Objective

To study the clinical study of Jingu Tongxiao Pill(筋骨痛消丸)combined with arthroscopy in the treatment of knee osteoarthritis(cold coagulation blood stasis syndrome).

Methods

A total of 120 patients diagnosed as knee osteoarthritis(cold coagulation and blood stasis syndrome)were selected and divided into two groups according to random number table.The reference group(60 cases)received arthroscopic treatment,and the study group(60 cases)received Jingu Tongxiao Pill and arthroscopic treatment.The clinical efficacy,TCM syndrome score,osteoarthritis index,inflammatory factor level and oxidative stress reaction of the two groupswere observed and compared.

Results

Compared with that(75.00%)of the reference group,the total effective rate of the study group was98.33%,and the difference was statistically significant(P<0.05).After treatment,the TCM syndrome score of the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).After treatment,compared with those of the reference group,the pain,stiffness and difficulty scores of daily activities in the study group were lower,and the difference was statistically significant(P<0.05).After treatment,compared with those of the reference group,the levels of tumor necrosis factor-alpha(TNF-α),interleukin-1 beta(IL-1β)and matrixmetalloproteinase-3(MMP-3)in the study group were lower,and the difference was statistically significant(P<0.05).

Conclusion

The patients with knee osteoarthritis(cold coagulation blood stasis syndrome)were treated with arthroscopy and Jingu Tongxiao Pill,and the therapeutic effect was good,which could effectively improve the clinical symptoms and promote the recovery of patients,and the clinical application value was high.

目的

研究筋骨痛消丸与关节镜联用治疗膝骨关节炎(寒凝血瘀证)的临床。

方法

抽取120例经检查被确诊为膝骨关节炎(寒凝血瘀证)的患者,按随机数字表分为两组,参考组60例予以关节镜治疗,研究组60例予以筋骨痛消丸、关节镜联合治疗。观察比较两组患者的临床疗效、中医证候积分、骨关节炎指数、炎症因子水平以及氧化应激反应。

结果

研究组总有效率与参考组相比,98.33%高于75.00%,差异有统计学意义(P<0.05)。治疗后,与参考组相对比,研究组中医证候积分更低,差异有统计学意义(P<0.05)。治疗后,与参考组相对比,研究组疼痛、僵硬、日常活动的难度评分更低,差异有统计学意义(P<0.05)。治疗后,与参考组相对比,研究组肿瘤坏死因子-α(Tumor Necrosis Factor-alpha,TNF-α)、白细胞介素-1β(Interleukin-1 beta,IL-1β)、基质金属蛋白酶-3(Matrix Metalloproteinase-3,MMP-3)水平均更低,差异有统计学意义(P<0.05)。

结论

膝骨关节炎(寒凝血瘀证)的患者采取关节镜、筋骨痛消丸联合治疗后,治疗效果好,能有效改善患者的临床症状,促进患者恢复,临床应用价值较高。

目的

研究筋骨痛消丸与关节镜联用治疗膝骨关节炎(寒凝血瘀证)的临床。

方法

抽取120例经检查被确诊为膝骨关节炎(寒凝血瘀证)的患者,按随机数字表分为两组,参考组60例予以关节镜治疗,研究组60例予以筋骨痛消丸、关节镜联合治疗。观察比较两组患者的临床疗效、中医证候积分、骨关节炎指数、炎症因子水平以及氧化应激反应。

结果

研究组总有效率与参考组相比,98.33%高于75.00%,差异有统计学意义(P<0.05)。治疗后,与参考组相对比,研究组中医证候积分更低,差异有统计学意义(P<0.05)。治疗后,与参考组相对比,研究组疼痛、僵硬、日常活动的难度评分更低,差异有统计学意义(P<0.05)。治疗后,与参考组相对比,研究组肿瘤坏死因子-α(Tumor Necrosis Factor-alpha,TNF-α)、白细胞介素-1β(Interleukin-1 beta,IL-1β)、基质金属蛋白酶-3(Matrix Metalloproteinase-3,MMP-3)水平均更低,差异有统计学意义(P<0.05)。

结论

膝骨关节炎(寒凝血瘀证)的患者采取关节镜、筋骨痛消丸联合治疗后,治疗效果好,能有效改善患者的临床症状,促进患者恢复,临床应用价值较高。

Objective

To study the clinical study of Jingu Tongxiao Pill(筋骨痛消丸)combined with arthroscopy in the treatment of knee osteoarthritis(cold coagulation blood stasis syndrome).

