Jujuboside B(JuB),one of themain constituents of herbal medicine Suanzaoren(Ziziphi Spinosae Semen),possess various pharmacological activities,including anti-inflammatory,antiplatelet and anti-restenosis potentials on the cardiovascular system.However,anti-arrhythmia and underlying mechanisms of JuB are uncertain.This study explored the effects of JuB on the sodium currents(INa)in rat ventricular myocytes using whole-cell patch-clamp techniques and molecular docking studies.The whole-cell patch-clamp technique showed that the current-voltage(Ⅰ-Ⅴ)curves of INa shifted along the positive axis in a dose-dependent manner with JuB.Besides,the administration of JuB caused the steady-state activation curve to shift positively along the voltage axis,i.e.,a depolarization trend but the steady-state inactivation curve of INa shifted negatively,which represented that JuB suppressed the opening process of the Na+channel and may had a strong affinity for binding to sodium channels during inactivation.Themolecular docking study exhibited that JuB was mainly docked in the trans membrane helices of voltage-sensing domain(VSD)ⅢandⅣ of rNav1.5 and rNav1.5/QQQ.The study preliminarily confirmed that JuB can inhibited voltage-gated sodium channel of cardiacmyocytes for the first time.

酸枣仁皂苷B(Jujuboside B,JuB)是安神药酸枣仁的主要成分之一,具有抑制血小板聚集和抗血管再狭窄作用等多种药理活性。然而,JuB的抗心律失常作用及其潜在机制尚不明确。研究通过全细胞膜片钳技术和分子对接研究探讨了JuB对大鼠心室肌细胞钠电流(sodium current,INa)的影响。结果 显示,JuB导致INa的电流-电压(current-voltage,Ⅰ-Ⅴ)曲线上移,激活曲线沿电压轴正向即膜电位的去极化趋势移动,稳态失活曲线向超极化方向移动,延长了从失活到恢复所需的时间,表示JuB抑制了Na+通道的开放过程,可能在失活期间对钠通道具有较强的结合亲和力。分子对接研究显示,JuB主要结合在rNav1.5和rNav1.5/QQQ的电压感应域(voltage-sensing domain,VSD)Ⅲ和Ⅳ的跨膜螺旋结构中。研究首次初步证实JuB能够呈浓度依赖性抑制大鼠心室肌细胞INa

酸枣仁皂苷B(Jujuboside B,JuB)是安神药酸枣仁的主要成分之一,具有抑制血小板聚集和抗血管再狭窄作用等多种药理活性。然而,JuB的抗心律失常作用及其潜在机制尚不明确。研究通过全细胞膜片钳技术和分子对接研究探讨了JuB对大鼠心室肌细胞钠电流(sodium current,INa)的影响。结果 显示,JuB导致INa的电流-电压(current-voltage,Ⅰ-Ⅴ)曲线上移,激活曲线沿电压轴正向即膜电位的去极化趋势移动,稳态失活曲线向超极化方向移动,延长了从失活到恢复所需的时间,表示JuB抑制了Na+通道的开放过程,可能在失活期间对钠通道具有较强的结合亲和力。分子对接研究显示,JuB主要结合在rNav1.5和rNav1.5/QQQ的电压感应域(voltage-sensing domain,VSD)Ⅲ和Ⅳ的跨膜螺旋结构中。研究首次初步证实JuB能够呈浓度依赖性抑制大鼠心室肌细胞INa

Jujuboside B(JuB),one of themain constituents of herbal medicine Suanzaoren(Ziziphi Spinosae Semen),possess various pharmacological activities,including anti-inflammatory,antiplatelet and anti-restenosis potentials on the cardiovascular system.However,anti-arrhythmia and underlying mechanisms of JuB are uncertain.This study explored the effects of JuB on the sodium currents(INa)in rat ventricular myocytes using whole-cell patch-clamp techniques and molecular docking studies.The whole-cell patch-clamp technique showed that the current-voltage(Ⅰ-Ⅴ)curves of INa shifted along the positive axis in a dose-dependent manner with JuB.Besides,the administration of JuB caused the steady-state activation curve to shift positively along the voltage axis,i.e.,a depolarization trend but the steady-state inactivation curve of INa shifted negatively,which represented that JuB suppressed the opening process of the Na+channel and may had a strong affinity for binding to sodium channels during inactivation.Themolecular docking study exhibited that JuB was mainly docked in the trans membrane helices of voltage-sensing domain(VSD)ⅢandⅣ of rNav1.5 and rNav1.5/QQQ.The study preliminarily confirmed that JuB can inhibited voltage-gated sodium channel of cardiacmyocytes for the first time.

