Objective

To explore the clinical effect of ultrasound-guided needle knife dissection combined with lumbar inclined plate method in the treatment of gluteal epithelial nerve compression syndrome based on the theory of“bone alignment and tendon flexibility”.

Method

Sixty patients with gluteal epithelial nerve compression syndrome who received treatment in the Orthopedic Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2023 to January 2024 were selected as the study subjects.Using the random number allocation method,the patients were randomly divided into a control group(30 cases)and an experimental group(30 cases).The control group was treated with ultrasound-guided needle knife dissection,while the experimental group was treated with a combination of ultrasound-guided needle knife dissection and lumbar inclined plate dissection under the guidance of the“bone alignment and tendon flexibility”theory.The treatment period for both groupswas 4 weeks.After treatment,the clinical efficacy,Visual Analog Scale(VAS)score,Oswestry Disability Index(ODI)score,shear wave Young'smodulus values of the lumbar multifidus and psoasmajor muscles,and the changes in surface electromyography were compared between the two groups.

Result

Compared with the control group,the experimental group had a higher clinical total effective rate after 4 weeks of treatment,but there was no statistically significant difference in the total effective rate between the two groups(P>0.05).Compared with those before treatment,VAS scores and ODI scores in both groups showed a decreasing trend after treatment and 4 weeks follow-up(P<0.05).Compared with those of the control group,the VAS score and ODIscore of the experimental group were lower after treatment and 4 weeks follow-up(P<0.05).Compared with those before treatment,the shearwave Young'smodulus values of the bilateral multifidus and lumbar muscles in the experimental group showed a decreasing trend after treatment and 4 weeks follow-up(P<0.05).Compared with those before treatment,there was no statistically significant difference in the shearwave Young'smodulus values of the bilateral multifidus and lumbar muscles in the control group after treatment and 4 weeks follow-up(P>0.05).Compared with the control group,the shearwave Young'smodulus values of the bilateral multifidus and lumbar muscles in the experimental group were lower after treatment and 4 weeks follow-up(P<0.05).Compared with those before treatment,the average amplitude of electromyography on both sides of the multifidus and lumbar muscles in the experimental group showed an increasing trend after treatmentand 4 weeks follow-up(P<0.05).Compared with those before treatment,there was no statistically significant difference in the average amplitude of surface electromyography of the multifidus and psoasmajormuscles in the control group after treatmentand 4 weeks follow-up(P>0.05).Compared with those in the control group,the average amplitude of surface electromyography of the multifidus and psoasmajor muscles in the experimental group was higher after treatment and 4 weeks follow-up(P<0.05).

Conclusion

The combination of ultrasound-guided needle knife dissection method and lumbar inclined plate method based on the concept of“bone alignment and tendon flexibility”can significantly improve the short-term and long-term clinical effects of treating gluteal epithelial nerve compression syndrome,with advantages such as high safety,minimal trauma and rapid recovery.

目的

基于“骨正筋柔”理论探讨超声引导下针刀解结联合腰椎斜板法治疗臀上皮神经卡压综合征的临床疗效。

方法

选取2023年1月—2024年1月,于辽宁中医药大学附属医院骨伤科接受治疗的臀上皮神经卡压综合征患者60例作为研究对象。采用随机数字分配法,随机将患者分为对照组(30例)和试验组(30例)。对照组采取超声引导下针刀解结法治疗,试验组在“骨正筋柔”理论指导下,采用超声引导针刀解结法联合腰椎斜板法进行综合治疗,两组的治疗周期均为4周。治疗结束后,比较两组患者的临床疗效、疼痛视觉模拟评分(Visual analogue pain scale,VAS)、Oswestry功能障碍指数问卷表(Oswestry disability index,ODI)评分、腰部多裂肌及腰大肌的剪切波杨氏模量值及表面肌电图的变化情况。

