Objective

To evaluate the impact of Yougui Pill（右归丸）and Modified Ditan Decoction（涤痰汤）on endocrine hormone metabolism and insulin resistance in the treatment of polycystic ovary syndrome（PCOS）.

Methods

A total of 102 patients with PCOSadmitted to the hospital from March 2020 to March 2024 were selected as the study objects，51 cases in the control group were treated with Western medicine alone，and a total of 51 patients in the observation group received treatment with Yougui Pill and Modified Ditan Decoction based on the standard therapy，and the clinical effects，sex hormone indexes，ovarian and menstrual conditions，and glucose metabolism indexes of the two groups were compared.

Results

The total effective rate of the observation group was 98.04%，higher than 78.43% of the control group，and the difference was significant（P<0.05）. After treatment，the serum levels of testosterone（T）and luteinizing hormone（LH）in the two groups were lower than those before treatment，and the levels of estradiol（E2），prolactin（PRL），follicle-stimulating hormone（FSH）and sex hormone binding globulin（SHBG）were higher than those before treatment（P<0.05），and the decrease/increase in the observation group was more significant than that in the controlgroup（P<0.05）. After treatment，the ovarian volume and follicle number of the two groupswere increased（P<0.05），and the increase was more significant in the observation group compared with that in the control group（P<0.05）. The recovery rate of menstruation in the observation group was96.08%，which was higher than that（74.51%）in control group（P<0.05）. The ovarian recovery rate of the observation group was 96.08%，higher than that of the control group（76.47%，P<0.05）. After treatment，the levels of fasting blood glucose（FBG），fasting insulin（FINS），2-hour blood glucose（2 h PG）and insulin resistance index（HOMA-IR）in both groups were decreased（P<0.05）. Compared with those in the control group，the levels of FBG，FINS，2 h PG and HOMA-IR in the observation group were significantly decreased（P<0.05）.

Conclusion

In patients with PCOS，the combined treatment with medication and the approach of warming kidney and resolving phlegm demonstrates the significant therapeutic efficacy. This regimen effectively regulates the endocrine hormone levels，improves the glucose metabolism parameters and facilitates the recovery of the patients.

目的

分析右归丸、涤痰汤化裁治疗多囊卵巢综合征对内分泌激素代谢和胰岛素抵抗的影响。

方法

选取2020年3月—2024年3月医院收治的多囊卵巢综合征患者102例为研究对象，51例对照组采用单纯西药治疗，51例观察组在上述基础上予以右归丸、涤痰汤化裁治疗，对比两组临床效果、性激素指标、卵巢及月经情况、糖代谢指标情况。

结果

观察组与对照组总有效率对比，98.04%高于78.43%，差异有统计学意义（P<0.05）。治疗后，两组血清睾酮（Serum Testosterone，T）、黄体生成素（Luteinizing Hormone，LH）水平较前降低，雌二醇（Estradiol，E2）、泌乳素（Prolactin，PRL）、卵泡刺激素（Follicle-Stimulating Hormone，FSH）以及性激素结合球蛋白（Sex Hormone-Binding Globulin，SHBG）较前升高（P<0.05），且与对照组比较，观察组降低/升高更为显著（P<0.05）。治疗后两组卵巢体积、卵泡数目均减少（P<0.05），且与对照组比较，观察组减少更为显著（P<0.05）；观察组月经恢复率96.08%，高于对照组的74.51%（P<0.05）；观察组卵巢恢复率96.08%，高于对照组的76.47%（P<0.05）。治疗后两组空腹血糖（Fasting Blood Glucose，FBG）、空腹胰岛素（Fasting Insulin，FINS）、2 h血糖（2-hour Plasma Glucose，2 hPG）、性激素结合球蛋白（Sex Hormone-Binding Globulin，SHBG）水平均有所降低（P<0.05），与对照组比较，观察组FBG、FINS、2 hPG以及胰岛素抵抗指数（HOMA-IR）水平降低更为明显（P<0.05）。

结论

被诊断为多囊卵巢综合征的患者使用药物、右归丸、涤痰汤化裁联合治疗后，治疗效果显著，可有效改善患者的激素水平，改善其糖代谢指标，促进患者恢复。

目的

分析右归丸、涤痰汤化裁治疗多囊卵巢综合征对内分泌激素代谢和胰岛素抵抗的影响。

方法

选取2020年3月—2024年3月医院收治的多囊卵巢综合征患者102例为研究对象，51例对照组采用单纯西药治疗，51例观察组在上述基础上予以右归丸、涤痰汤化裁治疗，对比两组临床效果、性激素指标、卵巢及月经情况、糖代谢指标情况。

