Objective

To explore the intervention and mechanism of YiqiBushen Tongluo Decoction(益气补肾通络方)in the treatment of varicocele infertility inmice.

Methods

A total of60 Sprague Dawley(SD)rats were selected and divided into sham operation group,model group,Yiqi Bushen Tongluo Decoction low dose group,medium dose group and high dose group according to random number table method,with 12 cases in each group.The drug was given by intragastric administration 48 h after the modeling was completed and continuously used for60 d.The testis and epididymis quality,sperm quality,spermatogenic cell apoptosis index and the levels of malondialdehyde(MDA),superoxide dismutase(SOD)and nitric oxide(NO)in testicular tissue were compared among all groups.

Results

The testis quality,epididymal quality,sperm concentration and sperm motility in the model group were lower than those in the sham operation group,and the spermatogenic apoptosis index was higher than those in the sham operation group,with statistical significance(P<0.05).Compared with those in the model group,the testis quality,epididymal quality,sperm concentration and sperm motility were significantly increased in the Yiqi Bushen Tongluo Decoction low dose,medium dose and high dose groups,and the spermatogenic apoptosis indexwas significantly decreased,with statistical significance(P<0.05).The levels of MDA and NO in the model group were higher than those in the sham operation group,and the SOD level was lower than that in the sham operation group,with statistical significance(P<0.05).Compared with those in the model group,the levels of MDA and NO were significantly decreased in the Yiqi Bushen Tongluo Decoction low dose,medium dose and high dose groups,and the SOD level was significantly increased,with statistical significance(P<0.05).

Conclusion

Yiqi Bushen Tongluo Decoction can improve the sperm quality of mice with varicocele infertility.Itsmechanism of action is mainly related to inhibiting spermatogenic cell apoptosis,anti-oxidative stress and reducing NO level.

目的

探讨益气补肾通络方对小鼠精索静脉曲张性不育症的干预及作用机制。

方法

选择SD(Sprague Dawley)大鼠共计60只,按照随机数表法分为假手术组、模型组及益气补肾通络方低剂量组、中剂量组、高剂量组,每组12例,造模完成后48 h开始灌胃给药,连续使用60 d。比较各组大鼠睾丸、附睾质量、精子质量、生精细胞凋亡指数及睾丸组织丙二醛(Malondialdehyde,MDA)、超氧化物歧化酶(Superoxide dismutase,SOD)、一氧化氮(Nitric oxide,NO)的表达。

结果

模型组睾丸质量、附睾质量、精子浓度、精子活力均低于假手术组,生精细胞凋亡指数高于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组睾丸质量、附睾质量、精子浓度、精子活力明显增加,生精细胞凋亡指数明显降低,均有统计学意义(P<0.05);模型组MDA、NO均高于假手术组,SOD低于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组MDA、NO均明显降低,SOD明显升高,均有统计学意义(P<0.05)。

结论

益气补肾通络方可改善小鼠精索静脉曲张性不育症的精子质量,其作用机制主要与抑制生精细胞凋亡、抗氧化应激、降低NO水平相关。

目的

探讨益气补肾通络方对小鼠精索静脉曲张性不育症的干预及作用机制。

方法

选择SD(Sprague Dawley)大鼠共计60只,按照随机数表法分为假手术组、模型组及益气补肾通络方低剂量组、中剂量组、高剂量组,每组12例,造模完成后48 h开始灌胃给药,连续使用60 d。比较各组大鼠睾丸、附睾质量、精子质量、生精细胞凋亡指数及睾丸组织丙二醛(Malondialdehyde,MDA)、超氧化物歧化酶(Superoxide dismutase,SOD)、一氧化氮(Nitric oxide,NO)的表达。

结果

模型组睾丸质量、附睾质量、精子浓度、精子活力均低于假手术组,生精细胞凋亡指数高于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组睾丸质量、附睾质量、精子浓度、精子活力明显增加,生精细胞凋亡指数明显降低,均有统计学意义(P<0.05);模型组MDA、NO均高于假手术组,SOD低于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组MDA、NO均明显降低,SOD明显升高,均有统计学意义(P<0.05)。

结论

益气补肾通络方可改善小鼠精索静脉曲张性不育症的精子质量,其作用机制主要与抑制生精细胞凋亡、抗氧化应激、降低NO水平相关。

Objective

To explore the intervention and mechanism of YiqiBushen Tongluo Decoction(益气补肾通络方)in the treatment of varicocele infertility inmice.

Methods

A total of60 Sprague Dawley(SD)rats were selected and divided into sham operation group,model group,Yiqi Bushen Tongluo Decoction low dose group,medium dose group and high dose group according to random number table method,with 12 cases in each group.The drug was given by intragastric administration 48 h after the modeling was completed and continuously used for60 d.The testis and epididymis quality,sperm quality,spermatogenic cell apoptosis index and the levels of malondialdehyde(MDA),superoxide dismutase(SOD)and nitric oxide(NO)in testicular tissue were compared among all groups.

