Objective

To study the efficacy and safety of Mongolian prescription in the treatment of ulcerative colitis with different physical characteristics of Mongolian medicine.

Methods

Sixty patients diagnosed with ulcerative colitis were selected from the Affiliated Hospital of Inner Mongolia University for Nationalities. According to different treatment methods，30 patients in the control group were treated with western medicine，and the other 30 patients in the study group were treated with Mongolian medicine. The clinical efficacy，main syndrome and secondary syndrome scores，inflammatory factor levels，recurrence rate and adverse reaction rate of two groups were observed and compared.

Results

The total effective rate of the study group was 93.33%（28/30），higher than that of the control group. The recurrence rate of the study group was 6.67%（2/30），lower than that of the control group（33.33%，10/30）. The total incidence of adverse reactions（6.67%，2/30）was lower than that of the control group（30.00%，9/30）. The difference was statistically significant（P<0.05）. After treatment，compared with those of the control group，the main syndrome scores in the study group were lower，and the difference was statistically significant（P<0.05）. After treatment，compared with those of the control group，the secondary syndrome scores in the study group were lower，and the difference was statistically significant（P<0.05）. After treatment，the levels of C-reactive protein（CRP），tumor necrosis factor-α（TNF-α）and interleukin-6（IL-6）in the study group were lower than those in the control group，and the difference was statistically significant（P<0.05）.

Conclusion

After taking Mongolian medicine to treat ulcerative colitis patients with different syndromes，the therapeutic effect is good. It can effectively reduce the clinical symptoms and promote the recovery of patients，and has high clinical application value.

目的

研究不同蒙医体质特征采用蒙方治疗溃疡性结肠炎不同证型的疗效和安全性研究。

方法

抽取内蒙古民族大学附属医院60例被确诊为溃疡性结肠炎的患者，按照治疗方式的不同分组，对照组（30例）予以西药治疗，研究组（30例）予以蒙西药治疗。观察比较两组患者的临床疗效、蒙医主症和次症证候积分、炎症因子水平、复发率、不良反应率。

结果

研究组总有效率（93.33%，28/30）高于对照组（73.33%，22/30）；研究组复发率（6.67%，2/30）低于对照组（33.33%，10/30）；不良反应总发生率为6.67%（2/30），低于对照组的30.00%（9/30）；差异均有统计学意义（P<0.05）。治疗后，与对照组相比较，研究组蒙医主症、次症证候积分更低，差异均有统计学意义（P<0.05）。治疗后，与对照组相对比，研究组C反应蛋白（CRP）、肿瘤坏死因子-α（TNF-α）以及白细胞介素-6（IL-6）水平均更低，差异有统计学意义（P<0.05）。

结论

不同证型的溃疡性结肠炎患者采取蒙西药治疗后，治疗效果较好，可有效减轻患者的临床症状，促进患者恢复，临床应用价值较高。

目的

研究不同蒙医体质特征采用蒙方治疗溃疡性结肠炎不同证型的疗效和安全性研究。

方法

抽取内蒙古民族大学附属医院60例被确诊为溃疡性结肠炎的患者，按照治疗方式的不同分组，对照组（30例）予以西药治疗，研究组（30例）予以蒙西药治疗。观察比较两组患者的临床疗效、蒙医主症和次症证候积分、炎症因子水平、复发率、不良反应率。

结果

研究组总有效率（93.33%，28/30）高于对照组（73.33%，22/30）；研究组复发率（6.67%，2/30）低于对照组（33.33%，10/30）；不良反应总发生率为6.67%（2/30），低于对照组的30.00%（9/30）；差异均有统计学意义（P<0.05）。治疗后，与对照组相比较，研究组蒙医主症、次症证候积分更低，差异均有统计学意义（P<0.05）。治疗后，与对照组相对比，研究组C反应蛋白（CRP）、肿瘤坏死因子-α（TNF-α）以及白细胞介素-6（IL-6）水平均更低，差异有统计学意义（P<0.05）。

结论

不同证型的溃疡性结肠炎患者采取蒙西药治疗后，治疗效果较好，可有效减轻患者的临床症状，促进患者恢复，临床应用价值较高。

Objective

To study the efficacy and safety of Mongolian prescription in the treatment of ulcerative colitis with different physical characteristics of Mongolian medicine.