结果

观察组与对照组总有效率对比，98.04%高于78.43%，差异有统计学意义（P<0.05）。治疗后，两组血清睾酮（Serum Testosterone，T）、黄体生成素（Luteinizing Hormone，LH）水平较前降低，雌二醇（Estradiol，E2）、泌乳素（Prolactin，PRL）、卵泡刺激素（Follicle-Stimulating Hormone，FSH）以及性激素结合球蛋白（Sex Hormone-Binding Globulin，SHBG）较前升高（P<0.05），且与对照组比较，观察组降低/升高更为显著（P<0.05）。治疗后两组卵巢体积、卵泡数目均减少（P<0.05），且与对照组比较，观察组减少更为显著（P<0.05）；观察组月经恢复率96.08%，高于对照组的74.51%（P<0.05）；观察组卵巢恢复率96.08%，高于对照组的76.47%（P<0.05）。治疗后两组空腹血糖（Fasting Blood Glucose，FBG）、空腹胰岛素（Fasting Insulin，FINS）、2 h血糖（2-hour Plasma Glucose，2 hPG）、性激素结合球蛋白（Sex Hormone-Binding Globulin，SHBG）水平均有所降低（P<0.05），与对照组比较，观察组FBG、FINS、2 hPG以及胰岛素抵抗指数（HOMA-IR）水平降低更为明显（P<0.05）。

结论

被诊断为多囊卵巢综合征的患者使用药物、右归丸、涤痰汤化裁联合治疗后，治疗效果显著，可有效改善患者的激素水平，改善其糖代谢指标，促进患者恢复。

Objective

To evaluate the impact of Yougui Pill（右归丸）and Modified Ditan Decoction（涤痰汤）on endocrine hormone metabolism and insulin resistance in the treatment of polycystic ovary syndrome（PCOS）.

Methods

A total of 102 patients with PCOSadmitted to the hospital from March 2020 to March 2024 were selected as the study objects，51 cases in the control group were treated with Western medicine alone，and a total of 51 patients in the observation group received treatment with Yougui Pill and Modified Ditan Decoction based on the standard therapy，and the clinical effects，sex hormone indexes，ovarian and menstrual conditions，and glucose metabolism indexes of the two groups were compared.

Results

The total effective rate of the observation group was 98.04%，higher than 78.43% of the control group，and the difference was significant（P<0.05）. After treatment，the serum levels of testosterone（T）and luteinizing hormone（LH）in the two groups were lower than those before treatment，and the levels of estradiol（E2），prolactin（PRL），follicle-stimulating hormone（FSH）and sex hormone binding globulin（SHBG）were higher than those before treatment（P<0.05），and the decrease/increase in the observation group was more significant than that in the controlgroup（P<0.05）. After treatment，the ovarian volume and follicle number of the two groupswere increased（P<0.05），and the increase was more significant in the observation group compared with that in the control group（P<0.05）. The recovery rate of menstruation in the observation group was96.08%，which was higher than that（74.51%）in control group（P<0.05）. The ovarian recovery rate of the observation group was 96.08%，higher than that of the control group（76.47%，P<0.05）. After treatment，the levels of fasting blood glucose（FBG），fasting insulin（FINS），2-hour blood glucose（2 h PG）and insulin resistance index（HOMA-IR）in both groups were decreased（P<0.05）. Compared with those in the control group，the levels of FBG，FINS，2 h PG and HOMA-IR in the observation group were significantly decreased（P<0.05）.

Conclusion

In patients with PCOS，the combined treatment with medication and the approach of warming kidney and resolving phlegm demonstrates the significant therapeutic efficacy. This regimen effectively regulates the endocrine hormone levels，improves the glucose metabolism parameters and facilitates the recovery of the patients.

多囊卵巢综合征（Polycystic Ovary Syndrome，PCOS）是临床上比较常见的内分泌疾病，主要是由于糖脂代谢障碍，导致患者生殖功能发生障碍[1]。目前，临床治疗多囊卵巢综合征常采取西药进行药物治疗。炔雌醇环丙孕酮片属于雌激素、孕激素的复合制剂，对改善PCOS临床症状具有一定疗效[2]。患者通常伴有明显的糖脂代谢异常以及胰岛素抵抗。使用二甲双胍可有效改善患者的胰岛素抵抗，有效促使排卵。但是单一使用西药治疗虽能改善患者的临床效果，但是常伴有一定的不良反应，临床疗效不佳。从中医角度上看，多囊卵巢综合征属于“不孕”“崩漏”“癥瘕”等范畴[3]。PCOS主要是由于肝、肾、脾功能失常，导致瘀、痰等病理产物阻滞于胞宫，导致生殖系统障碍和代谢异常[4]。治疗时，因以温肾补阳、健脾化湿、除瘀为主，予以右归丸、涤痰汤化裁治疗，进而改善患者的临床症状，促使患者尽快恢复，达到治疗目的。鉴于此，本次研究对PCOS的患者采用右归丸、涤痰汤化裁联合治疗后，获得较好效果。现报道如下。
1　临床资料

