Objective

To observe the clinical efficacy，recurrence rate and serum inflammation of patients with recurrent oral ulcer treated by Kouyanqing Granules（口炎清颗粒）combined with Kangfuxin Liquid（康复新液）.

Method

Sixty patients with mild recurrent oral ulcer treated in the stomatology department of the hospital from January 2020 to September 2022 were selected as the research objects. The patients were divided into conventional treatment group and Kouyanqing Granules group by random number table method，with 30 cases in each group. The conventional treatment group was given Kangfuxin Liquid. The Kouyanqing Granules group was orally treated with Kouyanqing Granules on the basis of conventional treatment group. The clinical efficacy，clinical symptoms，recurrence rate and adverse reactions of the two groups were compared as well as the changes of serum inflammatory factors and immune function before and after intervention.

Results

The effective rate of the conventional treatment group was 73.33%（22/30）and that of Kouyanqing Granules group was 93.33%（28/30）. Compared with the conventional treatment group，the effective rate of the Kouyanqing Granules group was significantly increased（P<0.05）. Compared with those of the conventional treatment group，the pain duration，wound healing time and feeding improvement time in the Kouyanqing Granules group were significantly shortened and the differences were statistically significant（P<0.01，P<0.05）. After the intervention，the levels of CD3+，CD4+and CD4+/CD8+in peripheral blood of the Kouyanqing Granules group were significantly higher than those of the conventional treatment group，while the levels of CD8+were significantly decreased（P<0.01，P<0.05）. After the intervention，the serum levels of IL-1，IL-6 and TNF-αin the Kouyanqing Granules group were significantly decreased compared with those in the conventional treatment group（P<0.01）. There was no significant difference in recurrence rate between the two groups at 3 and 6 months after the intervention（P>0.05）. However，compared with those of the conventional treatment group，the total recurrence rate of the Kouyanqing Granules group was significantly reduced（P<0.05）. No obvious drug-related adverse reactions or allergic reactions occurred in the two groups during the intervention.

Conclusion

Kouyanqing Granules combined with Kangfuxin Liquid have good clinical effects in the treatment of recurrent oral ulcer，which can significantly improve the clinical symptoms of patients and reduce the recurrence rate. Its mechanism of action is related to inhibiting serum inflammatory response and regulating the body's immune function.

目的

观察口炎清颗粒联合康复新液治疗复发性口腔溃疡临床疗效、复发率及其对患者血清炎症水平的影响。

方法

选择2020年1月—2022年9月于南阳医学高等专科学校第一附属医院口腔科治疗的60例轻复发性口腔溃疡患者作为研究对象，采用随机数字表法将上述患者分为常规治疗组和口炎清组，每组30例，常规治疗组予以康复新液含漱治疗，口炎清组在常规治疗组基础上予以口炎清颗粒口服治疗。比较两组患者的临床疗效、临床症状、复发率和不良反应情况，以及干预前后血清炎症因子和免疫功能变化。

结果

常规治疗组有效率为73.33%（22/30），口炎清组有效率为93.33%（28/30），与常规治疗组比较，口炎清组的有效率明显升高（P<0.05）。与常规治疗组比较，口炎清组疼痛持续时间、创面愈合时间以及进食改善时间均明显缩短，差异具有统计学意义（P<0.01，P<0.05）。干预完成后，口炎清组患者外周血中CD3+、CD4+以及CD4+/CD8+水平相较于常规治疗组均明显升高，而CD8+水平则明显下降（P<0.01，P<0.05）。干预完成后，口炎清组的血清白介素（interleukin，IL）-1、IL-6以及肿瘤坏死因子（tumor necrosis factor，TNF）-α水平相较于常规治疗组均明显下降（P<0.01）。干预完成后3个月、6个月时两组复发率差异无统计学意义（P>0.05）；但相较于常规治疗组，口炎清组患者的总复发率明显降低（P<0.05）。干预过程中两组均未发生明显药物相关不良反应或过敏反应。

