Objective

To explore the therapeutic effect of Xiaoer Feire Kechuan Granules（小儿肺热咳喘颗粒）combined with vitamin D in the treatment of mycoplasma pneumoniae pneumonia in children.

Methods

A total of 302 children with mycoplasma pneumoniae pneumonia（admitted from October 2021 to December 2022）were included in the study and divided into control group and observation group by random number table method，with 151 cases in each group.The children in the control group received conventional Western medicine treatment and supplemented with vitamin D treatment.Those in the observation group were treated with Xiaoer Feire Kechuan Granules on the basis of medication in the control group.During the treatment and study，9 cases fell off in the control group and 10 cases fell off in the observation group.The clinical data of children in each group was compared，including the clinical efficacy，the changes in TCM symptom scores（fever，chest tightness and distension，cough and asthma，yellow phlegm not easy to cough up，body heat sweating，etc.）before and after treatment，time for fever to disappear，time for cough to disappear，time for sputum relief，time for lung moist rale to disappear，changes in lung function indicators，inflammation and immune-related indicators，and adverse reactions.

Results

The clinical effect of the observation group was better than that of the control group（P<0.05）. Compared with that of the control group，the disappearance time of fever，cough，sputum relief and pulmonary moist rales was shorter in the observation group（P<0.05）.After treatment，TCM symptom scores（fever，chest tightness and distension，cough and asthma，yellow phlegm not easy to cough up，body heat sweating，etc.）in two groups were lower.The values of forced vital capacity（FVC），forced expiratory volume in one second（FEV1），FEV1/FVC and other pulmonary function indicators of two groups were significantly increased.The levels of C-reactive protein，CRPand procalcitonin（PCT）and other inflammatory indicators were decreased.The levels of immunoglobulin A（IgA），immunoglobulin G（IgG），immunoglobulin M（IgM）and other levels were all increased，and the scores of TCM symptoms（fever，chest tightness and distension，cough and asthma，yellow phlegm not easy to cough up，body heat and sweat，etc.）and inflammatory indicators（PCT and CRP）in the observation group were lower than those in the control group after treatment.The pulmonary function indexes（FVC，FEV1，FEV1/FVC）and immune indexes（IgA，IgG，IgM）were higher than those of the control group（P<0.05）.The adverse reaction rate of the observation group was lower than that of the control group（P<0.05）.

Conclusion

Xiaoer Feire Kechuan Granules combined with vitamin D has good curative effect in the treatment of mycoplasma pneumoniae pneumonia in children，with alleviated symptoms，rapid recovery，reduced inflammation，improved immune function and lung function and less adverse reactions，and the treatment is safe and reliable.

目的

探讨小儿肺热咳喘颗粒结合维生素D治疗儿童肺炎支原体肺炎疗效。

方法

研究纳入302例儿童肺炎支原体肺炎进行分组，采取随机数字表法将儿童分为对照组与观察组，每组151例，对照组儿童采取西医常规治疗，并辅助以维生素D治疗，观察组儿童在对照组用药基础上结合小儿肺热咳喘颗粒治疗，治疗与研究过程中对照组脱落9例，观察组脱落10例。对比各组儿童临床数据情况：临床疗效、治疗前后中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）变化、发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间、肺功能指标及炎症、免疫相关指标变化、不良反应。

结果

观察组儿童临床疗效优于对照组（P<0.05）；与对照组儿童发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间相比，观察组儿童各项时间均更短（P<0.05）；治疗后各组儿童中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）均降低，用力肺活量（Forced vital capacity，FVC）、第1秒用力呼气容积（Forced expiratory volume in one second，FEV1）以及FEV1/FVC等肺功能指标均显著提升，C反应蛋白（C-reactive protein，CRP）与降钙素原（procalcitonin，PCT）等炎症指标均下降，免疫球蛋白A（Immunoglobulin A，IgA）以及免疫球蛋白G（Immunoglobulin G，IgG）、免疫球蛋白M（Immunoglobulin M，IgM）等水平均提升，观察组儿童治疗后中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）、炎症指标（PCT、CRP）均低于对照组，肺功能指标（FVC、FEV1、FEV1/FVC）、免疫指标（IgA、IgG、IgM）均高于对照组儿童（P<0.05）；观察组儿童治疗不良反应率低于对照组（P<0.05）。