Methods

A total of 120 patients diagnosed as knee osteoarthritis(cold coagulation and blood stasis syndrome)were selected and divided into two groups according to random number table.The reference group(60 cases)received arthroscopic treatment,and the study group(60 cases)received Jingu Tongxiao Pill and arthroscopic treatment.The clinical efficacy,TCM syndrome score,osteoarthritis index,inflammatory factor level and oxidative stress reaction of the two groupswere observed and compared.

Results

Compared with that(75.00%)of the reference group,the total effective rate of the study group was98.33%,and the difference was statistically significant(P<0.05).After treatment,the TCM syndrome score of the study group was lower than that of the reference group,and the difference was statistically significant(P<0.05).After treatment,compared with those of the reference group,the pain,stiffness and difficulty scores of daily activities in the study group were lower,and the difference was statistically significant(P<0.05).After treatment,compared with those of the reference group,the levels of tumor necrosis factor-alpha(TNF-α),interleukin-1 beta(IL-1β)and matrixmetalloproteinase-3(MMP-3)in the study group were lower,and the difference was statistically significant(P<0.05).

Conclusion

The patients with knee osteoarthritis(cold coagulation blood stasis syndrome)were treated with arthroscopy and Jingu Tongxiao Pill,and the therapeutic effect was good,which could effectively improve the clinical symptoms and promote the recovery of patients,and the clinical application value was high.

