Objective

To explore the mechanism of Modified Qingre Shenshi Decoction(清热渗湿汤化裁)regulating TLR4/NF-κB signaling pathway in improving ulcerative colitis.

Methods

A total of150 patients with ulcerative colitis admitted to the hospital from January 2021 to January 2024 were selected and divided into control group(75 cases,treated with mesalazine enteric-coated tablets)and observation group(75 cases,treated with Modified Qingre Shenshi Decoction)according to a random number table.The efficacy and adverse reactions of the two groupswere compared.TCM syndrome scores,disease evaluation,serum factor and mRNA expressions of TLR4 and NF-κB in two groups were observed.

Results

After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).Before treatment,the scores of lower abdomen fullness and pain,burning anus,strong anal distension,diarrhea,mucus,pus and blood stool,heavy limbs,chest tightness and fever,dry mouth and bitter mouth and short urine red were not significant between the two groups(P>0.05).After treatment,the scores of the above symptoms were all lower than those before treatment.In addition,the scores in the observation group were significantly lower than those in the control group(P<0.05).Before treatment,the scores of Baron endoscopy and mucosal histology were not significant between the two groups(P>0.05).After treatment,the scores of Baron endoscopy and mucosal histology in the two groups were lower and the scores in the observation group were significantly lower than those in the control group(P<0.05).Before treatment,the levels of interleukin-1β,tumor necrosis factor-α,endotoxin and D-lactic acid were not significant between the two groups(P>0.05).After treatment,the levels of interleukin-1β,tumor necrosis factor-α,endotoxin and D-lactic acid in both groups were lower than those before treatment,and the levels in the observation group were significantly lower than those in the control group(P<0.05).Before treatment,the mRNA expressions of TLR4 and NF-κB not significant between the two groups(P>0.05).After treatment,the mRNA expressions of TLR4 and NF-κB in the two groups were lower than those before treatment,and the expressions in the observation group were significantly lower than those in the control group(P<0.05).There were no adverse reactions in both groups during the treatment.

Conclusion

By regulating TLR4/NF-κB signaling pathway,Modified Qingre Shenshi Decoction can improve the clinical symptoms of patients with ulcerative colitis,plays an anti-inflammatory role and promotes the recovery of patients,which is worthy of clinical application.

目的

探讨清热渗湿汤化裁调节TLR4/NF-κB信号通路改善溃疡性结肠炎的作用机制。

方法

选择2021年1月—2024年1月医院收治的溃疡性结肠炎患者150例,按照随机数字表分为对照组(75例,给予美沙拉嗪肠溶片治疗)和观察组(75例,给予清热渗湿汤化裁治疗),对比两组疗效及不良反应,观察两组患者中医证候积分、病情评价、血清因子及TLR4、NF-κBmRNA表达。

结果

治疗后,研究组总有效率高于对照组(P<0.05)。治疗前,两组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分比较(P>0.05)。治疗后两组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均低于治疗前,且研究组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均明显低于对照组(P<0.05)。治疗前,两组Baron内镜评分、黏膜组织学评分比较(P>0.05)。治疗后两组Baron内镜评分、黏膜组织学评分均低于治疗前,且研究组Baron内镜评分、黏膜组织学评分均明显低于对照组(P<0.05)。治疗前,两组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平比较(P>0.05)。治疗后两组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均低于治疗前,且研究组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均明显低于对照组(P<0.05)。治疗前,两组TLR4、NF-κB mRNA表达比较(P>0.05)。治疗后两组TLR4、NF-κBmRNA表达均低于治疗前,且研究组TLR4、NF-κB mRNA表达均明显低于对照组(P<0.05)。治疗过程中两组均未出现不良反应。

结论

清热渗湿汤化裁能够通过调节TLR4/NF-κB信号通路改善溃疡性结肠炎患者临床症状,发挥抗炎作用,促使患者康复,值得临床推广应用。

目的

探讨清热渗湿汤化裁调节TLR4/NF-κB信号通路改善溃疡性结肠炎的作用机制。

方法

选择2021年1月—2024年1月医院收治的溃疡性结肠炎患者150例,按照随机数字表分为对照组(75例,给予美沙拉嗪肠溶片治疗)和观察组(75例,给予清热渗湿汤化裁治疗),对比两组疗效及不良反应,观察两组患者中医证候积分、病情评价、血清因子及TLR4、NF-κBmRNA表达。