结果

与对照组相比,试验组治疗后临床总有效率较高,但两组总有效率差异无统计学意义(P>0.05)。与治疗前相比,治疗后、随访4周两组VAS评分、ODI评分呈降低趋势(P<0.05);与对照组相比,治疗后、随访4周试验组VAS评分、ODI评分较低(P<0.05)。与治疗前相比,治疗后、随访4周试验组两侧多裂肌、腰大肌剪切波杨氏模量值呈降低趋势(P<0.05);与治疗前相比,对照组治疗后、随访4周两侧多裂肌、腰大肌剪切波杨氏模量值差异无统计学意义(P>0.05);与对照组相比,治疗后、随访4周试验组两侧多裂肌、腰大肌剪切波杨氏模量值较低(P<0.05)。与治疗前相比,治疗后、随访4周试验组两侧多裂肌、腰大肌表面肌电图平均振幅呈升高趋势(P<0.05);与治疗前相比,对照组治疗后、随访4周两侧多裂肌、腰大肌表面肌电图平均振幅差异无统计学意义(P>0.05);与对照组相比,治疗后、随访4周试验组两侧多裂肌、腰大肌表面肌电图平均振幅较高(P<0.05)。

结论

基于“骨正筋柔”的理念应用超声引导下的针刀解结法与腰椎斜板法相结合治疗臀上皮神经卡压综合征能够显著提升近远期临床疗效,具备安全性高、创伤小、恢复迅速等优势。

选取2023年1月—2024年1月,于辽宁中医药大学附属医院骨伤科接受治疗的臀上皮神经卡压综合征患者60例作为研究对象。采用随机数字分配法,随机将患者分为对照组(30例)和试验组(30例)。对照组采取超声引导下针刀解结法治疗,试验组在“骨正筋柔”理论指导下,采用超声引导针刀解结法联合腰椎斜板法进行综合治疗,两组的治疗周期均为4周。治疗结束后,比较两组患者的临床疗效、疼痛视觉模拟评分(Visual analogue pain scale,VAS)、Oswestry功能障碍指数问卷表(Oswestry disability index,ODI)评分、腰部多裂肌及腰大肌的剪切波杨氏模量值及表面肌电图的变化情况。

结果

与对照组相比,试验组治疗后临床总有效率较高,但两组总有效率差异无统计学意义(P>0.05)。与治疗前相比,治疗后、随访4周两组VAS评分、ODI评分呈降低趋势(P<0.05);与对照组相比,治疗后、随访4周试验组VAS评分、ODI评分较低(P<0.05)。与治疗前相比,治疗后、随访4周试验组两侧多裂肌、腰大肌剪切波杨氏模量值呈降低趋势(P<0.05);与治疗前相比,对照组治疗后、随访4周两侧多裂肌、腰大肌剪切波杨氏模量值差异无统计学意义(P>0.05);与对照组相比,治疗后、随访4周试验组两侧多裂肌、腰大肌剪切波杨氏模量值较低(P<0.05)。与治疗前相比,治疗后、随访4周试验组两侧多裂肌、腰大肌表面肌电图平均振幅呈升高趋势(P<0.05);与治疗前相比,对照组治疗后、随访4周两侧多裂肌、腰大肌表面肌电图平均振幅差异无统计学意义(P>0.05);与对照组相比,治疗后、随访4周试验组两侧多裂肌、腰大肌表面肌电图平均振幅较高(P<0.05)。

结论

基于“骨正筋柔”的理念应用超声引导下的针刀解结法与腰椎斜板法相结合治疗臀上皮神经卡压综合征能够显著提升近远期临床疗效,具备安全性高、创伤小、恢复迅速等优势。

Objective

To explore the clinical effect of ultrasound-guided needle knife dissection combined with lumbar inclined plate method in the treatment of gluteal epithelial nerve compression syndrome based on the theory of“bone alignment and tendon flexibility”.