1.1　病例来源
所有病例来源于浙江中医药大学附属杭州市中医院妇科门诊部所收治的PCOS患者。
1.2　诊断标准
参照《妇产科学》《中医妇产科》《多囊卵巢综合征的诊断和治疗专家共识》作为诊断标准。
1.2.1　西医诊断标准
患者均符合《妇产科学》[5]、《多囊卵巢综合征的诊断和治疗专家共识》[6]中关于多囊卵巢综合征的相关针对标准。①稀发排卵或是没有排卵：即患者存在月经稀发、闭经或异常子宫出血；②表现为高雄激素的临床症状和（或）临床检查提示高雄激素血症；③彩超提示一侧或双侧的卵巢多囊，且卵巢的直径在2～9 mm，卵泡在12个及以上；卵巢的体积在10 mL及以上；上述三项中符合两项即可确诊。
1.2.2　中医辨证标准
参照《中医妇产科》[7]中关于多囊卵巢综合征的相关诊断标准，中医辨证为肾虚夹瘀（肾阳虚痰瘀互结）：月经后期，月经量少，继而闭经或是出现崩漏，色黯，有血块；经期腹痛，腰膝酸软，胸闷头晕；舌黯淡，舌质有瘀点，苔白厚腻，脉弦滑。
1.3　纳入标准
①所有患者均符合上述中医、西医诊断标准；②签署知情文件自愿参与；③年龄在18岁以上；④临床资料完善。
1.4　排除标准
①合并其余内分泌疾病的患者，包括甲亢、甲减、高密乳素血症、柯兴氏综合征等；②存在卵巢或输卵管病变的患者；③研究前3个月使用过激素治疗；④对研究药物过敏的患者；⑤不能配合治疗的患者。
1.5　一般资料
选取2020年3月—2024年3月浙江中医药大学附属杭州市中医院妇科收治的PCOS患者102例为研究对象。对照组51例患者中，年龄24～36岁，平均（29.28±3.49）岁；病程1～3年，平均（2.05±0.35）年；体质量46.7～73.2 kg，平均（59.57±9.87）kg；其中，已婚患者29例，未婚患者22例；月经紊乱20例，闭经20例，多毛5例，痤疮6例。观察组51例患者中，年龄24～37岁，平均（30.24±3.47）岁；病程1～3年，平均（2.10±0.40）年；体质量46.6～73.3 kg，平均（59.52±9.74）kg；其中，已婚患者27例，未婚患者24例；月经紊乱18例，闭经18例，多毛7例，痤疮8例。两组患者上述资料比较差异无统计学意义（P>0.05），可比性较好。本研究经伦理委员会审批通过。
1.6　分组方法
使用RAND Corporation的随机数字表随机选择数字中的任一数字作为抽样起点，从起点开始先取所需要数量的样本、并逐一对应调查数据集中病例，记录并选定102例患者，分为观察组、对照组各51例。
2　方法