结果

治疗后,研究组总有效率高于对照组(P<0.05)。治疗前,两组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分比较(P>0.05)。治疗后两组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均低于治疗前,且研究组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均明显低于对照组(P<0.05)。治疗前,两组Baron内镜评分、黏膜组织学评分比较(P>0.05)。治疗后两组Baron内镜评分、黏膜组织学评分均低于治疗前,且研究组Baron内镜评分、黏膜组织学评分均明显低于对照组(P<0.05)。治疗前,两组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平比较(P>0.05)。治疗后两组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均低于治疗前,且研究组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均明显低于对照组(P<0.05)。治疗前,两组TLR4、NF-κB mRNA表达比较(P>0.05)。治疗后两组TLR4、NF-κBmRNA表达均低于治疗前,且研究组TLR4、NF-κB mRNA表达均明显低于对照组(P<0.05)。治疗过程中两组均未出现不良反应。

结论

清热渗湿汤化裁能够通过调节TLR4/NF-κB信号通路改善溃疡性结肠炎患者临床症状,发挥抗炎作用,促使患者康复,值得临床推广应用。

Objective

To explore the mechanism of Modified Qingre Shenshi Decoction(清热渗湿汤化裁)regulating TLR4/NF-κB signaling pathway in improving ulcerative colitis.

Methods

A total of150 patients with ulcerative colitis admitted to the hospital from January 2021 to January 2024 were selected and divided into control group(75 cases,treated with mesalazine enteric-coated tablets)and observation group(75 cases,treated with Modified Qingre Shenshi Decoction)according to a random number table.The efficacy and adverse reactions of the two groupswere compared.TCM syndrome scores,disease evaluation,serum factor and mRNA expressions of TLR4 and NF-κB in two groups were observed.

Results

After treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05).Before treatment,the scores of lower abdomen fullness and pain,burning anus,strong anal distension,diarrhea,mucus,pus and blood stool,heavy limbs,chest tightness and fever,dry mouth and bitter mouth and short urine red were not significant between the two groups(P>0.05).After treatment,the scores of the above symptoms were all lower than those before treatment.In addition,the scores in the observation group were significantly lower than those in the control group(P<0.05).Before treatment,the scores of Baron endoscopy and mucosal histology were not significant between the two groups(P>0.05).After treatment,the scores of Baron endoscopy and mucosal histology in the two groups were lower and the scores in the observation group were significantly lower than those in the control group(P<0.05).Before treatment,the levels of interleukin-1β,tumor necrosis factor-α,endotoxin and D-lactic acid were not significant between the two groups(P>0.05).After treatment,the levels of interleukin-1β,tumor necrosis factor-α,endotoxin and D-lactic acid in both groups were lower than those before treatment,and the levels in the observation group were significantly lower than those in the control group(P<0.05).Before treatment,the mRNA expressions of TLR4 and NF-κB not significant between the two groups(P>0.05).After treatment,the mRNA expressions of TLR4 and NF-κB in the two groups were lower than those before treatment,and the expressions in the observation group were significantly lower than those in the control group(P<0.05).There were no adverse reactions in both groups during the treatment.

Conclusion

By regulating TLR4/NF-κB signaling pathway,Modified Qingre Shenshi Decoction can improve the clinical symptoms of patients with ulcerative colitis,plays an anti-inflammatory role and promotes the recovery of patients,which is worthy of clinical application.

