Objective

To study the clinical effect of Xinshuai No.1 Formula（心衰1号）combined with sacubitril valsartan in the treatment of chronic heart failure with hypotension.

Methods

A hundred patients with chronic heart failure and hypotension treated in the hospital from August 2022 to July 2023 were selected and divided into observation group and control group according to the random number table method，with 50 patients in each group. The patients in the control group were treated with sacubitril valsartan，and those in the observation group were treated with Xinshuai No.1 Formula combined with sacubitril valsartan for 4 weeks. The efficacy of cardiac function，blood pressure changes，serum inflammatory factors，quality of life and adverse reactions were observed and compared between the two groups.

Results

The effective rate of cardiac function in the observation group was 68.00%（34/50），which was significantly higher than that in the control group（48.00%，24/50）（P<0.05）. After treatment，the systolic blood pressure and diastolic blood pressure of the two groups were significantly increased（P<0.05），and the pressures of the observation group were significantly higher than those of the control group（P<0.05）. There was no significant difference in serum high-sensitivity C-reactive protein（hs-CRP），interleukin-6（IL-6）or tumor necrosis factor-α（TNF-α）between the two groups before treatment（P>0.05）. After treatment，the serum inflammatory factors of the two groups were significantly reduced，and the levels of the observation group were significantly lower than those of the control group（P<0.05）. After treatment，the physical domain score and total score of the Chinese version of the Minnesota Heart Failure Quality of Life Scale（MLHFQ）in the two groups were significantly decreased，and the physical domain score and total score of the MLHFQ in the observation group were significantly higher than those in the control group（P<0.05）. The incidence of adverse reactions in the observation group and the control group was 8.00%（4/50）and 10.00%（5/50）and the difference was not statistically significant（P>0.05）.

Conclusion

The clinical effect of Xinshuai No.1 Formula combined with sacubitril valsartan in the treatment of chronic heart failure with hypotension is satisfactory. It can better improve the cardiac function and blood pressure level，reduce the body's inflammatory response and improve the quality of life of patients，and the treatment is safe and reliable.

目的

研究心衰1号中药联合沙库巴曲缬沙坦治疗慢性心力衰竭伴低血压的临床效果。

方法

选取2022年8月—2023年7月医院收治的慢性心力衰竭伴低血压患者100例，按照随机数表法分为观察组和对照组，每组50例。对照组采用沙库巴曲缬沙坦治疗，观察组采用心衰1号中药联合沙库巴曲缬沙坦治疗，疗程4周。观察并比较两组患者的心功能疗效、血压变化、血清炎症因子、生活质量及不良反应情况。

结果

观察组心功能疗效显效率68.00%（34/50），显著高于对照组的48.00%（24/50）（P<0.05）。治疗后两组患者收缩压、舒张压均显著升高（P<0.05），且观察组显著高于对照组（P<0.05）。治疗前两组血清超敏C反应蛋白（Hypersensitive C-reactive protein，hs-CRP）、白细胞介素6（Interleukin-6，IL-6）、肿瘤坏死因子α（Tumour necrosis factor-α，TNF-α）比较差异均无统计学意义（P>0.05），治疗后两组以上各血清炎症因子指标均显著降低，且观察组显著低于对照组（P<0.05）。治疗后两组患者中文版本明尼苏达心功能不全生命质量量表（Minnesota living with heart failure questionnaire，MLHFQ）躯体领域评分及总分均显著降低，观察组MLHFQ躯体领域评分及总分显著高于对照组（P<0.05）。观察组、对照组不良反应发生率分别8.00%（4/50）、10.00%（5/50），差异无统计学意义（P>0.05）。

结论

心衰1号中药联合沙库巴曲缬沙坦治疗慢性心力衰竭伴低血压的临床效果满意，能够更好地改善心功能状况及血压水平，降低机体炎症反应，提高患者生活质量，并且治疗安全性可靠。

目的

研究心衰1号中药联合沙库巴曲缬沙坦治疗慢性心力衰竭伴低血压的临床效果。

方法

选取2022年8月—2023年7月医院收治的慢性心力衰竭伴低血压患者100例，按照随机数表法分为观察组和对照组，每组50例。对照组采用沙库巴曲缬沙坦治疗，观察组采用心衰1号中药联合沙库巴曲缬沙坦治疗，疗程4周。观察并比较两组患者的心功能疗效、血压变化、血清炎症因子、生活质量及不良反应情况。