Method

Sixty patients with gluteal epithelial nerve compression syndrome who received treatment in the Orthopedic Department of the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2023 to January 2024 were selected as the study subjects.Using the random number allocation method,the patients were randomly divided into a control group(30 cases)and an experimental group(30 cases).The control group was treated with ultrasound-guided needle knife dissection,while the experimental group was treated with a combination of ultrasound-guided needle knife dissection and lumbar inclined plate dissection under the guidance of the“bone alignment and tendon flexibility”theory.The treatment period for both groupswas 4 weeks.After treatment,the clinical efficacy,Visual Analog Scale(VAS)score,Oswestry Disability Index(ODI)score,shear wave Young'smodulus values of the lumbar multifidus and psoasmajor muscles,and the changes in surface electromyography were compared between the two groups.

Result

Compared with the control group,the experimental group had a higher clinical total effective rate after 4 weeks of treatment,but there was no statistically significant difference in the total effective rate between the two groups(P>0.05).Compared with those before treatment,VAS scores and ODI scores in both groups showed a decreasing trend after treatment and 4 weeks follow-up(P<0.05).Compared with those of the control group,the VAS score and ODIscore of the experimental group were lower after treatment and 4 weeks follow-up(P<0.05).Compared with those before treatment,the shearwave Young'smodulus values of the bilateral multifidus and lumbar muscles in the experimental group showed a decreasing trend after treatment and 4 weeks follow-up(P<0.05).Compared with those before treatment,there was no statistically significant difference in the shearwave Young'smodulus values of the bilateral multifidus and lumbar muscles in the control group after treatment and 4 weeks follow-up(P>0.05).Compared with the control group,the shearwave Young'smodulus values of the bilateral multifidus and lumbar muscles in the experimental group were lower after treatment and 4 weeks follow-up(P<0.05).Compared with those before treatment,the average amplitude of electromyography on both sides of the multifidus and lumbar muscles in the experimental group showed an increasing trend after treatmentand 4 weeks follow-up(P<0.05).Compared with those before treatment,there was no statistically significant difference in the average amplitude of surface electromyography of the multifidus and psoasmajormuscles in the control group after treatmentand 4 weeks follow-up(P>0.05).Compared with those in the control group,the average amplitude of surface electromyography of the multifidus and psoasmajor muscles in the experimental group was higher after treatment and 4 weeks follow-up(P<0.05).

Conclusion

The combination of ultrasound-guided needle knife dissection method and lumbar inclined plate method based on the concept of“bone alignment and tendon flexibility”can significantly improve the short-term and long-term clinical effects of treating gluteal epithelial nerve compression syndrome,with advantages such as high safety,minimal trauma and rapid recovery.