Methods

Sixty patients diagnosed with ulcerative colitis were selected from the Affiliated Hospital of Inner Mongolia University for Nationalities. According to different treatment methods，30 patients in the control group were treated with western medicine，and the other 30 patients in the study group were treated with Mongolian medicine. The clinical efficacy，main syndrome and secondary syndrome scores，inflammatory factor levels，recurrence rate and adverse reaction rate of two groups were observed and compared.

Results

The total effective rate of the study group was 93.33%（28/30），higher than that of the control group. The recurrence rate of the study group was 6.67%（2/30），lower than that of the control group（33.33%，10/30）. The total incidence of adverse reactions（6.67%，2/30）was lower than that of the control group（30.00%，9/30）. The difference was statistically significant（P<0.05）. After treatment，compared with those of the control group，the main syndrome scores in the study group were lower，and the difference was statistically significant（P<0.05）. After treatment，compared with those of the control group，the secondary syndrome scores in the study group were lower，and the difference was statistically significant（P<0.05）. After treatment，the levels of C-reactive protein（CRP），tumor necrosis factor-α（TNF-α）and interleukin-6（IL-6）in the study group were lower than those in the control group，and the difference was statistically significant（P<0.05）.

Conclusion

After taking Mongolian medicine to treat ulcerative colitis patients with different syndromes，the therapeutic effect is good. It can effectively reduce the clinical symptoms and promote the recovery of patients，and has high clinical application value.

随着社会的不断发展，人们生活方式的改变，导致溃疡性结肠炎患病率呈上升趋势发展。溃疡性结肠炎是一种病因尚不十分清楚的直肠和结肠慢性非特异性炎症性疾病。该病主要累及直结肠，呈连续非阶段性分布，约有30%患者可以累及整个结肠，较难治愈，严重影响其生活质量[1]。西药治疗该病目前常使用氨基水杨酸制剂、免疫抑制剂、激素类药物等方式进行对症治疗，在短期内控制和缓解症状，但停药后易复发，长期用药不良反应多，部分顽固性患者疗效不够理想。溃疡性结肠炎属中医学“痢疾”“肠澼”等范畴，中医认为该病与肝、脾、胃、肠等脏腑有关[2]。湿热阻滞、热毒蕴结兼脾肾阳虚是溃疡性结肠炎的主要病机。蒙医认为溃疡性结肠炎属于“大肠宝如病”范畴，蒙医认为机体内的三根七素发生紊乱，巴达干、血和希拉相互冲突，赫依产生病变，进而引发恶血与黄水增多的复杂病变[3]。根据大肠诱发宝如病的原因和机理，发生的病变主要考虑赫依和血，将大肠宝如病划分为三型，分别为奇素希拉、巴达干赫依、粘合并型。蒙医诊治轻度、中度、初发类型、处于缓解期、慢性复发类型的溃疡性结肠炎具有显著的治疗效果[4-5]。基于此，为探讨蒙西医给不同证型溃疡性结肠炎患者带来的临床效果，选取内蒙古民族大学附属医院60例被确诊为溃疡性结肠炎的患者作为研究对象，现报道如下。
1　临床资料