结果

观察组心功能疗效显效率68.00%（34/50），显著高于对照组的48.00%（24/50）（P<0.05）。治疗后两组患者收缩压、舒张压均显著升高（P<0.05），且观察组显著高于对照组（P<0.05）。治疗前两组血清超敏C反应蛋白（Hypersensitive C-reactive protein，hs-CRP）、白细胞介素6（Interleukin-6，IL-6）、肿瘤坏死因子α（Tumour necrosis factor-α，TNF-α）比较差异均无统计学意义（P>0.05），治疗后两组以上各血清炎症因子指标均显著降低，且观察组显著低于对照组（P<0.05）。治疗后两组患者中文版本明尼苏达心功能不全生命质量量表（Minnesota living with heart failure questionnaire，MLHFQ）躯体领域评分及总分均显著降低，观察组MLHFQ躯体领域评分及总分显著高于对照组（P<0.05）。观察组、对照组不良反应发生率分别8.00%（4/50）、10.00%（5/50），差异无统计学意义（P>0.05）。

结论

心衰1号中药联合沙库巴曲缬沙坦治疗慢性心力衰竭伴低血压的临床效果满意，能够更好地改善心功能状况及血压水平，降低机体炎症反应，提高患者生活质量，并且治疗安全性可靠。

Objective

To study the clinical effect of Xinshuai No.1 Formula（心衰1号）combined with sacubitril valsartan in the treatment of chronic heart failure with hypotension.

Methods

A hundred patients with chronic heart failure and hypotension treated in the hospital from August 2022 to July 2023 were selected and divided into observation group and control group according to the random number table method，with 50 patients in each group. The patients in the control group were treated with sacubitril valsartan，and those in the observation group were treated with Xinshuai No.1 Formula combined with sacubitril valsartan for 4 weeks. The efficacy of cardiac function，blood pressure changes，serum inflammatory factors，quality of life and adverse reactions were observed and compared between the two groups.

Results

The effective rate of cardiac function in the observation group was 68.00%（34/50），which was significantly higher than that in the control group（48.00%，24/50）（P<0.05）. After treatment，the systolic blood pressure and diastolic blood pressure of the two groups were significantly increased（P<0.05），and the pressures of the observation group were significantly higher than those of the control group（P<0.05）. There was no significant difference in serum high-sensitivity C-reactive protein（hs-CRP），interleukin-6（IL-6）or tumor necrosis factor-α（TNF-α）between the two groups before treatment（P>0.05）. After treatment，the serum inflammatory factors of the two groups were significantly reduced，and the levels of the observation group were significantly lower than those of the control group（P<0.05）. After treatment，the physical domain score and total score of the Chinese version of the Minnesota Heart Failure Quality of Life Scale（MLHFQ）in the two groups were significantly decreased，and the physical domain score and total score of the MLHFQ in the observation group were significantly higher than those in the control group（P<0.05）. The incidence of adverse reactions in the observation group and the control group was 8.00%（4/50）and 10.00%（5/50）and the difference was not statistically significant（P>0.05）.

Conclusion

The clinical effect of Xinshuai No.1 Formula combined with sacubitril valsartan in the treatment of chronic heart failure with hypotension is satisfactory. It can better improve the cardiac function and blood pressure level，reduce the body's inflammatory response and improve the quality of life of patients，and the treatment is safe and reliable.