臀上皮神经损伤,又称臀上皮神经卡压综合征,是骨科临床常见的引起腰腿痛的疾病之一,本病通常由腰臀部的急性、亚急性损伤或长期劳损诱发,导致臀上皮神经在髂嵴骨纤维管处受到压迫或嵌顿[1]。臀上皮神经卡压综合征的常见神经卡压区域包括臀上皮神经的出孔点、横突点以及入臀点。中医学通常将臀上皮神经卡压综合征模糊地归类为腰痛[2]病的范畴,根据中医学理论,其常被归类为“痹证”或“筋痹”,主要因肝肾亏虚、气滞血瘀及风寒湿邪对机体的侵袭所致。中医治疗策略通常包括补益肝肾、行气活血以及祛风散寒等治则,但纯中药方案无法完全满足临床治疗该病症的需求[3]。目前,临床上采用的超声引导下针刀解结治疗是一种中西医结合的微创治疗手段,对于确诊的臀上皮神经卡压综合征显示出了确切的疗效,随着微创治疗趋势的兴起和针刀可视化技术的进步,越来越受到骨科和疼痛科医生的推崇[4]。“骨正筋柔”这一理念源自《黄帝内经》,强调了筋骨系统的正常运行和经络气血的顺畅流动是维持健康的关键[5]。近期研究表明,“骨正筋柔”在中医治疗脊柱筋伤疾病中扮演着至关重要的角色,并且在预防和治疗急、慢性脊柱筋伤疾病方面得到了广泛的应用[6]。本项目通过对比超声引导下针刀与“骨正筋柔”理论下针刀解结联合腰椎斜板法在臀上皮神经卡压综合征中的临床疗效,为“骨正筋柔”理论治疗臀上皮神经卡压综合征提供循证依据。
选取2023年1月—2024年1月在辽宁中医药大学附属医院骨伤科收治的臀上皮神经卡压综合征患者共60例,根据随机数字表法进行随机分组,分为对照组(超声引导下针刀组,30例)与试验组(超声引导针刀结合腰椎斜板组,30例)。两组的一般资料(年龄、性别、病程)详见表1,经统计学比较,两组差异无明显统计学意义(P>0.05),具有可比性。
依据《神经卡压综合征针刀整体松解治疗与康复》[7]中疾病的诊断标准:①患者表现为同侧腰部和臀部的刺痛、胀痛或撕裂性疼痛,疼痛可能会沿着同侧大腿的后外侧扩散,通常不会延伸至膝关节以上;②患者可能会存在患侧臀部麻木感,下肢并无麻木现象;起坐困难,且在弯腰或下蹲的动作中疼痛感加剧;③在髂嵴的中点及其下方区域检测到一个固定的压痛点,在局部可能存在条索状或结节状的异常改变。
(1)符合上述诊断标准;(2)年龄在20~75岁;(3)接受并配合入组检查及治疗;(3)经过医院医学伦理委员会批准,患者知情同意,并签署知情同意协议。
(1)拒绝采纳本次治疗计划的患者;(2)存在神经功能障碍、沟通障碍等,无法顺利完成治疗过程的患者;(3)患有严重心脏、肝脏或肾脏功能不全等基础性疾病患者,处于妊娠期或哺乳期的妇女;(4)存在腰椎间盘突出、腰椎管狭窄、腰椎骨折、腰椎结核等严重腰腿痛相关疾病并发的情况;(5)在治疗过程中表现出较差的依从性,存在不良反应,不宜继续治疗的个体;(6)在治疗中途自行中断或无法持续接受治疗而退出者。
应用超声引导下针刀解结法进行治疗。具体操作:①患者俯卧位,腹部垫软枕,充分暴露腰臀部;②依据个体化临床症状和查体确定腰脊神经后支的卡压位置,并结合蒙世远等所确定的腰椎骨纤维管体表投影定位点[8]确定治疗靶点,并用记号笔标记;③常规皮肤消毒,铺无菌巾;④每个治疗点注射0.5%利多卡因1.5 mL浸润麻醉;⑤应用Terason3000c超声诊断仪,配备线阵探头,频率设定在5~10 MHz,无菌超声探头套扎在涂抹耦合剂的US高频线阵探头上;⑥采用平面内进针法,针刀从探头的一端与皮肤成一定角度进针。进针后,在超声显示屏上观察针体走向路线,利用Color模式明确血管位置,使针刀避开重要的血管、神经,熟练地识别局部解剖结构,通过可视化的方式指导针刀精确地对治疗点进行松解。在超声显示下针刀尖端到达关节突外侧与横突根部上侧形成的凹槽处。针刀沿着上关节突的外侧边缘,上下方向进行2~3次切割以松解粘连的组织。当感觉到针下有松动感时,即可拔针。术中若感觉到关节突关节、腰椎横突尖或臀上皮神经出口处有压痛,可在这些压痛点部位在超声实时动态监视下进行针刀2~3次的松解切割。⑦术后对手术区域进行无菌敷料覆盖包扎,并避免局部沾水和剧烈运动。每周治疗1次,4次为1个疗程。
在“骨正筋柔”理论下超声引导针刀解结联合腰椎斜板法进行治疗。
(1)“骨正筋柔”理论指导下采用超声引导针刀解结法:在对照组治疗的基础上,针刀治疗的靶点位置除对照组的治疗点外,还包括横突末端,在超声的引导下针刀刺入到第2~3腰椎横突末端后调整刀口线至90°角,然后沿着横突上缘进行水平方向的切割松解。
