel index.

Results

The total effective rate of the observation group was 97.62%（41/42），significantly higher than that（80.95%，34/42）of the control group，and the difference between two groups was statistically significant（P<0.05）. There was no statistically significant difference in muscle tone grading between the two groups before treatment（P>0.05），but after treatment，the observation group was significantly better than the control group，and the difference was statistically significant（P<0.05）. There was no significant difference in the H/M ratio of the radial nerve，ulnar nerve or median nerve reflection wave between the two groups before treatment（P>0.05）. After treatment，the H/M ratio of the radial nerve，ulnar nerve and median nerve reflection wave in the observation group was significantly lower than that in the control group，and the difference was statistically significant（P<0.05）. After treatment，the FMA score and Barthel index score of the two groups were significantly increased，and the scores of the observation group were significantly higher than those of the control group（P<0.05）. No serious adverse reactions occurred in both groups.

Conclusion

The clinical efficacy of acupuncture combined with Jiejing Decoction in the treatment of upper limb muscle tension after stroke is satisfactory. It can significantly improve the upper limb muscle tension，and improve the upper limb motor function and daily living ability of the patients，and it has good safety.
