目的

分析活血散风针刺法联合天麻钩藤饮治疗脑卒中合并高血压病的疗效及康复效能。

方法

纳入2022年5月—2024年5月收治的脑卒中合并高血压病患者208例作为研究对象，随机数字表法将其分为两组，对照组104例采用常规西药进行治疗，观察组104例在对照组基础上予以活血散风针刺法联合天麻钩藤饮治疗，对比两组疗效、24 h动态血压、神经功能、肢体功能、中医症状积分、血管内皮功能及Klotho蛋白（Klotho protein，Klotho）水平变化情况。

结果

观察组治疗总有效率为97.12%（101/104），远高于对照组的85.58%（89/104），组间比较差异有统计学意义（P<0.05）；治疗后观察组中医症状积分、美国国立卫生研究院卒中量表评分（National institutes of health stroke scale，NIHSS）、24 h动态血压低于对照组，弗格尔-梅耶尔运动功能评分（Fugl-meyer assessment，FMA）高于对照组（P<0.05）；治疗后观察组内皮素-1（Endothelin-1，ET-1）低于对照组，一氧化氮（Nitric oxide，NO）、Klotho高于对照组（P<0.05）。

结论

对脑卒中合并高血压病患者采用活血散风针刺法联合天麻钩藤饮治疗能有效降低血压水平，调节NO、ET-1及Klotho蛋白水平，改善血管内皮功能，促进神经功能及肢体功能恢复，对患者预后改善起到积极作用。

Objective

To analyze the curative effect and rehabilitation efficiency of activating blood and dispersing wind acupuncture combined with Tianma Guteng Yin（天麻钩藤饮）in the treatment of stroke patients with hypertension.

Method

A total of 208 patients with stroke combined with hypertension admitted to the hospital from May 2022 to May 2024 were included as the study objects. They were divided into two groups by the random number table method. A hundred and four patients in the control group were treated with conventional western medicine，and the other 104 patients in the observation group were treated with activating blood and dispersing wind acupuncture combined with Tianma Guteng Yin on this basis. The difference of curative effect，24 h ambulatore blood pressure，nerve function，limb function，TCM syndrome scores，vascular endothelial function and Klotho protein level were compared between the two groups.

Results

The total effective rate of the observation group was 97.12%（101/104），which was much higher than 85.58%（89/104）of the control group（P<0.05）. After treatment，the TCM symptom scores，National Institutes of Health Stroke Scale（NIHSS）score and 24 h ambulatore blood pressure in the observation group were lower than those in the control group，and the Fugl-Meyer Assessment（FMA）score was higher than that in the control group（P<0.05）. After treatment，endothelin-1（ET-1）in the observation group was lower than that in the control group，and nitric oxide（NO）and Klotho were higher than those of the control group（P<0.05）.

Conclusion

Activating blood and dispersing wind acupuncture combined with Tianma Guteng Yin can effectively reduce the blood pressure，regulate the levels of NO，ET-1 and Klotho protein，improve the vascular endothelial function，promote the recovery of nerve function and limb function，and plays a positive role in improving the prognosis of patients with stroke and hypertension.
