Objective

To study curative effect of Banxia Xiexin Decoction（半夏泻心汤）on gastric epithelial dysplasia and its influence on immune function and gastric mucosa inflammatory reaction.

Methods

Eighty cases with gastric epithelial dysplasia（dampness-heat syndrome of spleen and stomach）were divided into the control group and the study group with 40 cases in each by digital table method.The control group was treated with conventional western medicine，and the observation group was treated with Banxia Xiexin Decoction.The course was12 weeks.The changes of TCM syndrome scores，immune function and gastric mucosal inflammation were observed before and after treatment.

Results

There was no significant difference in fullness，stomachache，fatigue，loose stool，belching，lethargy and lack of thirst between the two groups before treatment（P>0.05）.After treatment，the score decreased（P<0.001），and the degree of decline in the observation group was greater than that in the control group（P<0.001）.There was no significant difference in the levels of CD+3，CD+4 or CD+8 between the two groups before treatment（P>0.05），and the levels of CD+3 and CD+4 were higher than those before treatment（P<0.001），and the increases in the observation group were greater than those in the control group（P<0.001）.There was no significant difference in serum levels of interleukin-6（IL-6），interleukin-2（IL-2），interleukin-17（IL-17）or tumor necrosis factor-α（TNF-α）between the two groups before treatment（P>0.05），and the levels of IL-6，IL-17 and TNF-αwere decreased compared with those before treatment（P<0.001）.The decrease of IL-2 in the observation group was greater than that in the control group（P<0.001），and the increase of IL-2 in the observation group was greater than that in the control group（P<0.001）.The therapeutic effect of the observation group was better than that of the control group（P<0.05）.

Conclusion

Banxia Xiexin Decoction in the treat-ment of gastric mucosal dysplasia（dampness-heat syndrome of spleen and stomach）can help to alleviate disease-related symptoms，improve the immune function，inhibit the inflammatory response and improve the therapeutic effect.

目的

探讨半夏泻心汤治疗胃黏膜异性增生的疗效及对免疫功能、胃黏膜炎性反应的影响。

方法

选取80例胃黏膜异型增生患者（脾胃湿热证），采用随机数字表法分为对照组与观察组各40例，对照组常规西药治疗，观察组加服半夏泻心汤治疗，疗程12周，观察治疗前后中医证候积分、免疫功能及胃黏膜炎性反应变化。

结果

两组患者治疗前心下胀满、胃痛、倦怠乏力、便溏、嗳气、纳呆、口淡不渴积分比较差异无统计学意义（P>0.05），治疗后积分下降（P<0.001），且观察组下降程度大于对照组（P<0.001）；两组患者治疗前CD3+、CD4+、CD8+ 水平比较差异无统计学意义（P>0.05），治疗后CD3+、CD4+ 较治疗前升高（P<0.001），且观察组升高幅度大于对照组（P<0.001），CD8+ 治疗后下降（P<0.001），且观察组下降程度大于对照组（P<0.001）；两组患者治疗前血清白细胞介素-6（interleukin-6，IL-6）、白细胞介素2（interleukin-2，IL-2）、白细胞介素17（interleukin-17，IL-17）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）水平比较差异无统计学意义（P>0.05），治疗后IL-6、IL-17、TNF-α较治疗前下降（P<0.001），且观察组下降幅度大于对照组（P<0.001），IL-2治疗后升高（P<0.001），且观察组升高程度大于对照组（P<0.001）；观察组治疗疗效优于对照组（P<0.05）。

结论

半夏泻心汤治疗胃黏膜异型增生（脾胃湿热证）可有助于缓解疾病相关症状，改善免疫功能，抑制炎性反应，提高治疗疗效。

目的

探讨半夏泻心汤治疗胃黏膜异性增生的疗效及对免疫功能、胃黏膜炎性反应的影响。

方法

选取80例胃黏膜异型增生患者（脾胃湿热证），采用随机数字表法分为对照组与观察组各40例，对照组常规西药治疗，观察组加服半夏泻心汤治疗，疗程12周，观察治疗前后中医证候积分、免疫功能及胃黏膜炎性反应变化。