(2)腰椎斜板法:在超声引导针刀解结治疗后实施腰椎斜板手法治疗,具体操作:①腰臀部松筋手法:患者选择俯卧姿势,医生使用双掌或单掌从上至下在患者的腰臀部及患侧下肢后外侧进行推摩手法,重复操作3~5次,然后进行扌衮、按、揉等手法,双手拇指或肘尖对肾俞、大肠俞、关元俞、居髎、环跳、殷门、委中等穴位进行点按,术者使用双手分开推按骶棘肌,并旋揉两侧腰三横突,对患侧的臀中肌和梨状肌进行点压和旋揉。沿着髂嵴寻找可能的卡压点(触诊时存在条索状或硬结感),以与肌纤维垂直的方向进行按揉、弹拨手法,力度从轻到重。术者使用双手拇指重叠或用肘部在患侧髂嵴最高点内侧2~3 cm处的条索状物进行操作,手法方向与纤维鞘垂直,向外和向上弹拨条索状物。操作力度应以患者能够承受为准,随后保持静按1~2 min。②腰椎斜板法:经上述手法放松周围肌肉后实施腰椎斜扳法,患者侧卧位,患侧位于上方,下方的下肢保持自然伸直,上方的下肢屈膝屈髋。若病变部位在下腰部,患者上半身需要有较大的屈曲;若病变部位在上腰部,则下半身需要有较大的屈曲。站在患者的正前方,用一只手放在患者的肩部前方,另一只手屈肘,用肘内侧支撑患者的髋部,拇指定位在之前触及的病变节段。双手向相反方向轻柔而缓慢地施加力量。当腰部旋转至限制位置时,病变节段成为扳动的支点,施加一个短暂而精细的推动力,通常会伴随关节的咔嚓声。部分患者可能听不到关节弹响声,若术者感觉到患者后关节发生位移,也证实调整成功。医生指导患者仰卧,医生站在患者患侧,一只手扶住患者的膝盖,另一只手握住患者的脚踝,进行患肢的屈髋屈膝内旋牵拉操作,目的是拉长肌纤维并松解臀部及下肢后侧的肌群。牵拉持续1 min,重复进行3次。
临床治愈:腰部疼痛、下肢放射痛基本消除,腰部功能恢复至正常水平,直腿抬高测试能达到70°或以上,症状改善率达到75%以上;显效:腰部疼痛和下肢放射痛显著缓解,腰部活动功能大体上恢复至正常,且症状改善率为50%~75%。有效:腰部疼痛和下肢放射痛有所缓解,腰部活动功能部分改善,且症状改善率为30% ~50%。无效:未达到上述标准。总有效率=治愈、显效和有效例数之和/总病例数。
治疗前、后及治疗完成后随访4周评估VAS评分,分值为0~10分,0分无痛,10分极度疼痛。用10厘米标尺量化患者主观疼痛,刻度背对患者。医生根据标尺标记位置评分,2分以下优秀,3~5分良好,6~8分一般,8分以上较差。
治疗前后使用Oswestry功能障碍指数问卷表(OswestryDisabilityIndex,ODI)问卷评估腰部功能,涵盖疼痛、自理、提物、行走、坐、站、睡眠、性生活、社交和旅行。每题6选项,最高5分,最低0分,得分高提示腰部功能障碍重。
治疗前、治疗后及治疗完成后随访4周应用法国声科影像公司的SupersonicImagine彩色多普勒超声诊断仪来进行剪切波弹性超声成像,检测杨氏模量值。选用的是SL15-4型号的高频线阵探头。采取俯卧姿势,确保背部肌肉保持放松状态,上肢自然平放于身体两侧。患者腰背部皮肤上均匀涂抹耦合剂,将探头垂直于腰部中线进行扫描,通过髂后上棘定位L4棘突,以此为参照定位L2、L3及L5棘突,左右移动探头,将探头置于L2-3、L3-4腰椎关节突及L3横突上方,选择超声剪切波弹性成像(SWE)模式,观察多裂肌与腰大肌的肌肉组织,记录其杨氏模量平均值。对左右两侧的肌肉分别采集3次图像并测量杨氏模量值(见插页ⅩⅩⅦ图1),最终取测量的平均值用于统计分析。
治疗前后及治疗完成后随访4周应用MyoMove-Cow表面肌电图仪,确保测试环境安静,室温维持在25℃,湿度控制在80%以下,避免周围电器的干扰。采取端坐姿势,在L3水平的双侧竖脊肌和腰大肌区域进行采样。肌电测试仪器记录下原始的肌电信号,将其上传到电脑上的sEMG数据采集分析软件中。通过快速傅里叶变换(FFT)进行频谱分析,最终计算出平均振幅值。
采用SPSS 26.0软件进行统计学分析,VAS评分、ODI评分、杨氏模量值、平均振幅等计量资料与临床总有效率、计数资料分别采取均数±标准差、%表示,并进行单一样本t检验、单因素方差法分析,P<0.05提示差异存在统计学意义。
与对照组相比,治疗后试验组临床总有效率较高,但总有效率相比差异无统计学意义(P>0.05)。见表2