Results

The testis quality,epididymal quality,sperm concentration and sperm motility in the model group were lower than those in the sham operation group,and the spermatogenic apoptosis index was higher than those in the sham operation group,with statistical significance(P<0.05).Compared with those in the model group,the testis quality,epididymal quality,sperm concentration and sperm motility were significantly increased in the Yiqi Bushen Tongluo Decoction low dose,medium dose and high dose groups,and the spermatogenic apoptosis indexwas significantly decreased,with statistical significance(P<0.05).The levels of MDA and NO in the model group were higher than those in the sham operation group,and the SOD level was lower than that in the sham operation group,with statistical significance(P<0.05).Compared with those in the model group,the levels of MDA and NO were significantly decreased in the Yiqi Bushen Tongluo Decoction low dose,medium dose and high dose groups,and the SOD level was significantly increased,with statistical significance(P<0.05).

Conclusion

Yiqi Bushen Tongluo Decoction can improve the sperm quality of mice with varicocele infertility.Itsmechanism of action is mainly related to inhibiting spermatogenic cell apoptosis,anti-oxidative stress and reducing NO level.

精索静脉曲张在男性中较为常见,可导致患者出现进行性的睾丸功能衰退,引起睾丸体积降低、精液质量异常等,是造成男性不育的重要疾病[1-2]。精索静脉曲张性不育症的相关作用机制较为复杂,但多数研究均认为氧化应激、细胞凋亡、一氧化氮(Nitric oxide,NO)毒性损伤等在其中占据着重要地位[3-4]。近年来,关于中药治疗精索静脉曲张性不育症的疗效也受到较多学者关注,中医学认为,该病与血瘀、气虚、肾虚等因素相关,病机表现为虚实夹杂,且兼具热、湿、瘀等,治疗原则应以“益气活血、补肾通络”等治法为主[5-6]。因此,本研究主要观察益气补肾通络方对小鼠精索静脉曲张性不育症的干预作用及机制,现报道如下。
选择无特定病原体(Specific pathogen free,SPF)级雄性SD(Sprague Dawley)大鼠共计60只,许可证号为SCXK(粤)2022-0023,3月龄,体质量150~200 g。所有大鼠均分笼喂养于普通级动物房中,温度控制在20~22℃,湿度为50% ~70%,明暗周期为12 h/12,大鼠全天均自由摄食进水。整个研究遵循实验动物福利3R原则,并经导科医药技术(广东)有限公司伦理道德委员会批准(IACUC-2023-7-29)。
益气补肾通络方组成如下,炙黄芪20 g,当归15 g,丹参15 g,槐花15 g,地榆15 g,牡丹皮15 g,生地黄15 g,熟地黄15 g,枸杞子15 g,覆盆子15 g,王不留行15 g,炙白术15 g,白芍15 g,柴胡10 g,升麻10 g,甘草10 g。药材均由本院中药房所提供,煎煮、过滤药材后,置于4℃的冰箱中储存备用。
将60例大鼠按照随机数表法分为假手术组、模型组及益气补肾通络方低剂量组、中剂量组、高剂量组,每组12例。采用Turner[7]创建的狭窄左肾静脉法进行造模,假手术组小鼠仅分离肾静脉不进行结扎,其余小鼠部分结扎左肾静脉、左肾总静脉的交通支。造模成功标准为:将大鼠剖腹,和右侧的精索静脉进行比较,左侧直径有迂回、扩张表现,且大鼠左肾无缺血、萎缩情况。本实验所有大鼠均造模成功。
假手术组、模型组使用10 mL/kg的双蒸水进行灌胃;益气补肾通络方的给药剂量根据人与大鼠的体表面积,换算成临床成年人用量的3、6、12倍,即低剂量组为6.3 g/kg、中剂量组为12.6 g/kg、高剂量组为25.2 g/kg,使用对应剂量的混悬液进行灌胃,给药体积为10 mL/kg,1次/d,连续使用60 d。