结果

两组患者治疗前心下胀满、胃痛、倦怠乏力、便溏、嗳气、纳呆、口淡不渴积分比较差异无统计学意义（P>0.05），治疗后积分下降（P<0.001），且观察组下降程度大于对照组（P<0.001）；两组患者治疗前CD3+、CD4+、CD8+ 水平比较差异无统计学意义（P>0.05），治疗后CD3+、CD4+ 较治疗前升高（P<0.001），且观察组升高幅度大于对照组（P<0.001），CD8+ 治疗后下降（P<0.001），且观察组下降程度大于对照组（P<0.001）；两组患者治疗前血清白细胞介素-6（interleukin-6，IL-6）、白细胞介素2（interleukin-2，IL-2）、白细胞介素17（interleukin-17，IL-17）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α）水平比较差异无统计学意义（P>0.05），治疗后IL-6、IL-17、TNF-α较治疗前下降（P<0.001），且观察组下降幅度大于对照组（P<0.001），IL-2治疗后升高（P<0.001），且观察组升高程度大于对照组（P<0.001）；观察组治疗疗效优于对照组（P<0.05）。

结论

半夏泻心汤治疗胃黏膜异型增生（脾胃湿热证）可有助于缓解疾病相关症状，改善免疫功能，抑制炎性反应，提高治疗疗效。

Objective

To study curative effect of Banxia Xiexin Decoction（半夏泻心汤）on gastric epithelial dysplasia and its influence on immune function and gastric mucosa inflammatory reaction.

Methods

Eighty cases with gastric epithelial dysplasia（dampness-heat syndrome of spleen and stomach）were divided into the control group and the study group with 40 cases in each by digital table method.The control group was treated with conventional western medicine，and the observation group was treated with Banxia Xiexin Decoction.The course was12 weeks.The changes of TCM syndrome scores，immune function and gastric mucosal inflammation were observed before and after treatment.

Results

There was no significant difference in fullness，stomachache，fatigue，loose stool，belching，lethargy and lack of thirst between the two groups before treatment（P>0.05）.After treatment，the score decreased（P<0.001），and the degree of decline in the observation group was greater than that in the control group（P<0.001）.There was no significant difference in the levels of CD+3，CD+4 or CD+8 between the two groups before treatment（P>0.05），and the levels of CD+3 and CD+4 were higher than those before treatment（P<0.001），and the increases in the observation group were greater than those in the control group（P<0.001）.There was no significant difference in serum levels of interleukin-6（IL-6），interleukin-2（IL-2），interleukin-17（IL-17）or tumor necrosis factor-α（TNF-α）between the two groups before treatment（P>0.05），and the levels of IL-6，IL-17 and TNF-αwere decreased compared with those before treatment（P<0.001）.The decrease of IL-2 in the observation group was greater than that in the control group（P<0.001），and the increase of IL-2 in the observation group was greater than that in the control group（P<0.001）.The therapeutic effect of the observation group was better than that of the control group（P<0.05）.

Conclusion

Banxia Xiexin Decoction in the treat-ment of gastric mucosal dysplasia（dampness-heat syndrome of spleen and stomach）can help to alleviate disease-related symptoms，improve the immune function，inhibit the inflammatory response and improve the therapeutic effect.

胃黏膜异型增生（Gastric epithelial dysplasia，GED）属于癌前病变，在病理上胃黏膜失去了自身单纯性增生形态，表现为腺体异常分化、黏膜异型增生形态，此阶段仍然为良性病变，患者多伴有慢性萎缩性胃炎，如积极进行治疗病变可停止发展甚至于逆转，但如进一步发展则可发展为胃癌。目前公认的正常胃黏膜发展成胃癌的模式为：正常胃黏膜—慢性浅表性胃炎—慢性萎缩性胃炎—肠上皮化生—异型增生—胃癌，因此积极干预对于降低胃癌的发生具有重要意义。胃癌前病变患者多可表现出嗳气、上腹部不适、疼痛、饱胀、恶心、食欲不振等症状，根据其症状可将其归属到中医学“胃痞”“胃脘痛”等范畴，脾胃虚弱、湿热阻络是其发病的病机[1-3]，因此治疗可采取调节脾胃、清热祛湿、解毒通络等法。半夏泻心汤最早见于《伤寒杂病论》，由东汉医家张仲景所创，为治“心下痞”之代表方，是古今医家治胃炎的常用方剂，具有良好的治疗疗效[4-5]，尤其近年来有研究显示半夏泻心汤治疗胃黏膜异性增生可促使异常增生的黏膜逆转[6-8]，基于临床相关报道，我院对诊治的胃黏膜异性增生患者采取半夏泻心汤治疗，观察了其治疗疗效及对免疫功能、胃黏膜炎性反应的影响，尝试分析其治疗胃黏膜异型增生的可能机制，为胃黏膜异常增生的治疗提供参考及科学依据，现将相关研究结果报道如下。