在中医学中,“骨”指的是骨骼和关节,“筋”则包括筋膜、韧带等软组织结构[9],“筋骨”最早见于《黄帝内经》,在中医理论中描述一个紧密结合、处于动态平衡状态的运动系统。《说文解字》曰“筋,肉之力也;腱,筋之本,附着于骨”,提示筋附骨上,骨连筋中,二者不可分割。《灵枢》指出筋在外,骨在内,筋骨损伤常相互影响。《素问》曰“诸筋者皆属于节”“宗筋主束骨而利关节”,认为筋为纽带,约束肌肉骨骼,骨节则支撑筋附着,保持人体稳定。在正常生理条件下,骨骼和筋膜之间维持着一种平衡。当这种平衡被打破时,就会导致病理状态的出现。筋膜的损伤可能会导致关节失衡,而骨骼的损伤则可能使筋膜失去正常的营养[10]。中医骨伤科特别强调骨骼和筋的同等重要性,并认为筋骨失衡是许多疾病的根本原因。在现代医学中,“筋”被理解为软组织,包括筋膜、肌肉等,其主要功能是带动关节活动并稳定骨骼结构[11]。而“骨”则指的是骨组织,为肌肉提供附着点,筋膜和肌肉提供动态平衡的动力,骨骼则提供静态平衡的支撑,整个平衡过程由神经系统进行调节。脊柱不平衡可能由异常情况引起,甚至导致病变[12]。臀上皮神经卡压综合征是筋骨力学失衡为代表的疾病之一,腰臀部的筋包括胸腰筋膜、椎旁肌、腰大肌、臀肌、棘上韧带等软组织,而骨则包括椎体、横突、关节突关节等骨结构。臀上皮神经起源于腰1~腰3脊神经后支的外侧支,本病的病理变化包括肌肉、筋膜和骨骼的损伤刺激或卡压邻近的脊神经后支或臀上皮神经,例如肌肉痉挛、水肿以及神经的移位等。从生物力学角度来看,动态和静态力学的失衡也是导致该病症的原因之一,这种失衡会影响脊柱的力学平衡[13]。曾有研究表明,通过调整筋痹痛处的筋膜层中结聚区域和软组织的张力,可恢复骨骼、关节以及周围筋膜的内外应力平衡[14]。腰椎不稳定会引起肌肉损伤,进而破坏生物力学的平衡,这也是导致腰臀部疼痛的原因之一[15]。因此维持筋骨力学平衡对于指导臀上皮神经卡压综合征的实验模型和临床治疗具有重要的价值。
本研究结果显示,治疗后及随访后两组VAS评分、ODI评分降低,提示超声引导下的针刀解结法通过松解臀上皮神经的出孔点、横突点及入臀点,不仅可以显著缓解近期减轻疼痛症状,而且还能明显改善远期腰部的功能和活动能力。相关文献资料证实,在超声可视化引导下,通过针刀对病变部位进行切刺、铲剥等操作,可以显著改善非特异性腰痛患者的疼痛状况和功能活动度,研究同样证实在超声可视化引导下观察到的臀上皮神经卡压综合征的病理变化,是腰部从平衡状态向失衡状态转变的关键因素[16]。本试验遵循“骨正筋柔”的理念,认为有效的软组织松解与追求在正常状态下筋骨平衡协调的理念是一致的[17]。还有研究指出,仅对臀上皮神经进行松解可能会引起后续的再次卡压现象[18]。根据“骨正筋柔”的理论以及解剖学结构的分析,臀上皮神经由第1~3腰神经的后外侧皮支构成,这些神经分支分布在椎间关节的外侧区域,涉及韧带、筋膜和肌肉等组织[19]。