结论

小儿肺热咳喘颗粒结合维生素D治疗儿童肺炎支原体肺炎疗效良好，患儿症状缓解，恢复快，炎症反应减轻，免疫功能提升，肺功能改善，且治疗不良反应少，治疗安全可靠。

目的

探讨小儿肺热咳喘颗粒结合维生素D治疗儿童肺炎支原体肺炎疗效。

方法

研究纳入302例儿童肺炎支原体肺炎进行分组，采取随机数字表法将儿童分为对照组与观察组，每组151例，对照组儿童采取西医常规治疗，并辅助以维生素D治疗，观察组儿童在对照组用药基础上结合小儿肺热咳喘颗粒治疗，治疗与研究过程中对照组脱落9例，观察组脱落10例。对比各组儿童临床数据情况：临床疗效、治疗前后中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）变化、发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间、肺功能指标及炎症、免疫相关指标变化、不良反应。

结果

观察组儿童临床疗效优于对照组（P<0.05）；与对照组儿童发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间相比，观察组儿童各项时间均更短（P<0.05）；治疗后各组儿童中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）均降低，用力肺活量（Forced vital capacity，FVC）、第1秒用力呼气容积（Forced expiratory volume in one second，FEV1）以及FEV1/FVC等肺功能指标均显著提升，C反应蛋白（C-reactive protein，CRP）与降钙素原（procalcitonin，PCT）等炎症指标均下降，免疫球蛋白A（Immunoglobulin A，IgA）以及免疫球蛋白G（Immunoglobulin G，IgG）、免疫球蛋白M（Immunoglobulin M，IgM）等水平均提升，观察组儿童治疗后中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）、炎症指标（PCT、CRP）均低于对照组，肺功能指标（FVC、FEV1、FEV1/FVC）、免疫指标（IgA、IgG、IgM）均高于对照组儿童（P<0.05）；观察组儿童治疗不良反应率低于对照组（P<0.05）。

结论

小儿肺热咳喘颗粒结合维生素D治疗儿童肺炎支原体肺炎疗效良好，患儿症状缓解，恢复快，炎症反应减轻，免疫功能提升，肺功能改善，且治疗不良反应少，治疗安全可靠。

Objective

To explore the therapeutic effect of Xiaoer Feire Kechuan Granules（小儿肺热咳喘颗粒）combined with vitamin D in the treatment of mycoplasma pneumoniae pneumonia in children.

Methods

A total of 302 children with mycoplasma pneumoniae pneumonia（admitted from October 2021 to December 2022）were included in the study and divided into control group and observation group by random number table method，with 151 cases in each group.The children in the control group received conventional Western medicine treatment and supplemented with vitamin D treatment.Those in the observation group were treated with Xiaoer Feire Kechuan Granules on the basis of medication in the control group.During the treatment and study，9 cases fell off in the control group and 10 cases fell off in the observation group.The clinical data of children in each group was compared，including the clinical efficacy，the changes in TCM symptom scores（fever，chest tightness and distension，cough and asthma，yellow phlegm not easy to cough up，body heat sweating，etc.）before and after treatment，time for fever to disappear，time for cough to disappear，time for sputum relief，time for lung moist rale to disappear，changes in lung function indicators，inflammation and immune-related indicators，and adverse reactions.

Results

The clinical effect of the observation group was better than that of the control group（P<0.05）. Compared with that of the control group，the disappearance time of fever，cough，sputum relief and pulmonary moist rales was shorter in the observation group（P<0.05）.After treatment，TCM symptom scores（fever，chest tightness and distension，cough and asthma，yellow phlegm not easy to cough up，body heat sweating，etc.）in two groups were lower.The values of forced vital capacity（FVC），forced expiratory volume in one second（FEV1），FEV1/FVC and other pulmonary function indicators of two groups were significantly increased.The levels of C-reactive protein，CRPand procalcitonin（PCT）and other inflammatory indicators were decreased.The levels of immunoglobulin A（IgA），immunoglobulin G（IgG），immunoglobulin M（IgM）and other levels were all increased，and the scores of TCM symptoms（fever，chest tightness and distension，cough and asthma，yellow phlegm not easy to cough up，body heat and sweat，etc.）and inflammatory indicators（PCT and CRP）in the observation group were lower than those in the control group after treatment.The pulmonary function indexes（FVC，FEV1，FEV1/FVC）and immune indexes（IgA，IgG，IgM）were higher than those of the control group（P<0.05）.The adverse reaction rate of the observation group was lower than that of the control group（P<0.05）.