选取2022年1月—2022年12月期间在浙江大学金华医院诊治的胃黏膜异型增生患者80例。纳入标准：（1）符合《中国慢性胃炎共识意见》中胃黏膜异型增生诊断标准[9]。中医诊断符合《慢性胃炎中医诊疗专家共识意见》中相关诊断标准[10]，中医辨证为脾胃湿热证，主症：心下胀满，胃痛，倦怠乏力，便溏；次症：嗳气，纳呆，口淡不渴；舌脉：舌质淡紫，苔薄黄腻，脉细滑。（2）胃镜及病理性确诊。（3）年龄≥18岁，性别不限。（4）病程时间≥1年。排除标准：（1）合并心、肝、肾疾病、血液系统疾病或凝血功能异常者。（2）合并消化性溃疡。（3）胃黏膜重度异型增生或疑似恶变。（4）有消化道疾病手术史者。（5）有精神疾病史者。（6）患有听力、语言障碍等影响正常沟通疾病。（7）服用非甾体类药物、抗血小板集聚药物等可能影响到治疗疗效、免疫功能药物者。（8）合并肺部感染、皮肤感染等其他急慢性细菌感染或病毒感染性疾病者。（9）妊娠期及哺乳期女性。将患者根据随机数字表法随机分为对照组与观察组，每组40例。对照组中男22例、女18例，年龄27～68岁，平均（46.12±6.33）岁；病程时间1～7年，平均（4.11±0.92）年；合并有幽门螺杆菌感染阳性者12例。观察组中男24例、女16例，年龄25～65岁，平均（46.65±6.24）岁；病程时间1～8年，平均（4.15±0.87）年；合并有幽门螺杆菌感染阳性者14例。两组患者的性别、年龄、病程时间、合并幽门螺杆菌感染情况等一般资料比较差异无统计学意义（P>0.05）。本研究取得患者知情同意，均签署知情同意书，报医院伦理委员会审核通过。
1.2　方法
1.2.1　对照组
采取西药治疗，奥克-奥美拉唑肠溶胶囊口服（生产商：常州四药制药有限公司，国药准字H20023053，规格20 mg/粒），20 mg/次，2次/d；可立硫糖铝混悬凝胶吸服（生产商：昆明积大制药股份有限公司，国药准字H20080322，规格1 g/支），1支/次，2次/d；治疗时间12周。幽门螺杆菌感染阳性者加克拉霉素分散片250 mg，2片/次，2次/d，甲硝唑片0.2 g，2片/次，2次/d，疗程14 d停药。
1.2.2　观察组
在对照组治疗基础上加服半夏泻心汤治疗，组方：半夏12g，黄芩9 g，干姜9 g，人参9 g，炙甘草9 g，大枣9 g，黄连3 g，随症加减：伴胃灼热反酸重可加煅瓦楞子、吴茱萸；舌苔呈白腻加藿香、砂仁；伴腹胀积食食欲差加神曲、山楂。每日1剂，水煎取汁400 mL早晚分服，治疗时间12周。
1.3　观察指标
1.3.1　中医证候积分
根据患者脾胃湿热证的主症及次症症状的有无及严重程度计算积分，无、轻、中、重度主症分别计0、2、4、6分，次症分别为0、1、2、3分。
1.3.2　免疫功能
抽取患者外周静脉血5 mL，使用BD FACSCalibur流式细胞仪检测外周血T淋巴细胞亚群CD+3、CD+4、CD+8 等指标水平。
1.3.3　炎性指标
抽取患者外周静脉血，采用酶联免疫吸附法检测血清白细胞介素-6（interleukin-6，IL-6）、白细胞介素2（interleukin-2，IL-2）、白细胞介素17（interleukin-17，IL-17）、肿瘤坏死因子α（tumor necrosis factor-α，TNF-α），检查试剂盒购于上海仁捷生物科技有限公司。
1.4　疗效判定标准[10]
临床痊愈：症状消失或基本消失，治疗后的中医证候积分较治疗前下降程度≥95%，同时胃镜复查胃黏膜组织学恢复正常；显效：症状明显缓解，治疗后的中医证候积分较治疗前下降程度在≥70%及<95%，同时胃镜复查胃黏膜组织学异常较治疗前改善程度为2度；好转：症状有所改善，治疗后的中医证候积分较治疗前的下降程度在≥30% 及<70%，同时胃镜复查胃黏膜组织学异常表现改善程度为1度；无效：症状无明显改善或加重，治疗后的中医证候积分较治疗前下降程度 <30%，同时胃镜复查胃黏膜组织学无明显改善或进一步发展。
1.5　统计学方法
采用SPSS22.0统计软件对结果进行统计分析，计量资料以均数±标准差（）表示，采用t检验，计数资料采用率或百分比表示，采用卡方检验，等级资料采用秩和检验，P<0.05为差异有统计学意义。
2　结果