2.1　治疗方法
2.1.1　对照组
对照组采用单纯西药治疗。在来月经时或是黄体酮撤药性出血的第5天开始，予以患者炔雌醇环丙孕酮片（拜耳医药保健有限公司广州分公司；国药准字20140114），1片/次，睡前服用，连续使用21 d。停药后7 d重复给药。盐酸二甲双胍片（寿光富康制药有限公司；国药准字H31020246），第1周每次0.25 g，2次/d，餐后服用；第2周开始增加至每次0.5 g，3次/d，餐后服用。连续治疗3个月。
2.1.2　观察组
在上述基础上予以右归丸、涤痰汤化裁治疗，方药如下：肉桂3 g，鹿角片9 g，菟丝子15 g，熟地12 g，山萸肉9 g，山药15 g，当归12 g，川芎12 g，法半夏9 g，苍术12 g，白芥子9 g，茯苓12 g，香附9 g。水煎服，1剂/d，早晚温服。经期需停药。连续治疗3个月。
2.2　观察指标及方法
2.2.1　临床疗效对比[8]
痊愈：患者卵巢功能、月经恢复正常，临床症状消失，中医证候积分降低90%及以上；显效：患者卵巢功能、月经趋于正常，但未完全恢复，临床症状明显减轻，中医证候积分降低70%～<90%；有效：患者卵巢功能、月经异常情况改善，临床症状较前缓解，中医证候积分降低30%～<70%；无效：患者卵巢功能、月经异常情况无改善，临床症状无减轻，中医证候积分降低30%以下。总有效率＝［（痊愈+显效+有效）/总例数］×100%。
2.2.2　性激素指标对比
在治疗前以及治疗3个月后，待月经来潮的第3～5天，抽取患者清晨空腹静脉血，采取化学发光法检测其血清睾酮（Serum Testosterone，T）、黄体生成素（Luteinizing Hormone，LH）、雌二醇（Estradiol，E2）、泌乳素（Prolactin，PRL）、卵泡刺激素（Follicle-Stimulating Hormone，FSH）以及性激素结合球蛋白（Sex Hormone-Binding Globulin，SHBG）。
2.2.3　卵巢、月经情况对比
在治疗前以及治疗3个月后，予以患者B超检查，检测其卵巢体积、卵泡的数目、排卵和月经恢复的情况。
2.2.4　糖代谢指标对比
在治疗前以及治疗3个月后，抽取患者清晨空腹静脉血，采取葡萄糖氧化酶法检测其空腹血糖（Fasting Blood Glucose，FBG）、2 h血糖（2-hour Plasma Glucose，2 hPG），采用化学发光法检测空腹胰岛素（Fasting Insulin，FINS），以及胰岛素抵抗指数（Homeostasis Model Assessment of Insulin Resistance，HOMA-IR）。HOMA-IR＝FBG×FINS÷22.5。
2.3　统计学方法
将采集数据采用SPSS 25.0处理分析，经图形检验（直方图/QQ图等）数据资料均符合正态分布，计数资料用［例（%）］表示，采用χ2检验；性激素指标、卵巢及月经情况、糖代谢等指标用（）表示，用t检验。P<0.05为差异有统计学意义。
3　结果

3.1　两组临床疗效比较
观察组与对照组总有效率对比，98.04%高于78.43%，差异有统计学意义（P<0.05）。见表1。
3.2　两组性激素比较
治疗前两组T、LH、E2、PRL、FSH、SHBG比较差异无统计学意义（P>0.05）；治疗后，两组T、LH水平较前降低，E2、PRL、FSH、SHBG较前升高（P<0.05），且与对照组比较，观察组降低/升高更为显著（P<0.05）。见表2。
3.3　两组卵巢、月经情况比较
治疗前两组卵巢、月经情况比较差异无统计学意义（P>0.05）；治疗后两组卵巢体积、卵泡数目均减少（P<0.05），且与对照组比较，观察组减少更为显著（P<0.05）；观察组月经恢复率96.08%，高于对照组的74.51%（P<0.05）；观察组卵巢恢复率96.08%，高于对照组的76.47%（P<0.05）。见表3。
3.4　两组糖代谢比较
治疗前两组FBG、FINS、2 hPG、HOMA-IR水平比较差异无统计学意义（P>0.05）；治疗后两组FBG、FINS、2 hPG、HOMA-IR水平均有所降低（P<0.05）。与对照组比较，观察组FBG、FINS、2 hPG、HOMA-IR水平降低更为明显（P<0.05）。见表4。
4　讨论