Conclusion

Xiaoer Feire Kechuan Granules combined with vitamin D has good curative effect in the treatment of mycoplasma pneumoniae pneumonia in children，with alleviated symptoms，rapid recovery，reduced inflammation，improved immune function and lung function and less adverse reactions，and the treatment is safe and reliable.

儿童肺炎支原体肺炎是儿童常见的呼吸道疾病，该病发病率近年显著上升，其由肺炎支原体感染导致，发病后患儿多出现发热、咳痰、咳嗽等症状，危害患儿健康，影响患儿正常生长发育，须积极进行治疗[1-3]。临床治疗儿童肺炎支原体肺炎多采取阿奇霉素治疗，可较好改善患儿症状，但单独用药治疗效果并不显著，临床不断探究儿童该病新的治疗方案[4-6]。维生素D辅助治疗儿童肺炎支原体肺炎取得较好的效果，该药可较好减轻或者预防免疫与炎症介导引起的组织损伤，达到治疗效果[7]。近年，中医药对儿童肺炎支原体肺炎具有较好作用，治疗效果良好，不良反应少，具有一定优势，临床十分重视[8-10]。小儿肺热咳喘颗粒属于中成药，该药具有较好止咳平喘、清热解毒、润肺祛痰等功效，治疗儿童肺炎支原体肺炎疗效显著，值得深入探讨。故而本次纳入302例肺炎支原体肺炎儿童进行分组，探讨小儿肺热咳喘颗粒结合维生素D治疗的效果，报道如下。
1　资料与方法

