目的

分析针刺疗法与高频重复经颅磁刺激对脊髓损伤神经病理性疼痛的治疗效果。

方法

纳入2021年6月—2024年5月西安国际医学中心医院脊柱脊髓康复科收治的92例脊髓损伤神经病理性疼痛患者为研究对象，随机数字表法分为两组，对照组46例予以高频重复经颅磁刺激治疗，观察组46例予以针刺疗法与高频重复经颅磁刺激联合治疗，对比两组治疗效果、疼痛情况、神经电生理指标、血清炎症因子水平变化、心理状态评分情况。

结果

观察组治疗总有效率为95.65%（44/46），远高于对照组的76.09%（35/46），组间比较差异有统计学意义（P<0.05）；治疗后1、2个月观察组疼痛数字视觉模拟评分量表（VAS）评分低于对照组（P<0.05）；治疗后观察组美国脊柱损伤协会（ASIA）脊髓损伤感觉、运动评分高于对照组，静息运动阈值（RMT）低于对照组，运动诱发电位（MEP）高于对照组（P<0.05）；治疗后观察组肿瘤坏死因子-α（TNF-α）、白介素-10（IL-10）、白细胞介素-6（IL-6）低于对照组（P<0.05）；治疗后观察组汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA）评分均低于对照组（P<0.05）。

结论

综上所述，对脊髓损伤神经病理性疼痛患者应用针刺疗法与高频重复经颅磁刺激联合治疗能有效减轻患者疼痛程度，调节血清炎症因子，对神经起到保护作用，促进神经功能恢复。

Objective

To analyze the therapeutic effect of acupuncture therapy and high-frequency repetitive transcranial magnetic stimulation (rTMS) on neuropathic pain of spinal cord injury.

Methods

A total of 92 patients with spinal cord injury-induced neuropathic pain treated at the Spinal Cord Rehabilitation Department of Xi'an International Medical Center Hospital from June 2021 to May 2024, were enrolled in this study. The patients were randomly divided into two groups using a random number table method. The control group (46 cases) received high-frequency rTMS treatment, while the observation group (46 cases) received a combination of acupuncture therapy and high-frequency rTMS. The two groups were compared in terms of treatment efficacy, pain levels, neuroelectrophysiological parameters, oxidative stress markers, serum inflammatory cytokine levels and psychological status scores.

Results

The total effective rate of the observation group was 93.48%, which was much higher than that of the control group (76.09%), and the difference between groups was significant (P<0.05). The visual analogue scale (VAS) score of the observation group was lower than that of the control group 1, 2 and 3 months after treatment (P<0.05). After treatment, American Spinal Injury Association (ASIA) sensory and motor scores, resting motor threshold (RMT) and motor evoked potential (MEP) in the observation group were higher than those in the control group (P<0.05). After treatment, the levels of tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10) and interleukin-6 (IL-6) in the observation group were lower than those in the control group (P<0.05). After treatment, the Hamilton Depression Rating Scale (HAMD) and Hamilton Anxiety Rating Scale (HAMA) scores of the observation group were lower than those of the control group (P<0.05).

Conclusion

In summary, acupuncture therapy combined with high-frequency rTMS can effectively reduce the degree of pain in the patients with neuropathic pain from spinal cord injury, regulate the serum inflammatory factors and oxidative stress response, play a protective role on nerves, and promote the recovery of nerve function.
