目的

研究增液汤治疗食积化热型小儿功能性便秘的疗效及对血清胃泌素（gastrin，GAS），胃动素（motilin，MLT），胃肠激素P物质（substance P，SP）指标的影响。

方法

选取2023年11月—2024年5月医院治疗的食积化热型小儿功能性便秘98例，按照随机数表法分为观察组、对照组，各49例。对照组给予乳果糖口服液、枯草杆菌二联活菌颗粒治疗，观察组给予增液汤治疗，2周为1个疗程。比较两组患儿疗效，中医证候积分、中医证候消失时间及血清GAS、MLT、SP指标变化和不良反应。

结果

观察组临床痊愈率（69.39%）显著高于对照组（46.94%）（P < 0.05）；观察组总有效率（97.96%）与对照组（93.88%）比较，差异无统计学意义（P > 0.05）。治疗后两组患儿排便间隔时间延长、大便干结、排便困难、大便失禁积分均显著下降，观察组排便间隔时间延长、大便干结、排便困难显著低于对照组（P < 0.05）。观察组患儿的主要中医证候排便间隔时间延长、大便干结、排便困难、大便失禁消失时间均显著短于对照组（P < 0.05）。治疗后两组患儿血清GAS及SP水平显著升高，观察组患儿MLT水平也显著升高、但对照组患儿MLT水平无显著变化，治疗后观察组血清GAS、MLT、SP显著高于对照组（P < 0.05）。观察组不良反应发生率（6.12%）与对照组（10.20%）比较，组间差异无统计学意义（P > 0.05）。

结论

增液汤治疗食积化热型小儿功能性便秘的疗效满意，能够提高患儿的临床痊愈率，缓解患儿症状，其通过有效调节其胃肠激素水平、促进患儿康复，具有安全、可靠的应用价值。

Objective

To study the curative effect of Zengye Decoction（增液汤）on functional constipation in children with dyspepsia and food accumulation transforming into heat type and its effect on serum gastrin（GAS）, motilin（MLT）and gastrointestinal hormone substance P（SP）.

Methods

From November 2023 to May 2024, 98 children with functional constipation of food accumulation and heat type treated in the hospital were selected and divided into two groups according to the random number table method, with 49 children in each group. The control group was treated with lactulose oral liquid and bacillus subtilis and enterococcus faecium granules. The observation group was treated with Zengye Decoction. Two weeks was a course of treatment. The curative effect, TCM syndrome scores, disappearance time of TCM syndrome, changes of serum GAS, MLT and SP and adverse reactions were compared between the two groups.

Results

The clinical cure rate of the observation group was 69.39%, which was significantly higher than that of the control group（46.94%）（P < 0.05）. After treatment, the scores of defecation interval, dry stool, difficult defecation and fecal incontinence in the two groups were significantly decreased, the scores of the defecation interval, dry stool and difficult defecation in the observation group were significantly lower than those in the control group（P < 0.05）. The time of defecation interval prolongation, dry stool, difficult defecation and disappearance of fecal incontinence in the observation group were significantly shorter than those in the control group（P < 0.05）. After treatment, the levels of serum GAS and SP in the two groups were significantly increased, and the level of MLT in the observation group was also significantly increased, but there was no significant change in the level of MLT in the control group, After treatment, the levels of serum GAS, MLT and SP in the observation group were significantly higher than those in the control group（P < 0.05）. The incidence of adverse reactions was 6.12% in the observation group and 10.20% in the control group. There was no significant difference between the two groups（P > 0.05）.

Conclusion

The curative effect of Zengye Decoction in the treatment of functional constipation in children with food accumulation transforming into heat type is satisfactory. It can improve the clinical cure rate and relieve the symptoms, and promote the rehabilitation of children by effectively regulating their gastrointestinal hormone levels, so it has safe and reliable application value.