结论

口炎清颗粒联合康复新液治疗复发性口腔溃疡具有良好临床效果，能够明显改善患者的临床症状、降低复发率，其作用机制与抑制血清炎症反应、调节机体免疫功能相关。

目的

观察口炎清颗粒联合康复新液治疗复发性口腔溃疡临床疗效、复发率及其对患者血清炎症水平的影响。

方法

选择2020年1月—2022年9月于南阳医学高等专科学校第一附属医院口腔科治疗的60例轻复发性口腔溃疡患者作为研究对象，采用随机数字表法将上述患者分为常规治疗组和口炎清组，每组30例，常规治疗组予以康复新液含漱治疗，口炎清组在常规治疗组基础上予以口炎清颗粒口服治疗。比较两组患者的临床疗效、临床症状、复发率和不良反应情况，以及干预前后血清炎症因子和免疫功能变化。

结果

常规治疗组有效率为73.33%（22/30），口炎清组有效率为93.33%（28/30），与常规治疗组比较，口炎清组的有效率明显升高（P<0.05）。与常规治疗组比较，口炎清组疼痛持续时间、创面愈合时间以及进食改善时间均明显缩短，差异具有统计学意义（P<0.01，P<0.05）。干预完成后，口炎清组患者外周血中CD3+、CD4+以及CD4+/CD8+水平相较于常规治疗组均明显升高，而CD8+水平则明显下降（P<0.01，P<0.05）。干预完成后，口炎清组的血清白介素（interleukin，IL）-1、IL-6以及肿瘤坏死因子（tumor necrosis factor，TNF）-α水平相较于常规治疗组均明显下降（P<0.01）。干预完成后3个月、6个月时两组复发率差异无统计学意义（P>0.05）；但相较于常规治疗组，口炎清组患者的总复发率明显降低（P<0.05）。干预过程中两组均未发生明显药物相关不良反应或过敏反应。

结论

口炎清颗粒联合康复新液治疗复发性口腔溃疡具有良好临床效果，能够明显改善患者的临床症状、降低复发率，其作用机制与抑制血清炎症反应、调节机体免疫功能相关。

Objective

To observe the clinical efficacy，recurrence rate and serum inflammation of patients with recurrent oral ulcer treated by Kouyanqing Granules（口炎清颗粒）combined with Kangfuxin Liquid（康复新液）.

Method

Sixty patients with mild recurrent oral ulcer treated in the stomatology department of the hospital from January 2020 to September 2022 were selected as the research objects. The patients were divided into conventional treatment group and Kouyanqing Granules group by random number table method，with 30 cases in each group. The conventional treatment group was given Kangfuxin Liquid. The Kouyanqing Granules group was orally treated with Kouyanqing Granules on the basis of conventional treatment group. The clinical efficacy，clinical symptoms，recurrence rate and adverse reactions of the two groups were compared as well as the changes of serum inflammatory factors and immune function before and after intervention.

Results

The effective rate of the conventional treatment group was 73.33%（22/30）and that of Kouyanqing Granules group was 93.33%（28/30）. Compared with the conventional treatment group，the effective rate of the Kouyanqing Granules group was significantly increased（P<0.05）. Compared with those of the conventional treatment group，the pain duration，wound healing time and feeding improvement time in the Kouyanqing Granules group were significantly shortened and the differences were statistically significant（P<0.01，P<0.05）. After the intervention，the levels of CD3+，CD4+and CD4+/CD8+in peripheral blood of the Kouyanqing Granules group were significantly higher than those of the conventional treatment group，while the levels of CD8+were significantly decreased（P<0.01，P<0.05）. After the intervention，the serum levels of IL-1，IL-6 and TNF-αin the Kouyanqing Granules group were significantly decreased compared with those in the conventional treatment group（P<0.01）. There was no significant difference in recurrence rate between the two groups at 3 and 6 months after the intervention（P>0.05）. However，compared with those of the conventional treatment group，the total recurrence rate of the Kouyanqing Granules group was significantly reduced（P<0.05）. No obvious drug-related adverse reactions or allergic reactions occurred in the two groups during the intervention.

