Clinical Efficacy of Patients with Severe Acute Respiratory Distress Syndrome Treated with Modified Xuanbai Chengqi Decoction（宣白承气汤加减）as adjuvant therapy

目的

分析宣白承气汤加减辅助治疗重症急性呼吸窘迫综合征的临床疗效及转归情况。

方法

选取重症急性呼吸窘迫综合征患者120例作为研究对象，随机数字表法分为两组，对照组60例采用常规治疗，观察组60例在对照组基础上予以宣白承气汤加减辅助治疗，对比两组血气指标、中医证候积分、炎症因子、急性生理和慢性健康状况（APACHE Ⅱ）、序贯性器官衰竭（SOFA）评分、机械通气时间、ICU住院时间等。

结果

治疗后3、7 d观察组动脉血二氧化碳分压（PaCO₂）低于对照组，动脉血氧分压（PaO₂）、氧合指数（PaO₂/FiO₂）水平高于对照组（P<0.05）；治疗后观察组中医证候积分低于对照组，机械通气时间、ICU住院时间短于对照组（P<0.05）；治疗后3、7 d观察组白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）、白细胞介素-1β（IL-1β）水平低于对照组（P<0.05）；治疗后APACHE II评分与SOFA评分低于对照组（P<0.05）。

结论

应用宣白承气汤加减辅疗重症急性呼吸窘迫综合征患者有利于改善患者氧合指数及血氧代谢情况，增强组织循环，调节炎症因子水平，更快纠正呼吸困难、低氧血症等表现，促使患者预后改善。

Objective

To analyze the clinical efficacy and regression of patients with severe acute respiratory distress syndrome treated with Modified Xuanbai Chengqi Decoction（宣白承气汤加减）.

Methods

A total of 120 patients with severe acute respiratory distress syndrome were selected for the study，60 cases of the control group were treated with conventional treatment，and 60 cases of the observation group were treated with auxiliary treatment of Modified Xuanbai Chengqi Decoction based on the above mentioned foundation. The blood gas indicators，traditional Chinese medicine syndrome scores，inflammatory factors，acute physiology and chronic health conditions（APACHE Ⅱ），sequential organ failure assessment（SOFA）scores，mechanical ventilation time and ICU stay time of the two groups were compared between the two groups.

Results

The level of partial pressure of carbon dioxide（PaCO₂）of the observation group was lower than that of the control group and the levels of partial pressure of oxygen（PaO₂）and PaO₂/fraction of inspiration O₂（FiO₂）were higher than those of the control group after 3 d and 7 d of treatment（P<0.05）. The traditional Chinese medicine syndrome scores of the observation group were lower than those of the control group，and the time of mechanical ventilation and ICU hospitalization were shorter than those of the control group after treatment（P<0.05）. The levels of interleukin-1β（IL-1β），tumor necrosis factor-α（TNF-α）and interleukin-6（IL-6）of the observation group were lower than those of the control group after 3 d and 7 d of treatment（P<0.05）. The APACHE Ⅱ scores and SOFA scores were lower than those of the control group after treatment（P<0.05）.

Conclusion

The application of Modified Xuanbai Chengqi Decoction adjuvant treatment for patients with severe acute respiratory distress syndrome is conducive to improving the patients＇ oxygenation index and blood oxygen metabolism，enhancing the tissue circulation，regulating the levels of inflammatory factors，correcting the manifestations of respiratory distress and hypoxemia faster，and contributing to the improvement of the patients＇prognosis.