目前,临床治疗膝骨关节炎的治疗方式包括保守治疗、手术治疗。根据膝骨关节炎的治疗原则来看,临床症状较为严重,且临床检查显示关节内的游离体较大,伴卡压等症状的患者,或是仅仅采取保守治疗无效的患者,通常需要采取手术治疗[1]。关节镜是临床治疗膝骨关节炎的常用方式,属于微创技术。与传统手术相比,该种方式具有创伤较小、出血少、恢复快、费用较低等特点,在临床广泛使用。从中医的角度上看,膝骨关节炎属于“痹证”“膝痹”等范畴,其中以寒凝血瘀证较为常见,主要是由于外感风寒湿邪气,内伴有瘀血、痰浊,导致经脉阻滞[2]。加上关节镜属于外源性创伤,极易导致局部出现气血不畅,瘀血产生,导致经脉不通,引起疼痛。因此,关节镜后需要采取相应治疗,以改善患者的临床症状。筋骨痛消丸属于复方制剂,具有活血行气、温经通络、消肿止痛的作用。对治疗血瘀寒凝证、骨质增生导致的膝关节疼痛、活动受限等症状具有较好作用。基于此,为探讨关节镜、筋骨痛消丸联合治疗给膝骨关节炎(寒凝血瘀证)患者带来的临床效果,选取我院120例经检查被确诊为膝骨关节炎(寒凝血瘀证)的患者作为研究对象,现报道如下。
①符合《关节炎诊断及治疗指南》[3]中对膝骨关节炎的诊断标准;②伴明显的膝关节疼痛,晨僵超过30 min,活动伴骨摩擦;③或骨性膨大。
参考《中医病证诊断疗效标准》[4]中对寒凝血瘀证的辨证;伴膝关节疼痛,夜晚加重,关节屈伸不利,舌质暗、有瘀斑,苔白,脉细涩。
①符合膝骨关节炎的诊断标准,且关节内的游离体较大;②根据Kellgren-Lawrence膝关节骨关节炎分级(Kellgren-Lawrence Grading Scale,K-L分级),患者均属于Ⅰ~Ⅲ级;③符合关节镜手术指征;④患者对该研究知情,签署相关文件参与。
①存在膝关节关节镜禁忌的患者;②合并其余膝关节病,如半月板损伤、骨结核等;③合并心脑血管、肝肾功能不全等疾病的患者;④合并膝关节外伤史的患者;⑤凝血功能异常的患者。
抽中2021年1月—2024年1月在该院收入治疗的膝骨关节炎(寒凝血瘀证)患者120例作为研究对象,按随机数字表法划分组别,各60例。研究组男30例,女30例;年龄50~75岁,平均年龄(63.44±5.22)岁;病程1~8年,平均病程(5.52±2.11)年;Kellgren-Lawrence分级:Ⅰ级20例,Ⅱ级20例,Ⅲ级20例。参考组男29例,女31例;年龄51~77岁,平均(63.32±5.60)岁;病程1~9年,平均病程(5.42±2.12)年;Kellgren-Lawrence分级:Ⅰ级19例,Ⅱ级22例,Ⅲ级19例。两组比较,差异无统计学意义(P>0.05)。
对照组予以关节镜治疗,采取膝关节镜清除术,操作如下:术前完善血常规、凝血功能、肝肾功、传染病等相关术前检查。使用美国Stryker关节镜系统对患者实施关节镜治疗。患者仰卧位,予以患者腰硬联合麻醉,在其患侧大腿中上约1/3处扎一个止血带,操作者将患侧髌骨向内侧推,于髌骨外上、股骨外髁的交界处,作一个4 mm切口,将灌注吸引针插至膝关节腔,注入80 mL 0.9%氯化钠注射液,充盈关节腔。将患者膝关节被动屈曲30°,在距离髌韧带1厘米处的膝关节内侧关节间隙和外侧关节间隙做一5 mm横行切口,将关节镜置入,探查膝关节内部的病变情况。使用器械(磨钻、刨削器等)处理病变部位,包括切除游离碎骨、软骨,去除软骨表层的绒毛,去除斑片状钙化,去除变性或撕裂的半月板,切除增生滑膜。清除完成后,使用大量0.9%氯化钠注射液冲洗关节腔至出现清亮液。最后,将切口进行缝合,置引流管,棉垫加压包扎,10 min后若无异常送回病房。术后予以抗炎、消肿、补液等对症治疗。
研究组在参考组基础上予以筋骨痛消丸(河南省洛正药业有限责任公司;国药准字Z10970117)治疗,6 g/次,2次/d,使用温水吞服。连续服用30 d。
(1)临床疗效。参照《中医病证诊断疗效标准》,治愈:患者膝关节疼痛、肿胀等临床症状消失,中医证候积分减少达95%以上;显效:患者膝关节疼痛、肿胀等临床症状明显减轻,中医证候积分减少在70% ~94%;有效:患者膝关节疼痛、肿胀等临床症状有好转,中医证候积分减少30% ~69%;无效:患者膝关节疼痛、肿胀等临床症状无改善,中医证候积分减少低于30%.治疗总有效率为治愈率、显效率与有效率总和。(2)中医证候积分。主要症状包括关节疼痛、屈伸障碍、膝周肿胀,分值0、2、4、6分。次要症状腰膝酸软、关节僵硬,分值0、2、4、6分。患者症状的严重程度和中医证候积分成正比。(3)骨关节炎指数。采取骨关节炎指数(Western Ontario and Mc-Master Universities Osteoarthritis Index,WOMAC)进行评分,内容包括疼痛(5项)、僵硬(2项)、日常活动的难度(17项)3个方面,分数越低,临床症状越有改善。(4)血清炎症因子。于治疗前后,抽取患者清晨空腹静脉血,采取酶联免疫吸附法检测肿瘤坏死因子-α(Tumor Necrosis Factor-alpha,TNF-α)、白细胞介素-1β(Interleukin-1 beta,IL-1β)、基质金属蛋白酶-3(Matrix Metalloproteinase-3,MMP-3)。
采用SPSS 22.0统计学软件分析数据。计量资料以()表示,两组治疗前后相比较,采用独立双样本t检验;同组治疗前采用配对样本t检验(paired samples t-test)进行分析。计数资料以率(%)表示,采用χ2检验。P<0.05表示差异有统计学意义。
研究组总有效率与参考组相比,98.33%高于75.00%,差异有统计学意义(P<0.05)。详见表1
治疗前,两组患者中医证候积分比较无差异(P>0.05);治疗后,与参考组相对比,研究组中医证候积分更低,差异有统计学意义(P<0.05)。详见表2

