Clinical Observation on Treatment of Stasis-Type Rheumatoid Arthritis with Modified Huoluo Xiaoling Formula（活络效灵方加减）

目的

观察活络效灵方加减治疗瘀阻型类风湿关节炎（rheumatoid arthritis，RA）的临床疗效。

方法

病例选取时间段2022年1月—2023年12月，研究人群为就诊于亳州市人民医院的120例瘀阻型RA患者，将上述患者采用简单随机法分为两组，单组60例。对照组给予甲氨蝶呤联合来氟米特治疗，观察组在对照组基础上联合活络效灵方加减治疗，连续用药12周。比较两组中医症状评分、血沉（erythrocyte sedimentation rate，ESR）、抗环瓜氨酸肽抗体（anti-cyclic peptide containing citrulline，CCP）、白细胞介素-6（interleukin-6，IL-6）、类风湿因子（rheumatoid factor，RF）变化，评估两组视觉模拟（visual analogue scale，VAS）评分、28个关节疾病活动度（Disease Activity Score using 28 joints，DAS28）评分差异，统计两组临床总有效率发生率。

结果

两组治疗前中医症状评分比较，差异无统计学意义（P > 0.05）。治疗12周与治疗前比较，两组关节肿胀、压痛、晨僵、畸形及肌肉刺痛、屈伸不利、面色暗淡、肌肤甲错评分均降低，观察组治疗12周较对照组更低（P < 0.05）。两组治疗前ESR、抗CCP抗体、IL-6、RF比较，差异无统计学意义（P > 0.05）。治疗12周与治疗前比较，两组抗CCP抗体、IL-6、ESR、RF均降低，观察组治疗12周较对照组更低（P < 0.05）。两组治疗前DAS28评分、VAS评分比较，差异无统计学意义（P > 0.05）。治疗12周与治疗前比较，两组DAS28评分、VAS评分均降低，观察组治疗12周较对照组更低（P < 0.05）。观察组临床总有效率发生率为93.33%，高于对照组的80.00%，差异有统计学意义（P < 0.05）。

结论

活络效灵方加减治疗瘀阻型RA可降低ESR、抗CCP抗体、IL-6、RF水平，提高疗效。

Objective

To observe the clinical efficacy of Modified Huoluo Xiaoling Formula（活络效灵方加减）in the treatment of stasis-type rheumatoid arthritis（RA）.

Methods

The time period of case selection was from January 2022 to December 2023, and the study population consisted of 120 patients with stasis-type RA attending the Bozhou People＇s Hospital, which was divided into two groups by simple randomisation method. The control group was treated with methotrexate combined with leflunomide, while the observation group was treated on the basis of the control group with Modified Huoluo Xiaoling Formula, and the medication was administered for 12 consecutive weeks. The two groups were compared in terms of TCM symptom scores, erythrocyte sedimentation rate（ESR）, anti-cyclic peptide containing citrulline（CCP）, interleukin-6（IL-6）and rheumatoid factor（RF）. The differences in visual analogue scale（VAS）scores, Disease Activity Score using 28 joints（DAS28）scores were assessed between the two groups, and the overall clinical effective rate and incidence were analyzed.

Results

There was no significant difference in TCM syndrome scores between the two groups before treatment（P > 0.05）. Compared with those before treatment, the scores of joint swelling, tenderness, morning stiffness, deformity, muscle tenderness, limited flexion and extension, pale complexion and dry skin were lower in both groups after 12 weeks of treatment. The observation group had lower scores than the control group after 12 weeks of treatment（P < 0.05）. There was no significant difference in ESR, anti-CCP antibodies, IL-6 or RF between the two groups before treatment（P > 0.05）. Compared with those before treatment, the levels of anti-CCP antibodies, IL-6, ESR and RF were lower in both groups after 12 weeks of treatment, with the observation group having lower levels than the control group（P < 0.05）. There was no significant difference in DAS28 scores or VAS scores between the two groups before treatment（P > 0.05）. Compared with those before treatment, the DAS28 scores and VAS scores were lower in both groups after 12 weeks of treatment, with the observation group having lower scores than the control group（P < 0.05）. The clinical effectiveness rate in the observation group was 93.33%, higher than 80.00% in the control group, with a significant difference（P < 0.05）.

Conclusion

The treatment of stasis-type RA with Modified Huoluo Xiaoling Formula can reduce the levels of ESR, anti-CCP antibody, IL-6 and RF, and improve the therapeutic efficacy.