1.1　一般资料
选取2021年1月—2023年6月在内蒙古民族大学附属医院收入治疗的溃疡性结肠炎患者60例作为研究对象，按照治疗方式的不同分组，每组30例。研究组男15例，女15例；年龄22～55岁，平均（33.44±10.22）岁；奇素希拉偏盛型10例，巴达干赫依偏盛型10例，粘盛型10例。对照组男18例，女12例；年龄21～57岁，平均（33.32±10.60）岁；奇素希拉偏盛型11例，巴达干赫依偏盛型11例，粘盛型8例。两组比较，差异无统计学意义（P>0.05）。
1.2　诊断标准
参照《内科学》[6]中对溃疡性结肠炎的诊断标准。
1.3　蒙医辨证标准
参照《中国医学百科全书-蒙医学》[7]，《蒙医内科学》[8]中对溃疡性结肠炎的辨证标准。
1.4　纳入标准
（1）符合溃疡性结肠炎的诊断标准。（2）生命体征平稳，没有其他心脑血管病、肾病、肝病等疾病。（3）所需的主要病历资料记录完整。（4）以溃疡性结肠炎“大肠宝如病”作为出院第一诊断。
1.5　排除标准
（1）不符合纳入标准。（2）随便服用溃疡性结肠炎药物的患者。（3）合并糖尿病、肝肾功能障碍、心脑血管等严重慢性疾病。（4）因各种原因住院期间转出科室以及外科手术的。（5）病例资料不齐全。
2　治疗方法

2.1　对照组
对照组接受美沙拉嗪肠溶片（葵花医药公司佳木斯鹿灵医药公司，国药准字H20171358）治疗，1 g/次，4次/d。总疗程为56 d。
2.2　研究组
观察组在对照组的基础上给予蒙医药辨证施治。总疗程为56 d。
（1）奇素希拉偏盛型：①早饭后，哈日嘎布日-10 3 g；②午饭后，吉召木道尔吉3 g；③晚饭后，利肝和胃丸3 g。
（2）巴达干赫依偏盛型：①早饭后，达格布-15 3 g；②午饭后，吉召木道尔吉3 g；③晚饭后，兴格格其满都拉丸3 g。
（3）粘盛型：①早饭后，利肝和胃丸3 g；②午饭后，吉召木道尔吉3 g；③晚饭后，哈敦嘎日迪-13 3 g。
3　观察指标与统计学方法

3.1　观察指标
（1）临床疗效[9]：显效：通过诊治，病人的临床治疗症状和体征全部减轻，恢复了健康。有效：经治疗，临床症状、体征好转。无效：经治疗，临床症状、体征没有改善。总有效率＝（显效例数＋有效例数）/总例数×100%。（2）蒙医证候积分[10]。主症：腹泻形态、腹泻次数、腹胀、腹痛、黏液及血便、里急后重感等，主症按照无、轻、中、重分别计0、2、4、6分；次症：食欲、头晕、饮食发病因素、起居发病因素、体质量减轻、结肠镜下表现等，次症按照无、轻、中、重分别计0、1、2、3分。（3）炎症因子水平：治疗前后，取患者清晨空腹静脉血，检测C-反应蛋白（CRP）、肿瘤坏死因子（TNF）以及白细胞介素6（IL-6）。（4）对照组及研究组复发率比较。
3.2　统计学方法
采用SPSS 22.0统计学软件分析数据。计量资料以表示，采用t检验；计数资料以率（%）表示，采用χ2检验。P<0.05表示差异有统计学意义。
4　结果