心血管疾病是全球主要死因之一[1]。正常的心脏功能与心肌细胞离子通道的开放和关闭以及调节跨膜离子的电位差相关[2]。大量的研究已经证实,通过心肌细胞膜离子通道的电流异常是诱发心律失常的重要因素[3],与心血管疾病的死亡率和发病率的增加[4]。心肌细胞膜的跨膜蛋白电压门控钠离子通道负责心脏的膜去极化和动作电位传导,并在维持正常心脏节律中发挥重要作用[5-7]。心肌细胞钠通道的功能异常可能导致各种心脏的病理生理异常[8-10],包括心室重塑、心肌损伤、心律失常和其他心脏疾病[11]。由钠电压门控通道α亚基5(sodium voltage-gated channel alpha subunit 5,SCN5A)基因编码的NaV 1.5被认为是心肌细胞膜表达的主要电压门控钠通道,并传导内向去极化钠电流(sodium current,INa[12-15]。心肌细胞NaV 1.5的功能异常是触发心肌病和心律失常的主要因素之一[12]
药物疗法是治疗心律失常的主要方法。当前抗心律失常药物的基本药理效应是通过影响心肌细胞膜的离子通道功能,改变离子电流并影响细胞的电生理特性,从而抑制心律失常的发生[16]。代表I类抗心律失常药物的Na+通道阻滞剂通过降低心脏组织的兴奋性发挥抗心律失常作用,从而防止异常的自发动作电位。然而,抗心律失常药物的疗效由于其潜在的严重不良反应而受到限制[17-18]。因此,研发更安全、更有效的抗心律失常药物仍然是治疗心律失常的重要方向。大量的研究在传统中药中寻找抗心律失常药物,包括针对INa的效应[19-20]
酸枣仁是鼠李科植物酸枣的干燥成熟种子,广泛用于治疗心悸、焦虑、失眠和多汗等症状。酸枣仁皂苷B(Jujuboside B,JuB)是酸枣仁的主要生物活性皂苷成分之一,具有镇静催眠[21],抗热性惊厥[22],抗炎[23]和保肝功能[24]。JuB分子式为C52 H84 O21图1)。一些研究已经证明了JuB在心血管系统方面的药理作用,包括抗血小板[25]、抗血管再狭窄[26]、保护血管内皮功能和降低血管张力作用[27]。最近的一项研究表明,JuB通过降低血管内皮生长因子受体2及其下游信号通路的激活,抑制血管新生和肿瘤生长,具有良好的耐受性和低毒性[28]。然而,JuB对心律失常的影响仍然是未知的。在这项研究中,我们通过全细胞膜片钳技术和分子对接分析初步得出JuB是大鼠心肌细胞电压门控钠通道的潜在抑制剂。
Sprague-Dawley大鼠,体质量(200~300)g,雌雄不限,由扬州大学比较医学中心提供,动物合格证[SYXK(苏)2017-0044)]。本实验通过扬州大学实验动物伦理委员会伦理审查,编号:YXYL-2022-135。
JuB(CAS号55466-05-2,纯度≥98%,成都埃法生物科技有限公司,批号AB0608)溶解在二甲基亚砜(DMSO)中,配制成浓度为25、50、100、200μmol/L的溶液;葡萄糖、肝素钠、L-谷氨酸(北京索莱宝科技有限公司,批号分别为405A096、301A028、405A096);牛磺酸(Taurine)、氯化铯(CsCl)、四乙基氯化铵(TEA-Cl)、腺苷5′-三磷酸镁盐(Mg-ATP)、4-羟乙基哌嗪乙磺酸(HEPES)(美国Sigmaaldrich公司,批号分别为101282014、101368097、101394223、1001571838、1001621577);河豚毒素(Tetrodotoxin,TTX)(Affix公司,批号AF02);牛血清白蛋白(bovine serum albumin,BSA,美国ICN公司,批号:8398F);乙二醇-双四乙酸(EGTA)(Fluka公司,批号101257840);胶原酶Ⅱ(美国Worthington公司,批号:44N15308B)。