相关资料指出[1],全世界溃疡性结肠炎患者高达500万例,且逐年呈上升趋势。该病的主要临床症状为里急后重、左下腹或下腹阵或全腹部疼痛、腹泻等,随着疾病的加重,部分患者溃疡面积会增大,诱发穿孔、坏死等症状,极难治愈,且可发展为结直肠癌。目前,针对溃疡性结肠炎应给予精准治疗,以控制溃疡面积,避免出现穿孔、坏死等症状。现常采用美沙拉嗪治疗该病,其能够有效降低炎症反应,缓解临床症状,但部分患者停药后易反复发作,影响其疗效[2]。中医药具有多成分、多通路、多靶点等作用特点,在治疗溃疡性结肠炎方面具有显著疗效[3-4]。清热渗湿汤出自《赤水玄珠》卷二,具有化湿健脾、清热渗湿的作用,笔者根据临床溃疡性结肠炎患者的病因病机特点在清热渗湿汤的基础上增加了马齿苋、仙鹤草、生芪、炒白芍、蛇舌草、积雪草、藿香、党参、槐花等药物,去掉了黄连、苍术,共奏健脾利湿、清热止泻的功效,从而发挥治疗溃疡性结肠炎的目的。
溃疡性结肠炎的发生与炎症反应密切相关,TLR4/NFκB信号通路是经典的炎症信号通路,其参与溃疡性结肠炎的发生、发展过程,因此调节TLR4/NF-κB信号通路对治疗该病发挥关键作用[5]。其中TLR4在内毒素触发的炎症反应初始化阶段过程中发挥核心价值;NF-κB参与促炎细胞因子等多种炎症相关介质的生成。既往相关研究指出[6-7],中医药能够有效调节溃疡性结肠炎的TLR4/NF-κB信号通路,但清热渗湿汤化裁是否能够通过调节溃疡性结肠炎的TLR4/NF-κB信号通路发挥治疗作用尚无报道。基于此,本研究将探讨清热渗湿汤化裁调节TLR4/NF-κB信号通路改善溃疡性结肠炎的作用机制研究,以期为临床防治该病提供依据。
选择2021年1月—2024年1月我院收治的溃疡性结肠炎患者150例,按照随机数字表分为对照组(75例)和观察组(75例)。对照组:男39例、女36例;年龄24~51岁,平均年龄(35.93±6.87)岁;体质量指数18~24 kg/m2,平均体质量指数(22.63±2.35)kg/m2;病程0.3个月~5.3年,平均病程(2.45±0.76)年;病变部位:27例广泛结肠型,25例左半结肠,23例直肠;病变程度:41例轻度,34例中度。观察组:男40例、女35例;年龄24~51岁,平均年龄(35.87±6.93)岁;体质量指数18~24 kg/m2,平均体质量指数(22.69±2.41)kg/m2;病程0.3个月~5.3年,平均病程(2.49±0.76)年;病变部位:28例广泛结肠型,26例左半结肠,21例直肠;病变程度:39例轻度,36例中度。两组一般资料临床比较差异无统计学意义(P>0.05),具有可比性。本研究经过医院伦理委员会审批通过后实施[批号:(研)2025430101]。
(1)西医诊断标准:根据《炎症性肠病诊断与治疗的共识意见》[8]制定:①里急后重伴黏液脓血等临床症状;②经结肠镜检查存在水肿、充血、上覆黏液脓血等典型特征者;③符合上述诊断标准且伴黏膜组织活检确诊。
(2)中医诊断标准:根据《溃疡性结肠炎中西医结合诊疗专家共识(2023)》[9]中大肠湿热证制定:主症:下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便;次症:四肢沉重、胸闷发热、口干口苦、小便短赤;舌脉:舌红、苔黄腻、脉滑数。
①符合上述诊断标准者;②年龄为22~60岁;③完成4周疗程,且依从性较好者;④临床资料完整者;⑤认知正常;⑥均签订知情同意书者。
①对本研究药物过敏者;②凝血或免疫系统疾病者;③其他肠道疾病者;④肿瘤患者;⑤重要脏器功能不全者;⑥精神疾病者;⑦伴中毒性结肠扩张、肠梗阻等严重并发症者;⑧自动退出研究者;⑨病情加重;○10未配合治疗者;○1妊娠期或哺乳期患者;○12失访病例。