随访1年，研究组复发率低于对照组，组间比较差异有统计学意义（P<0.05）。见表5。

溃疡性结肠炎属于慢性特殊胃肠炎症性病变，其原因尚未完全明确，多认为与自然环境因素、遗传因素和消化道微生态的改变等导致消化道的免疫功能失调所致[11-13]。其中，以青中年人群较为常见[14]。该病迁延难愈、极易反复发作，且具有较好的癌变性，严重危及患者的身体健康。目前，临床治疗溃疡性结肠炎多采取西药治疗。美沙拉嗪肠溶片属于5-氨基水杨酸制剂，经口服后，可在肠道中释放美沙拉嗪，作用到肠黏膜以及黏膜的下层组织[15]。同时，还可以抑制结肠当中环氧合酶、前列腺素的生成，进而减轻局部的炎症反应。虽然能减轻患者的临床症状，但是不同患者可能出现不同症状，难以有效改善溃疡性结肠炎的一系列临床表现。且长期单一使用，且极易导致病情反复发作，效果并不理想。因此，治疗时需要根据患者的实际情况，辨证施治，进而达到最佳的临床效果。
中医认为，本病的发病多由情志失调、肝郁所致；或饮食不当，吃食肥腻；或体内湿热，喜食生冷，导致脾胃受损，脾失健运[16]。或因感受暑湿、热毒等邪气，导致体内的湿热毒邪进入肠道，气血运行受损、凝滞，化脓；长此以往，精血流失，伤阴。邪气侵袭经络，形成瘀血内阻[17]。随着时间的推移，脾、肾阳虚，清阳不升，气机下沉，气血不足，胃关受损，从而引起疾病。而蒙医学认为[18]，溃疡性结肠炎是在病变过程中，肝脏中未被运化的精血，滞留在大肠的薄弱处，和赫依相融合，最终导致了大肠的损伤。发病初期，溃疡性结肠炎是一种慢性疾病，是一种血热内蕴的慢性疾病，主要潜伏于大肠之中。而出现加重热因的条件，血热亢盛，就会转化为大肠渗出型宝如。若日久凝固，即为陈，即有演变为大肠血痞的可能。根据蒙医辨证，又将溃疡性结肠炎分为3种证型，包括齐素希拉盛型、巴达干赫依盛型及粘盛型。将蒙药直接作用于溃疡的黏膜，可以促进新的黏膜细胞的生长，使溃烂的黏膜细胞脱落，从而恢复肠道菌群的平衡，提高机体免疫系统功能，进而彻底治疗溃疡性结肠炎[19-20]。吉召木道尔吉[21]属于蒙西药治疗大肠宝如病常用药物，具有清热解毒、止痛、消除黏热的作用，对不同证型的大肠宝如病均具有一定疗效。该药物中含有的制草乌[22]具有疏通经络、祛湿除寒的功效；红花[23]可活血化瘀、散瘀止痛；绵马贯众[24]清热解毒；牛黄[25]能清热解毒、祛风凉肝；麝香[26]通络散瘀、止痛；熊胆[27]封脉窍、防腐生肌、止血、除“希拉”。从现代药理学的角度上看[28-30]，制草乌、红花、牛黄具有镇痛、消炎的功效；绵马贯众具有抗菌、抗病毒、抗氧化等功效；麝香可抗炎、消肿止痛。而根据不同的证型，会使用到其余不同的蒙西药治疗。对于齐素希拉偏盛型和粘盛型的患者，可选用利肝和胃丸。利肝和胃丸属于蒙医传统制剂，包含20余种药材。其中含有的寒水石[31]具有清热泻火的作用，可助消化、清巴达干热；沙棘[32]、豆蔻[33]可健脾消食、活血散瘀；五灵脂[34]散瘀止痛、调理体素；木香[35]可调中导滞、行气止痛等。通过使用利肝和胃丸，可有效抑制齐素、希拉、赫依等发病因素。而巴达干赫依偏盛型的患者可采用兴格格其满都拉，具有祛巴达干、健脾开胃、补肾气的功效[36]。在本研究中，研究组总有效率更高，蒙医证候积分更低（P<0.05），说明通过辨证后采取蒙西药治疗后，可有效减轻患者的一系列临床症状，治疗效果提高。目前医学认为血清细胞因子有着免疫调节的作用，炎症因子水平增加与溃疡性结肠炎的发病密切相关[37-38]。血清细胞因子具有重要的免疫调节作用，炎症因子浓度的增高与溃疡性结肠炎的发生具有紧密的联系。而与对照组相对比，研究组CRP、TNF-α、IL-6水平均更低（P<0.05），说明采取蒙西药治疗后，减轻患者的炎性反应，促进患者康复。
蒙医药以药草植物为主，和中医药类似，亦隶属祖国医学的瑰宝，蒙医吸取中医药的经验并在长期诊疗中实践总结，独特地理位置生长的多样药草让蒙医有其独到之处。本项研究中，分型辨证施治，研究组患者的腹泻形态、次数、腹胀、腹痛、黏液及血便等证候积分均优于对照组，炎症因子CRP、TNF、IL-6水平治疗后均低于对照组，复发率、不良反应率低于对照组，治疗效果较好，可有效减轻患者的炎性反应、改善患者的临床症状。