细胞保存液(Kreb's,KB液)的配制如下(mmol/L):葡萄糖11.0、EGTA 1.0、L-谷氨酸70.0、牛磺酸10.0、NaH2 PO4 10.0、KCl25.0、KOH 89.0和HEPES 5.0,使用KOH调整溶液至pH 7.4。无钙台氏液的成分如下(mmol/L):葡萄糖10.0、MgCl2 1.0、NaH2 PO4 0.33、KCl 5.4、NaCl 135和HEPES 5.0,使用NaOH调整溶液至pH 7.3。有钙台氏液:向无钙台氏液中加入CaCl2(1.8 mmol/L)。电极内液(mmol/L):HEPES 10.0、EGTA 10.0、TEA-Cl20.0、NaCl 5.0、CsCl 133.0和Mg ATP 5.0,用CsOH调pH至7.30。CsOH(99%)用于调整溶液至pH 7.40。细胞外液(mmol/L):CdCl2 0.1、CsCl 5.4、CaCl2 1.8、葡萄糖10.0、NaCl135.0、MgCl2 1.0和HEPES 5.0,使用NaOH将溶液调整至pH 7.40。酶溶液:无钙台氏液中加入1 g/L BSA、0.4 g/L牛磺酸、0.4 g/L胶原酶Ⅱ。
EPC-10 USB/Patchmaster单通道膜片钳放大器(德国HEKA公司);IX73倒置科研级显微镜(日本Olympus公司);MP-225显微操作器系统、P-97微电极拉制仪(美国Sutter公司)。
SD大鼠心室肌细胞的急性分离[29]操作步骤中,先在大鼠腹腔注射肝素(2000 IU/kg),10 min后,使用2%的戊巴比妥钠(40 mg/kg)腹腔注射进行麻醉。随后,立即固定大鼠并切开胸腔,取出心脏并置于4℃的无钙台氏液中,修剪多余的组织,将心脏固定在Langendorff灌流装置中。通过有钙台氏液灌注心脏30 s,接着,给心脏灌注无钙台氏液,直到残余的血液被洗掉。大约经过25 min的100%氧饱和酶溶液灌流后,心脏变软并膨大。当流出物黏稠混浊、滴速较快时,终止消化。取出心室组织,解剖,并通过100目滤网过滤。所获得的单个心室肌细胞在KB溶液中洗涤3次,并在室温下保存10 min,待细胞沉淀后除去上清液。经过30 min的充氧后,静置2 h。
将细胞悬浮液放入含有细胞外液的培养皿中,静置,待细胞贴壁后,用细胞外液进行灌流以除去残存的KB液。具有光滑完整边缘、清晰横纹和立体外观且没有任何自发性收缩和震颤的细胞被认为处于良好状态,适合进行后续实验。用单通道膜片钳放大器记录INa
在单刺激方案中,在电压钳模式下,保持电位-80 mV,给予-30 mV、持续30 ms时间的方波刺激。在电压钳模式下,保持电位-80mV,给予-70~50mV、阶跃5mV、持续时间30 ms的方波串刺激,0.5 Hz刺激频率。以膜电位为横坐标,峰电流为纵坐标绘制出INa的电流-电压(current-voltage,Ⅰ-Ⅴ)关系曲线。
激活电流刺激方案中,在电压钳模式下,钳制电位-80 mV,给予-80~50 mV、阶跃5 mV、0.5 Hz频率、持续时间30 ms的方波串刺激。
在电压钳模式下,采取双脉冲刺激方案记录INa失活曲线,保持电位-80 mV,条件脉冲从-140~40 mV,阶跃10 mV,持续时间50 ms去极化心室细胞,然后再给予条件脉冲至-30 mV,持续时间25 ms。
双脉冲刺激方案用于检测INa失活后的恢复曲线,保持电位-80mV,给予去极化至-30mV的刺激,持续30ms,回到保持电位持续5 ms后,再给予一系列条件脉冲至-30 mV,持续25ms,脉冲总数为15,两个脉冲之间的设定间隔时间逐渐增加10 ms。
在有机小分子生物活性数据PubChem化合物数据库中查询天然化合物的化学结构(http://pubchem.ncbi.nlm.nih.gov/),JuB的化合物编号(compound identity document,CID)为24721031,从蛋白数据库(Protein Data Bank,PDB)(http://www.rcsb.org/pdb)获取大鼠电压门控钠通道NaV 1.5(PDB ID:6UZ3)和rNav1.5/QQQ(PDB ID:7FBS)的晶体结构。使用AutoDock-Vina软件进行分子对接计算。配体和蛋白质的pdbqt文件均按照AutoDock方案进行分子对接计算。除了最大能量评估次数和遗传算法运行次数(genetic algorithm,GA)外,所有对接参数都保持在默认值。对接网格作为受体的结合位点,网格大小为40Å×40 Å×40Å。网格间距值被调整为0.375Å。PyMol(https://pymol.org)用于可视化,以获得氢键、疏水和静电相互作用[30]。使用分子力学广义波恩表面积(molecular mechanics/generalized born surface area,MM/GBSA)结合自由能计算方法,打分函数为MMGBSAΔG Bind[31]