Conclusion

Kouyanqing Granules combined with Kangfuxin Liquid have good clinical effects in the treatment of recurrent oral ulcer，which can significantly improve the clinical symptoms of patients and reduce the recurrence rate. Its mechanism of action is related to inhibiting serum inflammatory response and regulating the body's immune function.

复发性口腔溃疡是多发于唇、颊以及舌缘等部位的口腔科常见疾病，又被称为复发性阿弗他口炎，患者主要表现为反复发作的口腔黏膜红肿、疼痛、破溃等，溃疡常呈圆形或椭圆形，触之易出血，表面常覆盖以灰白色假膜，虽然对患者生命安全构不成明显威胁，但严重影响其日常沟通、进食及健康[1-3]。复发性口腔溃疡约占普通人群口腔溃疡患病率的20%，而其在特定人群中的患病率高达60%以上，但临床发病无明显区域、民族、性别以及年龄差异，属于口腔溃疡的常见类型[4-5]。复发性口腔溃疡的病因复杂且发病机制尚不明确，目前认为与慢性微炎症状态、机体免疫功能失调、维生素缺乏、消化不良、心理应激以及上呼吸道感染等因素相关[6-7]。相关研究指出，相较于正常健康人，复发性口腔溃疡患者的细胞免疫功能受损，血清免疫细胞水平有一定程度的下降[8]。在治疗方面，目前尚无确切有效的根治办法，西医多以消炎、止疼、促进创面恢复并预防继发性感染等对症治疗为主，虽然能够暂时愈合溃疡面、缓解疼痛，但其疗效往往存在个体差异，且周期性和复发率较高，还有可能进一步损伤患者的免疫功能[9-10]。近年临床发现传统中医中药和外治法对复发性口腔溃疡具有较好的治疗效果，口炎清颗粒和康复新液均为治疗口腔溃疡的常用药[11]。本研究通过观察口炎清联合康复新液治疗复发性口腔溃疡的临床疗效和复发情况，分析其免疫和炎症方面的作用机制。
1　临床资料

1.1　一般资料
本课题经过南阳医院高等专科学校第一附属医院伦理委员会批准试验研究。选择2020年1月—2022年9月于南阳医学高等专科学校第一附属医院口腔科治疗的60例轻复发性口腔溃疡作为研究对象，采用随机数字表法将上述患者分为常规治疗组和口炎清组，每组30例，常规治疗组予以康复新液含漱治疗，口炎清组在常规治疗组基础上予以口炎清颗粒口服治疗。两组一般临床资料比较，差异无统计学意义（P>0.05）。见表1。
1.2　纳入标准
（1）病史及黏膜病变特点符合第3版《口腔黏膜病学》中复发性口腔溃疡的相关标准，中医诊断符合《消化系统常见病复发性口腔溃疡中医诊疗指南》中“口疮”的相关标准；（2）年龄18～65岁；（3）病程1年以上，每月发病1次以上；（4）溃疡数目5个及以下、此次发病尚未接受治疗；（5）临床资料完整，患者及家属签署知情同意。
1.3　排除标准
（1）由于创伤、感染、癌症、白塞病等免疫疾病或其他继发因素引起的口腔溃疡患者；（2）入组前1月内接受过抗生素、激素、免疫抑制剂等药物治疗者；（3）对本课题使用药物或成分过敏或存在禁忌者；（4）合并胃炎、慢性肠炎、消化性溃疡、胃肠道出血等疾病者；（5）合并心肝肾等重要脏器功能不全、血液系统、恶性肿瘤或全身性急慢性感染者；（6）妊娠或哺乳期女性；（7）存在意识障碍或精神类疾病者。
2　方法

常规治疗组：治疗期间调节饮食结构，严格戒烟酒、忌食辛辣，保证新鲜蔬果的摄入量、睡眠充足，同时适当配合运动。该组予以四川好医生攀西药业有限公司生产的康复新液（国药准字Z51021834）含漱治疗，即治疗前用江苏知原药业股份有限公司的复方氯己定含漱液（国药准字H32026694）漱口，然后含康复新液10 mL并维持5 min，治疗后30 min内禁食、水，3次/d，两组均连续治疗7 d。
口炎清组：在常规治疗组的基础上予以广州白云山和记黄埔中药有限公司的口炎清颗粒（国药准字Z44021730）口服，20 g/次，2次/d。
3　观察指标