《慢性心力衰竭加重患者的综合管理中国专家共识2022》[1]中指出我国心力衰竭患病率持续升高，心衰患者的高病死率、高住院率带来巨大的公共卫生负担，心力衰竭加重与慢性心力衰竭患者的死亡和再住院密切相关。慢性心力衰竭患者心脏的收缩和舒张功能会受到影响，心脏的射血量下降，外周有效循环血量就会减少，容易伴发低血压，成为诊治工作的重要问题[2-3]。有研究[4-6]表明中西医结合能够优势互补，对慢性心力衰竭伴低血压发挥更佳的治疗作用。本研究分析心衰1号中药联合沙库巴曲缬沙坦治疗慢性心力衰竭伴低血压的临床效果，现对相关病例资料进行总结并报道如下。
1　资料与方法

1.1　一般资料
选取2022年8月—2023年7月医院治疗的慢性心力衰竭伴低血压患者100例，纳入标准：均符合《慢性心力衰竭基层诊疗指南（2019年）》[7]中慢性心力衰竭诊断标准；纽约心脏病协会（New York Heart Association，NYHA）心功能Ⅱ～Ⅳ级，左室射血分数（Left ventricular ejection fraction，LVEF）<40%；患者舒张压<60 mmHg和/或收缩压<90 mm-Hg；病情稳定；知情同意研究内容。排除标准：血管紧张素转化酶抑制剂或血管紧张素Ⅱ受体拮抗剂用药患者；恶性肿瘤及严重肝肾疾病患者；起搏器植入的慢性心律失常患者。按照随机数表法分为观察组和对照组，每组各50例患者，两组患者的性别、年龄、病程、基础病因情况见表1，经统计学分析表明组间差异无统计学意义（P>0.05），一般资料均有可比性。
1.2　方法
两组患者均常规接受β受体阻滞剂、利尿药、洋地黄、硝酸酯类等抗心力衰竭治疗，同时对照组患者采用沙库巴曲缬沙坦治疗，起始剂量为每次25～50 mg，2次/d，根据患者耐受情况每2～4周倍增1次，直至达到每次200 mg，2次/d，维持剂量。观察组采用心衰1号中药联合沙库巴曲缬沙坦治疗，沙库巴曲缬沙坦用药同对照组，心衰1号中药组分：炙黄芪20 g，党参15 g，炮附子10g，茯苓15 g，炒白术15 g，桂枝20 g，干姜15 g，白芍15 g，麦冬20 g，葶苈子10 g，桃仁10 g，炙甘草9 g，1剂/d，水煎取汁300 mL，分早晚2次服用。两组患者均疗程4周。
1.3　观察指标
（1）心功能疗效。治疗后根据NYHA心功能分级改善情况判定疗效，心功能改善2级为显效，改善1级为有效，无改善或恶化为无效[8]。（2）血压变化。检查前确定电子血压计电量充足、袖带完好、充气密闭并完成校正以确保测量结果准确性，测量血压之前嘱患者静5～10 min，取坐位，充分暴露患者上臂，将电子血压计袖带固定至肘关节上距离肘窝1～2 cm处，保持手肘或手腕应与心脏处于同一水平面上，使血压计基本与心脏平齐，开始测量并读取数值，需要测量3次取平均值。（3）血清炎症因子。血清超敏C反应蛋白（Hypersensitive C-reactive protein，hs-CRP）采用免疫比浊法测定，白细胞介素6（Interleukin-6，IL-6）、肿瘤坏死因子α（Tumour necrosis factor-α，TNF-α）采用酶联免疫吸附试验法测。（4）生活质量。采用中文版本明尼苏达心功能不全生命质量量表（Minnesota living with heart failure questionnaire，MLHFQ）[9]评估患者生活质量，包括躯体领域8个条目、情绪领域5个条目、其他领域8个条目，共计21个项目，每个条目根据患者回答“没有”“很轻”“轻度”“中度”“重度”“很重”对应0、1、2、3、4、5分，总分0～105分，评分越高则生活质量越差。（5）不良反应。记录患者用药期间有无头晕、恶心、腹痛、耳鸣等不良反应发生。
1.4　统计学分析
以统计学软件包SPSS 23.0为数据分析工具，计数资料用［例（%）］表示，行χ2检验，计量资料正态分布用（）表示，行t检验，P<0.05表示差异有统计学意义。
2　结果

