Objective

To observe the curative effect of Shunao Jieyu Decoction（疏脑解郁汤）combined with moxibustion of regulating governor and relieving depression-transcranial magnetic stimulation（rTMS）on the rehabilitation of the patients with post-stroke depression（PSD）.

Methods

A total of PSD patients admitted to Hebei Provincial Hospital of Traditional Chinese Medicine from June 2021 to January 2023 were randomly divided into the combined group（60 cases）and the control group（60 cases）. Both groups were treated with Escitalopram Oxalate Tablets and the moxibustion method of regulating Du meridian and relieving depression-rTMS, and the combined group was intervened with Shunao Jieyu Decoction for 4 weeks. The changes of Hamilton Depression Scale（HAMD）, TCMsyndrome scores and modified Edinburgh-Scandinavian stroke scale（MESSS）before and after treatment were compared between the two groups. The side effects of the two groups were evaluated by Asperger＇s anti-depressant side effects scale, and the therapeutic effects of the two groups were evaluated after treatment.

Results

After 2 and 4 weeks of treatment, the HAMD scores, TCMsyndrome scores and MESSS scale scores of the two groups decreased compared with those before treatment, while the scores of the combined group were lower than those of the control group after 2 and 4 weeks of treatment（P<0.05）. The incidence of side effects in the combined group and the control group were 15.00%（9/60）and 18.33%（11/60）, respectively, and there was no statistical difference between the two groups（P>0.05）. The total effective rate of the combined group was 91.67%（55/60）, which was significantly higher than that of the control group（76.67%, 46/60）（P<0.05）.

Conclusion

On the basis of regulating Du and relieving depression moxibustion-rTMS therapy, the combined application of Shunao Jieyu Decoction can effectively regulate the depression, relieve the clinical symptoms and improve the neurological function without affecting the safety of treatment, so as to achieve a more ideal therapeutic effect.

目的

观察疏脑解郁汤联合调督解郁灸法-经颅磁刺激（Relieving depression-transcranial magnetic stimulation，rTMS）对卒中后抑郁（Post-stroke depression，PSD）患者康复治疗中的疗效。

方法

选取河北省中医院2021年6月—2023年1月收治的120例PSD患者，使用简单随机法分别纳入联合组60例、对照组60例，两组均接受草酸艾司西酞普兰片及调督解郁灸法-rTMS治疗，联合组加用疏脑解郁汤干预，持续4周。对比两组治疗前后汉密尔顿抑郁量表（Hamilton depression scale，HAMD）、中医证候积分、改良爱丁堡-斯堪的纳维亚神经功能缺损量表（Modified edinburg scandinavian stroke scale，MESSS）评分变化，使用Asberg抗抑郁剂不良反应量表评估两组治疗不良反应，并于治疗后评估两组治疗效果。

结果

两组治疗2、4周后HAMD评分、中医证候积分、MESSS量表评分均较治疗前下降，联合组治疗2、4周后HAMD评分、中医证候积分、MESSS量表评分均低于对照组（P<0.05）。联合组、对照组治疗不良反应发生率分别为15.00%（9/60）、18.33%（11/60），组间比较差异无统计学意义（P>0.05）。联合组治疗总有效率为91.67%（55/60），较对照组的76.67%（46/60）显著升高（P<0.05）。

结论

在调督解郁灸法-rTMS治疗的基础上，联合应用疏脑解郁汤能够在不影响治疗安全性的前提下，有效调节患者抑郁状态、减轻患者临床症状、改善神经功能，从而达到更为理想的治疗效果。

目的

观察疏脑解郁汤联合调督解郁灸法-经颅磁刺激（Relieving depression-transcranial magnetic stimulation，rTMS）对卒中后抑郁（Post-stroke depression，PSD）患者康复治疗中的疗效。

方法

选取河北省中医院2021年6月—2023年1月收治的120例PSD患者，使用简单随机法分别纳入联合组60例、对照组60例，两组均接受草酸艾司西酞普兰片及调督解郁灸法-rTMS治疗，联合组加用疏脑解郁汤干预，持续4周。对比两组治疗前后汉密尔顿抑郁量表（Hamilton depression scale，HAMD）、中医证候积分、改良爱丁堡-斯堪的纳维亚神经功能缺损量表（Modified edinburg scandinavian stroke scale，MESSS）评分变化，使用Asberg抗抑郁剂不良反应量表评估两组治疗不良反应，并于治疗后评估两组治疗效果。

结果

两组治疗2、4周后HAMD评分、中医证候积分、MESSS量表评分均较治疗前下降，联合组治疗2、4周后HAMD评分、中医证候积分、MESSS量表评分均低于对照组（P<0.05）。联合组、对照组治疗不良反应发生率分别为15.00%（9/60）、18.33%（11/60），组间比较差异无统计学意义（P>0.05）。联合组治疗总有效率为91.67%（55/60），较对照组的76.67%（46/60）显著升高（P<0.05）。

结论

在调督解郁灸法-rTMS治疗的基础上，联合应用疏脑解郁汤能够在不影响治疗安全性的前提下，有效调节患者抑郁状态、减轻患者临床症状、改善神经功能，从而达到更为理想的治疗效果。

Objective

To observe the curative effect of Shunao Jieyu Decoction（疏脑解郁汤）combined with moxibustion of regulating governor and relieving depression-transcranial magnetic stimulation（rTMS）on the rehabilitation of the patients with post-stroke depression（PSD）.

Methods

A total of PSD patients admitted to Hebei Provincial Hospital of Traditional Chinese Medicine from June 2021 to January 2023 were randomly divided into the combined group（60 cases）and the control group（60 cases）. Both groups were treated with Escitalopram Oxalate Tablets and the moxibustion method of regulating Du meridian and relieving depression-rTMS, and the combined group was intervened with Shunao Jieyu Decoction for 4 weeks. The changes of Hamilton Depression Scale（HAMD）, TCMsyndrome scores and modified Edinburgh-Scandinavian stroke scale（MESSS）before and after treatment were compared between the two groups. The side effects of the two groups were evaluated by Asperger＇s anti-depressant side effects scale, and the therapeutic effects of the two groups were evaluated after treatment.

Results

After 2 and 4 weeks of treatment, the HAMD scores, TCMsyndrome scores and MESSS scale scores of the two groups decreased compared with those before treatment, while the scores of the combined group were lower than those of the control group after 2 and 4 weeks of treatment（P<0.05）. The incidence of side effects in the combined group and the control group were 15.00%（9/60）and 18.33%（11/60）, respectively, and there was no statistical difference between the two groups（P>0.05）. The total effective rate of the combined group was 91.67%（55/60）, which was significantly higher than that of the control group（76.67%, 46/60）（P<0.05）.

Conclusion

On the basis of regulating Du and relieving depression moxibustion-rTMS therapy, the combined application of Shunao Jieyu Decoction can effectively regulate the depression, relieve the clinical symptoms and improve the neurological function without affecting the safety of treatment, so as to achieve a more ideal therapeutic effect.