臀上皮神经是由皮支穿过胸腰筋膜的后层形成的,进入臀部的浅筋膜层,负责支配皮肤。这些神经分为前、中、后3个分支,全部延伸至臀部,走行可以概括为“四段”“六点”和“一管”,其中“六点”和“一管”是容易出现卡压和需要治疗的部位[20]。当神经穿过深筋膜并被固定后,跨越髂嵴并在浅筋膜中行进,逐渐变得接近表面。在躯干旋转过程中,浅层结构的活动幅度较大,而深层结构的活动幅度较小,这种差异可能导致臀上皮神经受到损伤,进而成为腰腿痛的一个重要原因[21]。在传统针刀松解治疗中,仅对臀上皮神经的入臀点进行松解,治疗方式不彻底,易导致病情复发[22]。相比之下,基于“骨正筋柔”理论的超声引导针刀解结法则选择对臀上皮神经的出孔点及第2、3腰椎横突进行联合治疗。
在超声引导下进行的针刀治疗,在生物学层面上:一方面能够改善局部血液循环、抑制炎症因子以缓解疼痛,另一方面还能减缓纤维化和粘连的发展过程[23]。从力学角度来看:治疗通过减少肌张力、改善肌肉和肌腱的松弛度、增强肌肉和肌腱的弹性来恢复筋膜的柔韧性,从而实现“筋柔骨正”的效果,重建筋骨的静态平衡,最终帮助臀上皮神经卡压综合征患者恢复腰部功能。除了针刀治疗,本试验还结合了腰椎斜板法,能减轻腰背部肌肉和筋膜的紧张度,降低局部粘连的严重性,从而促进受损组织的恢复;此外还能促进多种活性物质的代谢和分解,导致血液中的镇痛成分增多,而致痛成分减少,从而发挥镇痛的作用;能够改善血液循环,促进局部组织水肿和炎性物质的吸收。通过弹拨手法,可解除周围软组织的粘连,减少肌肉对神经的牵拉和刺激,同时扩大骨纤维管的体积,有助于被压迫的神经恢复到正常位置。《素问·举痛论篇》中提及“……按之热气至,热气至则痛止矣。”弹拨后进行静按可达到复原和镇痛的作用。在进行弹拨操作之后,通过静按可以帮助恢复和达到镇痛的效果。腰椎斜扳法能够矫正关节错位,调整腰椎后关节,释放腰椎小关节的嵌顿,改善臀上皮神经起点和路径中软组织与小关节的病理状况,上述方法能促进僵硬的筋骨活动,使筋骨间的关系清晰,关节错位和嵌顿得以纠正,恢复其正常位置和顺序,进而消除对神经的刺激和压迫症状。本试验证实,治疗后,与对照组相比,试验组临床总有效率较高,两组总有效率相比差异无统计学意义;与对照组相比,治疗后及随访后试验组VAS评分、ODI评分较低。证实结合“骨正筋柔”理念的超声引导针刀解结法与腰椎斜板法,在显著提升临床近期的治疗效果的同时,实现筋骨平衡,有效缓解近远期疼痛及腰部功能障碍的问题。
