目的

探究八珍汤加减对脑卒中合并肌少症患者骨骼肌指数和下肢功能的影响。

方法

选择2021年3月—2024年3月收治的脑卒中合并肌少症患者196例，按照随机数字表法将其随机分为研究组和对照组，每组98例。对照组采用日常康复治疗，研究组在对照组基础之上联合口服八珍汤加减治疗。比较治疗前后两组患者中医证候积分、骨骼肌指数及握力变化，采用计时起立-行走（Timed up and go test，TUG）测试评估两组患者治疗前后行走能力，采用Tinetti平衡与步态量表（Tinetti performance oriented mobility assessment，POMA）评估两组患者治疗前后下肢平衡能力，记录两组患者治疗前后下肢运动耐力。

结果

治疗后，两组患者半身不遂、肢体软弱、偏身麻木、手足肿胀、气短乏力、心悸自汗积分均下降（P<0.05），TUG缩短（P<0.05），骨骼肌指数、握力、POMA及症状限制性递增负荷运动试验（Cardiopulmonary exercise testing，CPET）的最大摄氧量（Maximal oxygen uptake，VO₂ max）、最大功率、力竭时间上升（P<0.05），且研究组患者半身不遂、肢体软弱、偏身麻木、手足肿胀、气短乏力、心悸自汗积分较对照组更低（P<0.05），TUG更短（P<0.05），骨骼肌指数、握力、POMA及CPET的VO₂ max、最大功率、力竭时间更大（P<0.05）。

结论

八珍汤加减治疗脑卒中合并肌少症疗效确切，能够有效提升患者肌力水平及下肢的行走、平衡功能，提高患者运动耐力。

Objective

To explore the influence of Modified Bazhen Decoction (八珍汤加减) on skeletal muscle index and lower limb function in patients with stroke complicated with sarcopenia.

Methods

A total of196 stroke patients with sarcopenia in the hospital were selected from March 2021 to March 2024, and were randomly divided into study group and control group by using the random number table method, with 98 cases in each group. The control group was treated with daily rehabilitation therapy, while the study group was combined with oral administration of Modified Bazhen Decoction on the basis of the control group. The changes of TCMsyndromes scores, skeletal muscle index and grip strength were compared between groups before and after treatment. Timed Up and Go test (TUG) was used to evaluate the walking ability of the two groups before and after treatment. Tinetti Performance Oriented Mobility Assessment (POMA) was adopted to evaluate the lower limb balance ability of the two groups before and after treatment, and the lower limb exercise tolerance in the two groups was recorded before and after treatment.

Results

After treatment, the scores of hemiplegia, limb weakness, hemianesthesia, hand and foot swelling, shortness of breath, and palpitation and spontaneous sweating in the two groups were decreased (P<0.05), the TUG was shortened (P<0.05), and the skeletal muscle index, grip strength, POMA and maximum oxygen uptake (VO₂ max), maximum power and the exhaustion time of Cardiopulmonary Exercise Testing (CPET) were increased (P<0.05). The scores of hemiplegia, limb weakness, hemianesthesia, hand and foot swelling, shortness of breath, and palpitation and spontaneous sweating in the study group were lower than those in the control group (P<0.05), the TUG was shorter (P<0.05), and the skeletal muscle index, grip strength, POMA and VO₂ max, maximum power and the exhaustion time of CPET were higher (P<0.05) .

Conclusion

Modified Bazhen Decoction has the exact efficacy in the treatment of stroke with sarcopenia. It can effectively improve the muscle strength level and lower limb walking function and balance function, and enhance the exercise tolerance of patients.