膝骨关节炎属于临床上比较常见、高发的骨关节炎疾病,其主要病理表现为关节软骨的丢失、骨赘的形成、软骨下出现骨硬化[5]。其中,以中老年人群、肥胖人群较为高发,且男性发病率低于女性[6]。患者病发后,早期会出现膝关节的疼痛、肿胀,活动时有弹响、摩擦音,随着疾病的继续发展,患者还会出现关节活动受限,后期还会导致膝关节周围肌肉的萎缩、关节僵硬甚至畸形,严重影响患者的日常生活,给患者带来极大地痛苦。因此,需要采取科学、有效的治疗方式,以改善膝骨关节炎患者的临床症状,减轻患者的痛苦。
既往,临床上治疗膝骨关节炎主要采取保守治疗,包括药物治疗、非药物治疗[7-9]。保守治疗对于症状较轻的患者,可有效减轻其疼痛,改善其膝关节疼痛、肿胀等症。但是对症状较重或采取保守治疗依旧效果不理想的患者,通常需要采取手术治疗。现如今,随着微创技术的不断发展,关节镜的出现,为临床诊断、治疗膝骨关节炎取得较好的疗效。和传统开放手术相比,关节镜治疗的出血量更少、创伤更小,可有效延长患者膝关节的使用时间,患者的接受度更好[10]。采取关节镜治疗,主要是利用特有用的工具,将导致膝骨关节炎症状的因素彻底清除,恢复其关节软骨完整,使膝关节内部的环境恢复正常,进而减轻患者的一系列临床症状[11]。然而,手术治疗存在一定的创伤,加上膝骨关节炎主要发病人群为老年人群,其年龄较大,术后恢复相对较慢,仅仅采取关节镜治疗难以彻底改善其关节液,且极易导致局部出现经脉损伤,经络痹阻,进而出现疼痛等症。因此,为标本兼治,往往需要联合用药治疗。
中医认为,膝骨关节炎主要是由于体内的正气不足,导致风、寒、湿等邪气的夹杂,侵袭机体,进而出现经络痹阻,失去濡养,留滞在关节中,气血运行失常,形成瘀血,经脉不通则导致疼痛[12]。或由于外力的暴力损伤、慢性劳损等因素导致。引起膝骨关节炎的病因较多,其病理变化却与血瘀具有密不可分的联系,因此临床以寒凝血瘀证膝骨关节炎较为常见。因此,治疗时应以活血化瘀、温经通络、止痛为主,以改善患者的膝关节功能,减轻患者膝关节的疼痛、肿胀等症。筋骨痛消丸属于平乐郭氏的正骨经验方,主要是由丹参、乌药、香附(醋制)、威灵仙、鸡血藤、桂枝、川牛膝、地黄、白芍、秦艽、甘草所构成,对治疗寒凝血瘀证膝骨关节炎具有较好的功效[13]。方中丹参具有活血祛瘀、凉血止痛的功效[14-15],乌药[16]可行气止痛、散寒温肾,香附理气宽中、止痛[17],威灵仙[18]、川牛膝[19]、秦艽[20]可祛风除湿、通经络解滞气止痛功效,桂枝[21-22]温通经脉、祛风散寒、调和营卫,鸡血藤[23]可以活血舒筋,白芍[24]平抑肝阳、柔筋止痛,甘草[25-26]调和诸药、缓急止痛。诸药合用,达到活血行气、温经通络、消肿止痛的作用[27-30]。在本研究中,研究组总有效率更高,中医证候积分、骨关节炎指数更低(P<0.05),说明采取关节镜联合筋骨痛消丸治疗寒凝血瘀证膝骨关节炎可有效提高临床效果,减轻患者的临床症状,促进患者恢复。而炎症因子可以直接作用于膝关节的滑膜和软骨,引起一系列的临床症状。我们前期研究发现,TNF-α、IL-1β等炎性因子可通过激活软骨细胞[31],促进其分泌前列腺素,但MMP-3具有降解软骨的能力,从而抑制关节软骨的生长[32]。因此,抑制以上炎性因子可有效缓解炎症反应。治疗后,与参考组相对比,研究组TNF-α、IL-1β、MMP-3水平均更低。说明关节镜联合筋骨痛消丸治疗可有效减轻机体的炎性反应。究其原因,可能是由于筋骨痛消丸具有较好抗炎效果。从现代药理学的角度上看,香附具有镇痛、消炎等功效;威灵仙可以抗炎、止痛、保护关节软骨;桂枝可以解热、抗炎;而鸡血藤、丹参等可以改善局部的血流动力学障碍情况,改善局部微循环[33-37]
综上所述,膝骨关节炎(寒凝血瘀证)患者采用关节镜联合筋骨痛消丸治疗,对患者临床症状、机体炎性反应有减轻效果,对患者健康恢复有促进作用。