1.1　一般资料
研究纳入2021年10月—2022年12月收治的302例肺炎支原体肺炎儿童进行分组，采取随机数字表法将患儿分为对照组与观察组，每组151例。治疗与研究过程中对照组脱落9例，观察组脱落10例。对照组142例，男72例，女70例；年龄2～12岁，平均（6.94±1.65）岁；病程1～7 d，平均（4.62±0.59）年。观察组141例，男71例，女70例；年龄2～11岁，平均（6.98±1.68）岁；病程1～7 d，平均（4.58±0.64）年。两组患儿资料对比，差异无统计学意义（P>0.05）。
1.2　诊断标准
西医诊断儿童肺炎支原体肺炎参照《儿童肺炎支原体肺炎诊疗指南（2023年版）》[11]：患儿发热、咳嗽、咳痰等，经X线确诊，支原体感染诊断采取支原体抗体滴度检测，结果大于1∶32则为阳性。中医诊断儿童肺炎支原体肺炎（痰热壅肺证）参照《小儿肺炎喘嗽中医诊疗指南》[12]：发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、咽干、面赤、身热有汗、胸中烦闷、大便秘结、尿赤、舌红、苔黄、脉滑数。
1.3　纳入排除标准
纳入标准：入组者均符合儿童肺炎支原体肺炎；患儿家属知情，同意参与本次治疗实验；患儿可接受西医治疗、维生素D、小儿肺热咳喘颗粒治疗；患儿年龄2～12岁；伦理批准研究进行。排除标准：合并其他部位感染患儿；合并呼吸系统疾病患儿；精神障碍患儿；先天性语言、听力、视力及智力等发育障碍患儿；本次药物过敏患儿；无法配合研究的患儿等。
1.4　治疗方法
对照组：进行西医常规治疗。根据患儿具体情况给予患儿退热及平喘药物。1 mg布地奈德混悬液（厂家为长风药业股份有限公司；国药准字为H20213357）+0.9%氯化钠溶液4 mL雾化吸入治疗，1次/d，静注盐酸阿奇霉素注射液（厂家为东北制药集团沈阳第一制药有限公司；国药准字为H20000197）治疗，1次/d，10 mg/kg，连续用药5 d，停药3～4 d，患儿口服阿奇霉素干混悬剂（药物厂家为辉瑞制药有限公司；国药准字为H10960112）治疗，1次/d，10 mg/kg，口服3 d，停药4 d。同时患儿采取维生素D滴剂（厂家为青岛双鲸药业有限公司；国药准字为H20113033）治疗，100 IU/（kg·d）。疗程2周。观察组：在对照组治疗基础上结合小儿肺热咳喘颗粒（药物厂家为上海医药集团青岛国风药业股份有限公司，国药准字Z37021346）治疗。<3岁患儿服药3次/d，4 g/次；3～7岁患儿4次/d，4 g/次；>7岁患儿3次/d，8 g/次。治疗2周。
1.5　观察指标
①各组儿童疗效标准[13]：儿童治疗后发热及咳痰、咳嗽、肺部湿啰音等症状基本消失，胸片显示肺部阴影消失为显效；儿童治疗后发热及咳痰、咳嗽、肺部湿啰音等症状改善，胸片显示肺部阴影显著好转为有效；儿童治疗后的症状未见缓解为无效。总有效率＝显效率+有效率。②比较各组儿童治疗前后发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等中医证候积分，各项积分0～3分，分值越高表示证候越严重[14]。③比较各组儿童的发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间。④比较各组儿童治疗前后肺功能指标变化。采取肺功能测定仪器测定各组儿童用力肺活量（Forced vital capacity，FVC）与第1秒用力呼气容积（Forced expiratory volume in one second，FEV1）指标，计算FEV1/FVC指标。⑤比较各组儿童治疗前后炎症指标变化。采取酶联免疫吸附法测定各组儿童的C反应蛋白（C-reactive protein，CRP）以及降钙素原（procalcitonin，PCT）水平。⑥比较各组儿童治疗前后免疫指标。主要测定免疫球蛋白A（Immunoglobulin A，IgA）以及免疫球蛋白G（Immunoglobulin G，IgG）、免疫球蛋白M（Immunoglobulin M，IgM）指标，采取全自动免疫分析仪进行测定。⑦比较各组儿童不良反应。
1.6　统计学方法
数据录入SPSS 26.0软件处理，各组儿童数据实施t/χ2检验，当P<0.05时，说明数据比较差异存在统计学意义。
2　结果

2.1　治疗效果比较
表1显示，观察组儿童临床疗效（97.87%）优于对照组（92.96%），差异有统计学意义（P<0.05）。
2.2　治疗前后中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）比较
表2显示，各组儿童治疗后的中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）均降低，而观察组儿童治疗后的中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）均低于对照组儿童，差异有统计学意义（P<0.05）。
2.3　发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间比较
表3显示，与对照组儿童发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间相比，观察组儿童各项时间均更短，差异有统计学意义（P<0.05）。
2.4　治疗前后肺功能指标（FVC、FEV1、FEV1/FVC）比较
表4显示，各组儿童治疗后的肺功能指标（FVC、FEV1、FEV1/FVC）均提升，而观察组儿童治疗后的肺功能指标（FVC、FEV1、FEV1/FVC）高于对照组儿童，差异有统计学意义（P<0.05）。
2.5　治疗前后炎症指标（PCT、CRP）比较
表5显示，各组儿童治疗后的炎症指标（PCT、CRP）均降低，而观察组儿童治疗后的炎症指标（PCT、CRP）均低于对照组儿童，差异有统计学意义（P<0.05）。
2.6　治疗前后免疫指标（IgA、IgG、IgM）比较
表6显示，各组儿童治疗后的免疫指标（IgA、IgG、IgM）均提升，而观察组儿童治疗后的免疫指标（IgA、IgG、IgM）均高于对照组儿童，差异有统计学意义（P<0.05）。
2.7　不良反应比较
表7显示，观察组儿童治疗不良反应率（1.42%）低于对照组（6.34%），差异有统计学意义（P<0.05）。
3　讨论

