目的

探讨电针足三里联合当归饮子对血虚风燥型湿疹患者的临床疗效及其对神经-免疫-皮肤屏障交互作用的影响。

方法

采用前瞻性、随机对照试验设计，纳入2023年9月—2024年12月扬州大学附属医院中医科门诊确诊的血虚风燥型慢性湿疹患者60例，按照1∶1比例随机分为观察组（电针足三里+当归饮子）和对照组（单纯电针足三里），干预周期8周。主要结局指标为湿疹面积及严重度指数（Eczema area and severity index，EASI），次要指标包括夜间瘙痒视觉模拟评分（Visual analogue scale，VAS）、中医证候积分、血清白细胞介素-31（Interleukin-31，IL-31）/白细胞介素-4（Interleukin-4，IL-4）水平及皮肤屏障丝聚蛋白（Filaggrin，FLG）表达。神经免疫相关指标采用ELISA、qRT-PCR及免疫组化检测P物质（Substance P，SP）、神经生长因子（Nerve growth factor，NGF）、FLG mRNA。统计学分析采用混合效应模型、协方差分析（ANCOVA）及相关性分析。

结果

观察组EASI评分8周后降至（4.2±1.5）分，显著低于对照组［（7.8±2.1）分，P<0.001］；VAS评分下降63%，IL-31及IL-4水平分别降低57.3%和44.6%（P<0.001），均显著优于对照组。观察组FLG相对表达量提升2.15倍（95%CI［1.42，3.72］），且FLG上调与SP下降呈负相关（r=-0.67，P<0.001）。电针足三里联合当归饮子可能通过“抑制神经源性瘙痒-调控Th2炎症-促进屏障修复”的协同作用改善湿疹症状。

结论

电针足三里联合当归饮子可有效缓解血虚风燥型湿疹的临床症状，减少瘙痒、降低炎症水平并增强皮肤屏障功能。其机制可能与神经源性瘙痒信号下调、Th2炎症调节及FLG介导的屏障修复相关，为针药联合治疗湿疹提供了现代医学证据。

Objective

To investigate the clinical efficacy of electroacupuncture at Zusanli（ST36）combined with Danggui Yinzi in patients with chronic eczema of the blood-deficiency and wind-dryness pattern，and to explore its effects on the neuro-immune-skin barrier crosstalk.

Methods

This prospective，randomized controlled trial enrolled 60 patients diagnosed with chronic eczema of the blood-deficiency and wind-dryness pattern at the outpatient Department of Traditional Chinese Medicine，Affiliated Hospital of Yangzhou University，from September 2023 to December 2024. Participants were randomized（1∶1）to an observation group（electroacupuncture at ST36 plus Danggui Yinzi）or a control group（electroacupuncture at ST36 alone）for an 8-week intervention. The primary outcome was the Eczema Area and Severity Index（EASI）. Secondary outcomes included nocturnal pruritus assessed by the Visual Analogue Scale（VAS），Traditional Chinese Medicine（TCM）syndrome score，serum interleukin（IL）-31 and IL-4 levels，and filaggrin（FLG）expression as a marker of skin barrier function. Neuro-immune-related biomarkers were quantified using ELISA，qRT-PCR，and immunohistochemistry，including substance P（SP），nerve growth factor（NGF），and FLG mRNA. Statistical analyses were performed using mixed-effects models，analysis of covariance（ANCOVA），and correlation analyses.

Results

After 8 weeks，the EASI score in the observation group decreased to 4.2±1.5，significantly lower than that in the control group（7.8±2.1，P<0.001）. The VAS score decreased by 63%，and serum IL-31 and IL-4 levels decreased by 57.3% and 44.6%，respectively（all P<0.001），with superior improvements compared with the control group. The relative expression of FLG increased by 2.15-fold（95% CI［1.42，3.72］）in the observation group，and FLG upregulation was negatively correlated with SP reduction（r=-0.67，P<0.001）. Electroacupuncture at ST36 combined with Danggui Yinzi may alleviate eczema symptoms through synergistic actions of “suppressing neurogenic itch，modulating Th2-driven inflammation，and promoting barrier repair.”

Conclusion

Electroacupuncture at Zusanli（ST36）combined with Danggui Yinzi effectively improves clinical symptoms in patients with chronic eczema of the blood-deficiency and wind-dryness pattern by reducing pruritus，lowering inflammatory burden，and enhancing skin barrier function. The underlying mechanisms may involve inhibition of the TRPV1/SP/IL-31 axis，regulation of Th2 inflammation，and FLG-mediated barrier restoration，providing modern biomedical evidence for combined acupuncture-herbal therapy in eczema.