3.1　临床疗效比较
治疗有效率＝（显效+有效）例数/总例数×100%。其中治疗完成后，局部疼痛感消失，创面愈合面积在80%以上者，为显效；局部疼痛感有所改善，创面愈合面积在30%～80%之间，为有效；疼痛感未见好转甚至加重，创面愈合面积不足30%，为无效。
3.2　临床症状比较
比较两组溃疡疼痛持续时间、创面愈合时间以及进食改善时间。
3.3　免疫功能比较
于干预前后采用流式细胞仪检测两组患者的CD+3、CD+4以及CD+8等外周血T淋巴细胞亚群水平，计算并比较CD+4/CD+8比值。
3.4　血清炎症水平比较
分别于干预前后抽取两组患者空腹静脉血约5 mL，离心后分离上清，采用酶联免疫吸附法检测血清中白介素（interleukin，IL）-6、IL-1以及肿瘤坏死因子-α（tumor necrosis factor-α，TNF-α）水平，操作严格按照试剂盒说明书进行。
3.5　复发率比较
治疗完成后对两组进行为期6个月的随访，比较两组口腔溃疡的复发情况。
3.6　不良反应
治疗期间密切观察两组口腔溃疡局部病情，并记录有无药物不良反应发生。
4　统计学方法

应用SPSS 20.0软件进行统计学处理，以均数±标准差（）表示的计量资料采用独立样本t检验，以率（%）表示的计数资料采用χ2检验。P<0.05为差异有统计学意义。
5　结果

5.1　临床疗效比较
常规治疗组有效率为73.33%，口炎清组有效率为93.33%，与常规治疗组比较，口炎清组的有效率明显升高（P<0.05）。见表2。
5.2　临床症状比较
与常规治疗组比较，口炎清组疼痛持续时间、创面愈合时间以及进食改善时间均明显缩短，差异具有统计学意义（P<0.01，P<0.05）。见表3。
5.3　免疫功能比较
干预完成后，口炎清组患者外周血中CD+3、CD+4以及CD+4/CD+8水平相较于常规治疗组均明显升高，而CD+8水平则明显下降（P<0.01，P<0.05）。见表4、表5。
5.4　血清炎症水平比较
干预完成后，口炎清组患者的血清IL-1、IL-6以及TNF-α水平相较于常规治疗组均明显下降（P<0.01）。见表6。
5.5　复发率比较
干预完成后3个月、6个月时两组复发率差异无统计学意义（P>0.05）；但相较于常规治疗组，口炎清组患者的总复发率明显降低（P<0.05）。见表7。
5.6　不良反应比较
干预过程中两组患者均未发生明显药物相关不良反应或过敏反应。
6　讨论