实验数据使用Origin 7.0软件(Microcal Software)进行分析,使用GraphPad Prism 8软件进行曲线拟合。数据以均值标准偏差SD表示。使用t检验和方差分析(analysis of variance,ANOVA)进行均值比较。当P<0.05时被认为具有显著统计学意义。
在短时间内诱导产生内向电流,然后在短时间内达到最大电流。当外部溶液中加入选择性Na+通道阻滞剂10μmol/L TTX时,电流明显受到抑制。在用外部溶液灌洗去除TTX后,电流在一定程度上恢复,表明在刺激方案中诱发的电流是INa
监测和记录JuB和胺碘酮给药前后的INa结果,以研究JuB对不同电压条件下Na+通道电流的影响。JuB在25、50和100 μmol/L浓度下以浓度依赖性的方式抑制了INa。然而,与胺碘酮相比,200μmol/L的JuB显示出更强的抑制作用。为了防止过度抑制引起的心律失常,选择了25、50和100μmol/L JuB用于后续实验。如插页ⅩⅨ图2所示,记录了JuB和胺碘酮给药后Na+通道的电流及其随电压的变化。电流值以电流密度表示,即通过细胞膜的电流幅度除以细胞膜的面积,单位为皮安培/皮法拉(pA/pF)。25、50和100μmol/LJuB以及24.24μmol/L胺碘酮导致Na+通道Ⅰ-Ⅴ曲线向上移动和内向电流的减小,而不影响曲线形状。在施加电压约为-40 mV时,每个测试电压条件下电流密度显著减小。
监测和记录胺碘酮和JuB给药前后的INa变化(插页ⅩⅩ图3A、B),并绘制INa的激活曲线(插页ⅩⅩ图3C),以研究JuB对Na+通道激活和开放的影响。不同浓度的JuB和胺碘酮都导致稳态激活曲线向右移动,即去极化方向,并导致通道半数激活电压(half activation voltage,V1/2-ac)的增加。25、50和100μmol/L浓度的JuB分别使V1/2-ac值从(-53.22±1.30)mV(给药前)变为-50.50±1.17 mV、(-48.59±0.81)mV、(-44.88±1.29)mV(n=6;P<0.05)。因此,JuB可以提高钠通道激活的阈值。
插页ⅩⅩ图4A、B展示了在添加不同浓度JuB和胺碘酮前后获得的诱导INa失活电流的结果。电流与最大电流的比值用相对电流I/Imax表示,作为纵坐标,膜电位为横坐标,通过Boltzmann方程进行拟合稳态失活曲线:
k和V1/2分别代表失活曲线的斜率因子和通道半数失活时的条件脉冲电压(half inactivation voltage,V1/2)。
不同浓度JuB和胺碘酮均导致失活曲线向左移动,即向超极化方向移动(插页ⅩⅩ图4C)。JuB在25、50和100μmol/L浓度下,V1/2从(-72.96±1.08)mV减少到(-81.63±0.76)mV,(-86.14±1.56)mV,(-94.62±1.20)mV(n=6;P<0.05)。因此,JuB能够加速Na+通道失活。
插页ⅩⅪ图5A、B展示了胺碘酮和不同浓度JuB添加前后的INa恢复电流。插页ⅩⅪ图5C展示了INa失活后的恢复曲线。通过拟合单相关联方程来构建INa失活后的恢复曲线:
τ和t分别表示失活后再激活的时间常数和两个脉冲之间的时间间隔。在JuB(25、50和100μmol/L)的作用下,τ值分别从(18.18±1.13)ms变为(20.72±1.41)ms,(25.86±1.65)ms,(30.13±1.70)ms(n=6,P<0.05)。因此,JuB改变了INa失活后恢复的动力学,并延长了INa从失活到激活所需的时间。