对照组给予美沙拉嗪肠溶片(安徽东盛制药有限公司;国药准字H20020211;批号:2405271),4片/次,3次/d。
研究组给予清热渗湿汤化裁治疗,组方:马齿苋30 g,仙鹤草30 g,生黄芪15 g,炒白术15 g,茯苓15 g,炒白芍15 g,蛇舌草15 g,积雪草15 g,藿香15 g,党参12 g,槐花12 g,苍术9 g,苦参6 g,陈皮6 g,防风6 g,炒黄柏6 g,甘草6 g。辨证加减:腹胀明显者加厚朴6 g,槟榔6 g,木香6 g;腹泻明显者加补骨脂10 g,肉豆蔻10 g,升麻10 g;腹痛明显者白芍加倍;黏液脓血便明显者加秦皮10 g,紫草10 g,地榆炭10 g。日1剂,水煎服,早晚饭后30 min分服。
对比两组疗效及不良反应,观察两组患者中医证候积分、病情评价、血清因子及TLR4、NF-κBmRNA表达。
疗效:根据《溃疡性结肠炎中西医结合诊疗专家共识(2023)》[9]制定,治愈:下腹部胀满及疼痛、肛门灼热、肛门坠胀感强等症状消失,肠镜下黏膜病变基本消失;显效:下腹部胀满及疼痛、肛门灼热、肛门坠胀感强等明显缓解,肠镜下黏膜病变明显减轻;有效:下腹部胀满及疼痛、肛门灼热、肛门坠胀感强等有所缓解,肠镜下黏膜病变有所减轻;无效:下腹部胀满及疼痛、肛门灼热、肛门坠胀感强等未改善或加重,肠镜下黏膜病变未改善或加重。总有效率=治愈率+显效率+有效率。
中医证候积分:根据《溃疡性结肠炎中西医结合诊疗专家共识(2023)》[9]制定,根据下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便的严重程度,评分为0~6分;根据四肢沉重、胸闷发热、口干口苦、小便短赤的严重程度,评分为0~3分,得分越高则临床症状越严重。
病情评价:采用Baron内镜评分[10]及黏膜组织学评分[11]评估,Baron内镜评分:治疗前后评估肠镜结果,黏膜存在明显的溃疡,同时存在自发性出血为3分;黏膜出现颗粒样变化,中度接触性出血为2分;黏膜出现出血但不存在活动性出血,存在不明显的血管纹理为1分。黏膜组织学评分:治疗前后评估,存急性与慢性炎性细胞,黏膜表层存在溃疡及脓性黏液,隐窝脓肿为3分;血管纹理混乱,黏膜充血水肿,出现急慢性炎性细胞,黏膜上皮较完整为2分;出现小的淋巴细胞浸润灶,不存在黏上皮或脓肿、急性炎症为1分。
血清因子水平:治疗前后抽取患者静脉血3 mL,离心(1500 r/min,15 min)后送检,采用酶联免疫吸附法测定白细胞介素-1β、肿瘤坏死因子-α水平。内毒素、D-乳酸水平采用放射免疫法测定。
TLR4、NF-κBmRNA表达:治疗前后抽取患者静脉血3 m L,肝素抗凝,将外周血中的单核细胞分离,采用实时荧光定量反应测定TLR4、NF-κB mRNA表达,需严格按照说明书进行操作,采用2-△△Ct定量分析。
不良反应:统计治疗过程中出现的胃部不适、恶心等不良反应。
采用SPSS 26.0统计分析,中医证候积分、Baron内镜评分、黏膜组织学评分、白细胞介素-1β、肿瘤坏死因子-α水平等符合正态性、方差齐性的计量资料采用均数±标准差()表示,组间比较采用配对t检验,组内比较采用独立t检验;计数资料以率(%)表示,采用卡方χ2检验进行比较,以P<0.05为差异有统计学意义。
治疗后,研究组总有效率高于对照组(P<0.05)。见表1
治疗前,两组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分比较(P>0.05)。治疗后两组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均低于治疗前,且研究组下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均明显低于对照组(P<0.05)。见表2