2.1　心功能疗效分析
观察组心功能疗效显效率68.00%，显著高于对照组的48.00%，两组比较差异有统计学意义（P<0.05），两组有效率、无效率差异无统计学意义（P>0.05）。见表2。
2.2　血压变化分析
治疗后两组患者收缩压、舒张压均显著升高（P<0.05），且观察组显著高于对照组（P<0.05）。见表3。
2.3　血清炎症因子水平分析
治疗前，两组血清hs-CRP、IL-6、TNF-α水平比较差异均无统计学意义（P>0.05）；治疗后，两组以上各血清炎症因子指标均显著降低，且观察组显著低于对照组（P<0.05）。见表4。
2.4　生活质量分析
治疗前，两组MLHFQ各维度及总分差异均无统计学意义（P>0.05）；治疗后，两组患者MLHFQ躯体领域评分及总分均显著降低，观察组MLHFQ躯体领域评分及总分显著高于对照组（P<0.05），两组情绪领域、其他领域评分差异仍无统计学意义（P>0.05）。见表5。
2.5　不良反应发生率分析
观察组、对照组不良反应发生率分别8.00%、10.00%，差异无统计学意义（P>0.05）。见表6。
3　讨论

慢性心力衰竭是各种心血管病和非心血管病的终末阶段，在中医学中主要归于“心悸”“怔忡”“喘证”“水肿”“心水”等范畴，心主血脉，血脉运行全赖心中阳气的推动，心系疾患日久，心气不足，心阳不振，心之阳气推动无力，血行滞缓，血脉痹阻，气血推动无力，清窍失养，容易出现低血压症状[10-12]。患者心阳不足，经气不利，血行不畅，水气内停，泛滥肌肤，凌心射肺，则发为心悸、喘促、水肿等症。治则补气温阳，活血通络，利水消肿[13-14]。
中医药治疗心力衰竭具有悠久的历史，并形成了较为系统的理论体系，但相关工作者一直在不断探寻更佳的用药方案，以期降低慢性心力衰竭患者的住院率和病死率、减轻其给机体带来的危害[15-16]。心衰1号方是中国中医科学院徐贵成团队以《伤寒论》中温阳利水之经方为主体所创，经动物实验[17]、临床验证[18]其应用效果均得到认可。本研究中观察组增加心衰1号中药治疗效果满意，其中，炙黄芪益气补中；党参健脾益肺、养血生津；炮附子回阳救逆、补火助阳、散寒除湿，《本草正义》：“附子，本是辛温大热，其性善走，故为通十二经纯阳之要药，外则达皮毛而除表寒，里则达下元而温痼冷，彻内彻外，凡三焦经络，诸脏诸腑，果有真寒，无不可治”；茯苓利水渗湿、健脾宁心；炒白术健脾益气、燥湿利水、止汗，《医学启源》记载：“除湿益燥，和中益气，温中，去脾胃中湿，除胃热，强脾胃……”桂枝发汗解肌、温通经脉、助阳化气；干姜温中散寒，回阳通脉，温肺化阴；白芍养血敛阴、柔肝止痛、平抑肝阳，《本草求真》言：白芍有敛阴益营之力，能于土中泻；麦冬养阴润肺、益胃生津、清心除烦；葶苈子泻肺平喘、利水消肿；桃仁活血祛瘀、润肠通便、止咳平喘，《本草纲目》：桃仁行血，宜连皮、尖生用；炙甘草益气补中，润肺止咳，清热解毒，缓急止痛，调和药性；以上药物合用可以益气温阳、行血活血，使血脉通利、同时温煦运化水液。体现在本研究数据结果中更好地改善了患者心功能，又改善了低血压状况。现代药理研究表明，党参可增强机体免疫力，促进造血机能[19]；白术促进红细胞造血作用[20]；干姜能抗炎、抗凝血[21]；白芍含有芍药苷，对细胞免疫和体液免疫反应均具有功能性依赖性双向调节作用，具有抗心肌缺血的作用[22]；桃仁提取液能降低血管阻力，扩张血管，抑制血小板的聚集，还具有抗炎、免疫调节等作用[23-24]。因此，心衰1号中药的应用能够更好地改善研究中患者血清炎症因子水平，改善机体状态进而提高患者生活质量[25-26]。
综上所述，心衰1号中药联合沙库巴曲缬沙坦治疗慢性心力衰竭伴低血压的临床效果满意，能够更好地改善心功能状况及血压水平，降低机体炎症反应，提高患者生活质量，并且治疗安全性可靠。
