Effect of Danggui Dilong Guizhi Decoction（当归地龙桂枝汤）Combined with Acupuncture and Moxibustion in Treatment of Cervical Spondylosis Pain and Upper Limb Numbness

目的

研究当归地龙桂枝汤联合针灸治疗颈椎病疼痛及上肢麻木的效果。

方法

选取2021年6月—2023年10月医院针灸科诊治的颈椎病疼痛及上肢麻木患者128例作为研究对象，随机均分为两组，对照组（64例）进行针灸治疗，观察组（64例）进行当归地龙桂枝汤联合针灸治疗，疗程4周，观察并比较两组患者的疼痛及上肢麻木症状改善情况、颈椎功能、肌电图指标、炎症因子、不良反应情况。

结果

治疗后，观察组患者疼痛和上肢麻木评分均显著低于对照组［（0.87±0.14）分、（0.79±0.12）分vs（0.96±0.20）分、（0.88±0.17）分］（P<0.05），观察组颈椎功能障碍指数（Neck Disability Index，NDI）中颈痛及相关症状、日常生活活动能力及总分均显著高于对照组［（6.08±1.84）分、（9.74±1.69）分、（15.62±1.73）分vs（8.90±2.01）分、（10.83±2.30）分、（18.32±2.26）分］（P<0.05），观察组运动神经传导速度（motor nerve conduction velocity，MCV）和感觉神经传导速度（sensory nerve conduction velocity，SCV）均显著高于对照组［（49.22±4.30）m/s、（43.36±4.51）m/s vs（46.85±3.91）m/s、（39.40±5.48）m/s］（P<0.05），观察组血清白细胞介素（interleukin，IL）-8、IL-1β、肿瘤坏死因子-α（tumor necrosis factor-α，TNF-α）水平显著低于对照组［（59.73±11.42）ng/L、（3.87±1.08）ng/L、（9.77±2.38）ng/L vs（65.71±12.13）ng/L、（5.27±1.20）ng/L、（11.21±2.61）ng/L］（P<0.05）。两组均无严重不良反应发生。

结论

当归地龙桂枝汤联合针灸治疗颈椎病疼痛及上肢麻木的效果显著，能够改善患者疼痛及上肢麻木症状，增强颈椎功能、提高肌力，还可减轻炎症反应，且安全性好，值得应用。

Objective

To study the effect of Danggui Dilong Guizhi Decoction（当归地龙桂枝汤）combined with acupuncture and moxibustion on cervical spondylosis pain and upper limb numbness.

Methods

A total of 128 patients with cervical spondylosis pain and upper limb numbness diagnosed and treated in the acupuncture department of the hospital from June 2021 to October 2023 were selected as the research objects and randomly divided into two groups. Sixty-four patients in the control group were treated with acupuncture and moxibustion，and the other 64 patients in the observation group were treated with Danggui Dilong Guizhi Decoction combined with acupuncture and moxibustion. The course of treatment was 4 weeks. The improvement of pain and upper limb numbness，cervical spine function，electromyography index，inflammatory factors and adverse reactions were observed and compared between the two groups.

Results

After treatment，the scores of pain and upper limb numbness in the observation group were significantly lower than those in the control group［（0.87±0.14）points，（0.79±0.12）points vs（0.96±0.20）points，（0.88±0.17）points］（P<0.05）. The neck pain and related symptoms，activities of daily living and total scores of Neck Disability Index（NDI）in the observation group were significantly higher than those in the control group［（6.08±1.84）points，（9.74±1.69）points，（15.62±1.73）points vs（8.90±2.01）points，（10.83±2.30）points，（18.32±2.26）points］（P<0.05），The motor nerve conduction velocity（MCV）and sensory nerve conduction velocity（SCV）in the observation group were significantly higher than those in the control group［（49.22±4.30）m/s，（43.36±4.51）m/s vs（46.85±3.91）m/s，（39.40±5.48）m/s］（P<0.05），The levels of serum interleukin（IL）-8，IL-1β and tumor necrosis factor-α（TNF-α）in the observation group were significantly lower than those in the control group［（59.73±11.42）ng/L，（3.87±1.08）ng/L，（9.77±2.38）ng/L vs（65.71±12.13）ng/L，（5.27±1.20）ng/L，（11.21±2.61）ng/L］（P<0.05）. No serious adverse reactions occurred in both groups.

Conclusion

Danggui Dilong Guizhi Decoction combined with acupuncture has a significant effect on the treatment of cervical spondylosis pain and upper limb numbness，it can improve the symptoms of pain and upper limb numbness，enhance the cervical spine function，improve the muscle strength and reduce the inflammatory response，it is safe and worthy of application.