2.1　两组治疗前后中医证候积分变化比较
两组患者治疗前心下胀满、胃痛、倦怠乏力、便溏、嗳气、纳呆、口淡不渴积分比较差异无统计学意义（t＝0.108、0.097、0.610、0.406、0.709、0.745、0.363，P=0.914、0.923、0.497、0.686、0.480、0.459、0.717），治疗后积分下降（对照组t＝3.379、5.135、7.960、7.672、9.541、10.668、7.645，观察组＝13.247、10.392、16.135、13.097、13.097、16.011、16.826，P<0.001），且观察组下降程度大于对照组（t＝3.528、5.348、7.097、4.461、4.461、5.531、7.949，P<0.001），比较差异均有统计学意义。见表1。
2.2　两组治疗前后免疫功能指标变化比较
两组患者治疗前CD+3、CD+4、CD+8 水平比较差异无统计学意义（t＝0.054、0.101、0.069，P=0.957、0.920、0.945），治疗后CD+3、CD+4 较治疗前升高（对照组t＝4.457、5.853，观察组＝8.415、13.296，P<0.001），且观察组升高幅度大于对照组（t＝3.527、6.690，P<0.001），CD+8 治疗后下降（对照组t＝3.609，观察组＝10.764，P<0.001），且观察组下降程度大于对照组（t＝8.456，P<0.001），比较均差异有统计学意义。见表2。
2.3　两组治疗前后炎性反应指标变化比较
两组患者治疗前IL-6、IL-2、IL-17、TNF-α水平比较差异无统计学意义（t＝0.059、0.077、0.068、0.669，P=0.953、0.939、0.946、0.506），治疗后IL-6、IL-17、TNF-α较治疗前下降（对照组t＝15.286、8.225、14.054，观察组＝21.720、19.341、22.108，P<0.001），且观察组下降幅度大于对照组（t＝7.274、8.017、4.513，P<0.001），IL-2治疗后升高（对照组t＝12.372，观察组＝21.720，P<0.001），且观察组升高程度大于对照组（t＝7.274，P<0.001），比较均差异有统计学意义。见表3。
2.4　两组疗效比较
观察组治疗疗效优于对照组（P<0.05）。见表4。
3　讨论