治疗前,两组Baron内镜评分、黏膜组织学评分比较(P>0.05)。治疗后两组Baron内镜评分、黏膜组织学评分均低于治疗前,且研究组Baron内镜评分、黏膜组织学评分均明显低于对照组(P<0.05)。见表3
治疗前,两组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平比较(P>0.05)。治疗后两组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均低于治疗前,且研究组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均明显低于对照组(P<0.05)。见表4
治疗前,两组TLR4、NF-κB mRNA表达比较(P>0.05)。治疗后两组TLR4、NF-κB mRNA表达均低于治疗前,且研究组TLR4、NF-κBmRNA表达均明显低于对照组(P<0.05)。见表5
治疗过程中两组均未出现不良反应。
溃疡性结肠炎的发病机制较为复杂,其可能与线粒体功能缺陷、肠道微生物、免疫炎症、饮食习惯等相关。中医学根据溃疡性结肠炎的临床症状,将其归属于“下利”“肠风”“痢疾”“泄泻”,中医认为溃疡性结肠炎的关键在于脾虚和湿热,饮食不节及外感热邪致使脾运失常,无以运化水谷,引发湿热滞于肠间,壅滞肠道,损伤肠络,肉腐生脓,影响大肠传导,引发脓血、腹泻;脏腑之气不通,则肛门坠胀,诱发腹痛等症状;湿热下注,导致肛门灼热等症状。因此,临床治疗应首选清热祛湿止痢之法,同时结合健脾化瘀之法治疗[12-13]
经方鉴定了我国中医药的辨证论治体系基础、丰富了中医理论内涵,其用药精当、疗效显著,适应范围较广,为后世医家提供典范、培养了大批中医人才。本研究治疗溃疡性结肠炎,根据其临床症状选择清热渗湿汤化裁治疗,方中生芪能够疏通经络,可以帮助排出体内多余水分,治疗水肿、腹水、小便不利、虚胖等症状;又能够补充气力,提升阳气。党参补气兼能养血,故常用于平素倦怠乏力。炒白术能够补益脾气,增强脾的运化功能,适用于脾虚引起的泄泻等症状;又能燥湿和利尿,主要用于治疗水肿、痰饮等水湿停滞的病症。茯苓能够健脾补中、渗湿止泻,使中焦清升浊降,使湿无所聚,痰无所生,可用于治疗脾虚湿盛之食少体倦、便溏泄泻等症状。马齿苋可清热解毒、祛湿止带,玄府开通,小便将湿热之邪带出,津液通畅,不仅可用于治疗湿热下注,而且能够用于治疗湿热泄泻、痢疾等病症。苦参可清热燥湿,治疗湿热泻痢,缓解因湿热诱发的腹痛、里急后重等症状;同时能够利尿,可改善小便不利及水肿等症。陈皮可理气健脾,改善脾胃气滞;可燥湿化痰、化解祛湿,缓解湿阻中焦,改善肢体困倦、口黏腻等病症。炒白芍可止痛,用于缓解腹痛等症状;可养血调经,用于治疗月经不调、腹痛、崩漏等症状等。防风可止血止泻,对慢性腹泻有收敛止泻的效果;同时又可祛风解表、除湿止痛。蛇舌草、积雪草均具有清热解毒的作用,可用于治疗痈肿疮毒及咽喉肿痛等病症,又可解毒消肿。苍术可燥湿健脾,可用于治疗湿阻中焦诱发的脘腹胀满、疲倦乏力等症状;又可祛风散寒除湿。仙鹤草能够收敛止血,可用于便血、吐血等多种出血症;又可补虚止痢,可用于治疗身体虚弱引发的久泻久痢。炒黄柏可清热燥湿,可用于治疗湿热泻痢,缓解因湿热诱发的腹痛、里急后重等病证。槐花可凉血止血,能够治疗血痢、便血等各种出血症状;同时其性寒凉善清泻大肠之火而止血,尤其善治身体下部因血热诱发的出血。藿香可化湿解暑,可治疗湿阻中焦导致的身体困重、脘腹胀满等病证;又可治疗外感暑湿造成的泄泻、头痛等病证。甘草可调和诸药。上述诸药合用可健脾益气、清热利湿、开通玄府,除去肠道湿热之邪,恢复肠道内气血运行,濡养肠腑,通利肠道,止痢止血,故该方可治疗溃疡性结肠炎。