中医对儿童肺炎支原体肺炎认识较深，将其归属于“外感热病”“肺炎喘嗽”“喘咳”等范畴[15-18]。儿童肺炎支原体肺炎其主要病机为外感风邪及寒温失调，其中痰热壅肺证较为多见[19-20]。小儿脏腑娇嫩，中医认为，“心肝常有余，肺脾常不足”[21]。外邪入侵肺卫，肺宣发肃降失司，肺气郁闭，郁而化热，津液输布及水道异常，水湿内停，聚集成痰，痰热互结，壅阻于肺，致使气血运行不畅，故而发病[22-23]。治疗该病当以清热解毒以及止咳化痰、平喘等法[24-25]。小儿肺热咳喘颗粒为多种中药组成的中成药，该药中的麻黄可发汗散寒及宣肺平喘；石膏可清热泻火、除烦止渴；苦杏仁可降气及止咳平喘；连翘可清热解毒、疏散风热；甘草具有解毒、益气补中以及祛痰止咳、解毒等功效；金银花具有清热解毒以及疏散风热等功效；知母具有清热泻火以及滋阴润燥；板蓝根具有清热解毒以及凉血利咽等功效；黄芩具有清热燥湿以及泻火解毒等功效；麦冬具有润肺养阴以及益胃生津、清心除烦等功效；鱼腥草具有清热解毒等功效。诸药配伍具有止咳平喘、清热解毒以及宣肺解毒功效，用于治疗痰热壅肺证儿童肺炎支原体肺炎效果良好[26-27]。现代药理指出，麻黄具有较好抗炎、解热及抗病原微生物的作用；石膏可抗病毒及抗炎，解热、镇静、镇痉、止渴、增强免疫力；苦杏仁具有抗氧化、抗炎、免疫调节等作用；连翘具有广谱抗菌作用，还可解热、抗炎，镇吐、保肝、免疫调节及抗氧化；甘草可抗炎镇痛、调节免疫；金银花可抗炎及抗病毒，促进肾上腺皮质激素释放，抑制炎症，增加身体抵抗力；知母可抗炎、抗感染、抑菌；板蓝根具有抗病毒及抗炎、抗菌作用；黄芩可抗菌、抗炎、抗氧化；麦冬抗炎作用明显；鱼腥草抑菌作用显著。诸药合用可较好抗炎及抑菌、抗病毒及抗氧化，还可调节机体免疫功能[28-30]。
维生素D为机体必需脂溶性维生素，其介导钙磷代谢，有助于维持骨稳态，具有较好抗炎及调节免疫作用，可较好缓解免疫与炎症介导引起的组织损伤，其辅助治疗儿童肺炎支原体肺炎取得较好临床效果，可较好改善患儿病情，与小儿肺热咳喘颗粒结合可较好提升儿童肺炎支原体肺炎治疗效果[31-32]。本次研究数据显示，观察组儿童临床疗效（97.87%）优于对照组（92.96%），提示儿童肺炎支原体肺炎采取小儿肺热咳喘颗粒及维生素D治疗效果良好。与对照组儿童发热消失时间、咳嗽消失时间、咳痰缓解时间、肺部湿啰音消失时间相比，观察组儿童各项时间均更短，提示小儿肺热咳喘颗粒结合维生素D可促进儿童肺炎支原体肺炎更快好转。观察组儿童治疗后中医证候积分（发热、胸闷胸胀、咳嗽气喘、黄痰不易咳出、身热有汗等）、炎症指标（PCT、CRP）均低于对照组，肺功能指标（FVC、FEV1、FEV1/FVC）、免疫指标（IgA、IgG、IgM）均高于对照组儿童，提示儿童肺炎支原体肺炎采取小儿肺热咳喘颗粒及维生素D可较好缓解机体炎症反应，改善患者病症，提升免疫功能，改善患者肺功能。观察组儿童治疗不良反应率低于对照组，提示小儿肺热咳喘颗粒结合维生素D治疗儿童肺炎支原体肺炎安全可靠。
综上所述，儿童肺炎支原体肺炎采取小儿肺热咳喘颗粒以及维生素D治疗效果良好，患者症状好转，恢复快，炎症改善，免疫功能提升，肺功能改善，治疗安全可靠。