胃癌在我国的发病率及病死率均较高[11-13]，而且大部分患者确诊时病情已经处于进展期，早期发现者较少。癌前病变是病理概念，胃黏膜异型增生是胃癌前病变的一种病理表现形式，此阶段如果积极治疗，部分患者黏膜可逆性改变恢复为正常黏膜，从而阻止其发展为胃癌。在中医理论中并无癌前病变的描述，但是从胃黏膜异型增生的病机演变来看其属于中医的“痞”“积”病，综合古今医家的观点，大部分学者认为胃黏膜癌前病变进展为胃癌是脾胃亏虚-胃络瘀阻-毒损胃络这样的过程。清·李用粹《证治汇补·痞满》中也有记载：“大抵心下痞闷，必是脾胃受亏”，清·沈金鳌《杂病源流犀烛》有云：“痞满，脾病也，本由脾气虚，及气郁运化，心下痞塞满”，其病起多为虚，脾胃虚弱是发病之本，蕴久则化毒，而胃络瘀阻则是关键，毒损胃络是发病重要因素，在胃黏膜癌前病变的发病过程中存在气虚、肝郁、脾虚、胃虚、血瘀、毒损等不同变化，患者早期因劳倦过度、饮食不节等原因导致胃胀、胃脘痛等症状，为脾胃虚弱、纳运不佳、水谷精微化生无序的表现，是胃黏膜癌前病变的核心病机[1-3，14-16]，随着病情进展，脾胃阴阳失调，情志失常肝气郁结，反之伤脾胃，致胃气不降肝气不升，中焦气机不畅，初气结于经、久则伤血入络致气滞血瘀，中期渐成虚实夹杂之证，出现湿热寒等实质，病至后期脾胃虚弱与血瘀常互为因果，内生邪毒，因此胃黏膜癌前病变患者多见寒热错杂证及脾胃湿热证，同时也可兼夹其他证型，治疗则需辨证后采取祛湿清热、活血化瘀、健脾祛痰、解毒通络为治则，脾胃调理、补虚祛实、辛苦同施、平调寒热、寒热并用等“和解”法来调节人体阴阳气血，达到控制病情恶化的目的。
半夏泻心汤张仲景所创，为治“心下痞”之代表方，《伤寒论·辨太阳病脉证并治》：“但满而不痛，此为痞。”《金匮要略·呕吐哕下利病脉证治》言：“呕而肠鸣，心下痞者，半夏泻心汤主之。”方中半夏、干姜有辛温散寒功效，黄芩、黄连寒降泄热，人参、大枣、甘草益气补中，全方泻中有补，起到重启中焦、气机得畅、升降复常、除却痞满、调节阴阳作用。临床报道治疗胃黏膜癌前病变可改善症状，同时减轻异型增生、肠上皮化生及腺体萎缩[6-8]。幽门螺杆菌感染是胃癌及癌前病变发生的确切因素[17-18]，有报道显示半夏泻心汤对幽门螺杆菌有杀菌作用[19-20]，可明显提高三联及四联杀幽门螺杆菌的效果。半夏泻心汤对胃黏膜癌前病变的病理改变有逆转作用，促使病态的细胞凋亡，恢复正常胃黏膜细胞的增殖，起到逆转作用，而且还可抑制促癌基因活性、改善炎性反应微环境：（1）通过促进P3等抑癌基因的表达，起到逆转作用[21-22]；（2）对胃黏膜癌前病变中表皮生长因子受体的表达有下调作用，起到调节细胞增殖作用；对B淋巴细胞瘤-2等抗凋亡细胞有平衡调节作用，可能是逆转癌前病变的机制之一[23-24]；（3）相关研究显示[25-26]，半夏泻心汤可显著降低核蛋白基因-κB（NF-κB）、信号转导及转录活化因子3（STAT3）、促炎因子IL-1β的表达，改善炎性反应微环境，降低TNF-α等促炎因子的水平，改善胃黏膜内皮细胞的损伤从而达到治疗胃黏膜癌前病变的目的。现代医学研究发现半夏泻心汤中多种单药有改善胃肠道功能的作用，半夏有抑制胃酸分泌、增加肠蠕动作用[27]，黄芩、黄连等有抗幽门螺杆菌作用[28-29]，同时半夏泻心汤有调节免疫功能作用[30]。本研究对半夏泻心汤治疗胃黏膜异性增生的疗效及对免疫功能、胃黏膜炎性反应的影响进行了研究，研究结果显示观察组患者心下胀满、胃痛、倦怠乏力、便溏、嗳气、纳呆、口淡不渴等症状的改善程度明显优于对照组，提示加服半夏泻心汤对于改善胃黏膜异型增生患者的症状明显有益。CD+3、CD+4、CD+8 是反映机体T淋巴细胞免疫功能的主要指标，研究结果显示观察组患者治疗后CD+3、CD+4 升高幅度大于对照组，CD+8 治疗后下降程度大于对照组，提示观察组患者免疫功能改善情况优于对照组。IL-6、IL-2、IL-17、TNF-α等均能反应机体炎性反应状况，同时IL-2还可反映机体体液免疫功能，观察组IL-2的升高提示机体体液免疫功能得到了进一步恢复，IL-6、IL-17、TNF-α的下降提示机体炎性反应得到抑制，这对于胃黏膜损伤的恢复更为有利。分析治疗结果，观察组患者整体疗效优于对照组，进一步证实半夏泻心汤对于胃黏膜异型增生患者疗效是肯定的，这与相关临床报道结果一致。
综上所述，半夏泻心汤治疗胃黏膜异型增生（脾胃湿热证）可有助于缓解疾病相关症状，改善免疫功能，抑制炎性反应，提高治疗疗效。