在人体完全屈曲的状态下,椎旁肌不会发生收缩,脊柱的稳定性完全依赖于脊柱间的连接结构,如椎间盘、后纵韧带、棘间韧带、棘上韧带,以及肌肉的被动牵拉来维持稳定[24]。当人体处于完全伸直状态,也就是直立姿势时,椎旁肌会有轻微的肌电活动以帮助维持这一姿势。腰大肌是腰椎椎旁肌群中的一个重要部分,不仅参与髋关节的屈曲动作,还对脊柱和骨盆的运动与稳定性起着关键作用[25]。在进行屈曲运动时,腰大肌能确保动作的流畅性,其肌电活动的幅度和持续时间都比在伸直运动过程中要小和短[26]。在伸直运动中,腰大肌肌电活动的幅度较高[27]。因此评估腰大肌的功能对于辅助诊断臀上皮神经卡压综合征及评估治疗效果具有显著的临床重要性。多裂肌是腰部核心肌群中维持腰椎稳定性的关键肌肉,属于重要的腰椎稳定肌,这块肌肉的解剖学特征包括短纤维长度、大的肌肉横截面积、小的关节跨度以及丰富的I型肌纤维[28]。还具有受意识和下意识双重动作控制等生理学特点,特别是在L3~L5椎体的稳定性中发挥着重要作用[29]。在人体运动过程中,中枢神经系统能够接收来自不同感觉途径的信息输入,并通过前馈控制、反馈控制和随意运动控制机制,来激活和调节腰椎稳定肌的活动。这有助于保持腰椎的正常功能和稳定性。如腰部多裂肌过早激活或活动过于频繁,可能会导致肌肉疲劳[30]。相反,若腰部多裂肌激活延迟或激活力度不够,可能会导致腰椎在需要稳定支持时缺乏足够的力量,从而破坏稳定性,这可能是臀上皮神经卡压综合征患者感到疼痛的一个原因。
超声SWE技术能精确地对肌肉和软组织的硬度进行无创量化分析,技术通过测量超声诱导产生的剪切波在软组织中的传播速度来量化组织的弹性或硬度[31]。通过计算得到的剪切波速度,可以得出组织的杨氏模量。组织的硬度越大,相应的杨氏模量也越高,从而更直观地反映出组织的弹性特征[32]。表面肌电图是一种无创的影像学检查方法,它记录肌肉在活动时产生的生物电变化,并将其转换为一维信号图,用于定量分析椎旁肌的生理和病理状态。近年来的研究开始探索表面肌电图在腰背肌功能评估中的应用[33]。研究发现,在腰椎间盘突出症等慢性腰功能损伤的患者中,治疗后症状得到缓解的人,其表面肌电图的中位频率斜率有所下降,而健康侧没有明显变化,患侧则有所上升[34]。因此认为表面肌电图可以作为评估慢性腰功能损伤的一个客观指标。在本试验中选用了杨氏模量值测试和表面肌电图技术来对腰部多裂肌和腰大肌进行评估。与治疗前相比,治疗后、随访4周试验组两侧多裂肌、腰大肌剪切波杨氏模量值呈降低趋势,肌表面肌电图平均振幅呈增高趋势(P<0.05),而对照组治疗后、随访4周上述指标治疗后无差异。提示在“骨正筋柔”理念指导下,将超声引导针刀解结法与腰椎斜板法相结合应用于治疗臀上皮神经卡压综合征,不仅能够在治疗后提升即刻的腰部多裂肌和腰大肌的反应速度,还能在远期随访中增强腰部肌肉的反应力度,减轻肌肉的紧张状态,改善远期疗效。
本研究通过对臀上皮神经卡压综合征患者60例的临床疗效、VAS评分、ODI评分、剪切波杨氏模量值、表面肌电图平均振幅进行观察,证实遵循“骨正筋柔”的理念,应用超声引导下的针刀解结法与腰椎斜板法相结合治疗臀上皮神经卡压综合征,结果显示临床近远期疗效显著,同时具备安全性高、创伤小、恢复迅速的特点,值得临床推广应用。