使用分子对接方法探讨了JuB与大鼠电压门控钠通道(rNav1.5)和 rNav1.5 开放态结构(rNav1.5/QQQ)的相互作用(插页ⅩⅪ图6表1)。JuB与rNav1.5和rNav1.5/QQQ的亲和力结合能分别为-6.502 kcal/mol(吉布斯自由能为-64.75 kcal/mol)和-6.722 kcal/mol(吉布斯自由能量为-30.38 kcal/mol)。其中,JuB-rNav1.5的对接模式存在7个氢键,分别结合在Gly1717、Ala1688、Gly320、Arg896、Asp1425、Ser1720和Gln1416氨基酸残基,而JuB与rNav1.5/QQQ 在Thr321、Gly220、Ala1688、Glu1687、Lys1401和Asp908位点处形成氢键(插页ⅩⅪ图6表1)。
心脏钠通道在膜去极化的过程中迅速激活,导致Na+向内跨膜传导,心脏动作电位上升,以调节心脏组织动作电位的均匀性和传播速率[32]。心脏离子通道的紊乱属于离子通道病,可能导致心律失常和心脏结构异常,目前被认为是心血管疾病的重要危险因素以及致病机制[33]。在生理条件下,钠通道的失活和激活机制受到严格控制,钠通道的功能障碍可能对心律失常的发生和心脏电生理特性产生深远影响[9-10]。事实上,电压门控钠通道已经成为许多常用抗心律失常药物的靶点。心脏钠通道的高分辨率结构解析有助于研究心律失常的发病机制、离子选择性、电压依赖性激活以及抗心律失常药物在分子水平上的作用机制研究[14]。在哺乳动物中,NaV 1.5是用于启动和传递电信号的主要心脏钠通道亚型。rNav1.5通道的结构包括α-亚单位的Ⅰ、Ⅱ、Ⅲ、IFM基序和Ⅳ区域,以及β1和β2亚单位[34]。rNav1.5的开放态结构是通过引入IFM/QQQ突变来实现的,以去除快速失活门[35]。更具体地说,rNav1.5和rNav1.5/QQQ电压感应域(voltage-sensing domain,VSD)Ⅲ和Ⅳ的跨膜α螺旋已被确定为影响通道功能的核心区域[34-35]。因此,分析离子通道功能,揭示潜在的生物分子机制,并识别特定的靶向药物对于未来心脏保护和抗心律失常药物的研发至关重要。Ⅰ类抗心律失常药物的主要靶标是Nav1.5,其功能是阻断心脏动作电位的形成和传导[36]。膜片钳技术经常被用于研究细胞膜的详细电生理特性,尤其是膜电位随着药物作用的变化。因而,本论文使用原代分离的大鼠心肌细胞进行离子通道蛋白功能的研究[2037]。JuB已被广泛认为具有多种药理特性,包括心血管方面的功效[25-28]。目前,关于JuB对心肌细胞离子通道影响的研究较少,本论文初步验证了JuB对电压门控Na+通道的抑制作用,并且利用新鲜分离的大鼠心肌细胞展示了其潜在的抗心律失常机制。
在这项研究中,我们研究了不同浓度的JuB对SD大鼠心肌细胞的激活、失活和恢复动力学影响。JuB对INa的影响在不同的膜电位水平上变化。结果 显示:JuB能够抑制INa,引起Ⅰ-Ⅴ曲线上移,但没有影响曲线形状(插页ⅩⅨ图2);JuB对INa激活动力学的抑制作用表明JuB阻滞了Na+的流入(插页ⅩⅩ图3);在JuB的作用下,失活曲线向左移动(插页ⅩⅩ图4),而恢复曲线向右移动(插页ⅩⅪ图5)。膜片钳实验结果表明,JuB减缓了Na+通道的激活阶段,减少了通道开放程度,加速了失活阶段,并延长了Na+通道从失活到静息状态的恢复阶段,导致INa在心室复极时的恢复动力学延迟。影响作用于不应期可以降低Na+通道的间隙兴奋性并抑制快速心律失常,这与Ⅰ类抗心律失常药物中Na+通道阻滞剂的特性相似。Ju对钠通道的整体抑制作用表明其在未来具有很好地治疗应用前景。分子对接研究表明,JuB与rNav1.5的VSDⅢ的跨膜螺旋结构在Asp1425和Glu1416位点处形成2个氢键,另外3个氢键,即Gly1717、Ala1688和Ser1720,结合在VSDⅣ区域。JuB与rNav1.5/QQQ中VSDⅢ的Lys1410和VSDⅣ的Ala1688和Glu1687位点形成氢键(插页ⅩⅪ图6)。计算模型的结果进一步验证了JuB对大鼠心肌细胞电压门控Na+通道的抑制作用。
我们的研究初步表明,JuB影响心肌细胞电压门控钠通道的电流、减缓激活、加速失活,延长从失活到激活的恢复时间,提示JuB可能作为潜在的新型Ⅰ类抗心律失常和心肌保护药物的治疗前景。然而,理想的抗心律失常药物应该影响多离子通道电流,并同时调节相应的通道基因和蛋白表达水平,以恢复离子通道功能的平衡从而使异常的动作电位恢复正常。所以,后续应全面地探索JuB对心脏其他离子通道特异性调节作用的电生理机制和在体实验数据的研究。