本研究结果提示,治疗后,研究组总有效率高于对照组(P<0.05),且下腹部胀满及疼痛、肛门灼热、肛门坠胀感强、腹泻黏液脓血便、四肢沉重、胸闷发热、口干口苦、小便短赤评分均明显低于对照组(P<0.05),提示清热渗湿汤化裁能够有效缓解溃疡性结肠炎患者临床症状,疗效明显。本研究结果显示,治疗后,研究组Baron内镜评分、黏膜组织学评分均明显低于对照组(P<0.05),提示清热渗湿汤化裁能够有效降低溃疡性结肠炎患者Baron内镜评分、黏膜组织学评分,加快肠黏膜修复,促使患者康复。
溃疡性结肠炎发作时,多种致病因素共同诱发肠道黏膜局部炎症,影响肠道黏膜屏障功能,大量致病菌进入血液,诱发全身性炎症反应,导致白细胞介素-1β、肿瘤坏死因子-α等炎症介质水平升高[14-15]。白细胞介素-1β是常见的炎症因子,介导炎症反应,其参与肠道炎症的发生、发展。肿瘤坏死因子-α可介导炎症细胞向炎症部位聚集,释放炎症介质,导致局部组织发炎。既往研究指出[16],中医药能够有效降低溃疡性结肠炎患者炎症因子水平。D-乳酸是胃肠道菌群代谢产物,机体内发生病变后,肠道屏障功能受损,其进入血液中,导致患者病情加重[17]。内毒素存在于革兰氏阴性菌中,在细菌死亡裂解中,其进入机体内,可激活肿瘤坏死因子-α等炎症因子,引发炎症反应[18]。本研究结果显示,治疗后,研究组白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平均明显低于对照组(P<0.05),提示清热渗湿汤化裁能够有效降低溃疡性结肠炎患者白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平,降低炎症反应,修复肠道屏障功能。
激活TLR4/NF-κB信号通路是溃疡性结肠炎进展的核心机制,其激活后会大量释放白细胞介素-1β、肿瘤坏死因子-α等炎症因子,共同加速疾病的进展[19]。TLR4对肠道病原体入侵具有抵御作用,同时维持局部免疫因子的平衡。在溃疡性结肠炎中TLR4/NF-κB信号通路能够在肠上皮细胞中发挥交互作用,能够增强炎症反应,同时加重肠道黏膜损伤,增加肠道通透性,加重肠道损伤。既往研究指出[20],TLR4/NFκB信号通路在溃疡性结肠炎中呈高表达,给予中医药治疗后,可抑制TLR4/NF-κB信号通路,促使患者康复。本研究结果显示,治疗后研究组TLR4、NF-κBmRNA表达均明显低于对照组(P<0.05),提示清热渗湿汤化裁能够有效降低溃疡性结肠炎患者TLR4、NF-κB mRNA,有效减轻患者炎症状态。其原因可能是由于马齿苋的马齿苋粗多糖能够抑制痢疾杆菌等菌群[21]。生芪中的有效成分黄芪多糖能够抑制TLR4/NFκB信号通路,从而降低炎症反应[22]。积雪草中的积雪草苷能够有效调节TLR4/NF-κB信号通路,发挥抗炎作用[23]。藿香中的藿香水提取和醇提物具有抑菌的作用。藿香挥发油和提取物具有抗炎、镇痛的作用[24]。党参中的党参多糖能够调节肠道菌群、抗炎及免疫调节等作用[25]。苍术中的苍术素具有抗炎、利尿的作用,而苍术酮具有抗病毒的作用[26]。苦参中的苦参碱具有抑制炎症过程的各个阶段,同时能够抑菌的作用[27]。本研究进一步分析清热渗湿汤化裁对溃疡性结肠炎患者不良反应,结果提示,清热渗湿汤化裁治疗溃疡性结肠炎患者,不会增加不良反应,安全性高。
综上所述,清热渗湿汤化裁能够有效改善溃疡性结肠炎患者临床症状,Baron内镜评分、黏膜组织学评分,加快肠黏膜修复,降低白细胞介素-1β、肿瘤坏死因子-α、内毒素、D-乳酸水平,降低炎症反应,修复肠道屏障功能,同时能够抑制TLR4/NF-κB信号通路,发挥抗炎的作用,且不会增加不良反应,具有较高的安全性,可在临床推广应用。