口腔溃疡是发生于口腔黏膜表面角化程度较差部位的淡黄色或灰白色的溃烂，常伴有灼热疼痛感，复发性口腔溃疡还具有反复发作的特点[12]。研究表明，该病病因复杂，病机尚未完全明确，目前临床主要采用维生素、激素、消炎止痛药以及免疫调节制剂等化学药物治疗[13]。西医治疗本病虽能够缓解症状，但具有局限性，中医通过辨证论治、注重内外兼治，在改善中医证候、促进溃疡愈合的同时，将局部与全身治疗相结合，从而减少患者的复发率[14]。本研究中，常规治疗组有效率为73.33%，口炎清组有效率为93.33%，与常规治疗组比较，口炎清组的有效率明显升高（P<0.05）。干预完成后3个月、6个月时两组复发率差异无统计学意义（P>0.05）；但相较于常规治疗组，口炎清组总复发率明显降低（P<0.05）。复发性口腔溃疡患者常伴有微炎症状态，导致血清中相关炎症因子水平升高[15]。TNF-α、IL-6以及IL-1均为常见的炎症介质，能够促进免疫细胞分化、增殖，还参与炎症部位的中性粒细胞募集反应，在机体发生炎症反应或组织损伤时其表达水平上调并大量释放入血，且其水平通常与炎症反应程度呈正相关[16-17]。本研究结果显示，干预完成后，口炎清组患者的血清IL-1、IL-6以及TNF-α水平相较于常规治疗组均明显下降（P<0.01）；口炎清组患者外周血中CD3+、CD4+以及CD4+/CD8+水平相较于常规治疗组均明显升高，而CD8+水平则明显下降（P<0.01，P<0.05）。有研究指出，复发性口腔溃疡患者存在免疫功能失调，其外周血中CD3+、CD4+等T淋巴细胞亚群水平相较于正常人低，而通过积极有效的治疗后CD3+、CD4+水平均有所上升，表明复发性口腔溃疡患者存在免疫功能的下降[18-19]。
中医学根据复发性口腔溃疡的临床症状将其归属于“口糜”“口疮”“舌上生疮”等疾病范畴进行辨证论治，认为该病多由于先天禀赋异常、脏腑功能失调，加之外感六淫，或劳倦过度，或内伤情志，或过食辛辣、肥甘厚味以及嗜烟好酒等饮食不节因素而致[20-21]。其病位在口唇，因脾经系舌本，且脾开窍于口，故该病与脾胃关系密切[22]。多种病因相互影响、共同作用导致患者脾气虚弱、运化失调，升降失常，酿生湿热，虚火循经上炎，熏蒸肝胆，并使浊气冲于上焦，口舌失荣，发为口疮[23-24]。根据该病的关键病机，临床多以清热解毒，滋阴降火为治疗原则[25]。虽古代医籍中不乏治疗口疮效果良好的方剂，但由于其反复发作，目前临床上亟待研发出治疗复发性口腔溃疡疗效确切的中成药，以迅速缓解溃疡局部的疼痛感、降低复发率且方便患者服用。康复新液是临床治疗各类创面、溃疡的常用中药制剂，是提取干燥美洲大蠊虫体内有效成分制成的纯中药生物制剂，具有化腐生肌、消肿止痛、通利血脉的功效，临床研究证明该药能够促进创面愈合，有利于损伤创面的修复，临床广泛应用于胃及十二指肠溃疡、胃出血等疾病的治疗[26]。现代药理研究指出，康复新液中含有的多元醇、肽类以及氨基酸等有效成分，能够促进创面新生血管和肉芽组织的形成和生长，改善创面局部的微循环，还可促进损伤部位蛋白质胶原的合成，有利于创面黏膜的修复；同时还具有抑制炎症反应，抗菌、杀灭微生物的作用[23]。多年临床实践发现，反复发作是影响复发性口腔溃疡患者生活质量的主要原因，康复新液虽能促进口腔溃疡部位的愈合，但治疗时间短暂，对后续复发并无明显治疗和预防作用[27]。本研究中，与常规治疗组比较，口炎清组疼痛持续时间、创面愈合时间以及进食改善时间均明显缩短，差异具有统计学意义（P<0.01，P<0.05）。口炎清颗粒主要由天冬、玄参、山银花、麦冬、甘草等药物组成，是治疗“口糜”“口疮”“舌上生疮”等疾病的常用中成药，具有滋阴清热、消肿止痛的功效，方中金银花清热解毒；玄参清热凉血，滋阴解毒，利咽消肿；天冬、麦冬养阴润肺，益胃生津；甘草调和诸药。现代药理显示，该药有抗菌消炎、调节免疫功能的作用，并可有效降低口腔溃疡的复发率[28-29]。
综上所述，口炎清颗粒联合康复新液治疗复发性口腔溃疡具有良好的临床效果，能够明显改善患者的临床症状、降低复发率，其作用机制与抑制血清炎症反应、调节机体免疫功能相关，且无明显不良反应，适宜临床推广。