PCOS属于妇科常见病，其发病率约有5%～10%，也是导致女性不孕的一大原因[9-10]。PCOS的病因尚不清晰，现认为与环境因素、遗传因素有关。其主要特征为雄激素水平过高、无排卵、卵巢多囊样变，致使患者出现月经后期、痤疮、多毛，甚至不孕等症状[11]。若不及时干预治疗，随着病情发展，不仅会导致患者出现妇科肿瘤等疾病，还会增加对心脑血管疾病、糖尿病等并发症的发生率，危及患者的生命安全。因此，需要及时加以有效治疗。目前，临床上治疗多囊卵巢综合征常采取药物治疗。西医治疗PCOS常采取调经药物、改善血雄激素水平药物、改善胰岛素抵抗药物等进行治疗[12-13]。炔雌醇环丙孕酮片属于复方制剂，包括醋酸环丙孕酮和炔雌醇，可有效调节患者体内的激素水平。二甲双胍可用于治疗多囊卵巢综合征，通过药物提高对胰岛素的敏感性，进而恢复血糖代谢的稳定性，改善患者的胰岛素抵抗状态，进而促使患者月经规律，促进其排卵，改善其症状[14-15]。西药治疗PCOS虽有一定效果，但是长期使用极易出现耐药性，预后差，临床效果不理想。
现今，随着PCOS的发病率逐年增长，中医治疗的优势逐渐凸显。采取中医治疗PCOS已逐渐成熟运用到临床当中，备受重视。中医认为，该病的基本病机为肾亏，肾阳为其化源，是卵泡发育的重要物质基础；肾阳充足，气化旺盛，是卵泡发育和排卵的原动力[16-17]。因此，补肾为其基本治法，贯穿始终。PCOS的主要症状是月经迟、月经不调，甚至闭经。经血每月不能按时出现，月经不调，即瘀血停滞；痰浊阻塞，气血运行出现异常，气滞则血停，出现痰瘀互结、痰瘀互生；而痰浊壅盛，脂肪充溢，故见肥胖[18-19]。根据临床中医辨证进行治疗，多囊卵巢综合征患者治疗时应以温肾为基础，并加以健脾化痰除湿，行气活血，逐瘀通络。本病的病程较长，导致痰湿的原因主要是顽痰，采取涤痰汤化裁联用利于祛除顽痰[20]，右归丸主治肾阳不足，涤痰丸主治肥盛、不孕等症[21，22]。因此，治疗本病时需要温肾、涤痰同时进行。方中肉桂具有补火助阳、散寒止痛的功效；鹿角片补肾壮阳、益血生精；菟丝子补肾益精；而熟地黄是由生地黄所炮制而成，属于滋补肝肾阴血的药物，可达到滋阴补血、益精填髓的功效，对血虚、肝肾亏虚等症候具有较好疗效[23-24]。当归具有补血活血、止痛调经的功效。与熟地黄合用，可达到滋阴养血、补肾填精的效果[25]。山萸肉归肝肾经，具有补益肝肾、收敛止汗、涩精固脱的作用。苍术可以健脾燥湿、祛风散寒，治疗湿痰留饮。白术健脾益气、燥湿利水、止汗，为脾脏补气第一要药[26]。茯苓健脾安神、利水渗湿，为除湿圣药。白芥子利气祛痰、通络止痛、温中散寒。法半夏燥湿化痰、消痞散结，香附疏肝解郁、利气调经、止痛调中[27-28]。诸药合用，可达到补肾填精、涤痰之效。现代药理研究表明，补肾中药可以通过调控下丘脑-垂体-卵巢轴的平衡，起到促性腺激素样的作用。对治疗女性性激素失调症，往往可以获得与性激素相当甚至更好、更持久的效果[29]。
在本研究中，观察组总有效率、月经恢复率、卵巢恢复率更高，治疗后观察组卵巢体积、卵泡数目比对照组更高（P<0.05）。说明多囊卵巢综合征的患者使用右归丸、涤痰汤化裁联合治疗可有效提升临床效果，减轻患者的临床症状，促进患者月经规律、卵巢恢复正常。与对照组比较，观察组T、LH水平降低更为显著，E2、PRL、FSH、SHBG升高更为显著（P<0.05）。说明右归丸、涤痰汤化裁联用治疗可有效改善患者的内分泌指标，作用更好。胰岛素抵抗是由于胰岛素的产生低于正常情况，为了维持相对正常的血糖水平，机体会分泌出过多的胰岛素，进而形成代偿性的高胰岛素血征，此时即为胰岛素抵抗状态。而多囊卵巢综合征患者通常伴有明显的胰岛素抵抗特征[30-32]。与对照组比较，观察组FBG、FINS、2 hPG、HOMA-IR水平降低更为明显（P<0.05）。说明联合用药可有效改善患者的糖代谢水平，改善胰岛素抵抗状态，促使患者更快恢复。
综上所述，PCOS的患者使用西药治疗、右归丸合涤痰汤化裁治疗可有效提高治疗效果，减轻患者的临床症状，改善其内分泌水平以及胰岛素抵抗，促使患者尽快康复/患者接受度较好，但本研究也有不足之处，首先，样本量较小，且对于病情复杂的PCOS患者，治疗时间较短。其次，缺乏长期随访数据，无法评估长远的治疗效果。未来研究将考虑扩大样本量、延长观察治疗及随访时间，或采用多组学技术或内服药物递送系统等现代化科学技术手段，深入研究右归丸和涤痰汤化裁在调节内分泌和代谢方面的分子机制，为PCOS患者提供更精准更安全有效的治疗方案和药物选项。