所有大鼠给药结束并禁食12 h后,断头处死大鼠,迅速取出大鼠的左侧睾丸、附睾,称重后置于液氮中储存备检。其中一半睾丸用于测定睾丸及附睾质量、精子质量、生精细胞测定,另一半用于丙二醛(Malondialdehyde,MDA)、超氧化物歧化酶(Superoxide dismutase,SOD)、NO的测定。
摘除大鼠左侧睾丸及附睾,称重质量。
将左侧附睾剪碎后,放置于生理盐水中进行持续孵育,时间15 min,直至精子完全游出,将精子悬液混匀后,采用精液质量分析系统,分析精子浓度及活力;并采用TUNEL细胞凋亡试剂盒,测定睾丸组织中的生精细胞凋亡指数。
将睾丸组织匀浆并在冰上静置,采用南京建成生物研究所的试剂盒测定氧化应激指标(MDA、SOD)、NO表达。
应用SPSS 26.0软件包进行数据分析,计量资料以均数±标准差()表示,多组比较采用单因素方差分析,两两比较采用最小显著差异(Least significant difference,LSD)-t检验,计数资料以率表示,χ2检验,以P<0.05表示差异具有统计学意义。
益气补肾通络方低剂量组、中剂量组、高剂量组睾丸质量、附睾质量均比模型组更高,均有统计学意义(P<0.05)。见表1
模型组精子浓度、精子活力均低于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组精子浓度、精子活力明显增加均有统计学意义(P<0.05)。见表2
模型组生精细胞凋亡指数高于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组生精细胞凋亡指数明显降低,均有统计学意义(P<0.05)。见表3
模型组MDA均高于假手术组,SOD低于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组MDA均明显降低,SOD明显升高,均有统计学意义(P<0.05)。见表4
模型组NO均高于假手术组,均有统计学意义(P<0.05),与模型组相比,益气补肾通络方低剂量组、中剂量组、高剂量组NO均明显降低,均有统计学意义(P<0.05)。见表5
精索静脉曲张患者可表现出睾丸疼痛及睾丸功能障碍,从而降低精子质量,最终造成不育[8-9]。该病在男性人群中的患病率达到15%,不育的男性人群中,由于精索静脉曲张所致的患者则高达45%,对患者生活质量、心理健康等均有着较多不良影响[10]
精索静脉曲张不育症确切发病机制涉及到较多因素,研究显示,正常的精子在生成过程中,生精细胞增殖、凋亡过程之间表现较为平衡,但在精索静脉曲张患者中,在较多因素的刺激下可促使生精细胞凋亡率增加,造成少精、弱精症,继而导致男性生殖能力出现障碍[11-12]。也有报道显示,在精索静脉曲张患者中存在着明显的氧化应激失衡等表现,可直接影响到精子功能,导致生殖细胞凋亡[13]。而通过积极抗氧化应激在改善男性精索静脉曲张不育症中有重要价值[14]。NO也是近年来研究较多的一种效应分子,当患者发生精索静脉曲张时,随着血液淤积可对血管内皮细胞造成刺激,睾丸细胞中可生成大量的一氧化氮合酶(Nitric oxide synthase,NOS),导致NO水平增加[15-16]。研究也显示,NO对精子质量存在着双重影响,低水平的NO对正常生精具有保护作用,而当NO浓度升高,可损害正常细胞,导致生精细胞数量降低,并抑制精子活动度,从而对生育能力产生影响[17-18]
中医学指出,精索静脉曲张性不育症属于“不育”“筋瘤”等范畴,肾虚血瘀是该病发生的主要病机,属本虚标实之证,患者先天禀赋不足,致肾失所养,影响气血经脉运行,内生瘀血,而“肾”主生殖,可诱发不育,因此在治疗原则中应使用“益气活血、补肾通络”等方针[19-20]。本研究所采用的益气补肾通络方中,黄芪益气升阳,当归、丹参养血活血,化瘀通络,具有祛瘀不伤正的特点,牡丹皮、槐花、地榆凉血活血以祛除血中瘀热,枸杞子覆盆子、生熟地共同益肾生精,王不留行理气活血直达病所,柴胡疏肝行气,升麻益气升提、升举阳气,甘草调和诸药,全方符合精索静脉曲张性不育症的中医原则。
本研究结果显示,随着益气补肾通络方剂量的升高,大鼠睾丸组织中MDA、NO逐渐降低,SOD逐渐升高,且大鼠睾丸质量、附睾质量、精子浓度、精子活力明显提高,且生殖细胞凋亡指数也明显增加,可见益气补肾通络方在改善氧化应激、抑制NO过度生成、改善精子质量中效果明显,并具有剂量依赖性。经分析是由于,黄芪、当归能抑制诱导型NOS的表达,发挥保护血管内皮作用,有效调节NOS水平,两者配伍亦具有协同效应[21-22]。丹参中的丹酚酸B可加强SOD活性,并降低MDA含量,对机体内多余的氧自由基成分有清除作用,继而发挥抗氧化应激效应,并能改善血管内皮活力[23-24]。枸杞子中的枸杞多糖、多酚类成分均具有抗氧化效果,能调节MDA、SOD水平[25-26]。同时其有效成分能减轻睾丸组织损伤,促进生殖器官功能恢复,继而提高精子活力及数量[27-28]。白术中的挥发油成分具有较强的清除机体自由基能力,能降低脂质过氧化物的产生,抗氧化能力明显,且可能是通过抗氧化应激途径,抑制睾丸细胞凋亡[29-30]。且白芍、柴胡中的有效成分也具有较好的抗氧化活性[31-32]。由此可见,益气补肾通络方可通过抗氧化应激等途径改善患者精子质量。
综上所述,益气补肾通络方可改善小鼠精索静脉曲张性不育症的精子质量,其作用机制主要与抑制生精细胞凋亡、抗氧化应激、降低NO水平相关,本研究也为今后临床治疗该病提供了一定理论依据。
